Latitude-correlated genetic polymorphisms: selection or gene flow?

Latitude-correlated polymorphisms can be due to either selection-driven evolution or gene flow. To discriminate between them, we propose an approach that studies subpopulations springing from a single population that have lived for generations at different latitudes and have had a low genetic admixture. These requirements are fulfilled to a large extent by Ashkenazi and Sephardi Jews. The original population lived at a latitude of 35 degrees N, where the Sephardis still live. The Ashkenazis, however, moved to a latitude of 50 degrees N, starting about 10 centuries ago. The present study examines 3 latitude-correlated polymorphisms: PGP, PGM1, and AHSG. We found that PGP*2 and AHSG*2 alleles most likely underwent selection-driven evolution, but that PGM1*ts allele was not similarly affected. Since temperature might have been considered a reasonable selective factor, we also studied a population living at >800 m above sea level from Aosta Valley (Italy).

[Massive intestinal hemorrhage caused by leiomyoma of the jejunum. Report of a case].

Small-bowel leiomyoma is a rare tumour. The authors report a case complicated by severe bleeding. Esophagus-gastro-duodenal endoscopy was not decisive enough, while TC was the most reliable instrumental investigation. Because of dimensions basic than 10 cm, the presence of a pseudocapsule end the absence of cariokinetic figures, the histopathologic test supports the hypothesis that it was a benign disease. One year follow-up, negative for relapse, confirm that the lesion is benign.

[A case of leiomyosarcoma of the small intestine. Clinical and anatomopathologic considerations].

Leiomyosarcoma of the small intestine is a malignant mesenchymal tumour composed of smooth muscle cells from the muscular coat or, rarely, from muscularis mucosae, which in the majority of cases is characterised by a clinically silent development and by an unfavorable prognosis. The considerable difficulty of obtaining a histological definition of the degree of malignancy of the leiomyosarcoma is also typical. The paper reports a clinical case and summarises its main clinical and anatomopathological aspects. In particular, the difficulties of clinical and histological diagnosis of the degree of malignancy are underlined, together with the fact that these have a considerable influence on the possibilities and results of surgical therapy and survival rates.

[A case of leiomyoblastoma of the stomach. Clinical and anatomopathologic considerations].

Gastric leiomyoblastoma or Martin's myoid tumour or Stout's smooth muscle cell bizarre tumour is a rare benign mesenchymal neoplasia that may become malignant. A personal clinical case is reported and the most important anatomopathological and clinical aspects are reviewed, stress being laid on the importance of identifying the macroscopic and histological features that can point with certainty to malignity and so influence the choice of surgery. This, depending on the situation, may tumorectomy, gastric resection or gastrectomy with exeresis of any metastases.

[Isolated thrombosis of the left brachiocephalic venous trunk].

Only Flower (1976) has reported an isolated obstruction of the left brachiocephalic venous trunk following the observation of a unilateral distension of the left superior jugular vein secondary to compression of the homolateral innominate vein during a type I dissecting aneurysm of the aorta. The rarity of the finding is the reason for the report of two cases of isolated thrombosis of the venous trunk in question observed almost simultaneously some 19 months earlier. In both cases it was possible to identify the aetiopathogenic causes as congenital or acquired constriction of the "aortosternal strait" associated with thrombophilic alteration to the blood coagulation system.

[A case of splenic abscess in a drug addict with acquired immunodeficiency syndrome].

The complex diagnostic procedure involved in a case of splenic abscess in a young drug addict as a result of septicaemia caused by streptococcus fascalis is described and discussed with an examination of the criteria in support of a more aggressive approach to the removal of septic foci in such patients with AIDS. It is concluded that such criteria are valid even when, as in the case described, splenectomy further depresses the immune system.

[Use of vancomycin in the treatment of severe Staphylococcus aureus infection. Description of 3 cases].

In recent years, the diffusion of antibiotic multiresistant staphylococcus strains in hospitals (especially aureus and epidermis) has created serious drawbacks as regards the treatment of severe septic forms and the systemic spread of these bacteria. The results of endovenous vancomycin treatment in three cases of sepsis with secondary localisations (endocarditis, osteomyelitis, pneumonia) caused by gram positive staphylococcus aureus are examined. The pharmaceutical was found to be effective in all cases and free from significant side effects.
