[Combination chemotherapy of cisplatin, vincristine and peplomycin for patients with head and neck carcinoma].

The effectiveness of combined CDDP chemotherapy with other anticancer drugs in patients with head and neck carcinoma was investigated. The response rate for COP therapy (CDDP, Vincristine, Peplomycin) was better than those for CDDP administration only or CP (CDDP Peplomycin) therapy for carcinomas of the head and neck. Forty-seven cases treated with COP therapy, were analyzed in detail. (1) Non-treated fresh cases showed a better response than recurrent cases. In other words, the induction chemotherapy is useful. (2) The grade of differentiation of squamous cell carcinoma had no correlation with the response. (3) No relationship between the primary site and the respose rate was detected. (4) Nephrotoxicity was considered to be the dose-limiting factor of CDDP therapy. COP therapy did not show any remarkable side effects. COP chemotherapy is therefore concluded to be effective for the treatment of squamous cell carcinomas of the head and neck.

[The alternation of peripheral lymphocyte subsets in head and neck carcinoma].

Peripheral lymphocyte subsets of 115 patients with head and neck carcinomas were examined by the monoclonal antibody technique. In fresh tumor-bearing, the OKT 4 rate was decreased (P less than 0.01) and the OKT 8 rate was increased (P less than 0.05). Consequently the OKT4/OKT8 ratio was decreased (P less than 0.01) compared with normal healthy individuals. This result shows the abnormality of cellular immunity in carcinoma cases. The OKT4/OKT8 ratio reflected the clinical stages and courses sufficiently and correlated with other immunological parameters. The OKT4/OKT8 ratio is considered to be one of the parameters for elucidating the clinical conditions and immunities of carcinoma patients.

[Immunotherapy, with OK-432 for cases of nasopharyngeal carcinoma].

The prognosis of nasopharyngeal carcinoma (NPC) is very poor, due to the extreme failure of the immuno-surveillance mechanism in such cases. As an immunotherapeutic agent, OK-432 was administered to NPC patients. Cases were divided into two groups. One was given OK-432 for less than 6 months (short-term group), while a long-term group, was treated with OK-432 for over 6 months. These two groups were evaluated for immunological activity and prognosis. As immunological parameters, the numbers of white blood cells and lymphocytes were counted, and delayed skin reactivities with Su-PS and PPD were determined. No influence on the number of white blood cells and lymphocytes could be observed in either of the two groups. However, the skin tests showed better improvement of immunity in the long-term group than in the short-term cases. Furthermore, these reactivities correlated well with the clinical status of NPC patients. As to the absolute number of lymphocytes, improvement in the long-term NPC group was delayed in comparison to that of another head and neck carcinoma group given long-term OK-432 treatment. The prognosis of the long-term administered group was apparently better than that of the short-term group. Long-term administration of OK-432 is therefore indispensable for the treatment of NPC cases, because of the extreme decline of their immunity.

[The clinical value of long-term administration of OK-432 in head and neck cancer].

In order to evaluate the clinical effect of longer-term administration of OK-432, 72 cases of head and neck squamous cell carcinoma with stage III and IV were divided into two groups: a shorter-term group (25 cases), given OK-432 for less than 6 months mainly during the period of admission, a longer-term group (47 cases), given OK-432 for more than 6 months not only for the period of admission but also during ambulation. The immunological parameters and the cumulative survival rates were compared between the two groups. In the longer-term group, the absolute number of peripheral lymphocytes was significantly higher and the positive rate of skin reaction with PPD was higher than in the shorter-term group. However there was no difference in the positive rate of skin reaction with Su-PS between the two groups. OKT4/OKT 8 ratio and PHA blastogenic rate were higher, although not to a statistically significant extent, in the longer-term group. The cumulative survival rate was significantly higher up to 12 months (p less than 0.01) and 18 months (p less than 0.05) in the longer-term group than the shorter-term group. These results of the immunological parameters and the cumulative survival rates suggest the clinical efficacy of longer-term administration of OK-432.

Immunological basis and immunotherapy of nasopharyngeal carcinoma.

Clinically, the prognosis of nasopharyngeal carcinoma is very poor. It is considered that this depends on three factors. The first is the difficulty of early detection of this disease, because the symptoms of this cancer are latent. The second factor is the specificity of the histological character. The third is most important. This cancer cases fall into the extreme failure category of the immuno-surveillance mechanism. Immuno-responsiveness is extremely depressed. Immunological status has been examined and the clinical evaluation of immunotherapy with OK-432 (streptococcal preparation) and lymphocyte transfer has been made. Results are as follows: 1) Deficiencies of cellular immunity could be recognized through the various immunological parameters, such as subsets of peripheral blood lymphocytes. 2) Immunotherapy is indispensable in this cancer, for the above-mentioned reasons. Better results could be obtained with nonspecific immunotherapy, using OK-432. On the other hand, new immunotherapy, using immunologically enforced lymphocytes with Interleukin-2 prevented micrometastasis, one of the worst characteristics of this cancer. Nasopharyngeal carcinoma is a systemic disease. Therefore immunotherapy is indispensable for the treatment of this cancer.

[Evaluation of aclacinomycin-A in head and neck cancer].

Clinical effects of aclacinomycin-A (ACM) were evaluated in 30 patients with head and neck carcinoma. ACM was continuously administered intravenously at a dose of 20 mg/body for 4 days. After 2 weeks cessation the same courses were repeated. The maximum cumulative dose was 400 mg. Overall objective response rate was 53.3% for head and neck carcinoma, but, the effective rates were different according to histopathological classification of carcinoma. The most responsive tumor was adenocarcinoma. With squamous cell carcinoma the response rate was correlated with the degree of differentiation. Namely, 88.9% of anaplastic squamous cell carcinoma cases responded while the effective rate was only 14.3% for well and moderately differentiated types. In the evaluation of side effects, myelosuppresive toxicity, one of the dose-limiting factors in ACN trials, was slight using in this administration method. Also, any changes in electrocardiograms, occurring during ACM administration, were reversible and negligible. Manifestations of side effects with systemic ACM administration appeared to be less pronounced than those of adriamycin.

Lymphoproliferative response to membrane extracts of nasopharyngeal carcinoma.

The tumor-associated antigen (TAA) was extracted from cultured cells obtained from nasopharyngeal carcinoma (NPC), and the specific immune reaction was studied by utilizing the proliferation reaction of lymphocytes from NPC patients. The following results were obtained. (1) The stimulation of lymphocytes by membrane soluble antigen (MS-Ag) was significantly different in the group of patients with NPC and the control groups (patients with malignant tumors of the head and neck and healthy individuals). Lymphocytes from the NPC patients gave a significantly higher stimulation index (SI) value with NPC MS-Ag, but a lower value with PHA, as compared with those of normal controls. (2) No definite correlation was observed in anti-Epstein-Barr virus (EBV) antibody titers. (3) As for the correlation with various histological types, apparently enhanced reaction was noted irrespective of the histological type of the cancer. (4) In connection with stage classification, the reaction was always negative in patients in stage IV. A particularly close correlation was noted between the degree of the response and the N classification. Some inhibitory factor affecting the LP reaction might appear with the progress of N and M.

Benign and malignant oncocytoma of the salivary glands with an immunohistochemical evaluation of Ki-67.

We present two cases of benign oncocytoma derived from the parotid gland and minor salivary gland and one case of malignant oncocytoma from the parotid gland. The proliferative activity of the tumor cells was evaluated immunohistochemically for Ki-67. The average frequency of Ki-67-positive cells was 3.3% in the benign oncocytomas and 6.5% in the malignant oncocytoma. The higher frequency of Ki-67-positive cells in the malignant oncocytoma might reflect active cell proliferation. Ki-67 immunostaining may be useful in distinguishing a benign oncocytoma from a malignant oncocytoma.

Clinical efficacy of recombinant human granulocyte colony-stimulating factor in patients with head and neck cancer.

Recombinant human granulocyte colony-stimulating factor (rG-CSF) was used for the amelioration of neutropenia caused by cisplatin-based chemotherapy in head and neck cancer patients. For a dose-finding study of rG-CSF, 11 patients, who had levels of absolute neutrophil counts below 500/mm3 in peripheral blood, were administered rG-CSF subcutaneously at a dose of 1 or 2 micrograms/kg for 5 days. A 'useful' rate in 1 and 2 micrograms/kg was obtained in 71.4 and 100% of patients, respectively. In 6 patients, the clinical utility of rG-CSF at a dosage of 2 micrograms/kg from the day after chemotherapy for 10 consecutive days was observed. The percentage of 'very useful' evaluations was 83.3% (5/6). No side effects or abnormal laboratory findings were observed in any patients after administration of rG-CSF.

A case report of Sjögren's syndrome with repeated false cord swelling.

We present a 42-year-old female patient who had repeated swelling of the bilateral false cords. The patient complained of dryness of the eyes and oral mucosa, and was diagnosed as having Sjögren's syndrome. She underwent partial resections of the bilateral false cords 4 times in 3 years to evaluate the nature of the disease and to relieve airway obstruction. Histologic examinations revealed severe atrophic changes of the laryngeal glands and cystic dilation of the ducts of the glands, associated with marked lymphocytic infiltration and lymphoid hyperplasia. These findings suggest that the etiology of the swelling of the false cords was related to Sjögren's syndrome.

Secretion of a fibronectin-like substance from head and neck carcinomas.

The secretion of a fibronectin-like substance in culture supernatants of cell lines of head and neck squamous cell carcinomas was observed. Culture supernatants of some tumor cells had a high cell-attachment activity. Enzyme-linked immunosorbent assay with an antifibronectin antibody showed that the supernatant with cell attachment activity contained fibronectin. Furthermore, the cell attachment activity was clearly inhibited by the addition of a monoclonal antibody to fibronectin. These findings strongly suggest that the fibronectin produced by cancer cells is one of the potent mediators of the cell attachment.

Sinusitis associated with disorders of visual acuity.

We report a case of sinusitis associated with disorders of visual acuity. The patient's vision was dramatically improved by sinus surgery despite the fact that the changes in the ethmoidal cells were slight and had no direct observable relation to the optic nerve canal. Possible mechanisms involved in nasal optic neuritis are discussed.

Hypopituitarism and hyponatremia in a case with nasopharyngeal carcinoma.

We report a case of nasopharyngeal carcinoma (NPC) accompanied by hyponatremia and polyuria which was induced by tumor invasion into the pituitary gland. Magnetic resonance imaging showed the pituitary gland pushed upward by NPC. Hypopituitarism due to an intracranial tumor and the extracellular volume excess due to chemotherapy caused hyponatremia and polyuria. As the intracranial tumor was diminished by chemotherapy and irradiation, pituitary hormones were normalized. In patients with T4 NPC, pituitary-adrenocortical function should be endocrinologically evaluated before treatment.

Alteration of the Bcl-2/Bax status of head and neck cancer cell lines by chemotherapeutic agents.

The Bcl-2 family of genes includes some important regulators of apoptosis. Among these genes, Bcl-2 and Bax control cell death, thus contributing to both tumor growth and drug sensitivity. We determined the levels of protein and RNA expression of Bcl-2 family in 21 head and neck cancer cell lines. Then using four cell lines, which were KB (Bcl-2+/Bax+), KCC-L871 (Bcl-2+/Bax-), YCU-T891 (Bcl-2-/Bax+) and TC901 (Bcl-2-/Bax-), we investigated the impact of Bcl-2/Bax status on sensitivity to the following chemotherapeutic agents; paclitaxel, cisplatin, vincristine and 5-fluorouracil. Immunohistochemistry and RT-PCR showed that 71% of the cell lines were Bcl-2 positive, 62% were Bax positive, 38% were Bcl-XL positive and 62% were Bcl-XS positive. After treatment with all the chemotherapeutic agents, Bax expression changed from negative to positive in TC901 cells. In KB cells, Bcl-2 expression decreased only after treatment with paclitaxel. YCU-T891 cells were sensitive to all of the drugs. In conclusion, Bcl-2/Bax status was correlated with drug sensitivity and treatment with chemotherapeutic agents induced apoptosis in these cancer cells.
