Frequency, polymorphism and ultrastructural characteristics of the nuclear inclusion bodies encountered in cancers of the uterine cervix are reported and briefly discussed. The nuclear inclusions are grouped in three distinct types: a) nuclear bodies (comprising type I and II inclusions according to Bouteille et al.'s classification), b) inclusion of cytoplasmic origin, and c) particles of chromatic material. The ultrastructural aspects of the chromatic particles suggest an early structuration of viral chromatin into core material. There appears to be a direct relation between the frequency of chromatic particles and raised antiherpetic antibodies in the patient's sera.
Seminal vesicle cytology was studied in 18 patients admitted with diseases of the prostate, in whom fine needle aspiration by transrectal route was performed. Three cell-types are described: principal, basal and degenerative forms. Nuclear and cytoplasmic peculiarities of the principal and basal cells are revealed, discussing their value in the differential diagnosis from well-differentiated carcinoma of the prostate. It is emphasized the importance of pseudoatypical aspects of seminal cells--considered as a source for false positive results in FNAB of the prostate.
There were cytologically studied 42 patients admitted with radiologically suspected tumors of the upper urinary tract. Samples of voided urine and washing of the upper urinary tract were taken, and dried smears were stained with blue polychrome-tanin method. In all cases, the diagnosis was evaluated intraoperatively and cytologic results were compared with pathological ones. In 42 patients, there were found 34 transitional cell carcinomas (TCC) of the calices, renal pelvis and ureter and lithiasis in 8 cases. Cytology from voided urine identified malignant cells in 76.4% of cases and in washing samples in 97.1%. There were 8 false negative results with voided urine and only one with the upper urinary tract washing; the only false positive result was induced by an epidermoid metaplasia of the pelvic urothelium. The diagnostic value, as a primary procedure, and the importance of cytological method in the follow-up of patients operated with TCC of the upper urinary tract are discussed.
Four cases with small cell carcinoma of the prostate were studied pathologically and histochemically. A diffuse, homogeneous proliferation of small carcinomatous cells which usually replace normal architecture of the gland was the common pathologic feature in all cases. Histochemically, only double silver impregnation of Churukian-Schnenk was intensely positive. The value of early diagnosis in small cell carcinoma of the prostate is discussed, having in view that, virtually, only radical postectomy is an effective treatment.
Bioptic fragments of adipose white tissue taken from trochanterian area from children of 2-22 months old were ultrastructurally investigated. Children were of both sexes, 5 normal and 22 with clinical diagnosis of malnutrition. There were studied many interadipocyte spaces signalling out in cases with malnutrition modifications of different components, some of them related with the degree of malnutrition. There were noted: disorganisation and disappearance of basal membranes of capillaries and glycolema; modifications of endothelial cells with lesions of the capillary wall and free degraded red blood cells; disorganization of the ground substance in small areas or sometimes extended to all matrix of the space; collagen fibres reduced in number and size, and in two cases the presence of collagen fibrils with severe lesions, realeasing an electrondense material, fibrinoid-like; matrix infiltration, in some cases with lipids. In only one interadipocyte space a synaptic button was noted in contact with capillary. In malnutrition lesions of cellular elements of the white adipose tissue the following were observed: adipocytes, fibroblasts, fibrocytes, endothelial cells, mast cells--which in their turn are responsible for modifications of macromolecular structures of the extracellular matrix--glycosaminoglycans, proteoglycans, components of which biosyntheses are cell-dependent.
Biopsies taken by core-needle biopsy and/or transurethral resection to 68 patients with prostatic neoplasia stage III and IV, were studied morphohistochemically. Morphological observations showed high incidence of mixed histopathological forms. The Churukian-Schenk and Garvey silver impregnations were used in order to identify associated neuroendocrine differentiations. Three cases presented monomorphic small cell proliferation which contained numerous argyrophilic cytoplasmic granulations. The prognosis in these cases was poor--the patients died because of neoplastic extension within 5 to 9 months following diagnosis. Neuroendocrine cells were identified in 15 cases, 6 of which presented a malignant pattern. Taking into consideration the hormonal resistance of malignant cells with argyrophilic granulations, the necessity of chemotherapy associated to hormonotherapy is discussed.
There were studied 68 patients admitted with clinical signs of chronic gastritis. Biopsies were taken by endoscopical route from the antral and fundic levels in all cases, and paraffin slides were stained with haematoxylin-eosine, modified Giemsa, Warthin-Starry and blue cresyl methods. Microscopical results were compared with those obtained with the urease test performed from the gastric mucus. Results noted the presence of Helicobacter pylori in 41 from 68 cases in the antral mucosa, like curved bacteria located in the mucus on the excretory segment and often on the surface, covering the epithelium. Intracellular Helicobacter pylori was not noted. Its presence is related especially with the active chronic gastritis and rarely with inactive forms; it was not possible to establish a relationship between the morphology, incidence and features of their infiltrate, and the presence of Helicobacter pylori. The modified Giemsa method is easier to perform, faster and cheaper, and it may be currently applied in practice, but from the sensitivity viewpoint, best results have been achieved with Warthin-Starry silver impregnation.
There were investigated thymic biopsies taken from 14 children deceased of intercurrent diseases. After routine paraffin procedures, slides were stained with hematoxylin-eosin, Gordon-Sweet, Wilder and Laidlaw methods. Reticular fibers are described in the stroma and parenchyma of the thymus. Their number and thickness are higher in connective septa, perivascular sheaths, at the cortico-medullary junction and around Hassall's bodies. Their involvement in a basal membrane like structure located between epithelio-reticular cells and thymocytes is discussed.
There were investigated 51 patients admitted with suspicion of chronic hepatitis and secondary tumours of the liver. In all cases, fine needle aspiration biopsy was performed, and smears were fixed by drying and stained with BPT-Dragan and May Grunwald-Giemsa. In chronic hepatitis, the needle aspiration biopsy has a limited importance in the diagnosis because it cannot notice architectural features. The nucleo-cytoplasmic lesions are better observed. In secondary tumours of the liver, the aspiration biopsy, through simplicity and fidelity, represents the main method in the diagnosis. Cytological results were compared with histological ones. On smears we observed hepatocytes and atypical cells.
Eighteen patients with chronic hepatitis of viral and alcoholic etiology investigated and core needle biopsy of the liver was performed. Microscopical investigation consisted of pathological diagnosis, grading and staging of chronic hepatitis and identification of ito cells with smooth muscle cell anti-actin. Ito cells were noted mainly around portal spaces as small cells with vacuolated cytoplasm. Their number is higher in normal liver and chronic persistent hepatitis. Few ito cells were observed in active forms and they are absent in almost all cases with cirrhosis. In chronic hepatitis of alcoholic origin, ito cells are large and their cytoplasma contains many vacuoles. A strong correlation between the number of ito cells and the stage of the chronic hepatitis was noted but it was not possible to correlate the same parameter with Knodell score.
There were investigated 583 cases with tumors of the urinary bladder and 612 patients with non-malignant diseases of the urinary tract. Samples of voided urine were taken from all cases and direct smears fixed by drying were stained by rapid blue polychrome-tanin Drăgan method Cytological results were compared with endoscopical and pathological findings. The overall rate of real positive results was 91.7% and false negative results were noticed in 8.3% of cases. A direct relationship between real positive results and histological "G" was found. Causes for false negative results were: tumor developed in a bladder diverticulum, calcified tumor, irradiated tumor, insufficient quantity of voided urine, chronic urinary infections and underestimation of cytological criteria of cellular malignancy. There were 9 false positive results in patients with nonmalignant diseases, due to lithiasis, chronic renal failure and chronic urinary infections. The cytological grade of differentiation was performed by the method purposed by Friedman and Ash, and concordance with the standard histological finding was 76.4%. Urine cytology is thought to be a useful method in the primary diagnosis and recurrences of transitional cell carcinoma of the urinary bladder, in all patients with hematuria, recurrent infections of the urinary tract and neglected lithiasis.
The authors report two leiomyomas of small intestine compressive tumoral mass. Aspects of symptomatology, histopathology, surgical management and prognostic factors with reference in childhood are discussed.
