Serum interleukin 18 and tumour necrosis factor-alpha levels are increased in Behcet's disease.

Inflammation in Behcet's disease is thought to be mediated by cytokines derived from T-helper type 1 (Th1) lymphocytes. In this study, we tried to determine serum interleukin (IL)-18 and tumour necrosis factor (TNF)-alpha levels of patients with Behcet's disease. Twenty-seven patients with active Behcet's disease, and 20 healthy control subjects were included in this study. Differences between mean serum IL-18 and TNF-alpha level of patients with Behcet's disease were significantly increased when compared with the control group. A significant correlation was found between serum IL-18 and TNF-alpha levels of Behcet patients (rs = 0.627, P < 0.0001). IL-18 and TNF-alpha levels may be related to disease pathogenesis. Increased levels of IL-18 also support Th1 predominance in Behcet's disease.

Idiopathic perianal pruritus: washing compared with topical corticosteroids.

BACKGROUND: Idiopathic perianal pruritus is a poorly managed condition. Topical corticosteroids are effective in idiopathic perianal pruritus, but they may cause some side effects. In this study, the effectiveness of topical steroids were compared with perianal cleansers in the treatment of idiopathic perianal pruritus. PATIENTS AND METHODS: Seventy two patients with the complaint of perianal pruritus were evaluated; 60 of the 72 patients were found to be idiopathic. Twenty eight patients were treated with topical steroids and 32 patients were treated only with a liquid cleanser. RESULTS: At the end of this two week period, cleansers were found to be as effective as topical corticosteroids. CONCLUSION: This study shows that perianal cleansers can be used as a safe first step treatment in idiopathic perianal pruritus.

Nitric oxide levels in Behçet's disease.

Behçet's disease (BD) is a chronic multisystemic disorder which is characterized by a relapsing systemic inflammatory process. In certain inflammatory conditions such as rheumatoid arthritis, over production of nitric oxide (NO) could damage host cells and tissues, either directly and/or following reaction with other free radicals, such as superoxide anion to form species including peroxynitrite or hydroxyl radicals. Excessive superoxide radical production and impaired antioxidant mechanism in both the neutrophils and plasma of patients with BD have been reported. Our study was designed to investigate the role of NO in BD. NO is an extremely unstable molecule and rapidly converted in vivo and in vitro to nitrate (NO3-) and nitrite (NO2-). For this reason serum NO2- and NO3- have been used as an index of NO generation. We measured serum nitrate + nitrite levels, by using an enzymatic one-step methodology based on the reduction of nitrate to nitrite by nitrate reductase from Aspergillus species, in the presence of beta-NADPH. When compared to healthy controls, serum nitrate + nitrite levels were found to be higher in active periods of BD patients (P < 0.01). It was concluded that increased NO production in patients with BD might have critical biological activities relevant to vasculitic events in the active period of disease.

Temporary holiday "tattoos" may cause lifelong allergic contact dermatitis when henna is mixed with PPD.

Temporary tattoos are becoming more popular especially among teenagers. For this purpose henna tattoos are usually used. Although pure henna is relatively safe, combining it with other colouring agents such as para-phenylenediamine (PPD), may cause skin sensitisation. Several cases of contact allergy from temporary tattoos have been reported, each with positive reactions to PPD. Six cases of allergic contact reaction to temporary tattoos are reported. In all cases, the responsible agent was PPD. Two were also allergic to black 'henna' hair dye which also contained PPD. They had been sensitised on earlier holidays by their temporary tattoos. Temporary holiday tattoos may cause lifelong contact allergy. Although the allergenicity of pure henna is low, the allergenicity of PPD is high. Holiday 'tattoo' reactions are going to be common until PPD is removed from the mixture.

The multiple faces of Behçet's disease and its aetiological factors.

Behçet's disease (BD) is a chronic, inflammatory multisystemic condition of unknown aetiology. It is clinically characterized by recurrent orogenital ulcerations and skin eruptions; ocular manifestations; arthritis; vasculitis and in some cases neurological and large vessel involvement. Aetiology has not been defined, but genetic, environmental, viral, bacterial and immunological factors have been proposed as causative agents. Treatment includes colchicine, thalidomide, steroids and immunosuppressive agents and it is based on the severity of systemic manifestations, such as central nervous system involvement, arterial aneurysms and thrombosis of the major veins. Mortality is related to major system involvement. In this article the different clinical features, the multiple faces of BD and a list of currently suspected aetiological factors of the disease are discussed, and treatment modalities summarized.

Isotretinoin-induced nail fragility and onycholysis.

A 23 year old woman who presented with severe acne had been treated with 40 mg/day isotretinoin. On her monthly control, severe cheilitis and bilateral toe nail onycholysis were observed. It is well known that systemic retinoids have several side effects. Although dystropic nail, paronichia-like changes, median nail dystrophy have previously been reported with isotretinoin therapy, onycholysis is rare. In this report, we describe a case with isotretinoin induced nail fragility and onycholysis.

Pulmonary involvement in Behçet's disease: evaluation of 123 I-MIBG retention.

Pulmonary involvement in Behçet's disease (BD) is reported to indicate poor prognosis and high mortality. Our aim was to evaluate the pulmonary vascular status in BD. As 123I-meta-iodobenzylguanidine (123I-MIBG) shares the same uptake, storage and release mechanisms as norepinephrine, associated with the functional state of pulmonary endothelium, it may reflect endothelial injury. Twenty-five patients (13 males, 12 females; mean age, 36.44 years) and 12 age-matched controls were included. After intravenous injection of 123I-MIBG, thorax images were taken at 15 min and 4 h. Appropriate regions of interest were drawn, and heart to mediastinum (H/M) ratios and lung retention indices (LRI) were calculated. The difference between the LRI of controls (75.6+/-4%) and patients (87.7+/-10%) was found to be extremely significant (P<0.005). The LRIs of active (92.85+/-12%) and inactive (83.65+/-5%) states of BD were significantly different (P<0.05) from each other. There was no significant difference between the H/M ratios of controls (2.4+/-0.27) and patients (2.39+/-0.31) or between patients in active and inactive disease states. Our study revealed prolonged lung retention of 123I-MIBG in BD, probably reflecting the severity of the disease. In conclusion, 123I-MIBG lung retention is a valuable finding in the evaluation of pulmonary vascular status and may be a potential marker of prognosis in BD.

Depression, anxiety levels and general psychological profile in Behçet's disease.

BACKGROUND: Behçet's disease (BD) is a multisystem disease with unknown etiology. Until today, the role of emotional stress and the real incidence of psychiatric symptoms in this disease have not been clarified yet. OBJECTIVE: In this study, we aimed to evaluate the general psychological profile of the patients diagnosed as having BD as well as their depression and anxiety levels in order to investigate the psychiatric aspects of this disease. MATERIAL AND METHODS: Twenty-three patients with BD and 17 patients with chronic plaque-type psoriasis who made up the control group were examined by the Beck Depression (BDI), Beck Anxiety (BAI) and Brief Symptom Inventories (BSI). RESULTS: The mean BDI score of the BD group was 11.69 +/- 6.93, and the corresponding value was 9.11 +/- 7.34 for the psoriasis group. The mean BAI score of the BD group was 21.39 + 13.68 and this level was 12.41 +/- 10.70 for psoriasis patients. The mean BSI score of the BD group was 61.30 +/- 43.66 and it was 30.41 +/- 28.77 for the control group. The mean BDI, BAI and BSI scores of the patients diagnosed as having BD were significantly higher than those of the control group (F = 0.234, F = 0.508, F = 0.549 and p < 0.05). CONCLUSION: BDI, BAI and BSI scales are useful to evaluate the psychiatric aspects of BD. The results of our study revealed that the collaboration of a dermatologist and psychiatrist is essential for the follow-up of patients with BD.

A new surgical therapeutic approach to pincer nail deformity.

BACKGROUND: Pincer nail deformity (PND) is an important health problem that causes discomfort in daily life. Until now, some surgical and conservative treatment modalities were applied with success. However, there is still no consensus on the common and accepted form of treatment. OBJECTIVE: To present a new surgical therapeutic approach to PND. METHODS: Ten patients, four with bilateral and six with unilateral PND, were treated with a new surgical technique which is a combination of chemical matricectomy with phenol and nail bed repair. RESULTS: No recurrence was observed in any of the patients after 12-16 months follow-up. No serious complication was encountered. The cosmetic improvement was also marked and satisfactory. CONCLUSION: This new surgical approach was found to be effective and is presented as an alternative treatment modality for PND.

Stress and skin diseases in musicians: evaluation of the beck depression scale, general psychologic profile (the brief symptom inventory [BSI]), beck anxiety scale and stressful life events in musicians.

We had previously investigated skin problems of musicians (N = 117) working in a professional orchestra. The results of this survey suggest a significant incidence of occupational and stress-related skin problems in musicians such as hyperhydrosis, lichen planus, psoriasis, seborrheic dermatitis and urticaria. It was thought that emotional factors exacerbate their problems [6]. This article discusses the importance of psychiatric consultations in dermatologic problems.

Skin problems of musicians.

OBJECTIVE: We investigated the skin problems of high level musicians in a professional orchestra. This study was prompted by our observation of violinists with skin changes. METHODS: Ninety-seven musicians were evaluated. Twelve reporting dermatitis associated with the playing of their instruments were patch tested. We also examined 20 singers (nonplayers). RESULTS: Positive patch test reactions to nickel and colophony were found in only three patients. Musicians were subject to a variety of skin problems from playing their instruments, however, such as hyperhidrosis, cheilitis, and calluses of the fingertips. Singers had skin problems including lichen planus, psoriasis, seborrheic dermatitis, and urticaria. It was thought that emotional factors exacerbate their problems. CONCLUSIONS: The results of this survey suggest a significant incidence of occupational- and stress-related skin problems in musicians.
