The ultrastructural aspects of neoplastic myoepithelial cell in pleomorphic adenomas of salivary glands.

The purpose of this study has been to establish the major ultrastructural aspects of the myoepithelial cell and the myoepithelial-like cells proliferated in the pleomorphic adenomas of salivary glands. Thus, twelve benign pleomorphic adenomas of salivary glands have been studied by electron-microscopy transmission techniques. Our analysis has proved the proliferation of two major cellular populations, one of ductal type and one of myoepithelial type, which tried to reproduce the tubulo-acinar cytoarchitecture from the normal salivary glands. We have also noticed the key role of the so-called 'modified' myoepithelial cells from the periphery of the proliferating epithelial units in the genesis of the myxoid and chondromyxoid tumoral stromal areas. All these ultrastructural aspects have explained the great histological diversity of these salivary gland neoplasms as well as the key role of the myoepithelial cell in its histogenesis.

Urethral and neobladder cytologic survey in patients with total cystectomy.

Fourteen patients with total cystectomy and sigmoidian ureterostomy, 10 with cutaneous ureterostomy and 6 with substitution bladder were studied from the cytologic point of view; from all patients samples of voided urine, urethral and neobladder washing were taken and smears stained with blue polychrom-tanin Drăgan; the smears which belong to the last group were cytochemically investigated, too. From morphohistochemical point of view biopsies taken from cases with recurrences and one case with substitution bladder which died in the 10th day after surgery were studied. The cytologic study revealed two recurrences: one urethral and one at the pelvis level at 18 and respectively 4 months from total cystectomy, both being transitional cell carcinoma. Cytologic and histochemical aspects of the intestinal wall used as neobladder are described, revealing epithelial modifications and chronic inflammatory infiltrate with marked decrease of mucous synthesis. The importance of urinary cytology in oncologic survey of these patients is discussed.

Multiple inverted papilloma of the urinary bladder associated with glandular metaplasia of colic type. A case report.

A case of inverted papilloma of the urinary bladder associated with glandular metaplasia is presented. A bladder urothelial carcinoma was suspected after clinic and endoscopic findings, but the diagnosis was established only on histological picture. Morphological and histochemical peculiarities of the lesions are described, considering the case being the first with such an association-according to our knowledge. The malignant potential of inverted papilloma and glandular metaplasia are discussed.

A new trichrome staining method: its practice and application in normal tissues.

A new trichrome staining method is presented. The staining solution contains: chromotrope RH, lissamine green SF, phosphomolibdic acid and acetic acid in aquous solution. The steps of the technique are described, insisting on the staining solution and dehydration. The authors revealed the staining properties of the method in different tissue and organs. The importance of the method for the histological study and its possible applications in pathology is discussed.

Quantification of the chronic portal infiltrate in chronic hepatitis.

We investigated 50 patients admitted with chronic hepatitis in whom we performed core liver biopsy. The fragment were prepared with the usual histological technique, and the sections were stained with hematoxilin-eosin, in order to prove the pathological and immunohistochemical diagnosis. In order to point out the cell population of the inflammatory infiltrate we used the immunohistochemical method with DAKO monoclonal antibodies (anti-LCA and CD3), Gömöri's trichrome stain and with Alcian Blue and safranin method. In the chronic persistent hepatitis (CPH) B lymphocytes are predominant, while in the chronic aggressive hepatitis (CAH), the predominant cells are of T type (lymphocyte inversion). But there are also cases of persistent chronic hepatitis, in whom the inflammatory infiltrate is represented especially by T lymphocytes, these have a great potential of evolution in CAH. The quantification of the elements of the chronic portal inflammatory infiltrate is a importance factor of prognosis in chronic hepatitis.

Urine cytology in the follow-up of patients with transitional cell carcinoma of the urinary bladder: is that enough as a single method?

There were studied 130 patients with Ta, T1 and T2 transitional cell carcinomas of the urinary bladder, all treated by transurethral resection. After transurethral resection the follow-up was performed in 48 cases by cystoscopy and urine cytology and in 82 cases by cytology alone. In the first group there were 27 recurrences; cystoscopy failed to identify recurrences in 3 cases and cytology in one of them; all 3 patients with apparently false positive cytology recurred at 4, 9 and 11 months from the cytodiagnosis. From 82 patients of the second group 42 developed recurrences. Urine cytology was positive in 80.9%, suspect in 14.2% and false negative in 4.7% of cases. There were only 2 unsuspected recurrences with a consecutive delay in the correct diagnosis of recurrence in another one. Our results suggest that urine cytology can replace cystoscopy only when it exists a good correlation between its results with pathological findings (over 80% real positive results).

Thymus and thymoma: what's new?

The thymus is the prototype of lymphoid and epithelial organ that consists of lymphoid and epithelial cells. In spite of remarkable progresses made in the field of the immunohistochemical characterisation of the thymus parenchyma, the diagnosis of thymoma largely depends on the interpretation of conventional morphologic aspects. Histogenesis of this organ is a multi-step process, and many stages reproduce lesions and changes found in the adult thymus. The normal structure and its variants are extremely helpful to differentiate normal from pathologic aspects. Particular aspects of the thymus structures were shown in myasthenia gravis, despite the behaviour of thymoma in these patients is not clearly understood. Authors performed a detailed description of the conventional pathology of the thymoma, based on the new classifications, recently adopted. The immunohistochemical profile could be helpful in the diagnosis of many cases, and also seems to be useful in prediction of invasion that is the most important criterion in prognosis.

The immunohistochemical profile of luminal epithelial neoplastic component from pleomorphic adenomas of salivary glands.

The immunohistochemistry study made on 30 cases of salivary glands pleomorphic adenomas aims to establish the antigen profile of luminal epithelial neoplastic component proliferates into these tumors. We ascertain that luminal epithelial neoplastic cell were almost exclusively positive to antibody peculiar to epithelial differentiation (AE1/AE3, MNF 116, CK 8, CK 19, CK20, EMA) and occasional to S-100 protein and negative to vimentin, alpha-smooth muscle actin, calponin, Glial Fibrillary Acidic Protein (GFAP). This kind of immunoreactivity was similar to ductal cells from striated channels of salivary glands, suggesting the origin of such pleomorphic adenomas from this place.

Overexpression of cytokeratin 34beta E12 in thymoma: could it be a poor prognosis factor?

We included in our study nine normal human thymuses and sixteen thymomas (1 type A, 2 type AB, 3 type B1, 3 type B2, 5 type B3 and 2 type C). The 25 patients were between 7 days and 75 years old, and were submitted to open surgery for correction of congenital heart defects or for mediastinal tumor mass. Biopsies were formalin fixed for 24 hours and then embedded in paraffin using routine procedure. Five micrometers step sections were mounted on silanized slides. Slides from each case were stained with haematoxylin and eosin method for morphological diagnosis. Additional sections were immunostained using monoclonal antibodies against high molecular weight cytokeratin clone 34beta E12. We found a very strong positive reaction for this type of cytokeratin in thymomas type B2, B3 and inconstant in type C comparative with normal thymus. Also, the number and distribution of positive epithelial cells in normal thymus versus thymomas were different. We found positive cells into capsular vessels of thymomas. This could be an invasion marker for apparent encapsulated thymomas. Strong positive reaction in almost all epithelial cells of thymomas types mentioned above was correlated in part with invasion. We concluded that expression of 34beta E12 cytokeratin is a useful marker for thymomas with high grade of malignacy, correlated with vascular and capsular invasion.

Experimental skin calciphylaxis induced by iron citrate sorbitol in young dogs.

An experimental model of skin calciphylaxis using iron citrate sorbitol is presented. There were used for the experiment 12 young dogs sensitized with D3 vitamin and then injected with 0.2 ml Jectofer on the internal face of the shank. Macroscopic lesions become evident after 2-3 days last drug was administered. Nodular calcifications occurred after 7-8 days as white, hard and irregular lesions, when sectioned, presenting dissociable crystals. From the microscopic point of view fatty cysts, dermic granuloma and an amorphous irregular material are described. In the early period Perls stain is positive and becomes negative after 3 or 4 days. Von Kossa reaction is positive after 4 or 5 days and alizarin S after 7. Degenerative lesions of elastic fascicles are noticed and discussed in relationship with localization of calcium salts. On the basis of these data the possible succesion of skin calciphylaxis steps is discussed, but many things remain unknown.

Fine needle aspiration of the prostate: a histo-cytological correlation.

There were studied 84 patients with benign and malignant hypertrophia of the prostate from a cytomorphological point of view. Fine needle aspiration and transurethral resection was practised and a comparison between cytologic and histologic results was made. There were 14 adenomas, 3 adenomas with associated atypic canalicular hyperplasia, 6 adenomas with associated carcinoma and 61 carcinomas. Cytologic observations revealed malignant cells in 59 from 61 cases of carcinoma and in 4 from 6 adenomas with carcinoma. General accuracy of the method was appreciated at 96.8%. The incidence, causes and significance of false negative and one false positive results are discussed. Using Gaeta system as standard for histologic grading we may conclude that information (as well as histological form of the tumor and invasion) is difficult or even impossible to obtain only by the study of the smears. The importance of the method for detection of "early cancer", its inocuity, repeatability and the reduced number of complications recommend it as the most useful in the diagnosis of prostate carcinoma.

Trichomonas vaginalis--identification by rapid BPT-Drăgan staining.

The blue-polychrome-tannin rapid staining (BPT-Drăgan, 1968) applied on fresh and dried vaginal smears makes possible the identification of different forms of Trichomonas vaginalis (TV) parasite. The nucleus is coloured in blue-violet and the cytoplasm in various degrees of blue-green, violet or bichromatical, with blue and violet zones at the cell periphery. In the cytoplasm, the staining allows the identification of many granules and vacuoles. All the stainable substrates have an acid character and represent morphohistochemical aspects which reflect the parasite metabolism in different functional and evolutive stages. Our results allow the recommendation of the method in the screening actions of the TV identifications.

[The value of urinary cytology in the diagnosis and follow-up of patients with primary bladder tumors].

The authors have investigated the cyto-histologic correlation in 146 patients with primary urinary bladder tumours, and the causes which favor the false negative results. The authors found malignant cells in 92% of all cases, and stress the optimal conditions for the cytological investigation. It is considered that the study of the urinary sediment could replace successfully urinary bladder washing for the cytological surveillance of the patients operated for urinary bladder tumours. Diagnostic difficulties are presented, and the causes of the false negative results, which include: associated affections, special pathological conditions, therapeutical procedures, and technical details. The importance is discussed, of the urinary cytology study for the application of the associated immunostimulating therapy. On the basis of their personal experience the authors present succinctly the indications of urinary cytodiagnosis.

Microscopic changes of the urinary bladder in patients with primary tumors locally treated with Corynebacterium parvum.

The study covered 34 patients with tumors of the urinary bladder treated surgically by transurethral resection and, as an adjuvant therapy, with Corynebacterium parvum intravesically administered. Microscopic observations were performed on smears stained with blue polychrome-tannin and the histologic study used preparata stained with hematoxylin-eosin and van Gieson. After the second series of instillations, many biopsies from the same patient were taken. The investigation revealed the negativity for malignant cells on the cytologically studied smears, and histologically a chronic infiltrate formed of lymphocytes and plasma cells along the basement membrane could be observed; sometimes the infiltrate was follicularly organized. The morphologic changes of the urinary bladder mucosa were correlated with the immunostimulation potential of Corynebacterium parvum.
