Development of a standard protocol for in vitro cytogenetic testing with Chinese hamster ovary cells: comparison of results for 22 compounds in two laboratories.

A major problem of cytogenetics testing in mammalian cells is lack of agreement of results among laboratories. Our objective was to develop a sensitive in vitro test protocol that was applicable to large-scale chemical screening and yielded comparable results in two laboratories. We used sister chromatid exchange (SCE) and chromosome aberration (CAb) tests in Chinese hamster ovary (CHO) cells. The initial protocol used standard cell densities, medium, batch of rat liver S9 for metabolic activation; positive, negative, and solvent controls; staining and scoring techniques; and fixation times. Treatment without S9 was for 8-12 hr (CAb) or 26 hr (SCE), and with S9 for 2 hr in serum-free medium. Bromodeoxyuridine (BrdUrd) (10 microM) was added to SCE cultures only, 2 hr after addition of the test chemical. Doses were based on the 50% toxicity level in a preliminary test of cell survival 24 hr after treatment. One hundred cells (CAb) or 50 cells (SCE) were scored from each control and from five dose levels. Five clastogens were tested in the first two-laboratory comparison: mitomycin-C, triethylenemelamine, N-methyl-N'-nitro-N-nitrosoguanidine, cyclophosphamide, and benzo(alpha)pyrene. There was quite good agreement between laboratories. Seventeen compounds were then tested "blind" in the two laboratories. As testing proceeded, some discrepancies occurred between the laboratories, and the protocol was modified in attempts to improve the resolution of marginal responses and make dose selection more consistent. The preliminary test for cell survival was omitted. A 10(5) dose range in a half-log series was tested, and cells were scored at the highest dose at which sufficient mitotic cells were obtained, and at the next two lower doses. By delaying fixation times, SCE and CAb were scored at doses that inhibited cell cycle progression. This protocol gave comparable results in the two laboratories in many cases and by testing up to a maximum dose, limited by solubility and/or toxicity, should detect a high proportion of clastogens and SCE inducers.

Influence of chorioamnionitis on long-term prognosis in low birth weight infants.

The contribution of obstetric management to quality of life of surviving low birth weight infants is unclear. A possible association between maternal chorioamnionitis and development outcome was evaluated. One hundred twenty-seven mother/infant pairs with infant birth weight less than 2000 g were studied. The antenatal course was complicated by chorioamnionitis, premature rupture of membranes without chorioamnionitis, premature labor, or abruptio placenta. Analysis of variance was performed using these four diagnosis groups. After potentially confounding variables were taken into account, the overall difference in the four groups in Mental Development Index (Bayley Scales) was borderline (P = .138). However, significant differences remained between the group with chorioamnionitis and the group with premature rupture of membranes without chorioamnionitis (P = .017). The potential advantage of leaving infants in utero after premature rupture of membranes may be offset by disadvantage of chorioamnionitis with respect to future development in surviving infants.

Droperidol vs. haloperidol in the initial management of acutely agitated patients.

In a double-blind clinical study, 27 acutely agitated patients were treated with an intramuscular injection of 5 mg of droperidol or 5 mg of haloperidol from identical appearing vials. At 30 minutes following treatment, 81% of the patients treated with haloperidol but only 36% treated with droperidol required a second injection (p less than .05). Thus, droperidol, a safe butyrophenone neuroleptic, appears to be a drug of choice for rapid and reliable control of acute agitation.

Penile reconstruction with gracilis pedicle grafts.

Penile loss results from various external and self-inflicted catastrophic events. Many types of reconstructive efforts have been used successfully. Recently, bilateral pedicle grafts of the gracilis muscle have been successful and have provided serviceable results in our hands. Technique, complications and indications are discussed and illustrative cases are reported.

The appropriateness of health services for adolescents: youth's opinions and attitudes.

Minnesota high school students were surveyed about their attitudes, beliefs, and opinions about health, illness, and medical care. Data were obtained from small group discussions conducted by the adolescents themselves in schools and agencies throughout the state. This paper reports their views toward adolescent medical services. Teenagers emphasized the idea of service appropriateness as central to promoting service utilization. Key to the concept of appropriateness were the components of staff, cost and confidentiality. Each of these dimensions is examined from the view of youth as to implications for increased appropriateness of services for both utilization and improved health. Findings are discussed in terms of differences in problem definition between the adolescent and professional, and their implications for medical care in conventional and alternative settings.

Taurine transport in epilepsy.

Previous studies of plasma taurine concentrations in epileptics have yielded equivocal results. We measured plasma and urinary taurine in 41 epileptic and 68 control subjects and found plasma concentrations among epileptics to be comparable in general to those of controls, but that two or three classes of plasma taurine concentrations, possibly genetically regulated, occur in both epileptic and control subjects. Our previous studies and data from the present study on taurine excretion revealed three excretion classes under genetic control. The principal finding is that epileptics include disproportionate numbers of low excretors (high reabsorbers), who are presumptive homozygotes for the allele effecting higher reabsorption. If confirmed, these findings suggest that the transport of taurine, rather than absolute taurine concentration, may explain the efficacy of taurine administration in some epileptics but not in others. The locus involved may be one component in the polygenic diathesis to the idiopathic epilepsies.

Transrectal ultrasonography in the evaluation of cervical carcinoma.

Transrectal ultrasonography has proved useful in the evaluation of genitourinary disease. Experience with prostate scans has shown that such conditions as prostate carcinoma and benign prostatic hyperplasia can be diagnosed and assessed with a high degree of accuracy. In patients with carcinoma of the bladder this method is useful in determining the degree of tumor invasion within the bladder wall. Our experience with scans of the uterus is in its early stages. We have studied 30 patients with documented cervical carcinoma and are studying 1) the presence and extent of parametrial involvement, 2) the ability to distinguish benign from malignant lesions, and 3) the identification of bladder invasion.

Preoperative diagnosis of unilateral multicystic kidney with hydropelvis.

We present 2 patients with congenital unialteral multicystic kidney disease with hydropelvis. In the first patient the diagnosis was made by precutaneous puncture of a renal cyst followed by injection of contrast medium; in the second the diagnosis was confirmed by percutaneous puncture of the renal pelvis and injection of contrast medium, although an earlier ultrasonic examination had been strongly suggestive. Since in this condition the cysts and the renal pelvis communicate, either can be punctured to make the diagnosis. The procedures herein described are definitive for the diagnosis and should be followed whenever the urologist desires such a diagnosis.

Death of Mycobacterium lepraemurium after multiplication in CBA, BALB/c and nude mice.

The unexpected death of Mycobacterium lepraemurium in the course of systemic infection of mice, previously noted in the spleens of CBA mice, has been demonstrated in the spleens of BALB/c nu/+ and BALB/c nu/nu mice and also in the livers and other organs of mice of all three strains. That the same phenomenon was observed in nu/nu mice indicates that the mechanism of bacterial death does not involve a T-lymphocyte-mediated cellular immune response on the part of the mice.
