In the article we outlined the basic principles of some recent methods of lung function testing, suitable for the assessment of airway obstruction in children and adolescents. There is also a brief description of various methods, normal values expressed in a form of regression equations, significance of different methods for the assessment of airway obstruction in patients with respiratory diseases and in children from a region with increased levels of air pollution. Among the various functional indices we found the most sensitive for the assessment of airway obstruction maximum expiratory flow rates measured at lower lung volume levels. Airway conductance appeared as the most sensitive method for the evaluation of bronchodilation following inhalation of some drugs. The measurements of airway conductance in a body plethysmography during normal quiet breathing and of maximum expiratory flow rates from flow-volume curves represent the basic methods for the detection of airway obstruction, i.e. in the larger central airways, and in the peripheral airways respectively.

Muscle blood flow was studied in 28 patients after Blalock-Taussig anastomosis using the radioactive xenon clearance method. Insignificant differences in blood flow between the arms were found at rest, but there was a highly significant lowering of the peak blood flow on the operated side. Peak blood flow values in the first years after the anastomosis were close to the values in obliterative artery disease. The difference in maximum blood flow became less apparent beyond the first 5 years after the operation. A prolonged time of onset and duration of hyperemia after ischemic exercise were observed on the side of the operation. Statistically significant differences in circumference and length between the arms were measured and a lower strength of contraction of the affected hand was demonstrated. Abnormally low blood supply to muscles is thought to be responsible for the anatomical changes in the operated extremity.
Using the method 133Xe clearance we investigated blood flow and calculated vascular resistances simultaneously in the muscles of the upper and lower extremities in 58 patients following successful surgical correction of aortic coarctation carried out at age 11.5 (+/- 2.9) years. The interval from operation to investigation was 11.5 (+/- 4.5) years. Resting and maximal ischemic exercise blood flows in the upper extremity were decreased and the duration of maximal blood flow was shortened. Values recorded from the lower extremities did not differ from normal controls. The difference between upper and lower extremities was statistically significant. Vascular resistance during maximal blood flow was higher in the upper extremities than in the lower. Differences between upper and lower extremities did not change after vasodilation elicited by amyl nitrite. The degree of differences was not dependent upon the age at operation, the age of the patients at investigation, or on the time interval between operation and investigation.

Aplasia, hypoplasia and stenosis of the pulmonary artery showed, on scintigraphic investigation, unilateral, homogenous decreased in lung perfusion. In 3 of 4 children with pulmonary sequestration there was a difference in blood flow between the two lungs, or in one part of the lung. Isolated anomalous drainage of pulmonary veins does not produce deviations from the normal distribution of blood flowing from the right ventricle into the lungs. Analysis of scinitigraphic findings in 1450 children with cardiopulmonary disease has shown that with an otherwise unexplained marked homogeneous unilateral decrease in flow, we can with great certainty diagnose a developmental defect of the pulmonary vessels. Differences in lung area, judged from the scintigram, are a sensitive indicator of associated disturbances of development of the lung parenchyma. In 26 of 42 such developmental anomalies of the pulmonary vascular bed this finding was demonstrated. Pulmonary scintigraphy is a very valuable complementary method, because of its simplicity and relative safety for the patient, in the diagnosis of congenital anomalies of the pulmonary vasculature, especially in infants and young children.

Radioisotope lung scanning was used to investigate the distribution of pulmonary blood flow after banding of the pulmonary artery in children with a left-to-right shunt and pulmonary hypertension. An abnormal distribution of blood flow in the lung on the side of the operation approach was observed in all patients in the first three weeks following surgery. Abnormalities were still observed in 17 of 21 children 10 months to more than 8 years after the banding operation. There was no significant relation between the occurrence of these abnormalities and time after surgery. Diminished flow to the zones of the right lung was observed less frequently. The incidence of abnormalities in flow distribution was also high preoperatively. Respiratory complications in infants with large left-to-right shunts were considered to be responsible for most of the abnormal blood flow distributions observed. Radioactive lung scanning was found to be a valuable diagnostic method in the early and late postoperative period in infants and small children. It was more sensitive than the other techniques used in revealing deviation of blood flow from one lung in those cases with shifting of the applied band.
