Increased urinary nitrate excretion in inflammatory bowel disease.

Urinary nitrate is known to be a measure of environmental exposure. Only recently an endogenous metabolic pathway resulting in nitrate synthesis was described. We have determined the concentration of nitrate expressed as nitrate/creatinine ratio in the urine of patients in whom a significant environmental nitrate load was presumed to be absent. A significant elevation of nitrates in patients with inflammatory bowel disease compared with patients with non-inflammatory disorders was observed (0.173 +/- 0.155 vs. 0.037 +/- 0.016 mol/mol creatinine, P < 0.01). Increased nitrate excretion in the urine of patients with inflammatory bowel disease may reflect the endogenous nitrate synthesis induced by the immune activation.

Increased urinary zinc and copper excretion in colorectal cancer.

Although several studies have described an increase in urinary zinc output in cancer patients, little attention has been paid to the excretion of copper. In this study, we investigated urinary excretion of zinc and copper, and serum levels of alpha-1 acid glycoprotein, an acute phase protein, in 22 patients with colorectal cancer. Urinary excretion of copper and zinc were significantly increased compared to the reference group (77 +/- 72 vs. 32 +/- 14 mumol/mol creatinine, P < 0.05, and 1.50 +/- 0.62 vs. 0.70 +/- 0.41, P < 0.002, respectively). In addition, a negative correlation was observed between urinary copper and serum alpha-1 acid glycoprotein (rs = -0.5256, P < 0.02). In conclusion, urinary excretion of copper, as well as that of zinc, appears to be elevated in colorectal cancer. The finding of a negative correlation between urinary copper and alpha-1 acid glycoprotein supports the postulated role of the latter in regulating renal glomerular permselectivity.

[Actinomyces viscosus in subdural empyema].

The authors describe the finding of Actinomyces viscosus in pus from a subdural empyema in a child patient. When searching for the probable primary source of this uncommon infection of the central nervous system the authors identified by cultivation the same bacterial strain in a periapical dental granuloma. Bacterial diagnosis and radical surgical and antimicrobial therapy led to normalization of a clinical condition with an initially unfavourable prognosis.

Effects of essential phospholipids on the amino acid metabolism in whole body irradiated rats.

Ionizing radiation induces injury to cell membranes resulting in an increase of plasma amino acids concentration and catabolic response of the body. Essential phospholipids (EPL) administered before radiation protect the cell structures and can be used for their renewal. For this reason EPL are expected to have some effect on amino acid metabolism in radiation sickness. The adult male Wistar rats were injected with EPL in a dose of 125 mg EPL/kg b.w. (Essentiale) or saline solution. Three h later whole-body gamma irradiation with a single dose of 10 Gy was performed. Untreated pair-fed rats served as control. The rats were sacrificed 2 days after irradiation. In irradiated rats a decrease in body weight, increased concentrations of plasma amino acids, higher oxidation of branched chain keto acids in liver mitochondria, higher incorporation of leucine into liver proteins and decreased leucine incorporation into proteins of small intestine were observed. In EPL pretreated rats a longer survival time, a higher body weight, lower levels of plasma amino acids and higher protein synthesis in small intestine and in liver when compared with irradiated, saline treated rats were found. No significant changes were observed in branched chain amino acid oxidation measured by means of labeled ketoisocaproate in liver mitochondria.

[Myocyte transformation in autologous transplantation of colonic smooth muscle in domestic pigs].

Autologous gut smooth muscle graft of two piglets was used for creating a ring in sigmoidostomy. The graft was investigated several times by histology and electron microscopy. A sort of myofibroblasts were observed the 7th day in the smooth muscle. Three months later the smooth muscle was completely transformed into a cellular connective tissue comprising myofibroblasts and fibroblasts. The experiment points to possible relation of smooth muscle cell, myofibroblast and fibroblast.

[Thrombocytes as an indicator of activity in Crohn's disease treated with total parenteral and/or enteral nutrition].

UNLABELLED: Inflammatory bowel disease can be associated with increased platelet count. Aim of our study was focused on assessing thrombocyte count in evaluation of the activity in Crohn's disease (CD). The study included a total of 19 patients [6 men, 13 women (aged 18-57 years, mean 33, median 28]. All patients were malnourished in active stage of the disease on total parenteral and/or enteral nutrition (TP/EN). Thrombocytes were estimated at the beginning and after 3 weeks of TP/EN in venous blood sample using automatic analyser Coulter Counter JT3 (normal range: 130-380.10(9)/l). Serum prealbumin, transferrin, haptoglobin, alpha 1-orosomucoid and C-reactive protein were measured simultaneously. Thrombocytes count below 400.10(9)/l was found only in 1/19 at the beginning and in 7/19 patients in the end of the study. After 3 weeks of TP/EN a significant platelet count fall was found (mean +/- sd: 472 +/- 98 vs. 354 +/- 110, p < 0.01). A correlation was found between thrombocytes and serum prealbumin (r = -0.3871, p < 0.05), haptoglobin (r = 0.5247, p < 0.001), alpha 1-orosomucoid (r = 0.4857, p < 0.01) a C-reactive protein (r = 0.4354, p < 0.01). CONCLUSION: Thrombocyte count helped in the assessing of disease activity in CD on 3-week-TP/EN. Improvement of clinical status, increase of nutritional parameters and decrease of positive acute phase reactants were associated with a fall of platelet count in 15/19 patients.

Two-dimensional gel electrophoresis of four serum samples from patients with IgD myeloma.

Sera containing IgD paraprotein present problems in identifying patients with monoclonal gammopathies because only a small or even no spike may be present on standard serum protein electrophoresis. We have detected heavy chains of IgD monoclonal protein by means of high resolution two-dimensional gel electrophoresis. Besides clearly identifying delta heavy chains in maps of serum proteins, we also found size and charge heterogeneity of monoclonal immunoglobulins. The results demonstrate the usefulness of two-dimensional gel electrophoresis in the analysis of selected cases of immunoglobulin malignancies.

[The first selective coronarographies in the former Czecho-Slovakia].

The study observes the history and the beginning of selective coronarographies performed in the territory of the Slovak Republic. The first aortocoronary bypass in Slovakia was performed in Bratislava. It was realized by the team of the Surgical Clinic of Academician Dérer in november 1969, namely on the basis of coronarographic examinations which were performed by Smrecanský, Cagán and Krajcovic. Coronographies have been performed in the Central X-ray Department of the Hospital and Clinic of Academician Dérer since the end of the year 1968. During the period of december 1968 to june 1970 selective coronographies were realized in 70 patients and they represented the first performances of this method in former Czechoslovakia. (Ref. 8.)

Immunoglobulin G subclasses in Crohn's disease with and without bowel resection.

Increased immunoglobulin G2 [IgG2] in Crohn's disease in contrast with increased IgG1 in ulcerative colitis was reported in literature data. The aim of our study has been focused on serum IgG subclasses in Crohn's disease with resected bowel (11 patients) and without previous surgery for Crohn's disease (12 patients). All patients were in active stage of the disease on total parenteral nutrition. Serum IgG subclasses were estimated by means of radial immunodiffusion. There was no significant differences between in IgG subclasses in Crohn's disease with and without bowel resection. IgG2 over 4.5 g/l was found only in 2/12 with resected bowel and 1/11 without previous surgery for Crohn's disease. Surprisingly, IgG1/IgG2 ratio below 1.5 was found only in 5/23 persons. In 3/23 patients IgG1 was over 10.5 g/l although there was no doubt about the diagnosis of Crohn's disease. IgG subclasses failed to help in confirmation of the diagnosis of Crohn's disease. Serum IgG subclasses alterations probably are not influenced by bowel resection and/or nutritional status.

Neopterin in the diagnosis of disorders associated with immune system activation.

While the knowledge concerning the role of the immune system in many internal disorders has grown rapidly in recent years, there are few methods to assess immune system activation in clinical practice. Measurement of urine neopterin, product of a metabolic pathway controlled by interferon-gamma, has been found useful in many clinical conditions. The present study concerns neopterin excretion in 157 patients with different internal disorders. As expected, we found an increase in urine neopterin in patients with malignant tumors, autoimmune disorders like systemic lupus erythematosus or inflammatory bowel disease, and infections. Elevated neopterin levels were also observed in acute pancreatitis and in acute myocardial infarction. In addition, significant correlations between urine neopterin and zinc and neopterin and copper excretion were found suggesting a physiological role of neopterin as urine antioxidant.

IgD paraproteinemias.

In a series of 2038 paraproteinemic sera, 9 IgD paraproteins (0.44%) were found. Clinical diagnosis of all the 9 was multiple myeloma. One patient had an IgD-lambda plasmacytoma which developed into plasmocellular leukemia. Our series of IgD paraproteins consisted of 6 females and 3 males with a mean age of 58.5 years. The mean concentrations of IgD paraproteins were 6.7 +/- 4.26 g/l and the mean proteinuria was 5.1 +/- 4.76 g/24 h. Eight IgD paraproteins had light chains lambda and one had light chains kappa.

Urinary zinc excretion in patients with different disorders: the acute phase response in the kidney.

While depressed circulating zinc levels constitute a well-characterized part of the acute phase response, relatively little attention has been paid to the changes in urinary zinc excretion, although urine zinc output has been reported to be elevated in various disorders, mainly those known to be accompanied by inflammatory phenomena. In order to assess the significance of urinary zinc loss and its relationship with the acute phase response, zinc concentration was determined by atomic absorption spectrophotometry together with creatinine in urine samples of patients with different disorders. Plasma zinc, C-reactive protein, alpha-1 acid glycoprotein, haptoglobin, prealbumin, transferrin, and iron have also been determined in some patients. In accordance with the results of previous studies, we report an increase in urinary zinc, notably in solid tumors, hematologic malignancies, autoimmune rheumatic disorders, bacterial infections, diabetes mellitus and nephropathy. We also found a significant positive correlation between urinary zinc and acute phase proteins alpha-1 acid glycoprotein (rs = 0.4649, P < 0.005), haptoglobin (rs = 0.4688, P < 0.005) and C-reactive protein (rs = 0.3636, P < 0.025), as well as a negative correlation with plasma zinc (rs = 0.3640, P < 0.025). As the role of lipid peroxidation in renal tubular cell injury has been proposed, the increased urinary levels of zinc, which has antioxidant properties, may be an important protecting mechanism, representing a part of the acute phase response in the kidney.

[History of the Surgery Clinic at the Slovak University Medical School in Bratislava during the war years (1941-1945)].

The aim of the present study was to contribute to a good and objective understanding of the development of the Department of Surgery of the Medical School in Bratislava over the war years of 1941-1945. After Prof. MUDr. Kostlivý had left Slovakia, his student, Prof. MUDr. Konstantín Cársky was appointed head of the Clinic of Surgery, as it was officially named. Already during the period of World War II, Prof. Cársky became a remarkable and reknown personality of the School of Medicine of the Slovak University in Bratislava. The first Slovak school of surgery developed under his guidance. On the basis of archival sources and documents, the paper presents statistical data concerning some aspects of the pedagogical activities of the Clinic of Surgery, its staffing, and the complicated process of competence growth, particularly appointments to the highest academic ranks.
