The in-vitro effects of medroxyprogesterone acetate on acidic pH induced apoptosis of periferal blood mononuclear cells.

Medroxyprogesterone acetate (MPA) which has a steroid structure, is widely used in oncology practice in the treatment of the cachexia of cancer and to reduce hematologic toxicity in patients receiving chemotherapy. However, the mechanisms of MPA on these effects are unclear. In this study, we investigated the effects of two different doses of MPA (10(-5) and 10(-6) M/L) on acidic pH induced apoptosis of periferal blood mononuclear cells (PBMC) derived from 10 healthy volunteers. Compared with the control group, we found that MPA at the dose of 10(-5) M/L had a negative effect on apoptosis (32.88 +/- 4.61 and 20.7 +/- 1.53% respectively, p < 0.05), a positive effect on cell count of PBMC (1395 +/- 151 x 10(3) and 1100 +/- 139 x 10(3) respectively, p < 0.05) and no effects on cell viability and its proliferation. More comprehensive trials are needed to clarify this effect of MPA.

Variation of the ulnar variance with powerful grip.

Causal relationships between ulnar variance and wrist disorders are known. Gripping and pronation cause proximal translation of the radius with respect to the ulna, leading to a statistically significant increase in ulnar variance. The purpose of this study was to investigate variation of the ulnar variance with powerful grip. A total of 41 male volunteers aged between 19 and 25 years (mean, 21.2+/-1.7 years) were studied. Posteroanterior X-ray films of all wrists were taken in the standardized position. After neutral posteroanterior X-ray films had been taken, subjects were asked to grip a Takei hand dynamometer with maximum force while repeated standardized posteroanterior X-ray films were obtained. Ulnar variance values were measured using the perpendicular method. Mean maximum grip force was 38.1 kg (range, 26.6-47.9 kg). Mean values of force-free (neutral) and forced ulnar variances were 0.06+/-0.21 mm and 1.87+/-0.23 mm, respectively. The difference in ulnar variance between the two groups was statistically significant ( P<0.001). The increase in ulnar variance with grip observed varied between 0.00 mm (minimum) and 3.97 mm (maximum), with a mean of 1.81 mm. Gaining an understanding of normal limits of ulnar variance modification with grip may be helpful in planning surgical treatment.

Plasma lipoprotein (a) concentrations in hypothyroid, euthyroid and hyperthyroid subjects.

OBJECTIVE: Alterations of the lipid profile are a well known phenomenon in thyroid dysfunction. Thyroid hormones regulate lipid metabolism through various mechanisms, but a key role is played by the LDL receptor pathway. Thyroid hormone influence on lipoprotein (a) [Lp(a)] metabolism is known. METHODS AND RESULTS: Therefore we studied Lp(a) concentrations in a group of 16 hypothyroid patients and in a group of 22 hyperthyroid patients. Twenty-six euthyroid subjects were used as a control group. Plasma Lp(a) concentrations in hyperthyroid patients (23.2 +/- 28.1 mg/dl) were significantly lower than those of the hypothyroid patients (27.1 +/- 19.2, p < 0.05). There were negative correlations between plasma Lp(a) concentrations and total T4 levels in patients with hyperthyroidism and hypothyroidism (r: -0.49, p < 0.05; r: -0.40, p < 0.05, respectively). Also, decreased HDL-C levels, increased LDL-C, total cholesterol and apo B levels in the hypothyroid patients according to euthyroid subjects were observed (p < 0.05). Decreased LDL-C levels, increased HDL-C and apo Al levels in the hyperthyroid patients according to euthyroid subjects were determined (p < 0.05). CONCLUSIONS: It was concluded that plasma Lp(a) concentrations increase in hypothyroid patients and the observed relationships between thyroid status and Lp(a) levels can be explained by impaired catabolism of apo B and Lp(a) in hypothyroidism.

Plasma lipoprotein (a) levels in Turkish NIDDM patients with and without vascular diabetic complications.

OBJECTIVE: Plasma concentrations of lipoprotein (a) [Lp(a)], an independent risk factor for atherosclerosis, were measured in 59 non-insulin-dependent diabetes mellitus (NIDDM) patients with and without vascular complications, and 21 non-diabetic healthy subjects. RESULTS: The plasma log Lp(a) levels were found to be significantly increased in the NIDDM patients (1.40 +/- 0.36) compared with the healthy subjects (1.02 +/- 0.53; p < 0.05). Plasma Lp(a) levels in NIDDM patients with diabetic vascular complications (1.51 +/- 0.27) were significantly higher than those of the NIDDM patients without diabetic vascular complications (1.23 +/- 0.43) and healthy subjects (p < 0.05). There were significant correlations between plasma log Lp(a) levels and apolipoprotein B (apo B) in all NIDDM patients (r: 0.68, p < 0.05). No correlation was observed between Lp(a) levels and age, sex, duration of diabetes, fasting blood glucose, haemoglobin Alc, the mode of treatment, triglycerides, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and apolipoprotein Al levels in all patients. CONCLUSIONS: It was concluded that Lp(a) was a risk factor for angiopathy in NIDDM patients and the patients who have a high plasma Lp(a) concentration should be kept under strict glycaemic control.

Smart cards: a specific application in the hospital.

Computers have the ability to process and access tremendous amounts of information in our daily lives. But, now, individuals have this ability by carrying a smart card in their own wallets. These cards provide us the versatility, power, and security of computers. This study begins with a short description of smart cards and their advantages. Then, an electronic circuit that is designed for healthcare application in hospitals is introduced. This circuit functions as a smart card holder identifier, access controller for hospital doors and also can be used as a smart card reader/writer. Design steps of this electronic circuit, operation principles, serial communication with P.C., and the software are examined. Finally a complete access control network for hospital doors that functions with smart cards is discussed.

[Elastase of polymorphonuclear granulocytes. Determination in plasma in pregnancy and following premature rupture of the membranes].

Between May and October 1984 in the Department of Obstetrics of the Frauenklinik Berlin-Neukölln, 147 blood samples were taken from 106 women who were pregnant or already in labor. The purpose was to determine the concentration of the elastase of the polymorphonuclear leukocytes. Towards the end of pregnancy (from the beginning of the 28th gestational week till birth of the child), the range of 66 ng/ml-122 ng/ml with a median of 92 ng/ml was considered normal. It was shown that the concentration of elastase in the plasma of pregnant women is affected neither by the increasing gestational age nor by such pregnancy-associated diseases as the following: fetal hypotrophy, cervix insufficiency, premature onset of labor, or gestational diabetes. In addition, the elastase concentration in pregnant women who had had a premature rupture of the membranes was determined. The values were compared with other clinical and laboratory infection parameters, such as temperature, the total number of leukocytes as well as the number of rod-shaped leukocytes, and the duration of the opening of the membranes. The results indicated that under 135 ng/ml, there is most likely no infection present. Elastase concentrations between 135 ng/ml and 180 ng/ml lie in the pre-pathological range, concentrations above 180 ng/ml nearly always indicate the presence of an infection.

Systematic, combined treatment approach to nasolacrimal duct obstruction in different age groups.

PURPOSE: To report the outcome of a step-by-step treatment approach for congenital nasolacrimal duct obstruction (CNDO). METHODS: Three-hundred and fifty eyes with CNDO were included in the study. A number of treatment methods were applied systematically until a successful outcome was achieved. Listed in order from simple to more complex, the following methods were used: conservative management (massage and topical antibiotics), high-pressure syringing, probing, and silicone intubation. Treatment efficacy was determined according to age (Group 1: 0-6 months, Group 2: 7-12 months, Group 3: 13-24 months, Group 4: 25-72 months) and success rates were compared. RESULTS: Conservative management was applied only in children less than 1 year of age, and was successful in 91.8% of Group 1 and 60% of Group 2 eyes. The difference between these two success rates was significant (p = 0.003). High-pressure syringing was performed in children under 24 months of age, with success rates of 41.7% in Group 1, 33.3% in Group 2, and 12.5% in Group 3. The overall success rate for first probing in all groups was 76.1%, with a range of 69.4% to 80.9%. After second probing, the overall cure rate for the entire cohort was 88.0%, with a range of 74.9% to 94.8%. There was no real difference in probing cure rates relative to age (p > 0.05). Silicone intubation was indicated and performed in two eyes of Group 2 children, three eyes of Group 3, and nine eyes of Group 4. Two ducts in Group 4 eyes remained obstructed after silicone intubation. CONCLUSIONS: The systematic treatment approach to CNDO, including conservative management and minimally invasive procedures such as high-pressure syringing, probing, and silicone intubation, is highly successful. In this study, the cure rate for this combined approach was 100% in youngsters under 2 years of age and 94.5% in children 2 to 6 years old.
