In 28 patients with cystic fibrosis aged 5--24 years the values of maximum expiratory flow rates (Vmax) at lower volume levels were assessed, as well as the "specific" conductance of the respiratory pathways (Gaw/TGV), the vital capacity (VC), total lung capacity (TLC), residual volume (RV), functional residual capacity (FRC), RV/TLC and FRC/TLC ratios and the one-second forced expiration of the vital capacity (FEV1) to evaluate obstruction of the respiratory pathways. Most markedly and most frequently abnormal were the values of Vmax, RV and of the RV/TLC ratio. These findings revealed that in almost all patients with CF there was already during the initial examination an obstruction of the peripheral respiratory pathways and hyperinflation of the lungs. During repeated measurement of the above values of lung function in 15 patients with cystic fibrosis during a period of 1--5 years when the patients increased in height by 10 cm on average, the above values did not deteriorate on average. There was, however, an individual variability of the investigated values during this period. The comprehensive treatment provided in our country prevented in patients with CF a deterioration of obstruction of the respiratory pathways during the period of investigation.
Two cases of the hypoplastic left heart syndrome with premature closure of the foramen ovale and levoatriocardinal veins are described and the literature is reviewed. One of the patients is the only case with both mitral and aortic valve atresia. The other is associated with the asplenia syndrome, double-outlet right ventricle without ventricular septal defect, and an anomalous connection of the left atrium to the right auricle.

Anomalies in the course and drainage of systemic veins, in particular the inferior vena cava, markedly complicate or even prevent cardiac catheterization and some surgical procedures. Such anomalies have been observed in 32 out of 1,266 children subjected to catheterization and angiocardiography, i.e. 2.5 per cent. Severe anomalies of a superior vena cava were observed in six children, one of them having an associated anomaly of an inferior vena cava. Anomalies of the inferior vena cava were observed in 27 children. The latter included persistent paired development of the inferior vena cava, aplasia of suprarenal portion with azygos or hemiazygos continuation, its persistence at the side of the cardiac apex and combination of these findings. In 19 of the children with an anomaly of the inferior vena cava there was a visceral situs indeterminatus associated, often with severe congenital heart disease and with a large intracardiac shunt (e.g. transposition of the great arteries and stenosis or atresia of the pulmonary artery). Suspicious of such combinations of visceral situs indeterminatus with anomalies of the inferior vena cava and severe congenital heart disease can be suspected on plain chest films, but the diagnosis can be definitely determined only by angiocardiography.
Resting and maximal values of blood flow through limb muscles are higher in children than in adults; on the contrary, the time necessary for reaching maximal blood flow and the latter's duration are shorter. Simultaneous measurements in both upper or in both lower limbs, and in the right upper and right lower limbs, revealed no statistically significant differences among the values.
