[Detection of pulmonary tuberculosis. Special characteristics of patients detected while receiving treatment for other diseases].

Tuberculosis (TB) detection is mainly classified as being either passive case-finding, at a hospital, or positive case-finding through a mass screening. Pulmonary TB patients detected while being examined for other complaints at a hospital are classified as passive findings. It is suspected that these patients have special characteristics which separate them from patients that are detected only after the onset of TB symptoms. The aim of this study is to elucidate those special characteristics of patients where detection occurred while being examined for other diseases by comparing them with patients detected by the other means. A survey was conducted on 686 pulmonary TB patients, diagnosed between 1986 and 1988, from the area served by the Kochi Prefectural Chuo Health Center, with the following results. 1) Among the 533 patients who were detected at a hospital, and excluding those detected by mass screening, 331 patients were detected only after the onset of TB symptoms (group B), with 202 patients being treated for other diseases before being diagnosed as having TB (classified as group A). In group A, 130 patients were detected because of manifesting additional symptoms related to tuberculosis such as fever and cough (group A-1), while the remaining 72 patients were detected by chance while being examined for other diseases (group A-2). 2) The number of elderly people and relapse cases was higher in group A compared to B group and the group detected by mass screening. 3) Symptoms, the rate of tubercle bacillus positives, and the period from the onset of symptoms to diagnosis (defined delay) in group A-1, were similar to group B. In group A-2, symptoms and the rate of tubercle bacillus positives were less than those of the other groups. 4) From the above findings, it is concluded that passive case-findings should be classified into 3 distinct groups.

Terpenoid biotransformation in mammals. V. Metabolism of (+)-citronellal, (+-)-7-hydroxycitronellal, citral, (-)-perillaldehyde, (-)-myrtenal, cuminaldehyde, thujone, and (+-)-carvone in rabbits.

1. The metabolism of (+)-citronellal, (+-)-7-hydroxycitronellal, citral, (-)-perillaldehyde, (-)-myrtenal, cuminaldehyde, thujone, and (+-)-carvone was studied in rabbits. 2. In (+-)-7-hydroxycitronellal, (-)-perillaldehyde, (-)-myrtenal and cuminaldehydes, both primary alcohols and carboxylic acids were formed. 3. In (-)-citronellal and citral, regioselective oxidation was found and a trans-positioned methyl group was carboxylated in each case. 4. In o-cuminaldehyde, reduction but not oxidation, was found.

Acyclic Monoterpenes↗

Biotransformation of 1,4-cineole, a monoterpene ether.

1. The metabolism of 1,4-cineole, a monoterpene ether, was studied in the rabbit. 2. Four neutral and one acidic metabolites were isolated from the urine and shown to be 9-hydroxy-1,4-cineole, 3,8-dihydroxy-1,4-cineole, 8,9-dihydroxy-1,4-cineole, 1,4-cineole-8-en-9-ol and 1,4-cineole-9-carboxylic acid.

Terpenoid biotransformation in mammals. IV Biotransformation of (+)-longifolene, (-)-caryophyllene, (-)-caryophyllene oxide, (-)-cyclocolorenone, (+)-nootkatone, (-)-elemol, (-)-abietic acid and (+)-dehydroabietic acid in rabbits.

The metabolism of (+)-longifolene, (-)-caryophyllene, (-)-caryophyllene oxide, (-)-cyclocolorenone, (+)-nootkatone, (-)-elemol, (-)-abietic acid and (+)-dehydroabietic acid was studied in rabbits. Each of these sesquiterpenoids was converted to primary, secondary or tertiary alcohols, among which the primary alcohol was predominant. A vinylic methyl group and an exomethylene group were easily hydroxylated and converted to a glycol via an epoxide in many cases. Eight new metabolites were determined by chemical and spectroscopic methods.

Effects of morphine and ketamine anaesthesia and surgery on plasma concentrations of luteinizing hormone, testosterone and cortisol in man.

Thirty patients received morphine and 27 received ketamine. Morphine anaesthesia did not influence plasma concentrations of luteinizing hormone (LH) significantly, but there was a significant increase during ketamine anaesthesia. Plasma LH increased in the early stage of surgery under both anaesthetics. A significant decrease in the plasma concentration of testosterone was found during and following both techniques of anaesthesia, the lowest concentration being detected on the first day after surgery. Ketamine anaesthesia, but not morphine, stimulated adrenocortical function significantly. Furhter stimulation was detected in the periods during and after operation with both anaesthetic techniques.

Isolation of the Epstein-Barr virus in scirrhous gastric cancer by efficiency-monitored representational difference analysis.

We applied an improved representational difference analysis (RDA), efficiency-monitored RDA (EM-RDA), to analyze the genetic alterations of scirrhous gastric cancer. Clinical samples of this cancer are difficult to analyze genetically because of the large number of normal cells in the stroma. The entire 3rd difference product obtained from EM-RDA was subcloned. This is equivalent to the scirrhous gastric cancer DNA with the normal gastric tissue DNA subtracted from it. The difference clones, which were confirmed by Southern blot as being derived from the cancer, were sequenced. As a result, two difference clones were identified as DNA fragments of the Epstein-Barr virus (EBV). The existence of EBV in the scirrhous gastric cancer cells was proved by EBV-encoded small RNA 1 in situ hybridization. EM-RDA made it possible to detect the first case of EBV-related gastric cancer in a young woman.
