[Pediatric otorhinolaryngology in Russia: state of the art, problems and prospects].

Achievements of present-day pediatric otorhinolaryngology are demonstrated: novel techniques of diagnosing hypoacusis and deafness providing objective evidence on hearing at any age; development of cochlear implantation in children; introduction of diagnostic and therapeutic ENT endoscopy, therapeutic and surgical laser procedures into wide practice, new approaches to ENT pediatrician training. It is suggested to register officially pediatric otorhinolaryngology.

[The role of active audiological examination of infants in detection and prevention of hearing disorders].

In screening examination of 202 young children attending kindergartens, acoustic impedance audiometry has detected children (44%) with hearing problems. Endoscopic examination of the nasopharynx demonstrated that conductive hypoacusis resulted from hypertrophy of adenoid vegetations of different degree and chronic adenoiditis. These data emphasize the role of active audiological examination of young children in detection and prophylaxis of acoustic disorders.

[Etiology and rational therapy of severe otitis media purulenta acuta in children].

Etiology and antibiotic susceptibility of the pathogens of otitis media purulenta acuta in children was studied within 2000-2005. A total of 161 children at the age of 1 to 14 years were examined. The middle ear discharge collected during paracentosis (79.5%) or spontaneous perforation of the tympanic membrane (20.5%) was used in the bacteriological tests. The microflora growth in the culture was detected in 80% of the cases. The leading pathogen was Streptococcus pyogenes (47.5%), the part of Streptococcus pneumoniae amounted to 36.6% of all the etiologically significant microflora, the part of Staphylococcus aureus amounted to 6.9% and that of Haemophilus influenzae amounted to 4.0%. The part of the associations of 2 microorganisms equaled 5.0%. The species composion of the pathogens differed from that described in the literature. The isolates of S. pneumoniae and S. pyogenes were characterized by low resistance to macrolides (4.0-6.3%). As for the S. pneumoniae isolates, 97.3% of them was susceptible to penicillin. The results of the etiology study and the pathogen antibiotic susceptibility showed that the drugs of choice for the empirical antibacterial therapy of otitis media purulenta acuta in children should be amoxicillin and 1st generation cepholosporins. When the antibacterial therapy within the first 3 days fails, it is advisable to use protected aminopenicillins or 2nd generation cephalosporins. In case of the drug intolerance, macrolide antibiotics should be used.

[Impact of sound signals of high intensity on the acoustic analyzer].

Guinea-pig experiments were made for objective assessment of the effects of high-intensity noise on the acoustic analyzer. Noise with intensity above 100 dB for 11 hours is accompanied with thresholds rise by 20 dB. In some animals hearing loss was observed for several weeks after discontinuation of a sound impact. In the presence of initially high conductive hypoacusis intensive sound load may cause short-term sound threshold lowering with further damage to neurosensory cochlear elements.

[Hearing function in children with speech retardation].

Hearing function was studied in 140 children aged between 2 and 5 years with speech retardation and perinatal pathology for formulation of further treatment policy and rehabilitation. Impedance audiometry, SEAAE, game audiometry identified hearing loss of the first, second, third, forth degree in 6 (4%), 10 (7%), 24 (17%), 31 (21%) children respectively. Deafness was registered in 17 (12%) children, 52 (37%) examinees were audiologically normal.

[The role of chlamydial infection in development of acute respiratory diseases in children].

The examination of 245 children with respiratory diseases has detected chlamydia in 45 (18.4%) of them. Chlamydial species were identified and clinical symptoms of acute respiratory diseases (ARD) of chlamydial etiology are described. Five serological groups of patients depending on the presence or absence of chlamydial infection markers were recognized. Antibacterial drug treatment showed that most effective were sumamed, rulid and clindamycin. Long-term outcomes in relation to the kind of treatment are analysed.

[Effects of respiratory allergic pathology on voice function in children].

The article deals with respiratory allergic pathology in children with a focus on the course of different forms regarding anatomo-physiological changes of the growing organism, on laryngeal status and voice function. Difficulties of examination of the larynx in children especially in young children are described as well as methods of such examination. It is emphasized that children with different forms of respiratory allergic pathology need endoscopic ENT examination. Original clinical experience is analysed.

[Efficiency and safety of rhinoseptoplasty in childhood].

180 children were examined and operated for deformation of the nasal ceptum (NS). By intensity of NS growth, three age groups were distinguished: 5-7 years of age (12.7%), 8-13 years (34.4%), 14-15 years (52.9%). External and constitutional signs, characteristic complex of otorhinolaryngological and general symptoms, concomitant and complicating diseases in such children are described as well as methods of correction and plastic repair of nasal septum deformation in children. A total of 265 operations were made. The operations were effective not only in reestablishment of free nasal breathing but also for improvement of functional nasal indices, sharp reduction of the number of concomitant diseases, general conditioning. In the presence of indications, deformation of the NS should be operated in early childhood with consideration of the age specific features of NS growth, it is not necessary to wait for older age and stopping of face skeleton growth.

[ENT lesions in children with AIDS].

The number of AIDS patients in Cambodia is on the increase. The examination for AIDS covered 50,000 children and 23,000 adults. HIV-seropositive results were obtained in 1800 and 1250 children and adults, respectively. ENT lesions in children with AIDS are characterized.

[Clinical effectiveness of Duracef in children with ENT inflammation].

106 children aged 4 to 14 years received duracef for different acute and chronic (exacerbation) ENT inflammation. The drug proved highly effective in acute diseases of the parynx, paranasal sinuses and ear. In exacerbations of chronic inflammatory diseases it was insufficiently effective. In acute inflammation use of duracef is beneficial as it is effective, inexpensive, low toxic, convenient in administration, is well tolerated, friendly to the intestine.

[Role of macrolides in antibacterial therapy of otitis media in children].

A new macrolide vilprafen (jozamicine) was used as monotherapy or in combination with rhinopront and mucopront in 50 children with inflammation in the middle ear. Otorrhea ceased in 41 patients. Vilprafen is recommended after failure of penicilline or cephalosporine treatment, in allergic reaction to these antibiotics, in simultaneous adenoiditis and exacerbation of chronic tonsillitis.
