Interpeduncular high-density spot in severe shearing injury.

Thirty cases of severe shearing injury were analyzed utilizing serial computed tomography scans and clinical observations. Fatalities occurred in the majority of patients whose Glasgow Coma Scale scores at admission were 6 or less. The presence of perimesencephalic subarachnoid hemorrhage, hemorrhage in the corpus callosum, acute brain swelling, and intraventricular hemorrhage on computed tomography scans resulted in high mortality rates. In 13 patients, interpeduncular high-density spots were observed on computed tomography scans, which were performed in the acute stage of injury. Eight of these patients died, and those surviving had poorer neurological outcomes than corresponding patients with identical Glasgow Coma Scale scores on admission. In the management of severe shearing injury patients, more attention should be given to the presence of interpeduncular high-density spots as well as other important diagnostic computed tomography findings regarding diffuse brain injury.

Subrenal capsule assay as a test to predict the effectiveness of chemotherapy in choriocarcinoma.

The value of the 6-day subrenal capsule assay (SRCA) in predicting the effectiveness of chemotherapy was tested using three human choriocarcinomas. The SRCA method proved to be a valuable chemopredictive test for choriocarcinoma. The results of experimental chemotherapy in nude mice and the results of the SRCA in two types of choriocarcinoma correlated well. Two of three choriocarcinomas showed sensitivity to MAC combination chemotherapy [methotrexate, actinomycin D, cyclophosphamide), and all three showed greater sensitivity to VP-16. The administration of VP-16 for treatment of all three types of choriocarcinoma significantly reduced tumor size in a dose-dependent fashion. From these observations, we speculate that the SRCA may be applied to assess the effectiveness of chemotherapy against human choriocarcinoma.

[Fundamental studies on the subrenal capsule assay as chemosensitivity test for nonsolid tumors].

Fundamental evaluation of the subrenal capsule assay (SRCA) method in nonsolid tumors was made, using two types of murine malignant ascites. Malignant ascites were obtained from mice bearing M-5076 ovarian reticular cell sarcoma or MH-134 hepatoma. These tumor cells were allowed to settle by standing at 4 degrees C to form a jelly-like clot. This clot was cut into fragments about 1mm3 in size and one of these fragments was mashed in trypan blue to estimate the viability grade of the implanted tumor cells. The rest of the fragments were implanted beneath the renal capsule of the mice. On the 6th day after implantation, the assay mice were killed, the increase in the size of the tumor was determined and histological examination was carried out. The results were as follows: (1) The clot was formed reproductively by allowing ascites to settle for one or two days and there was a high viability rate for the tumor cells: 79.9 +/- 11.0% of M-5076 and 90.1 +/- 5.9% of MH-134. (2) The ascites clot thus implanted grew rapidly in the control groups but growth was inhibited by chemotherapy: Tumors were reduced significantly (p less than 0.05-0.005) in the group treated with a single agent. This trend towards a suppressive effect of carcinocidal agents on the tumor growth was more conspicuous as a combination regimen was utilized, a combination of three agents producing the maximum effect. (3) The clot grew more quickly than the solid tumor in both the control and the treated groups. There was a high correlation (r = 0.93 in M-5076, and r = 0.64 in MH-134) between the growth rates of ascites and solid tumor in SRCA. (4) Histological examination revealed that viable tumor cells infiltrated widely under the renal capsule in both types of tumors. These results suggest that ascites and solid tumor are useful materials for the subrenal capsule assay method.

[Radiotherapy of uterine cervix carcinoma].

Seventy-eight patients with uterine cervix cancer were treated radically with standardized radiation therapy at Teikyo University Hospital in Tokyo from January 1979 to December 1985. The age of these patients ranged from 32 to 88 years old (average age 66.0). The pathology and the stage of them were 71 cases of squamous cell carcinoma (1 in stage I, 11 in stage II, 58 in stage III and 1 in stage IV) and 7 cases of adenocarcinoma (6 in stage III and 1 in stage IV). The cumulative survival rates for 5 years by Kaplan-Meier method were 71.5% for squamous cell carcinoma stage II, 47.4% for squamous cell carcinoma stage III and 0% for adenocarcinoma stage III. Radiation injury was studied by the grading system of Kottmeier-NIRS in Japan. The incidence of the injuries for grade 2 and 3 was 12.8% (10/78), and the items of those were rectal injury 5.1% (4/78) and sigmoidal colonic injury 7.7% (6/78). The results of survival rate were fair for the squamous cell carcinoma compared with the other reports but poor for adenocarcinoma stage III. Concerning the therapy for advanced adenocarcinoma of the uterine cervix, special consideration should be given for irradiation doses or infusion chemotherapy.

[An electron microscopic study of malignant giant cell tumor of bone].

Three cases of malignant giant cell tumors were studied by light microscope and electron microscope. Light microscopic observation revealed three different types of tumor cells, namely, malignant stromal cells, fibrosarcomatous cell and osteosarcomatous cells. Electron microscopic observation revealed that fibrosarcomatous cells had ultrastructure similar to that of malignant stromal cells. Both types of cells showed nuclear pseudo-inclusions (NPI), and well developed organellae related to matrix synthesis in the cytoplasm. Polysaccharide staining of the same cells revealed positive products in the cytoplasm. Osteosarcomatous cells showed irregular nuclei which contained NPI and well developed r-ER. The cells were located in the non-calcified matrix which consisted of collagen fibers. No alkaline phosphatase activity was observed in this area. These facts suggest that: (1) malignant stromal cells and fibrosarcomatous cells synthesize collagenous stroma as normal fibroblasts do, and (2) osteosarcomatous cells are devoid of the ability to promote matrix calcification.

Small dose of 2-deoxy-D-glucose completely excludes the overgrowth of fibroblasts.

The present work has been undertaken to assess the effects of a small dose of 2-deoxy-D-glucose on proliferation of fibroblasts in culture by morphological criteria. Fibroblasts in culture medium with 2-deoxy-D-glucose proliferated only to a slight degree, but cell confluence was not achieved. They became swollen and the cytoplasm was increasingly occupied by lucent area. These changes became prominent on the 10th day of incubation. When cell mixtures of fibroblasts and TYK-nu (cell line of undifferentiated adenocarcinoma of the ovary) or TYK-nu only were incubated under the influence of 2-deoxy-D-glucose, the degenerative changes aforementioned were observed only in fibroblasts, while no significant changes were ascertained in the cell line of the cancer by morphological criteria. These results suggest that a small dose of 2-deoxy-D-glucose shows considerable promise in specific inhibition of fibroblastic proliferation during the process of establishing the cell line of the cancer.

Acoustoelectric technique for assessing the mechanical state of the dental implant-bone interface.

A simple technique is described for in vivo assessing the interfacial rigidity between a dental root implant and the bone surrounding it. The method is based on estimating the frequency (10 to 150 KHz) and amplitude of the vibration of the implant induced by a small pulsed force. Application of the force to the implant and detection of the vibrational signal from the implant are performed by lightly touching it with two fine needles connected with piezoelectric elements. A distinct signal difference observed in animal tests of bioactive implant and nonbioactive one is shown to well agree with that obtained from corresponding ex vivo models.

Plasma thromboxane and prostacyclin: comparison during normal pregnancy and pregnancy complicated by hypertension.

Plasma levels of thromboxane B2 (TXB2) and 6-keto-prostaglandin F1 alpha (6-keto-PGF1 alpha), stable metabolites of two prostanoids with opposing biological effects, TXA2 and prostacyclin, were measured by radioimmunoassay in normal pregnancy (controls) and pregnancy complicated by hypertension (PIH) from 32 to 36 (Period 1; P1) and from 36 to 40 (Period 2; P2) weeks of gestation. The plasma concentration of each compound in the control subjects was 265.6 +/- 58.4 (TXB2), 132.4 +/- 16.5 (6-keto-PGF1 alpha) for P1 (n = 10) and 142.6 +/- 11.8 (TXB2), 68.5 +/- 5.2 (6-keto-PGF1 alpha) for P2 (n = 10) respectively (pg/ml, mean +/- s.e). In the patients with PIH, TXB2 concentrations increased moderately for P1 (419.2 +/- 21.2; n = 7) and significantly (p less than 0.005) for P2 (452.8 +/- 31.0; n = 7) respectively (pg/ml, mean +/- s.e), while the plasma levels of 6-keto-PGF1 alpha revealed a slight to moderate decrease both for P1 (84.5 +/- 4.0; n = 7) and P2 (59.7 +/- 8.1; n = 7) respectively (pg/ml, mean +/- s.e). The physiological balance of TXB2 to 6-keto-PGF1 alpha was significantly greater (p less than 0.005) in the patients with PIH, where the TXB2/6-keto-PGF1 alpha ratio was 5.2 +/- 0.7 for P1 and 9.4 +/- 2.3 for P2 respectively (mean +/- s.e) compared with that of the controls, where it was 2.4 +/- 0.4 for P1 and 2.0 +/- 0.2 for P2 respectively (mean +/- s.e).(ABSTRACT TRUNCATED AT 250 WORDS)

Comparison of plasma prostanoid levels in the human cord artery in normal and fetal distressed deliveries.

Prostaglandin E2 (PGE2), thromboxane B2 (TXB2; as a stable metabolite of TXA2), prostaglandin F2 alpha (PGF2 alpha) and 6-keto-PGF1 alpha (as a stable end product of prostacyclin) have been measured by using specific radioimmunoassay in the plasma of the cord artery immediately after delivery before the cord was clamped. Plasma prostanoid concentrations in normal deliveries (n = 8, as controls) were 24.8 +/- 2.6 (PGE2), 246.8 +/- 37.0 (TXB2), 122.2 +/- 13.3 (PGF2 alpha) and 82.1 +/- 7.7 (6-keto-PGF1 alpha) respectively (pg/ml, mean +/- s.e). On the other hand, in fetal distressed deliveries showing continuous bradycardia (n = 6), they increased significantly to 275.4 +/- 20.1 (PGE2), 948.6 +/- 102.5 (TXB2), 218.0 +/- 21.4 (PGF2 alpha) and 1498.6 +/- 298.4 (6-keto-PGF1 alpha) respectively (pg/ml, mean +/- s.e, p less than 0.005). However, both PGF2 alpha/PGE2 and TXB2/6-keto-PGF1 alpha ratios declined significantly from 4.70 +/- 0.33 to 0.68 +/- 0.05 and from 3.07 +/- 0.37 to 0.68 +/- 0.12 respectively (mean +/- s.e, p less than 0.005) in the fetal distressed group compared with those of the controls. From these results, it may be concluded that the cord artery, which is known as the patent source for the production of PGE2 and prostacyclin, did exert a sufficiently strong reaction to overcome the undesirable haemodynamic changes to maintain the fetal well-being in utero.

[Effects of oral contraceptives on fibrinolytic system among Japanese and American women].

Neither urinary plasmin-activity nor whole plasmin changed during the menstrual period of healthy women. Among Japanese women, pill-takers' recorded a markedly higher level of plasmin-activity than non-takers. No such trend was observed with whole plasmin and FDP. Among American women, no significant difference between non-users and users concerning the levels of whole plasmin, plasmin activity and FDP was observed. Among Japanese women, a significantly high level of plasmin activity was observed after administering pills for one year. Five out of six rabbits treated with thrombin intramuscularly showed a level of urinary plasmin activity which is five times higher than the control. The number of platelets decreased in two rabbits, the amounts of fibrinogen in one, and prothrombin time in one. Plasmin activity did not change in rabbits treated with 1/20 X and 1 X maximal tolerated dose (MTD) of norethisterone and mestranol, but was significantly increased by 50 X MTD of norethisterone and mestranol. On the other hand, when treated with both norethisterone and mestranol, plasmin activity did not change with 1/20 X but significantly increased with 1 X and 50 X. The activity of plasminogen activator in glomeruli of rats was increased dose-dependently by thrombin. However, norethisterone, mestranol, and norethisterone-mestranol failed to activate production of plasminogen activator. In conclusion, looking at the comparison of the fibrinolytic system of American and Japanese women, the fibrinolytic system in endothelial cells was found to react well to thrombin among Japanese but weakly among Americans. The weak reaction of the fibrinolytic system among Americans is suggested to be a cause of the increase in thrombosis.

[A case of hepatoma with a remarkable response to gamma-interferon administration].

A 59-year-old male was admitted to our hospital with complaints of general fatigue, abdominal distension and edema in the legs in February, 1985. Laboratory findings were as follows: GOT 152 IU/l, GPT 129 IU/l, LDH 555 IU/l, ALP 1147 IU/l, gamma-GTP 413 IU/l, T.-Bil 2.1 mg/dl and AFP 422.6 ng/ml. Multiple SOLs were recognized in both lobes of the liver by abdominal CT scan and echography. Interferon-gamma (gamma-IFN: KW-2202; Kyowa Hakko Co.) therapy was started in March from an initial dose of 1 X 10(6) units and was increased up to 4 X 10(6) units, 2 X 10(6) units being administered as a maintenance therapy for 12 weeks. The tumors became remarkably smaller in size, AFP was decreased to 38.8 ng/ml, and PR was obtained. The only side effect was temporary fever. The patient was subsequently followed without gamma-IFN at an outpatient clinic for about 100 days, but finally died due to rupture of esophageal varices and hepatic failure.

Comparison of plasma prostaglandins in the non-pregnant and pregnant women: a longitudinal study.

Plasma levels of PGE2 and PGF2 alpha were determined in women from 24 to 40 weeks of pregnancy with non-pregnant women as controls. The plasma levels of PGE2 showed no significant changes as gestation advanced except for late pregnancy only. Furthermore, there were no significant differences observed between the control and pregnant subjects except late in pregnancy. On the other hand, the concentrations of PGF2 alpha in the pregnant subjects revealed a significant increase (p less than 0.005) compared with those of the controls, though no significant changes were observed with the advance of gestation except for late pregnancy. The PGF2 alpha/PGE2 ratio showed a six to ten times rise during pregnancy (p less than 0.005) compared with that of the controls. Therefore, the results obtained in the present study may explain the effectiveness of prostaglandin synthetase inhibitors in suppressing preterm uterine contraction and they also indicate the involvement of some factors in the initiation and inhibition of the myometrium contraction.

Moisture-conserving efficiency of condenser humidifiers.

The moisture-conserving efficiency of commercially available condenser humidifiers (small, disposable, airway humidifiers) was studied. A comprehensive comparison of all currently available condenser humidifiers utilising a laboratory system which simulated a breathing, intubated patient in temperature, humidity, dead space, and respiratory pattern characteristics was undertaken. The percentage of moisture conserved, or efficiency, of the humidifiers was calculated gravimetrically from the water loss of the testing system with and without the condenser humidifiers. The Portex Humid-Vent and Trach-Vent, Siemens Servo 150 and the Terumo Brethaia were evaluated. The data revealed an inverse correlation between efficiency and tidal volume. The most efficient were the Portex Humid-Vent at low tidal volumes and Siemens Servo 150 at the mid and high tidal volumes. To achieve a minimum inspired water content of 33 mg H2O/litre gas, a 79% condenser humidifier efficiency is necessary. None of these units met this requirement; however, they may be appropriate for limited clinical application.

Productivity of immunoreactive prolactin (IR-PRL) by the human decidual explants: a new sampling method.

In the present study, we used an explant culture system of the human decidual tissues involving a new sampling method for investigating the productivity of immunoreactive prolactin (IR-PRL) for a 5 day period in labored and nonlabored deliveries. The maximal release of IR-PRL in the incubation medium for each 24 hour interval was achieved from the 2nd to the 3rd day of culture in both groups, which was 145.2 +/- 14.0 ng/ml/10 mg w.w. (mean +/- s.e.; n = 7) in the labor group and 101.5 +/- 16.3 ng/ml/10 mg w.w. (mean +/- s.e.; n = 4) in the nonlabor group. The release of IR-PRL in both groups was not significantly different for the first 3 days. However, the amount of IR-PRL released in the nonlabor group was 50 to 70% of that of the labor group. The tissue content of IR-PRL in both groups ranged between 3.0 and 5.0 ng/ml/10 mg w.w. From these results, it was concluded that 1) our explant culture system for the human decidual tissues produced considerably more IR-PRL than those previously reported and 2) productivity of IR-PRL was lower in nonlabored delivery than in labored delivery and 3) since the tissue content of IR-PRL for each 24 hour interval was very small, it should be strongly emphasized that the production and release of IR-PRL takes place simultaneously in the human decidual tissues.
