[Risk factors and clinical variables in patients hospitalized for epistaxis. Review of 200 patients].

It is reported a review of 200 adult patients admitted for epistaxis in our Hospital during a 10 years period (1992-2002). This study evaluates several factors included in the clinical histories: HTA, infection of the upper airway, EPOC, cardiovascular pathology, use of anticoagulant medication or other drugs, tobacco, alcohol, diabetes, etc. Besides we collected variables like age, sex, bleeding location, type of nasal packing made and evolution. Less than 20% of the cases were classified as idiopatic and the hospitalized patients had one or more known risk factors.

[External severe laryngeal trauma with multiple fractures and airway compromise].

External laryngeal traumas are rare but clinically can cause important life-threatening injuries. We report the case of a 33 yeras old male who suffered a severe traffic accident: TCE, facial trauma with multiple fractures and laryngeal trauma with fracture of the thyroid cartilage and the hyoid bone. Because an important edema in the airway, it was not possible an orotracheal intubation and he required urgent coniotomy with later reconstruction surgery and tracheostomy. Actually our patient has been decanulated and the fibroscopic image of the larynx and its functionality is correct.

[Sinonasal cystic adenoid carcinoma with epiphora and orbital involvement. Report of a case and review of the literature].

We report the clinical case of a 41 years old male with nasal obstruction of 1 year, epistaxis and epiphora. The ENT exam showed a bleeding red mass in left nasal fossa and CT joint to IRM revealed a tumoral process on that level and informed about its extension to adyacents structures (cavum, ethmoides, sphenoids and maxillary sinus). The biopsy was positive for cystic adenoid carcinoma. Our patient was operated by paralateronasal rhinotomy with removal of the tumor. One year later we found recurrence on the left orbital floor and maxilar sinus. The Oncology Department informed that it was not possible a treatment with radiotherapy or chemotherapy because the low sensitivity of that lesion those treatment.

[Thyroglossal duct cysts and fistulae. Review of 30 consecutive operated patients].

Thyroglossal duct cysts represent a very common pathology among the benign neck masses. An own review of 30 consecutive operated patients between 1994-2003 by our ENT Department is reported. There have been analysed different parameters like age, sex, personal antecedents, clinical symptoms, complementary explorations, surgical treatment, evolution and anatomopathologic result (AP). We think the Sistrunk procedure is the best operatory technique or strategy to perform to avoid recurrences.

[Association between primary hyperparathyroidism and Paget's disease. Report of a case].

Although the association between hyperparathyroidism and Paget's disease is known, in the clinical practice occurs very rarely. We report the case of a 68 years old female who was diagnosed as such disease more than 5 years ago and later she got an increment of PTH levels and hypercalcemiajoint to other biochemical disorders that made to suspect a primary hyperparathyroidism. Besides the osteoarticular manifestations, clinically the patient suffered recurrent kidney lithiasis. She was operated by removal of one adenoma (1,5 cm) located in lower right parathyroid gland and her evolution was satisfactory without complications.

[Multiple brown tumours in a patient with tertiary hyperparathyroidism].

We describe the case of a 49-year-old female with chronic renal failure and secondary hyperparathyroidism that not dissapeared after kidney transplant and turned into an autonomous form (tertiary hyperparathyroidism). The xRays and CT showed the presence of multiple brown tumors in iliac bones and tibia. We performed a parathyroidectomy removing a lower left adenoma confirmed in the histological exam the renal. After the surgery the osteoarticular manifestations improved and lithiasis dissapeared. There were not any postoperative complications such as hypocalcemia or recurrential palsy although Nephrology Department had to control her renal function.

[Olfactory neuroblastoma in an old patient. Report of a case].

Olfactory neuroblastoma or esthesioneuroblastoma is a rare embrionary tumor, much more in old patients, which uses to present as a pollipoid mass with nasal obstruction, epistaxis and anosmia of long evolution. We report the case of a 82-year-old male with such symptoms diagnosed by biopsy as neuroblastoma class III according the histologic Hyams grading. Due to the age of the patient and grade of the tumor we did not perform surgery and, although a chemotherapy-radiotherapy treatment was suggested, the patient was not agree with it and he has not been attended or followed in our consulting rooms.

[Recidived medullary thyroid carcinoma with neck metastases and tracheal invasion].

Medullary carcinoma represents about 5% of all malignant tumors located in thyroid gland and it can appear as sporadic or hereditary form. We report the case of a 58-year-old female who was initially operated by General Surgery Department of an infiltrating medullary carcinoma with cervical metastasic nodes having its origin on left thyroid lobe (LTL). The patient did not want any kind of control and 5 years later she suffered dyspnea due to the tumoral recurrence with invasion of the trachea and recurrential left palsy. Our ENT Service only performed a tracheostomy on fourth tracheal ring because the woman did not accept another surgical intervention. For 6 years she has been followed and reviewed in our consults beeing on alive up to now.

[Hürthle cells carcinoma. Report of two operated cases and review of the literature].

Hürthle cells carcinomas are considered a variant of folicular carcinomas and represent about 2% of all malignant thyroid tumours. We report 2 cases of 2 women of 46 and 43 years old confirmed by histological exam after total thyroidectomy with good postoperative evolution. The presence of capsular and vascular invasion is the most important criteria to distinguish this entity from the Hürthle cells adenomas.

[Usefulness of computed tomography and magnetic resonance in the preoperative diagnosis for hyperparathyroidism].

Sonnography and Tc-99m sestamibi scintigraphy are the most requested preoperative imaging tests nowdays in the surgery of hyperparathyroidism. The aim of our article is to know if Computerized Tomography (CT) and Magnetic Resonance Imaging (MRI) are useful as a location study and in which cases it would be more justified to ask these radiologic techniques. For that we report our results with 29 patients at all diagnosed as hyperparathyroidism (26 primary forms and 3 secondary ones) and operated by our E.N.T. Department later. On 20 of them a cervical CT was asked before the parathyroidectomy and on the rest 9, a MRI with sensitivities of 65% and 88.9% respectively. We think both complementary explorations must not be solicited by routine but they can represent a help in the cases in that sonnography and scintigraphy are not able to show the possible adenoma or hiperplasia, particularly in recurrent hyperparathyroidisms, reinterventions or suspect of parathyroid glands in an atypical location.

[Internal jugular vein spontaneous thrombosis. Clinical case and review of the literature].

Internal jugular vein thrombosis (IJT) represents a potentially serious pathology that can be spontaneous or secondary to head and neck infections, surgery, venous catheterization and intravenous drug abuse among others. We report the case of a male affected with IJT without clear cause after performing a complete exam which included ultrasonography, Doppler and contrast-enhanced CT scanning to confirm the diagnosis. Actually its treatment is mainly mediacl by antibiotic therapy and anticoagulants. Besides, it must be excluded an associated neoplasm.

[Melanoma of sphenoid sinus. Case presentation and review of literature].

The primary malignant melanomas of sphenoidal sinus are very uncommon tumours. We are reporting a clinical case of a 56 years-old male who was sent from the Neurology Department with ocular pain and headache of several months evolution and without personal history. Exploration revealed a VIth palsy, diplopia and ptosis. The complementary imaging tests (CT and MR) showed and expansive tumour of the sphenoidal sinus involving the right cavenous sinus. A F.E.S.S. with biopsy was performed to confirm the diagnosis. Our patient was treated by Radiotherapy and after a 4 year follow-up period he is still alive. We have carried to a bibliographic review of this pathology whose differential diagnosis must be done with epidermoid carcinomas, adenocarcinomas and lymphomas on that location.

[Nasal angiofibroma in a 60 year-old female. A clinical case and literature review].

We are reporting an atypical and very rare clinical case of nasal angiofibroma in a 60 year-old female who presented with recurrent epistaxis and nasal blockage. The tumour was located on the right inferior turbinate C.T scans., M.R.I. and Angiography were required as complementary studies. After trying without success a previous arterial embolization, the patient underwent surgery by Functional Endoscopic Nasal Surgery (F.E.N.S.) with removal of the mass in the right nasal fossa. The anatomopathologic result confirmed the histology of angiofibroma and the clinical evolution was satisfactory. Angiofibromas in women and in general in adults are exceptional as we have verified through a literature review.

[Second hemithyroidectomy for thyroid carcinoma. Our experience].

Carcinomas as casual findings can be found in thyroid surgery. After the definitive AP result of malignant pathology, if a partial technique was performed, we can face problem of completing or not to a total thyroidectomy. We are reporting our 12 years experience about reinterventions because a diagnosis of thyroid carcinoma (hemithyroidectomies on previous one) was made after an anatomopathologic (AP) study on a sample from the first surgery for a supossed benign pathology. 18 patients have been studied, all of them were women, 45 year-old average and we have analysed the initial symptoms, results of basical complementary tests, diagnosis AP for first surgery and final result for second one. The incidence of malignancy showed in our series after the second intervention was 40% so we feel that a total thyroidectomy must be performed after a casual finding of thyroid carcinoma, because this allows oncological safety and a better control of the patient.

[Laryngo-tracheal estenosis in adults. A restrospective study of 21 cases and literature review].

We are reporting a retrospective study of 21 adults diagnosed of laryngo-tracheal stenosis and admitted to our Department between 1990 and 2001. In 9 cases (42.85%) it was due to prolonged orotracheal intubation; other 10 were secondary to tracheostomy (47.62%) and the rest, to coniotomy. We have studied and analysed these variables: Age, gender, personal history, level of stenosis, its grade according to Cotton's clasification, treatment and clinical evolution with an average follow-up of 12 months. Our main goal was to get people to know on experience with this pathology and to carry out a literature review in order to compare our data and results with other published series.

[Amyloidosis of the larynx. A clinical case and literature review].

The larynx is a rare site of deposition for amyloidosis. We are reporting the clinical case of a 73 years female with hoarseness of 2 years evolution due to a laryngeal mass which was informed as located primary amyloidosis after anatomopathologic exam (AP) and its extension study. A laryngeal microsurgery was performed and then we proceeded to send the patient to Internal Medicine Service with periodic controls by ORL. Surgery is the treatment of choice by microsurgery, laryngofissure or Laser with local excision. The prognosis of localized forms is better than systemic ones although recurrences are common and it is recommended a long-term follow-up. A bibliographic review about this pathology has been made.

[Otogenic intracranial abscesses. Description of 6 cases].

We are reporting 6 clinical cases corresponding to admitted patients with intracranial abscesses (4 in brain and 2 in cerebellum) due to chronic otogenic pathology. Five were men and one woman with ages between 25 and 74 years. We have analysed in each case the initial symptoms, otoscopic exam, the otic and cranial CT informed, diagnosis, treatment and evolution. In all of them, it was performed a drainage of the abscess by Neurosurgery and Radical mastoidectomy by our Service with positive result. Although it is an uncommon complication nowdays, the abscesses of otological cause must be always suspected in uncontrolled chronic otitis or poor response to medical treatment. Symptoms can be no characteristics and must be asked for a CT or IRM in case of doubt.

[Post-tonsillectomy hemorrhage in adults. A retrospective study and literature review].

We are reporting a review of 21 adults admitted with diagnosis of post-tonsillectomy hemorrhage between 1992 and 2001 (10 years). 14 were male and 7 female with 29 years-old average. We have studied the number of days from intervention to hemorrhage beginning, bleeding location, urgent analysis, the medical or surgical treatment that was performed and clinical evolution. A 85.7% were late bleeding (5th day or afterwards) being two times more frequent on the left fossae. It was required a hemostasia under general anestesia in 57.14% of the cases and the incidence of bleeding post-tonsillectomy has been estimated in 5.8% respect to global surgery of that kind.
