[Doppler flow changes in maternal and fetal blood vessels before and during peridural anesthesia].

26 women with an uneventful singleton pregnancy near term underwent scheduled Caesarean section with lumbar epidural anaesthesia (PDA). Doppler-flow measurements of the foetal umbilical artery and the maternal hypogastric artery were taken before and after sympathicolysis was established. The mean arterial blood pressure was maintained unchanged between the measurements. Both measurements were performed by the same examiner. Three patients received additional general anaesthesia during the operation because of inadequate analgesia. The resistance index (RI) with PDA decreased significantly in 23 patients by a mean of 7.6% (p less than 0.05). The RI in those patients (n = 3) with inadequate analgesia increased by a mean of 27%. The physiologically reversed diastolic flow in the maternal hypogastric artery was mathematically taken into consideration by applying the quotient of the systolic flow and the diastolic reversed flow (V/R-quotient). The V/R-quotient increased with established sympathicolysis by more than 180% in all but one patient. The neonates' mean umbilical artery pH was 7.32 (7.27 to 7.41), the mean five minute Apgar score was 10 (8 to 10).

[Pain therapy and depression in cancer patients].

In a pilot study involving eleven terminal tumor patients suffering pain, the effect of monotherapy with oral, slow-release morphine on analgesia, somatogenic components, depression and state of psychic health was investigated. A significant decrease in pain intensity was observed. A significant correlation was found between analgesia and changes in depression. Individual assessment on the basis for the Hamilton Depression Scale revealed a significant association between the treatment of pain and sleep disturbances, depressive states, restlessness and suicidality. In addition, analgesia led to an improvement in general psychic health as indicated by the v. Zerssen scale. A reactive-depressive symptomatology in tumor patients suffering pain can be positively influenced by selective opiate therapy.

[Salazosulfapyridine-induced eosinophilic pneumonia with pulmonary and cutaneous epithelioid cell granulomatosis in Sjögren syndrome].

A 68-year old woman suffering from Sjögren's syndrome for the last 30 years took sulphasalazine (SSP) for severe signs and symptoms at the joints. Soon after the start of this medication she developed progressive cough and shortness of breath. After two years she was referred for evaluation of a hemoptysis and a reddish skin lesion. The chest radiograph showed wide spread interstitial infiltrates in the lower lobes and some fibrotic changes. FVC was slightly reduced, DLCO markedly reduced. There was a high percentage of eosinophils in the bronchoalveolar lavage (55.2%). Transbronchial lung biopsy and skin biopsy demonstrated epitheloid granulomata. SSP was discontinued. After a short period of prednisone treatment the patient's condition improved considerably. After two months of followup neither pulmonary infiltrates nor any skin lesions were found. History and the clinical course after discontinuation of SSP indicate the relation of these infiltrates to SSP treatment. The previously published case reports of SSP-related lung disorders are reviewed. Three of these case reports included bronchoalveolar lavage. Our data suggest that patients with SSP related pulmonary infiltrates may have a marked increase of eosinophils in the bronchoalveolar lavage fluid.

[Transbronchial biopsy of the lung and bronchoalveolar lavage in the diagnosis of sarcoidosis].

The outcomes of a complex study of sarcoidosis and other interstitial pulmonary diseases in 1325 patients are presented. An important role of intrapulmonary biopsy and bronchoalveolar lavage methods in defining the diagnosis and the process activity, especially when they are used in combination, is demonstrated. Complications in the form of bleedings and pneumothorax were registered in a limited number of the patients, mainly as a result of rigid bronchoscope procedures. Differential and diagnostic signs of sarcoidosis and some other disseminations in the lung, obtained by means of biopsy, bronchoalveolar lavage and clinical data studies, are given.

[Clinical aspects and diagnosis of chronic berylliosis--a case report].

Three case reports of patients with chronic berylliosis are presented. The examination findings of these three cases are all unspecific. Merely the lymphocyte transformation test (LTT), which has, in the meantime, become an established procedure, is capable of demonstrating, at a high level of sensitivity and specificity, the presence of sensitisation towards beryllium, a condition that accompanies the disease itself. All three patients showed a positive LTT. A modification of the diagnostic criteria worked out by Sprince, is proposed, which takes into account the LTT performed in blood and bronchoalveolar fluid.

[IgA antibodies in exogenous allergic alveolitis are more specific than IgG antibodies].

Nineteen sera from patients suffering from extrinsic allergic alveolitis (bird fancier's lung, farmer's lung) and 19 sera from asymptomatic persons demonstrating IgG-antibodies were examined for IgA-antibodies against pigeon serum, Micropolyspora faeni and Aspergillus fumigatus. The IgA-antibodies against pigeon serum and Micropolyspora faeni were less of ten false-positive than the corresponding IgG-antibodies. Hence the IgA-antibodies were more specific than the IgG-antibodies. However, the sensitivity of the IgA-antibodies was lower than that of the IgG-antibodies. On the other hand IgA-antibodies against Aspergillus fumigatus were scarcely more specific than the corresponding IgG-antibodies against pigeon and M. faeni. It would thus seem that IgA-antibodies can supplement the IgG-serodiagnosis of extrinsic allergic alveolitis.

[Systemic pharmacotherapy in backache. Indications and practical applications].

The treatment of acute and chronic low back pain is a problem frequently encountered in the doctor's office. Selective drug therapy has a useful role to play within the framework of the overall therapeutic strategy. Indications and practical application of the major analgesic agents in various low back pain conditions are discussed.

Chronic bronchiolitis in adults.

We describe seven entities which could be comprised under the heading of "chronic Bronchiolitis in adults". In particular clinical, radiological and functional details are described. In a graphic presentation we tried to give a synopsis of morphological and clinical entities under the three morphologic notions bronchiolitis, alveolitis and bronchitis and emphysema.

Prognostic value of chest radiograph, serum-angiotensin-converting enzyme and T helper cell count in blood and in bronchoalveolar lavage of patients with pulmonary sarcoidosis.

We studied the prognostic value of the initial radiologic stage, the serum-angiotensin-converting enzyme (SACE) and T helper cells in blood (OKT-4-Bl) and in bronchoalveolar lavage (OKT-4-BAL) for patients with biopsy-proven pulmonary sarcoidosis. Thirty-seven patients without prior treatment were followed up for a period of 2 years. A BAL was performed in 22 of them as part of the diagnostic workup. Clinical examination, chest radiographs, vital capacity, diffusion capacity for CO, airway resistance and PaO2 at rest and during exercise were determined initially and after 6, 12 and 24 months. According to these results, patients were classified as having progressive or nonprogressive disease. The radiologic stage and the initial SACE (38.3 +/- 10.2 vs. 43.7 +/- 13.0 nmol/ml/min) could not discriminate between the two groups. Patients with progressive disease had significantly fewer OKT-4-Bl cells (403.3/microliter +/- 146.7/microliter vs. 842.0/microliter +/- 430.1/microliter) and more OKT-4-BAL cells (24.5 +/- 15.4% vs. 7.0 +/- 2.6%) than patients with stable disease (p less than 0.01). A negative correlation between OKT-4-Bl and OKT-4-BAL cells was shown (Rs = -0.79; p less than 0.001). We conclude that the number of OKT-4-Bl and OKT-4-BAL cells can be used as prognostic parameter for patients with sarcoidosis.
