[Diode lasers in ophthalmic surgery].

The paper outlines current aspects of the use of diode lasers in ophthalmology. The Ufa Research Institute of Eye Diseases is the first in Russia to develop and use endoscopic cyclic laser coagulation and laser endoscopic antegrade dacryocystorhinostomy. The introduction of new laser microendoscopic technologies into practical ophthalmology has increased the efficiency of ophthalmological care in the treatment of patients with non-compensated glaucoma and lacrimal duct diseases.

[Vitrectomy in traumatic cataracts and in the complications of surgery in congenital cataract in children].

Vitrectomies were carried out in 35 children with traumatic cataracts and complications of surgery for cataracts, caused by injury to the posterior lenticular capsule and incorporation of its fragments to the vitreous. Complete removal of lenticular rudiments rapidly eliminated phacogenic iridocyclitis and improved visual acuity. Improvement of visual functions was attained in 66.6% cases; in 33.4% cases visual acuity did not change. Hemorrhages to the vitreous cavity occurred in 4 cases with pronounced iridocyclitis; therefore, a corneal approach is preferable for cases with pronounced iridocyclitis.

[Endoscopic laser coagulation of ciliary processes in patients with severe uncompensated glaucoma].

Endoscopic laser coagulation was used in 85 patients with severe uncompensated glaucoma. A manifest hypotensive effect (decrease of intraocular pressure from 44.21 +/- 3.17 to 23.12 +/- 4.58 mm Hg 1 year after the operation) and analgesic effect were attained. As a result of treatment, intraocular pressure normalized in 88.2% patients and the eyeball was preserved in 98.8% patients subjected to surgery.

[Immunological status and its changes in children during surgery on anterior eye segment].

Nonspecific and specific immunity values were studied in 70 children before and in various periods after 74 operations on the anterior segment of the eyeball. In children with persistent autosensitization to ocular tissue autoantigens (alpha-crystalline and S-antigen) postoperative uveitis presented as an immunocomplex disease with signs of exhaustion of the functional potential of the phagocytosis and complement systems. In children without autosensitization uveitis was associated with immunity activation. Phagocytosis system and cooperation of lymphocyte subpopulations, including normal killers (CD16+), play an important role in prevention of sensitization to ocular tissue autoantigens. Therefore, complex immunological examinations of children before surgery and regular check-ups after it will help differentiate immunodeficiency from compensatory immunosuppression preventing autoimmune disease, which is important in prescription of immunostimulating therapy.

[High performance liquid chromatography identification of chloracetophenone in ocular tissues in burns induced by gas balloons (an experimental study)].

The kinetics of chloracetophenone (CN) in rabbit eyes was studied by high-performance liquid chromatography on a model of experimental third-degree burn. By the end of the first hour after exposure to the lacrimate, CN content was high in all the studied tissues: 28.5 +/- 0.563 x 10(-1) mg/kg in the cornea, 5.08 +/- 0.193 x 10(-1) mg/kg in the anterior chamber humor, and 3.26 +/- 0.123 x 10(-1) mg/kg in the vitreous. After 6 h the content of the irritant dropped almost threefold and was 8.5 +/- 0.403 x 10(-1) mg/kg in the cornea, 1.23 +/- 0.062 x 10(-1) mg/kg in the anterior chamber humor, and 0.017 +/- 0.006 x 10(-1) mg/kg in the vitreous. By the end of 24 h these values were 6.6 +/- 0.221, 1.46 +/- 0.123, and 0.015 +/- 0.005 x 10(-1) mg/kg, respectively, and by day 14 only trace amounts of CN were detected. Hence, CN in the Cheremukha gas balloons can penetrate into the inner structures of the eye and cause severe injuries. Persistence of CN for up to 14 days disordered the metabolic processes and anatomy of the eye. High content of CN during the first 7 days after burn requires adequate pathogenetic therapy.

[Ultraviolet irradiation of blood in combined treatment of traumatic endophthalmitis].

Thirty-five patients (35 eyes) with traumatic endophthalmitis were treated. Ultraviolet exposure of autoblood was used in 16 patients, the rest 19 were treated routinely (antibiotics, etc.). Use of ultraviolet exposure of the blood in combined therapy of traumatic endophthalmitis more rapidly (12.6 vs. 22.1 days) and effectively (93.7 vs. 68.4%) arrested intraocular infection and more often preserved the objective vision (31.3 vs. 10.5%).

[Treatment of chlamydial conjunctivitis].

A new method for treating chlamydial conjunctivitis consists in injections of ciprofloxacine for intravenous injections, which is injected (1 ml) under the inferior vault of the conjunctiva on novocaine pad (0.5 ml 0.5% solution) and intravenously by drip infusion (99 ml) twice daily for 7-10 days. The treatment led to clinical cure in all patients, no relapses were observed. This method ensures a high therapeutic effect and accelerates the recovery of patients with chlamydial conjunctivitis by 4.6 days. The method is recommended for patients with complicated forms of chlamydial conjunctivitis in severe and protracted cases with concomitant urogenital chlamydial infection and concomitant chronic gastrointestinal diseases, when oral antibiotic therapy is undesirable or precluded.

[Study of the effectiveness of ultraviolet irradiation of blood in the treatment of traumatic uveitis].

Sixty-five patients (65 eyes) with traumatic uveitis were treated. Ultraviolet irradiation of autoblood was included in therapeutic complexes of 28 patients. 37 patients received traditional therapy (corticosteroids, nonsteroid inflammatory agents, etc.). Addition of UV exposure of autoblood to combined therapy for traumatic uveitis more effectively (92.9 vs. 75.7%) and sooner liquidated posttraumatic inflammatory reaction (8.10 +/- 1.5 vs. 12.7 +/- 1.7 days), decreased the hospital stay (11.0 +/- 2.0 vs. 15.8 +/- 1.3 days), and eventually more often improved the visual acuity (in 42.9 vs. 24.3% patients). Hence, UV exposure of autoblood is an effective, safe, and virtually atraumatic method of treatment.

[Ultrasonic tomography of neurogenic tumors of the orbit].

The results of a comprehensive ultrasonic examination of 12 patients with tumors of the eye orbit are described. A low echogenicity as well as heterogeneity of the echo-structure and of the contours' sharpness are the main sonographic features of the mentioned tumors. An examination of a vascularization degree in patients with gliomas of the optic nerve showed no blood circulation inside tumors; while a collateral blood circulation was detected in patients with meningiomas of the optic nerve. A compression of the ocular artery by the tumor and reduced hemodynamic indices were found to be typical. Finally, a triplex examination of the orbit makes it possible to diagnose tumors of the optic nerve.

[Experimental study of the effectiveness of a silicone protector designed for the vitreous body and retina in phacoemulsification].

A phacoemulsification technique is suggested that involves the use of a silicone protector. It was proven experimentally that a temporary implantation of a silicone plate, i.e. protector, into the retronuclear space, while performing the lens nuclear phacoemulsification, is an effective means of protecting the retina and vitreous body from a negative ultrasound impact.

[Anti-inflammatory activity of hyaluronic acid].

The results of in vivo experiments showed that hyaluronic acid possesses antiexudative activity comparable with that of nonsteroidal anti-inflammatory drugs. Hyaluronic acid exhibits antagonism with respect to histamine-induced inflammation.
