Alum irrigation in the treatment of severe haemorrhage from the bladder.

Ten patients in whom severe haemorrhage from the bladder persisted after clot evacuation and fulguration were treated by continuous irrigation with 1% alum solution. Bleeding ceased in 8 patients, and was temporary in only one of these cases. In 2 patients with thrombocytopaenia the treatment failed. No side effects were noticed. Serum aluminium level was measured in one patient and the value was normal.

Prognostic significance of symptoms and findings in renal adenocarcinoma.

The occurrence of different symptoms and signs and their relation to prognosis were studied in a series of 324 cases of renal adenocarcinoma. The most common symptoms were gross haematuria and loss of weight, but 17% of the patients were asymptomatic at presentation. The incidence of asymptomatic patients had doubled in ten years. Of the symptoms and signs studied in a multivariate survival analysis, only erythrocyte sedimentation rate had prognostic significance independently of other factors.

Reliability of angiography in the evaluation of renal adenocarcinoma.

The accuracy of angiography in TNM classification and its significance in the assessment of prognosis was evaluated in a series of 178 consecutive renal angiograms performed on patients with renal adenocarcinoma. The overall accuracy in the T category was 74.2%. Angiography was reliable in the assessment of the renal vein but not reliable in nodal involvement. Radiographic signs associated with an unfavorable prognosis were an additional vascular supply from sources other than the lumbar artery, nonvisualization of the renal vein and a poorly defined tumor margin.

Angiography in renal oncocytoma: a report on 13 cases.

Angiographic features of 13 surgically treated oncocytomas are presented. A sharp rim was demonstrated in all cases; a 'spoke-wheel' arterial pattern in only 3 cases, and a homogenous nephrographic phase in 2 cases. None of these findings is specific for renal oncocytoma.

Reliability of aspiration biopsy cytology in the grading of renal adenocarcinoma.

Needle aspiration biopsy and histologic samples were taken from three corresponding regions of the surgical specimens immediately after nephrectomy from 150 renal parenchymal tumours. At review 144 of these were diagnosed as adenocarcinomas and 6 as oncocytomas. The adenocarcinomas were classified into three grades according to the degree of differentiation: High, moderate, poor. The grade, as evaluated on cytologic grounds, was in agreement with the histologic grade in 75.9% of the cases and in disagreement by one grade in 24.1%. The cytologic assessment was undergraded in 19.1% and overgraded in 5.0%. The 5-year survival rate for patients with histologically highly differentiated carcinomas was 82.7%, for moderately differentiated 48.7% and for poorly differentiated 28.1%. For corresponding cytologic groups the survival rates were 72.7%, 49.9% and 22.2%. The grade was variable in the same tumour in 24.2% of the cases. The assessment demands several samples, from which the least differentiated is significant. The prognostic significance of needle aspiration biopsy is reliable when cytologic differentiation is poor, but declines as the differentiation improves.

Prognosis of metastatic renal adenocarcinoma.

The prognosis for metastatic renal adenocarcinoma is evaluated on the basis of a consecutive series of 99 initially metastatic patients and 79 patients who developed distant metastases after radical nephrectomy. Patients with only osseous metastases survived longer, but the differences in survival between these and patients with metastases at other sites were not statistically significant. The survival for patients who developed metastases after a disease-free interval of less than 6 months was significantly lower than for others. A solitary distant metastasis was removed from 7 patients and in 4 of these cases no recurrence has been observed during 1 to 9 years' follow-up. In 54% of patients with distant metastases cytostatic agents or hormones or both in combination were administered. Only two partial responses were demonstrated, both concerning pulmonary metastases. Radiation therapy had an appreciable effect in relieving pain resulting from osseous metastases.

Hand injuries in Finland.

The study is based on patients with hand injuries treated in the casualty department of Turku University Central Hospital during the years 1971-75 and 1978; there were 4 800 of these patients annually. Hand injuries accounted for 26% of all accidents. Of these patients, 14% were younger than 15, 78% were of working age and only 8% were over 65. Of the adult patients, 3/5 were men. The average age of the men was 35 and of the women 45. Calculated on the basis of our material, the annual number of hand injuries requiring medical attention in Finland was 33 000/10(6) inhabitants. Most hand injuries among adults, 70%, occurred in the home, 28% at work and only some 3% in road accidents. Lacerations of varying degrees of severity constituted 45% of the injuries, fractures 26% and lesser contusions, excoriations and distortions 20%. There were relatively few serious injuries, e.g. 2% of the cases were nerve and tendon lesions; however, the actual workload caused by these injuries was considerable.

Healing of femoral neck fractures; influence of fracture reduction and age.

In the present study, the influence on healing of femoral neck fractures of reduction of the fracture and the age of the patients were investigated in 446 consecutive cases treated by internal fixation. If the fracture was reduced in valgus position anteroposteriorly, 90% of the cases healed, and if anatomical reduction was achieved the figure was 77%. However, if the fracture remained in varus position bone union was obtained in only 63%. After a good lateral reduction internal fixation resulted in failures in 22% of cases, if a satisfactory in 26%, but poor lateral reduction was followed by failure in 49% of cases. There was correlation to reduction position in all Garden's Stages. With regard to the healing of the fracture, reduction in lateral as well as anteroposterior views were of equal importance. The patient's age did not influence the failure rate in 22% of cases. With increasing age progressive decrease of healing was encountered. The number of patients attending for follow-up until bony union decreased also due to a high mortality rate in these patients. Internal fixation was found to be the treatment of choice in femoral neck fractures, provided that a satisfactory reduction could be obtained.

Radiologic diagnosis of renal colic: the role of plain films, excretory urography and sonography.

The accuracy of plain films, excretory urography and ultrasound for the clinical work-up of renal colic for detecting urinary calculi was evaluated prospectively in 49 patients. Excretory urography was the most sensitive and specific test. Plain films and sonography each had a sensitivity of approximately 60%, but combined yielded a sensitivity of 80%; specificity did not improve. A diagnostic algorithm where sonography was performed first followed by an excretory urography in case of a negative sonography was highly sensitive (93%) and rather specific (79%). This algorithm appears also to have a good cost-benefit rate.

The limited access paradigm: description of one method.

Restricting access to alcohol to a short period daily causes rats, in effect, to drink on command. They usually begin drinking alcohol immediately when it is first made available each day and consume a rather constant amount during each access period. The procedure thus has a variety of useful applications. The specific method reported here in detail provides continual access to food and water, but access to unflavored 10% ethanol solution only 1 h/d, all in the home cage, and produces mean alcohol intakes from 0.5-1.0 g/kg in the access hour.

Brain ethanol in AA, ANA, and Wistar rats monitored with one-minute microdialysis.

A microdialysis system for measuring the ethanol concentration curve in the nucleus accumbens of the rat brain was developed and tested in three different rat lines (AA, ANA, and Wistar) after an intraperitoneal (IP) and an intragastric (IG) dose of 1.0 g/kg ethanol. The flow rate of the modified Ringer solution was set at 5 microliters/min; samples were taken every minute after ethanol administration and analyzed with headspace gas chromatography. After IP administration, the brain ethanol levels rose much more rapidly than tail blood ethanol levels in the same animals. The maximum brain ethanol level after IG administration was lower and occurred later, and were similar to the tail blood levels in AA and ANA rats. No clear difference between the lines was found after IP administration but there was some indication that ANA rats may absorb alcohol after IG intubation faster than AA or Wistar rats.
