[Latero-pharyngeal phlegmons and abscesses in adults. Reports of 6 cases and literature review].

Lateropharyngeal phlegmons and abscesses are an uncommon complication actually but they are requiring an adequate knowledge to get later an adequate treatment. We are reporting a clinical review of 6 cases corresponding to adults admitted by our ENT service in 11 years period. Four were men and two women with ages between 21 and 92 years old. In most of them it was found a pharyngotonsillar focus and after the diagnosis they were treated by endovenous antibiotic therapy, corticoid and, in one case, surgery by incision and drainage. All the patients had a satisfactory evolution standing between 6-12 days in the hospital. We have performed a bibliographic review on this subject.

[Cervical metastases of melanoma. A case report and review of literature].

We are reporting the clinical case of a 56 years-old female who was diagnosed and operated of a stage V Clark melanoma in temporal zone that showed, 21 months after the surgery, several left cervical lymph nodes. The F.N.A.B. informed as melanoma metastases, result that was confirmed by AP study then a functional neck dissection. Later she has followed treatment by Oncology-Radiotherapy. Melanoma can isolate between a 5%-10% from all the cases of cervical lymph node metastases but in most of the patients it is not possible to find the primary lesion. Its vital prognosis is bad and the role of adjuvant irradiation remains controversial.

Bone Neoplasms↗

[Cerebellar and pulmonary metastases from papillary thyroid carcinoma. Report of a case].

We are reporting the clinical case of a male who was diagnosed as papillary thyroid carcinoma with cervical, cerebellar and lung metastases. Although in theory it was a well differentiated tumour and slow progression with a good prognosis, there was a very aggressive spread in our patient up to his death. We decided not to operate him and send him to the Oncology Service for chemotherapy that was unsuccessful. The intracranial metastases due to papillary thyroid carcinoma are very uncommon (about 1%) on the contrary of cervical ones, but their behaviour is more aggressive and frequently associated to other metastases on bone or lung.

[Hereditary angioneurotic edema (Quincke's edema). Report of a case and literature review].

Familiar angioneurotic edema or Quincke's edema is an uncommon variant of urticaria, associated or not, that involves the subcutanean cell tissue and mucous of the face and upper airdigestive tract. It can produce an acute dyspnea and risk of suffocation with intubation or tracheostomy up to a 20% of the cases. The disease is a result of deficit in C esterasa inhibitor which autosomal dominant inheritance. We are reporting one clinical case confirmed as such corresponding to a 81 years-old male whom was operated by tracheotomy after sending from UCI with an emergency coniotomy and failure for orotracheal intubation.

[The incidence of thyroid carcinoma in Hashimoto's thyroiditis. Our experience and literature review].

The incidence of thyroid carcinoma in Hashimoto's thyroiditis has been widely debated and it is notorious in many published series. The possibility of an immunological and autoimmune mechanism in the etiopathogenia of papillary carcinomas stimulating lymphocytic infiltration has been suggested. We are reporting an our review of 4 cases of Hashimoto's concomitant with papillary thyroid carcinoma recorded between 1991 and 2001. The clinical data and complementary tests performed before the surgery are reported. Three patients were operated by total thyroidectomy and the other by hemithyroidectomy on a previous one whose diagnosis was malignant. We estimate the coexistence of papillary Ca. and Hashimoto's disease in 36.4% (4 of 11) in our case history.

[Cervical abscess after hemithyroidectomy. An unusual complication].

Cervical abscesses secondary to hemithyroidectomy are an uncommon complication. We report the clinical case of a 37 years-old male, diagnosed and operated for nodular goiter, who suffered a paratracheal purulent collection three weeks later. He was reoperated by drainage of the neck abscess and antibiotic hospitalary treatment during 8 days. In a control CT showed an improvement despite the presence of a seroma on the surgical wound. Cervical exploration was normal two weeks later. Thyroid surgery is catalogued as clean and does requires antibiotic therapy. This must only be kept for cases of surgical wound infection or local abscess and both of them are rare in the literature.

[Tertiary hyperparathyroidism. Report of 5 cases].

Tertiary hyperparathyroidism represents an autonomous proliferation state of the parathyroid glands that takes place in spite of performing a renal transplantation in patients with chronic renal failure due to the illness. We are reporting 5 own clinical cases collected in a 14 years period corresponding to 2 men and 3 women, with ages between 43 and 53, that were sent from Nephrology for surgical intervention. The AP result was hyperplasia in 4 samples and 1 adenoma in the rest. 2 transitory hypocalcemias were registered but not recurrential palsies. After the surgery we got the normalization of hypercalcemia in all of them and improvement of skeletal symptoms.

[Maxillary fungal sinusitis by Zygomicetos. Report of a case in an immunocompetent patient].

We report the clinical case of a 36 years old female without significant previous history who was diagnosed with an acute maxillary sinusitis with not responding to antibiotic treatment. After her admission we asked for a CT which showed an occupation in the left maxillary sinus with a calcified area inside suggesting a mycetoma. The patient was operated by Functional Endoscopy Sinusal Surgery (FESS) plus Caldwell-Luc techniques sending the obtained sample for microbiological study, that was reported as sinusal mycosis by Zygomicetos. The postsurgical evolution was satisfactory.

[Sino-nasal undifferentiated carcinoma. Report of 1 case and literature review].

Sinonasal undifferentiated carcinomas are rare neoplasms but highly malignant and aggressive. We are reporting the case of a 75 year-old female who had related recurrent epistaxis, facial pain and progressive nasal obstruction. The clinical data collected by the exploration joint to CT showed us the presence of a tumoral mass located in the right nasal fossa and homolateral maxillary sinus, eroding structures around. Its biopsy was informed as undifferentiated carcinoma and the patient was sent to Oncology Department, where she was treated by Radiotherapy (68 Gy) and weekly carboplatin during 2 months. After a short improvement of her symptoms and due to poor prognosis in this type of tumors, she died a few months later.

[Association among hypocalcemia, AP result and surgical technique in hyperparathyroidism. A statistical study].

The main goal of this article was to know if there was any relations statistically significant among the post-operative hypocalcemia, type of surgery for hyperparathyroidism in our series and its anatomopathological result (AP). 122 patients affected of such illness (30 men and 92 women) were studied and operated and we obtained 30 hypocalcemias (27 transitory and 3 permanent ones). The subtotal and total parathyroidectomies compared with adenomas removal have showed us a percentage for that complication significantly higher (p<0.001). Besides we have found a clear association between parathyroid hyperplasia and hypocalcemia (p<0.05).

[Maxillary sinusitis by Fusarium sp. Report of a case and literature review].

We report the clinical case of a 54 year old male who was diagnosed as left maxillary sinusitis with poor evolution in spite of several treatments. All the tests performed on this patient are shown and they allowed the final anatomopathologic diagnosis of infection by Fusarium sp., an opportunist fungal pathogen very rare as cause of an ENT infection. It was necessary to practice a Caldwell-Luc technique or extern antrotomy and an antifungal therapy (itraconazol) to obtain a significant improvement. Finally a bibliographic review has been made at respect of this pathology.

[Posterior laryngeal stenosis in patient with Meige's Syndrome].

Meige's Syndrome is a combination of blepharospasm and oromandibular dystonia (cranial dystonia) that can affect the pharyngeal and respiratory musculature. We are reporting a case of a 61 year-woman with this diagnosis who presented a laryngeal stenosis due to sinequia on posterior commissure. The symptoms were an association of hoarseness and dyspnea with a clear movement in all pharyngo-laryngeal structures because of her illness. She was operated by Laser surgery with section of such sinequia and going on treatment in Neurology.

[Severe epistaxis secondary to ticlopidine. Report of 3 cases].

Ticlopidine (Ticlid@) is a platelet aggregation inhibitor that can produce bleeding or severe hemorrhage as a side-effect. The cases of three patients on previous treatment with that drug that required admission in our hospital because of recurrent epistaxis are reported. We have analysed their personal antecedents, complementary explorations, type of nasal packing used, clinical evolution and stay.

[Idiopathic supraclavicle abscess with extension to the cervico-thoracic intersection].

The incidence of deep neck abscesses has clearly decreased thanks to antibiotic therapy. In most of the cases we can find an odontogenic or pharyngotonsillar cause and the infection can extend from its origin to deep cervical spaces or mediastinum. We are reporting the clinical case of a 71 year-male diagnosed as right supraclavicle and idiopatic abscess that reaches the cervico-thoracic intersection. The patient was urgently operated by drainage of the purulent collection being carried later to ICU where he suffered an adult respiratory distress syndrome (ARDS) which required a change on the treatment up to the resolution.

[Severe dysphagia secondary to Plummer-Vinson Syndrome. Report of a case and review of the literature].

The Plummer-Vinson Syndrome is characterized by the presence of dysphagia, iron deficiency anemia and esophageal webs. We report the clinical case of a 67 year-female who was admitted with aphagia, glositis and important sialorrea. After performing a digestive endoscopy we found a double membrane located on Killiam region (upper esophageal tract) and proceeded, under general anestesia, to neumatic dilatation. The patient followed periodic controls yearly by Digestive Department and Internal Medicine with a significant clinical improvement.

[Acute confusional syndrome as first manifestation of primary hyperparathyroidism].

Hyperparathyroid crisis is an uncommon clinical state characterized by severe hypercalcemia which can produce an acute confusional syndrome (ACS) or delirium. We are reporting the case of a 58-year male with calcemias greater than 14 mg/dl, that were due to a parathyroid adenoma (primary hyperparathyroidism), and caused his admission in UCI with change in the conscience level, global afectation of the cognitive functions, letargia and disorentiation among other clinical symptoms. The patient was operated by our Service on lower left parathyroidectomy with PTH rapid or turbo assay and decreasing such hormone levels about 75%. After the surgery his evolution was satisfactory obtaining the normocalcemia.

[Chronic otitis media in adults complicated with meningitis. Report of two cases].

Although rare, meningitis secondary to chronic media otitis represent an intracranial complication of importance nowdays. We are reporting two clinical cases diagnosed by symptoms, CT and lumbar punction as meningeal syndrome with otogenic cause that required endovenous wide spectrum antibiotic treatment and surgery later. We consider essential an early diagnosis and close cooperation with other departments to try to decrease the associated mortality, greater than 30% in some series.

[Eagle's syndrome. Report of a case and literature review].

Eagle's syndrome produces recurrent throat pain, foreign body sensation, dysphagia/odinophagia or facial pain due to an elongated styloid process or calcified stylohyoid ligament. We report the clinical case of a 56 years old female who had consulted several times to our by Emergency Service relating these symptoms and she was finally diagnosed as this uncommon pathology. Up to now the patient has been controled with analgesics but not surgical treatment. We have performed a short bibliographic review about this syndrome.
