[Prevention of venous thromboses and cutaneous necroses using physical methods and pressure therapy in the surgery of chronic venous insufficiency of the lower limbs].

The surgical treatment of chronic venous insufficiency in the lower limbs, whether it is a case of so-called 'essential' varices or of post-phlebitic disorders, presents a certain number of risks of venous thrombosis and especially of cutaneous necrosis or of cicatricial detachment in operations of the Lindon or Felder type. Given a less significant set-back, it has been suggested that restorative surgery of the deep venous network, valve repair or insertion of a valvulated section, should be used in the treatment of post-phlebitic illness. Promising results have been obtained through specific kinesitherapy, involving a Leduc type manual lymph drainage together with pressotherapy, in the preparation of patients and in post-operative follow-up care.

[Dynamic popliteal phlebography].

Since 1984, dynamic popliteal phlebography is an investigative fundamental technique in chronic venous insufficiency of the lower extremities. Our experience includes more than 2,500 examinations. After an anatomical reminder, the technique and methodology of this examination are described. Then its indications are specified: a) in essential varicose veins when a surgical procedure is contemplated in the popliteal fossa: insufficient external saphenous vein, insufficient gemellar veins, etc... b) before repeated surgery for essential varicose veins, c) in the pre-operative evaluation of some post-phlebitic syndromes. Finally, the informations provided by this examination are compared with those provided by clinical examination, Doppler and ultrasonography.

[Diagnostic and therapeutic problems posed by the association of arterial disease and chronic venous insufficiency of the lower limbs].

The association of arterial disease and chronic venous insufficiency of the lower limbs poses a number of diagnostic and therapeutic problems. It is useful to define, at the time of diagnosis, which patients are suitable for further investigations (vascular function tests, angiography), in order to able to demonstrate the predominant pathophysiological disorder. The combination or these two diseases may influence the treatment modalities.

[Surgery in postphlebitic disease. Apropos of 157 surgical interventions].

Surgery occupies only a restricted position in post-phlebitic illness. The authors analyse 157 surgical operations carried out on 138 patients between 1969 and 1983. A detailed report is always given pre-operatively as a matter of course by Functional examination and phlebography, in order to locate the predominant physiopathological disorder or disorders: superficial venous deficiency--reflux via the perforators--the obliteration syndrome or devalvulation syndrome of the deep venous system. After a short listing of the surgical methods, the results of these different operations are analysed. They are hard to assess. Where there is a relapsing ulcer, surgery of the perforators produces 70 percent good results. The therapeutic indications are discussed, finally, on the basis of recently published series and the authors' experience.
