Demography of sexual orientation in adolescents.

This study was undertaken to explore patterns of sexual orientation in a representative sample of Minnesota junior and senior high school students. The sample included 34,706 students (grades 7 through 12) from diverse ethnic, geographic, and socioeconomic strata. Five items pertaining to sexual attraction, fantasy, behavior, and affiliation were embedded in a self-administered survey of adolescent health. Overall, 10.7% of students were "unsure" of their sexual orientation; 88.2% described themselves as predominantly heterosexual; and 1.1% described themselves as bisexual or predominantly homosexual. The reported prevalence of homosexual attractions (4.5%) exceeded homosexual fantasies (2.6%), sexual behavior (1%), or affiliation (0.4%). Gender differences were minor; but responses to individual sexual orientation items varied with age, religiosity, ethnicity, and socioeconomic status. Uncertainty about sexual orientation diminished in successively older age groups, with corresponding increases in heterosexual and homosexual affiliation. The findings suggest an unfolding of sexual identity during adolescence, influenced by sexual experience and demographic factors.

Demographic and risk factors associated with chronic dieting in adolescents.

A comprehensive, school-based survey was administered to 36,320 Minnesota public school students in grades 7 through 12 during the 1987-1988 school year. Self-reported chronic dieting was much higher in girls than in boys (12.1% of all girls vs 2.1% of boys). For girls, the percentage of chronic dieters was significantly less in grades 7 and 8 (7.8%) than in grades 9 and 10 (13.5%) or grades 11 and 12 (14.3%). There were no differences among urban, suburban, or rural youth. Black girls were less likely to diet compared with white girls. Chronic dieters were more likely than other students to report maladaptive weight-loss techniques, such as self-induced vomiting (relative risk, 9.92 for girls and 9.40 for boys), laxative use (relative risk, 7.18 for girls and 11.00 for boys), ipecac use (relative risk, 8.33 for girls and 11.00 for boys), and diuretic use (relative risk, 7.30 for girls and 13.5 for boys). It is suggested that chronic dieting may serve as a screening marker for more severe eating and weight-loss behaviors.

Ca(2+)-independent change in phosphorylation of the myosin light chain during relaxation of ferret aorta by vasodilators.

1. The effects of the vasodilators atrial natriuretic peptide (ANP) and forskolin were determined on isometric force, intracellular ionized Ca2+ concentration ([Ca2+]i) as indicated by aequorin, and myosin light chain (MLC) phosphorylation in ferret aorta. 2. Atrial natriuretic peptide (10(-7) M) inhibited intrinsic tone with an associated significant decrease in [Ca2+]i. ANP also inhibited the contraction induced by KCl with a significant decrease in [Ca2+]i. MLC phosphorylation induced by KCl was inhibited by ANP. 3. Forskolin (10(-6) M) decreased the intrinsic tone without significantly decreasing [Ca2+]i, although MLC phosphorylation was significantly decreased. 4. A calcium-force curve was constructed by plotting the calibrated aequorin light signal against the resulting force. The control (potassium-generated) calcium-force curve was not shifted by ANP, but was significantly shifted to the right by forskolin. Forskolin also shifted the phosphorylation-calcium curve to the right without changing the phosphorylation-force curve. 5. We conclude that the vasodilatory effect of ANP on vascular smooth muscle is mainly due to a decrease in [Ca2+]i. On the other hand, the effect of forskolin is via both a decrease in [Ca2+]i and a change in the Ca2+ requirement for MLC phosphorylation.

Change of Ca2+ requirement for myosin phosphorylation by prostaglandin F2 alpha.

The mechanism of contraction of vascular smooth muscle by prostaglandin F2 alpha (PGF2 alpha) was examined by simultaneous measurement of the intracellular Ca2+ concentration [( Ca2+]i), force, and myosin light-chain (MLC) phosphorylation in ferret aorta. In the presence of 2.5 mM extracellular Ca2+, PGF2 alpha (10(-5)M) produced a tonic contraction with a transient spike in [Ca2+]i, followed by a relatively small sustained increase in [Ca2+]i (from a basal level of 2.32 +/- 0.07 x 10(-7) to 2.72 +/- 0.05 x 10(-7) M). In Ca(2+)-free bathing media, PGF2 alpha also produced a tonic contraction with a small spike in [Ca2+]i, indicating a release of Ca2+ from intracellular store sites, followed by no significant increase in [Ca2+]i. Ca(2+)-force curves were constructed by plotting the calibrated steady-state aequorin light signal against the resulting steady-state force. The curve was significantly shifted to the left by PGF2 alpha. PGF2 alpha also shifted the Ca(2+)-phosphorylation curve to the left. These results suggest that PGF2 alpha causes contraction by both elevating [Ca2+]i and decreasing the Ca2+ requirement for MLC phosphorylation. The data are consistent with a mechanism where there is either an increase in activity of MLC kinase or a decrease in phosphatase activity. Additionally, there was a smaller, but statistically significant, effect to increase force at any one phosphorylation level, pointing to the possibility of regulation of contractile force separate from MLC phosphorylation.

Psychiatric consultation to a state board of medical examiners.

This article describes the evolution of psychiatric consultation to the Oregon Board of Medical Examiners. The board is charged with the licensing and regulation of physicians in the state of Oregon in order to protect the public and uphold the standards of the medical profession. Psychiatric consultation has focused on the board's investigations of physicians with mental illness and/or substance abuse and physicians who inappropriately prescribe psychoactive drugs or sexually abuse patients. Each of these physician groups is described, and remedial programs for each group are discussed. The authors conclude that psychiatric consultation to medical boards is a feasible and productive activity that can make a positive contribution to the lives of a large number of physicians and patients.

Detection of interleukin-3 in the serum of mice infected with Mycobacterium lepraemurium.

Infection of mice by Mycobacterium lepraemurium is accompanied by ablation of erythropoiesis in the bone marrow and gross enlargement of the spleen. This, together with increased monocytopoiesis and the earlier demonstration of macrophage colony-stimulating factor in the serum of infected mice, suggested the activity of additional cytokines. Eight weeks after infection of mice by M. lepraemurium, interleukin-3 (IL-3) activity was demonstrated in the serum (titer, 1:3200). The serum titer of IL-3 activity was maximal after 13 weeks (greater than 1:6400) and was slightly reduced after 18 weeks (1:6400). That the IL-3 activity detected in the serum of the M. lepraemurium-infected mice reflected the presence of IL-3 itself was confirmed by a neutralization assay using anti-murine IL-3 antibodies; IL-3 activity in the serum of mice 13 weeks after infection was completely abolished by the anti-IL-3 antibodies. Finally, a 1-kb signal of IL-3 RNA was detected in the spleens of M. lepraemurium-infected mice 13 weeks after infection.

Oligoclonal bands in acute schizophrenia: a negative search.

Numerous immunologic abnormalities have been reported in patients with schizophrenia, suggesting that this disorder may in some cases represent a viral or immunologic disorder of the brain. To further examine this question, evidence for intrathecal production of immunoglobulin (IgG) and immunoglobulin oligoclonal bands (OCB) was sought in patients with acute or early schizophrenia. Using high resolution zone electrophoresis and immunofixation, there was no evidence for intrathecal IgG generation in 5 of 5 patients tested. One of 9 patients had OCBs in CSF. This man, although he had a typical history of schizophrenia, tested positive for human immunodeficiency virus antibody in serum.

Why do so many people drop out from auto-injection therapy for impotence?

Although intracavernous injection of vasoactive substances has been shown to be a reasonable and reliable method of reversing impotence, studies assessing patient acceptance of this new technology are lacking. By prospectively following patients and willing partners over a 2-year period, this study sought to document patient utilization rates and the reasons why patients decline or discontinue injection therapy. The cumulative dropout rate was 46%, with patients being most at risk for leaving the program after evaluation or during the trial dose phase. Patients decline treatment because they are unable to accept the idea of injecting themselves or because of potential side effects. Patients discontinue treatment because of perceived lack of efficacy. Patient and programatic obstacles to utilization of self-injection therapy are analyzed.

The malpractice claims experience of physicians investigated for inappropriate prescribing.

Inappropriate prescribing of controlled substances, primarily opiates and benzodiazepines, is the most common complaint brought before the Oregon Board of Medical Examiners. We describe the malpractice claims experience of 120 physicians previously investigated by the Oregon board for inappropriate prescribing. These physicians were matched with a comparison group by age, specialty, and practice location. We found that a mean of one malpractice claim had been filed against each physician in our study, with the specialties of obstetrics and gynecology, neurosurgery, and orthopedics having the most claims. A significantly higher mean number of malpractice claims had been filed against 31 physicians disciplined by the board. Our study suggests a role for state regulatory boards in the malpractice area. We propose that such bodies do practice reviews based on the convergence of two events, a disciplinary action such as those described in this article and the filing of more than one malpractice claim against a physician. Further research is needed on inappropriate prescribing by physicians and its possible association with malpractice.

Physicians investigated for inappropriate prescribing by the Oregon Board of Medical Examiners.

We retrospectively reviewed all allegations of inappropriate prescribing investigated by the Oregon Board of Medical Examiners from 1981 through 1986. Inappropriate prescription writing accounted for 51% of all investigations during this period, with controlled drugs, primarily opiates and benzodiazepines, accounting for most complaints. Of 130 physicians investigated, more than half had previous complaints; 50 were ultimately restricted or disciplined by the board. Inappropriate prescribing of controlled drugs is probably underdetected and frequently repeated. Available literature suggests that inappropriate prescribing of other drugs, especially antibiotics, is extremely common, but such problems were rarely identified by the current discovery and review processes of the Oregon board. Inappropriate prescribing will require increased attention from physician educators and licensing boards.

Lysis of murine macrophages infected with intracellular pathogens by interleukin 2-activated killer (LAK) cells in vitro.

To determine if murine lymphocytes activated by interleukin 2 (IL-2) were cytotoxic against syngeneic elicited peritoneal macrophages (M phi) infected with intracellular pathogens, splenocytes that had been cultured with IL-2 for 5 or 10 days were studied in vitro. These cells, [lymphokine-activated killer (LAK) cells] showed significantly greater cytotoxicity against M phi infected with Leishmania major or Legionella pneumophila than against uninfected M phi. Preferential cytotoxicity against infected M phi was best shown using effector-to-target-cell ratios of 1:1-20:1; when ratios greater than or equal to 40:1 were employed, uninfected M phi were also lysed. The extent to which M phi that had been incubated with L. major were lysed depended upon the proportion of M phi containing intracellular organisms. After infection with L. major, the duration of incubation did not appear to influence the degree of lysis by LAK cells.

Haemophagocytosis and other haematological aspects of Mycobacterium lepraemurium disease of mice.

CBA mice inoculated i.v. with 2 x 10(8) viable MLM were observed at intervals for 5 months. In the bone marrow, haematopoietic cells were progressively displaced by MLM-laden phagocytes; depletion of erythroid cells began earlier and was more pronounced than that of myeloid cells. Transiently, mild anaemia and profound leucopenia were noted. The spleen was enlarged and the site of increasing histiocytosis and extramedullary haematopoiesis which was accompanied by displacement of splenic parenchyma. The liver was enlarged and its parenchyma contained scattered islands of haematopoietic elements. Lymph node cells had been largely replaced by MLM-laden macrophages by the end of the process. Thus, MLM infection exerts important effects on haematopoiesis of susceptible mice and is accompanied by active extramedullary haematopoiesis. In addition, the haemophagocytosis, observed most commonly in immuno-compromised patients infected with certain viral or mycobacterial pathogens, was observed late in the course of MLM disease of mice.

Response of the murine hematopoietic system to chronic infection with Mycobacterium lepraemurium.

Mycobacterium lepraemurium infection of mice produces a chronic lethal disease that is characterized by massive accumulation of macrophages throughout the mononuclear-phagocyte system. We studied the influence of M. lepraemurium infection on the composition and function of the hematopoietic system. Medullary erythropoiesis was virtually abolished, as reflected by a small number of erythroid elements and a decrease in the number and frequency of erythroid progenitors in the bone marrow, together with reduced uptake of 59Fe into bone marrow hemin. On the other hand, erythropoiesis was observed in the spleen, as demonstrated by a large number of erythroid cells, a sixfold increase of 59Fe uptake, and a pronounced increase in the number of erythroid progenitors. A considerable increase of monocyte progenitors was observed in the spleen, and a more modest increase was observed in the bone marrow. This increase may be accounted for, at least in part, by greatly increased levels of macrophage-colony-stimulating factor in the serum of infected mice. Thus, M. lepraemurium infection produces important changes in the hematopoietic system, during the course of which the spleen becomes the major hematopoietic organ.

Prediction of chemical carcinogenicity in rodents from in vitro genetic toxicity assays.

Four widely used in vitro assays for genetic toxicity were evaluated for their ability to predict the carcinogenicity of selected chemicals in rodents. These assays were mutagenesis in Salmonella and mouse lymphoma cells and chromosome aberrations and sister chromatid exchanges in Chinese hamster ovary cells. Seventy-three chemicals recently tested in 2-year carcinogenicity studies conducted by the National Cancer Institute and the National Toxicology Program were used in this evaluation. Test results from the four in vitro assays did not show significant differences in individual concordance with the rodent carcinogenicity results; the concordance of each assay was approximately 60 percent. Within the limits of this study there was no evidence of complementarity among the four assays, and no battery of tests constructed from these assays improved substantially on the overall performance of the Salmonella assay. The in vitro assays which represented a range of three cell types and four end points did show substantial agreement among themselves, indicating that chemicals positive in one in vitro assay tended to be positive in the other in vitro assays.

Intracavernosal injection in the treatment of impotence: a prospective study of sexual, psychological, and marital functioning.

This study assessed the sexual, psychosocial, and medical impact of intracavernosal injection of papaverine hydrochloride and phentolamine mesylate in the treatment of impotence. Ninety-six percent of patients initially achieved satisfactory erections with injections. Common side effects of self-injections included bruising (26%), nodule development (21%), abnormal liver function tests (7%), and pain (5%). No nodules resulted in penile bending or pain, and no patient discontinued treatment due to nodule development. Dropout rate to date was 35%. Self-injection treatment was associated with increases in erection quality, intercourse frequency, and sexual satisfaction, and decreases in masturbation frequency for patients. Partners also reported increases in intercourse frequency, coital arousal, and satisfaction. Decreases in general psychiatric symptomatology and increases in relationship satisfaction and self-esteem were apparent for patients, but not for partners.
