[Phase II study of THP patients with gastrointestinal cancer].

A phase II study on THP((2''R)-4'-0-Tetrahydropyranyladriamycin) was performed in 47 patients with advanced or recurrent gastrointestinal cancer through the cooperation of nine institutions in Hiroshima Prefecture from April 1982 to November 1984. THP was given by means of intravenous infusion and/or intraaortic infusion and the 47 cases were divided into two groups according to the method of administration: (A) 40-60 mg/body every 3 or 4 weeks, or (B) 30 mg/body every week. Among 24 evaluable cases, partial response (PR) was observed in two cases of recurrent metastatic lymph nodes in gastric cancer patients. The (A) method of administration was more effective than (B). Subjective side effects observed were appetite loss, nausea, vomiting and general fatigue, but these were not so severe. Leukocyte nadir occurred at the 1st or 2nd week of THP administration, but thrombocytes were not appreciably decreased.

In vitro chemosensitivity tests on human tumor xenografts by clonogenic assay: the combined use of mitomycin C with alpha-interferon or gamma-interferon.

Using the human tumor clonogenic assay technique, the effects of Mitomycin C plus either alpha-interferon or gamma-interferon were studied against five human tumor xenografts serially transplanted into nude mice (three gastric and two colon cancers). When the survival fraction found with the drug combination was smaller than the multiplication of survival fractions with either drug alone, the combined effect was defined as synergism. Similarly, antagonistic effect was defined when the survival fraction of drug combination was larger than the larger one observed in either interferon or Mitomycin C alone. Four out of five human tumor xenografts (three gastric and one colon cancers) showed synergistic effects in combination of alpha-interferon with Mitomycin C. Though two gastric cancer xenografts exhibited synergistic effects in combination of gamma-interferon with Mitomycin C, antagonistic effects, which was not found in combination of alpha-interferon with Mitomycin C, were observed in one gastric cancer and one colon cancer xenografts.

[Combination chemotherapy with cis-diamminedichloroplatinum (CDDP) and other anticancer agents for patients with advanced esophageal cancer].

Combination chemotherapy with cis-diamminedichloroplatinum (CDDP) and other anticancer agents was performed in patients with advanced esophageal cancer. From July 1982 to September 1984, 16 patients were entered into this study and divided into two regimens. In regimen I, 5 patients were treated with a daily dose of 20 mg CDDP and 10 mg Peplomycin for five consecutive days and 10 mg Mitomycin C on the first day. This course was repeated 2 or 3 times every 4 weeks. As for evaluation, 1 PR (20%) and 4 NC (80%) were observed. In regimen II, nine patients were treated with daily doses of 25 mg CDDP and 2 mg Vindesine for 5 consecutive days, a course which was repeated every 4 weeks. As for evaluation, 1 CR (9%), 1 PR (9%), 4 MR (36%), 2NC (18%), and 3 PD (27%) were observed. Major side effects were renal failure, bone marrow suppression, nausea and vomiting, which were mostly transient. However, more severe bone marrow suppression was observed in regimen I composed with regimen II.

[Assessment of the combined effects of mitomycin C with alpha-interferon or gamma-interferon by the clonogenic assay technique].

The combined effects of Mitomycin C (MMC) with alpha-interferon (HLBI) or gamma-interferon (TRP-2), which have become attractive drugs for use as Biological Response Modifiers, were investigated using the human tumor clonogenic assay technique. Tumors in this study were five human tumor xenografts serially transplanted into nude mice (three gastric cancer, two colon cancer). When the survival fraction occurring with the combination was smaller than that obtained by multiplication of the survival fractions occurring with either drug alone, the combined effect was considered to be synergism. Four out of five xenografts (three gastric cancer, one colon cancer) showed synergistic effects for the combination of MMC with alpha-interferon. Although two gastric cancer xenografts showed synergism for the combination of MMC with gamma-interferon, antagonistic effects were observed in one gastric cancer and one colon cancer xenograft.

[Postoperative adjuvant immunochemotherapy with bestatin for stomach cancer. Randomized controlled trial: first report].

The effectiveness of Bestatin combined with Mitomycin-C, FT-207 for treating gastric cancer was investigated in a prospective randomized controlled study. All patients had undergone gastrectomy during the period from November 1980 to July 1983. Three-year survival rates revealed no difference between groups given or not given Bestatin. The effectiveness of Bestatin was confirmed by postoperative changes revealed by liver function test, especially GOT and GPT. No side effect due to Bestatin was detected.

[Comparison of human tumor clonogenic and nude mouse-isotope assays in anticancer-agent sensitivity tests].

Comparative studies between the in vitro human tumor clonogenic assay (HTCA) and our original in vivo method (NM-IA) in which the final evaluation was made with 3H-TdR incorporation of tumor cells transplanted into nude mice were simultaneously performed on 44 fresh human solid tumors (22 gastric cancers, 6 breast cancers, 4 gall bladder cancers, 4 liposarcomas and 8 other tumors). Mitomycin C (MMC), 5-fluorouracil (5-FU), adriamycin (ADM) and cyclophosphamide (CPM) were tested. The evaluable rate was 73% (32/44) in HTCA and 89% (38/44) in NM-IA. Although correlation between the results of HTCA and those of MN-IA was obtained for MMC and ADM, no correlation was observed for 5-FU and CPM.

[Phase II study on MCNU in patients with advanced or recurrent gastrointestinal cancer].

A phase II study on MCNU (Methyl 6-[3-(2-chloroethyl)-3-nitrosoureido]-6-deoxy-alpha-D-glucopyranoside) was performed in 33 patients with advanced or recurrent gastrointestinal cancer under the cooperation of eight institutions in Hiroshima Prefecture. MCNU was given by means of intravenous drip infusion and the 33 cases were divided into three groups according to the method of administration; (A) 50mg/m2 every 1 or 2 weeks, (B) 70mg/m2 every 2 weeks or (C) 90mg/m2 every 6-8 weeks. Among 28 evaluable cases, 1 complete response (CR) and 1 partial response (PR) were observed and these two cases were gastric cancer patients. Platelet nadir occurred at the 3rd or fifth week after MCNU administration, but the leukocyte count was not so decreased. Subjective side effects were nausea, general fatigue and vomiting, but these were observed to be only mild.

[Clinical studies on plasma exchange in advanced cancer patients with special reference to the postcentrifugal filter].

Plasma exchange was undertaken to remove the immunosuppressive factors in 25 advanced cancer patients who did not respond to immunochemotherapy. In the present studies, a postcentrifugal filter was tried, and the filtrated autologous plasma was replaced in 10 patients replaced by about 1000 ml in eight and by about 1600 ml in two. The improvement of subjective symptoms and reduction of tumors were observed in 15 (60%) and seven (28%) patients, respectively. It was suggested that immunosuppressive factors of a large molecular size could be selectively removed by the postcentrifugal filter.

Levamisole in postoperative adjuvant immunochemotherapy for gastric cancer. A randomized controlled study of the MMC + Tegafur regimen with or without levamisole. Report I.

The usefulness of LMS in postoperative immunochemotherapy of gastric cancer was investigated. In compliance with the protocol, MMC was given at a dose of 20 mg on the day of gastrectomy, and an additional 10 mg on the next day IV. The patients receiving 600 mg Tegafur daily were then divided into two groups according to whether LMS was also given or not. LMS was administered for 3 days before the operation in a daily dose of 150 mg and for 1 year or more after operation according to a schedule of 3 days' administration followed by an 11-day interval. The 2-year follow-up demonstrated that in stage III patients, the LMS (+) regimen was superior to the LMS (-) regimen, since the former prolonged the relapse-free interval significantly. The survival rate for stage III disease was also significantly higher in the LMS (+) than in the LMS (-) group. There was no significant difference in the incidence of subjective or objective side-effects between two groups. The incidence of agranulocytosis was comparable in the two groups.

Early gastric cancer with widespread lymph node metastases; a case report.

The resected stomach from a 58-year old male patient showed a 4.3 X 3.2 cm, depressed (II c) lesion in the body, histology of which revealed moderately differentiated tubular adenocarcinoma with invasion reaching the submucosa. Widespread metastases of lymph nodes including superior mesenteric and para-aortic ones were noted. The patient once worked in a mustard gas (Yperite) factory, and had chronic bronchitis. The exposure to this toxic substance was discussed in relation to the diffuse metastases of cancer.

[Surgical treatment of Borrmann type 4 (diffuse) gastric cancer].

A total of 435 cases with stage II-IV gastric cancer resected in our department was investigated. Cases with Borrmann 4-type cancer showed the poorest prognosis in all stages compared with other types of cancer. None of three drugs-PS-K, FT-207 and Levamisole-was specifically effective on Borrmann 4-type cancer in two randomized controlled trials. A study of in-vitro lymphocyte responsiveness to mitogens suggested the presence of a suppressive factor in the serum of the patient with poorly differentiated cancer. An immunofluorescent study using monoclonal antibodies against large and small intestinal mucin antigens showed strong positivity in poorly differentiated cancers.

[Sensitivity test of anticancer agents using human tumor xenografts in nude mice with special reference on correlation between clinical effects and chemotherapy].

In order to improve the response rate of chemotherapy, we have investigated to the method for rapid and accurate prediction of the most appropriate anticancer agents. Thus, a rapid method using nude mice has been established as in vivo model for assessing chemosensitivity of individual human tumors, in which the final evaluation was made with 3H-thymidine incorporation and histological changes of tumor cells treated. Correlation between the sensitivity of anticancer agents and retrospective results of gastrectomy in stage IV gastric cancer was investigated. Our sensitivity test indicated a good correlation with clinical therapeutic and immunological effects. From these results, it seems reasonable to conclude that the sensitivity test using human tumor/nude mouse system may be useful for selection of appropriate agents to treat patients with cancer.

[Increased cytotoxic effects of various anticancer drugs by alpha-interferon (HLBI) on human tumor xenografts in nude mice].

Since interferon (IFN) has a mechanism of action very different from chemotherapeutic agents, it is possible that a combination of two may be of therapeutic value. The authors studied increased cytotoxic effects of anticancer drugs by IFN on human tumors xenografts in nude mice. Tumor used in this study were "SH-10", "S-7379" (gastric cancer) and "O-7294" (malignant melanoma), serially transplanted subcutaneously. IFN was injected, 5 X 10(5) mu/mouse, every day for 2 weeks and a single drug was administered 3 times every fourth day. Cytotoxic effect was determined by tumor size on day 16 after treatment. Of the 7 drugs, MMC and ADM were most effective. Other drugs showed a slight inhibition of tumor growth by combination therapy with drugs and IFN.

[Local immunotherapy for cancer].

As for the non-specific cancer immunotherapy concerned, the effect is limited and local use of immunopotentiators which could collect effector cells around tumor tissues might be the best way of cancer immunotherapy. Intratumoral or intraperitoneal administration of large-dose of OK-432 (100 KE) has been investigated since many years with favorable results. Side effects were minimum with a few days continuing slight fever elevation. From the immunohistological examinations using monoclonal antibodies, participation of killer T cells and NK cells was confirmed after intratumoral administration of large-dose OK-432. On the other hand, after intraperitoneal administration of OK-432, neutrophil leucocytes appeared at first on the 2nd to 4th day and they were followed by lymphocytes on the 4th to 5th day and finally appeared lot of macrophages on the 6th and later days. From the results of in vitro and in vivo experiments, these macrophages seemed to play the leading role in the cytostatic activities after intraperitoneal OK-432 administration. As for the fear of introducing suppressor cells after large-dose OK-432 administration, detailed studies on suppressor activities before and after operation for gastric cancer patients revealed no particular increase of suppressor cell activities after intratumoral and intraperitoneal administration of 100 KE OK-432 as compared with curative resection cases with no OK-432 administration.

[Four gastrectomized cases associated with chronic leukemia].

We reviewed 4 gastrectomized cases associated with chronic leukemia. Three patients had chronic myelogenous leukemia, one had hairy cell leukemia. Subtotal gastrectomy was performed in all cases; two operations were for gastric cancer and two for bleeding gastric ulcers. The spleen was removed in three patients without any trouble. There were no difficulties in managing the patients during and after surgery and their leukemia was not aggravated during these periods.

[A case of minute carcinoma associated with an atypical epithelial lesion of the stomach and histopathological analysis].

A patient with minute carcinoma associated with an atypical epithelial lesion (ATP) of the stomach is reported. The resected stomach from this 81-year-old male exhibited a 1.3 X 1.0 cm elevated ATP; microscopic examination revealed a minute (0.4 mm) carcinoma. The lesion was differentiated tubular adenocarcinoma and was suggestive of one-gland carcinoma. It consisted of one faveola in the upper half and multibranched , compound alveolar glands in the lower half, structurally mimicking the initial stage of intestinal metaplasia. The lesion existed in small metaplastic foci; the surface foveolar epithelia of the lesion showed no cancerous changes. These findings suggested that this minute carcinoma occurred in the initial stage of intestinal metaplasia.
