Progestinic treatment of mastodynia. New suggestion for the follow-up.

The problem of progestin treatment for benign breast disease and mastodynia is still dividing Clinicians and Searchers: the traditional methods of clinical investigation and follow-up do not allow any further step in the debate. In this work we evaluated the role of E2/Progesterone R.I.A. and thermography in monitoring the effect of progestin on BBD and found that only the latter can give useful informations, insufficient, however, to direct the treatment. If we want to be able to select the patients suitable to be treated by progesterone, we must know the tissutal hormone receptivity and be able to reexamine it during the treatment to verify the effectiveness of the administered drug. These informations can be obtained through the assay of Estradiol and Progesterone Receptors in the most accessible hormone-dependent tissue: the endometrium. Our data point out a quite different behaviour of Estradiol and Progesterone cytoplasmic Receptors in the endometrium of women with mastodynia and controls. The knowledge of these informations in the single patient will let us come out from subjectivity when deciding about BBD therapy or when trying to correct unsatisfactory treatments.

Manometric study on anal sphyncteres and intestinal MTT before and after extensive surgery on patients affected by cervical carcinoma. Clinical experiences.

The Authors report the preliminary results of a study performed on patients submitted to radical hysterectomy according to Wertheim-Meigs for cervical-carcinoma, in order to evidence possible changes of the intestinal functionality, by means of a manometric investigation of the anal sphyncteres and measurement of the intestinal transit times. The manometric survey showed that no essential parameter was uniformly altered whereas the transit time measurement proved in some cases the slowing-down of the intestinal progression at the level of the ascending and transverse colon, and a normal transit in the descending colon as well as in the rectum in all examined cases.

Uptake and metabolism of [3H] pteroylglutamic acid by developing chick embryo.

Chick embryos at 9 and 12 days of development were injected into the air space with [3', 5', 7, 9 -3H]pteroylglutamic acid. After various time periods the incorporation of radioactivity into folate derivatives was measured. In the 9-day embryo the label incorporation into longer chain polyglutamates, expressed per gram, was higher than in the 12-day embryo. Since the endogenous content of these compounds was lower at 9 days, the results suggest that, at this stage of development, the turnover of longer pteroylpolyglutamates is more rapid.

Pteroylpolyglutamates in liver of phenobarbitone-treated rats.

The effects of "acute" phenobarbitone treatment on some aspects of folate metabolism in rat liver, that is total folate activity, protein bound and free folate, and various-length pteroylpolyglutamate amounts, were studied. The administration of two doses of the anticonvulsant drug (8 mg/100 g of body weight), caused a severe depletion in folate pool, free folate and in the percentage of longer-chain polyglutamates, while the amount of protein bound folates was unchanged. Such modifications are more evident in the animals killed at 24 h after the last drug injection rather than in those killed 12 h later. The possible mechanisms by which phenobarbitone might interfere with the synthesis of pteroylpolyglutamates, the actual coenzymic forms of folic acid, are discussed.

Androgen, estrogen and progesterone receptors in vulvar dystrophy.

Assays have been developed for the quantitation of androgen, estrogen and progesterone receptors in the cytosol of 45 vulvar dystrophic tissues. In 27 patients we assayed only cytoplasmatic receptors, while in 18 patients both cytoplasmatic and nuclear receptors. The workers verified the possibility to assay these carriers, finding significant correlations neither with patient's age nor between atrophic and hypertrophic dystrophies. The research concerning the cytoplasmatic depletion and nuclear accumulation allowed us to understand the action mechanism of the androgen in vulvar tissues in patients in menopause.

[Effect of folic acid deficiency on the synthesis of pteroylpolyglutamates].

In order to evaluate the effect of folate deficiency on pteroylpolyglutamate synthesis in rat liver, the hepatic and urinary content of reduced folates, the distribution of pteroylpolyglutamates and the incorporation of i.p. injected 3H folic acid in polyglutamates have been studied. In deficient rats the hepatic content of reduced folates and the urinary amounts of reduced metabolites are lower and do not increase after vitamin treatment. Longer chain pteroylpolyglutamates are severely depleted and their content is not significantly modified by folic acid treatment. The percentage of labelled folic acid incorporated into pteroylglutamates is markedly decreased. The inability of deficient rat liver to synthesize polyglutamates might be ascribed to the decrease of pteroylpolyglutamate synthetase activity, and/or to the lower availability of tetrahydrofolates, the preferred substrates for the enzyme.

Swelling and binding effects in a polymeric microfiber model for controlled drug release.

The diffusion of model molecules from water dispersions of ethylenevinyl-N,N-diethylglycinate copolymer microfibers has been studied in different conditions. The influence of pH on diffusion rate has been related to water association and swelling effects following structural transitions induced in the copolymer by the acidic medium, rather than to difference in binding of the diffusing species to the macromolecule.

Non mutilant radical vulvectomy vs radical vulvectomy.

20 women, matched for age, site of primary tumor, histotype and stage, have been selected among patients affected with vulvar carcinoma for a study comparing classic radical vulvectomy vs "non mutilant" radical surgery. Detailed results actually available are discussed, showing that "non mutilant" technique gives results comparable to classic radical vulvectomy in terms of survival and recurrence rate; with minor functional and aesthetic damage, minor side effects and shorter post operative course.

Angular displacements in the upper body of AK amputees during level walking.

The angular displacements of the longitudinal axis of the trunk, and of the latero-lateral axes of pelvis and shoulder girdle were measured in five normal subjects and four AK amputees during level walking at different speeds. Amputees used single axis prostheses with the SACH foot. Spatial measurements were carried out in three dimensions by means of a photogrammetric technique. The time functions of the target angles underwent harmonic analysis. Based on the Fourier coefficients, comparison was made between normal subjects' and amputees' angular displacements. Relevant findings permitted the identification of compensatory mechanisms adopted by amputees at trunk level as well as the assessment of the relationship between these latter mechanisms and those put into action at lower limb level.

[Folic acid metabolism in rapidly growing tissues: synthesis of pteroylpolyglutamates].

Pteroylpolyglutamates are regarded as the functional coenzyme form of folate: hence their synthesis may represent important regulatory step in the folate-dependent processes. To verify this hypothesis the pteroylpolyglutamate content was studied in rapidly growing tissues, which require a large availability of folate coenzymes, essential as carbon atom donors in the synthesis of purine and pyrimidine nucleotides and in the metabolism of some aminoacids. In the developing chick embryo, the higher polyglutamates are the only folate forms present and their content increases progressively during the development of the embryo. In rat liver regenerating after partial hepatectomy the higher polyglutamate forms decrease below the normal values from 12 to 72 h after the operation; the normal values are restored after 180 h. The initial drop is not related to a lower synthesis, but to a greater utilization of these coenzymes during the early periods of regeneration, when biosynthetic processes are markedly increased. In fact the content of polyglutamates in regenerating liver can be maintained at normal levels by increasing the availability of precursors via folic acid administration to hepatectomized rats.

Introduction to chemotherapy and antineoplastic pharmacology in breast cancer.

Tumor cell biology and growth-kinetics of human breast cancer are reviewed with reference to the different action mechanisms of main antitumor drugs. Particular emphasis is given to pharmacokinetics, dosage, mode of administration, effectiveness and side-effects of drugs most commonly used in breast cancer. The importance of tumor Estrogen and Progesterone Receptor assay for the therapy planning is considered and a short review of estrogens, androgens, progestins, antiestrogens, corticoids and aminogluthetimide is presented. The rationale for combined chemotherapy and/or hormonotherapy in breast cancer is finally discussed.

Clinical re-staging in cervicocarcinoma after clinico-instrumental staging.

A review of 193 cases of invasive carcinoma of the cervix was undertaken to determine the efficacy of the procedures used in the staging of this disease. All patients underwent pretreatment chest, pelvis, and spine radiographies, intravenous pyelogram, cystoscopy and proctoscopy. Bone scanning was performed too. These patients were retrospectively staged according to findings of the physical examination only. Each additional examination was then evaluated comparing its results with the initial staging. The column Xray did never change the stage; the other radiologic examinations changed the staging as follows: chest Xray 0.5%, pelvis 1%, intravenous pyelogram 2%. Cystoscopy and proctoscopy changed the clinical stage in 5.1% and 2.5% of the cases, respectively. Bone scanning in 1.5%. A review of 54 selected cases was undertaken to determine the presence of positive nodes in patients with carcinomas at stages I and II. In 16 cases of cervicocarcinoma the hystologic examination showed positive pelvic lymph-nodes. The AA, recommend all the reviewed procedures in the staging of carcinoma of the cervix in initial stages too.
