[Blood coagulation activity and fibrinolysis in umbilical vein blood of healthy and asphyxiated newborn infants].

In two groups of 35 healthy and 15 asphyctic newborns the factors of blood coagulation and fibrinolysis were determined and compared with the normal values of non-pregnant women. The study demonstrates an increased coagulability and increased fibrinolytic activity at decreased levels of most of the single factors in the umbilical vein blood of the newborn. There is a statistically significant decrease of the concentration of plasminogen and increase of the concentration of fibrin degradation products and fibrin monomers in the groups of asphyctic newborns as compared with healthy newborns. These results may be considered as factors in the etiology of the respiratory distress syndrome of the newborn resulting in the formation of hyaline membranes. An increased tendency to hemorrhages in asphyctic newborn due to a hypocoagulation of the umbilical vein blood cannot be suggested by these results. The study confirms and supplements previous research findings from our laboratory and from others reported in the literature.

[Clinical aspects and diagnosis of intrauterine growth retardation].

Low birthweight infants add to a great extent to perinatal mortality. Among this group children with intrauterine growth retardation differ from premature children with respect to their pre-, intra- and postnatal risk. Early prenatal diagnosis and monitoring is of great importance for the outcome and later development of these children.

[Prevention of idiopathic respiratory distress syndrome. Influence of betamethasone on the frequency of respiratory distress syndrome in a premature infant collective at the Bonn University Gynecologic Clinic].

Out of a group of 189 pregnant women, who were in the 28th to 37th week of gestation, 57 women received i.m. injections of betamethasone. 132 premature infants whose mothers were not treated served as controls. Betamethasone caused a significant decrease in established respiratory distress syndrome (RDS). In the betamethasone group there was an incidence of 7% of respiratory distress syndrome as compared to 23,5% in the control group. Neonatal mortality was 1,8% in the treated group versus 10,6% in the control group. The occurrence of suspected respiratory distress syndrome also was significantly lowered after betamethasone. In the control group premature rupture of membranes or administration of tocolytic drugs did not have any effect on the frequency of respiratory distress syndrome. However, the premature babies who were delivered by caesarian section had a significantly higher incidence of respiratory distress syndrome than those who were delivered vaginally. After betamethasone treatment the occurrence of respiratory distress syndrome after caesarian section was as low as in the group with vaginal delivery. The present study of a larger group of premature babies has confirmed previous positive results of betamethasone treatment.

[Quantitative IgM-testing and prenatal rubella infection (author's transl)].

Cord blood of 686 newborns was tested for IgM-levels in connection with an epidemic occurrance of rubella infection in Bonn and its vicinity. In 17 samples IgM was higher than 30 mg/100 ml and in 102 samples higher than 20 mg/100 ml. In 121 cases the cord blood was examined for rubella antibodies and in 85 children it was also checked for the persistence of rubella antibodies at the end of the first six months of life. No unexpected prenatal rubella infection was found. Two children of this group had clinical and serological criteria for rubella embryopathy at birth. In addition 39 cases of verified rubella infections during pregnancy were investigated. In 13 cases abortion was performed during the first trimenon following a rubella infection; the presence of rubella virus embryotic tissue could be proven in 9 cases. A prenatal rubella infection became apparent by illness of the mother during the pregnancy and/or embryopathy of the newborn in 26 children. In 16 of these children prenatal rubella infection was confirmed by the detection of rubella specific IgM which was associated in all instances with elevated levels of IgM, and/or the persistence of rubella antibodies after the sixth month of life. The remaining 10 children had neither clinical nor serological signs. The mothers of 9 of these children had been ill after the 16th week of pregnancy. In at least 21 cases (81%) of 26 maternal rubella infections during the first trimenon of pregnancy the virus passed the placenta. In 13 cases the pregnancy was interrupted; of the remaining 13, 6 children (46%) had embryopathy.

[A mobile reanimation model (reanimationmobile) for initial newborn infant care].

A mobile unit for reanimation of the newborn is reported to be used alternatively in the deliveryroom or in the surgical tract. The following adventages are achieved: a large working plate easily accessible from three sides, a complete and variable instrumentation, an efficent warmth supply and the possibility for easy cleaning and desinfection.

Familial balanced (7;11;21) translocation and Down's syndrome in two siblings.

Cytogenetic studies of a family with two children with Down's syndrome have revealed a balanced reciprocal translocation between chromosomes No. 7, 11 and 21 in the mother. One of three daughters has inherited this translocation. two mongoloid daughters have a supernumerary chromosome No. 21 in addition to the translocation.

[The role of rubella specific IgM within the serological diagnosis of rubella (German measles) (authors transl)].

The sera of 327 patients were investigated for rubella specific IgA after separation of the IgM fraction by sucrose density gradient centrifugation. 1. In 96 out of 137 cases suspicious of clinical rubella, where the time for demonstration of a significant rise of the antibody titer had been missed, the demonstration of rubella specific IgM confirmed the diagnosis of German measles. This demonstration was still possible in 9 out of 10 cases 5 to 7 weeks after the onset of the disease, but in only 2 out 7 cases 8 to 15 weeks after the onset of the disease. 2. In 48 patients who had contact with patients with German measles but did not develop any symptoms, a primary infection could not be excluded by the examination of the whole serum. In 5 out of them rubella specific IgM indicted a recent rubella infection. 3. Rubella specific IgM could not be demonstrated in 47 patients who had high antibody titers but no other hints for a recent rubella infection. 4. In 89 newborns, the investigation was prompted, when elevated IgM levels were found in umbilical cord blood, or when the newborn was sick, or when a rubella infection of the mother was suspected or proven. Rubella infection of the newborn was confirmed by rubella specific IgM only if the mother had contracted German measles during the first trimester of the pregnancy (6 out of 7 cases). The diagnosis of primary rubella infection during pregnancy is proven only if 2 out the fose in antibody titer, demonstration of rubella specific IgM.
