We studied the effects of simultaneous combination therapy with UFT and radiation for 69 head and neck cases with the same protocol in 6 universities in Kanagawa. Completed cases (cases in which therapeutic effects could be evaluated) totaled 67 patients who could receive more than 40 Gy as a given radiation dose and more than 8.4 g as a total UFT dose. Responder rate at 90th day after completion of radiation in 67 completed cases (CR 57 cases + PR 6 cases/67 cases) was as favorable as 94%. A comparison between 52 cases of the most frequent laryngeal cancer and historical control 113 cases received single radiation therapy by means of Kaplan-Meier method resulted significantly (P less than 0.05) favorable in UFT combination group as for disease free rate in all cases, disease free rate in Stage IV cases, disease free rate in glottic cancer, and survival rate and disease free rate in supraglottic cancer. No severe adverse reactions were observed in all cases. This combination therapy with radiation and UFT was judged effective for head and neck cancer, especially laryngeal cancer.

Ionic and non-ionic contrast media were compared for use in intravenous drip infusion excretory urography. Sixty consecutive cases were classified alternately into two groups, ionic and non-ionic, excluding cases which were known to have factors affecting contrast ability adversely, such as hydronephrosis, renal failure and so on. Each group consisted of 30 cases. A hundred ml of the ionic contrast medium (Diatrizoate--60% Urografin) or non-ionic contrast medium (Iopamidol--Iopamiron 300) was administered I.V. by drip infusion, each infusion taking less than 10 minutes, using a 18 G needle, and 0-minute, 10-minute and 20-minute films were obtained. Visibility of nephrogram, calyceal system, renal pelvis, ureters and bladder were evaluated, ranging from 0 to 3, by six radiologists who were not informed of the contrast medium used. Some detailed factors such as radiographic contrast and fullness of the collecting system and of the bladder were also analyzed. Visualization rate of the ureters was defined as the length of visualized ureters divided by the distance between the renal pelvis and the ureteral orifice of the bladder. There was no significant difference in visualization of nephrogram between the two groups. However, the non-ionic group was superior to the ionic group in visualization of the calyceal system and ureters with statistical significance, probably due to higher radiographic contrast of non-ionic medium. There was no statistical significance between the two groups, in fullness of the calyceal system and visualization rate of the ureters, whereas the ionic group was superior to the non-ionic group in fullness of the bladder.(ABSTRACT TRUNCATED AT 250 WORDS)
We evaluated the significance of redistribution in 123I-IMP SPECT study using PET. Twelve lesions in ten patients were selected. These lesions were classified into the following three groups; ischemia, infarction of subacute phase and infarction of chronic phase. All lesions with ischemia or subacute infarction showed good or moderate redistribution. Three of four lesions with chronic infarction showed no redistribution. The mean values of rCBF and rCMRO2 were highest in the lesions with good redistribution, but there was no differences in rOEF with the degree of redistribution. In conclusion, the tissue with good redistribution is not always viable and it was impossible to predict a region with increased rOEF by redistribution alone.
Combination effect of UFT and gamma-irradiation was examined using KB cell transplanted in nude mice. KB cell line is known as derived from human malignant tumor of the mouth floor. Although treatment with UFT or with gamma irradiation did not suppress the tumor growth, combination of these two treatments caused a marked suppression. Especially, when UFT was administered to the mice successively from the time of transplantation and the irradiation was applied after tumor growth, both suppression of the tumor growth and necrosis of the tumor tissue were observed.
Creatinine clearance and beta-D-acetyl-glucosaminidase (NAG) were useful markers for the evaluation of the renal function of 57 cases of head and neck cancer treated with 60 mg/m2 cis-platinum (CDDP). There was a significant correlation between creatinine clearance and NAG values for (r = -0.4820, p less than 0.01). The nadir of creatinine clearance and NAG values were observed between 2 and 3 weeks after administration of CDDP. After 2 courses of chemotherapy with CDDP, more nephrotoxicity was observed in the cases received prior CDDP therapy, than cases without chemotherapy statistically. Combined chemotherapy with CDDP and 5-FU (CF therapy) caused more renal damage, comparing with other combined chemotherapy such as COP therapy (CDDP, vincristine and peplomycin), and CAP therapy (CDDP, aclacinomycin and cyclophosphamide). Early recovery from the renal damage following CDDP therapy was detected in the cases given Fosfomycin (FOM). This results suggest that FOM would be effective for decreasing the nephrotoxicity by CDDP.

Protein kinase C (PKC) from human platelets was resolved into two fractions by hydroxyapatite column chromatography. One of the enzymes was indistinguishable from the brain type III PKC having alpha-sequence in its kinetic and immunological properties. The other enzyme was kinetically different from any of the brain PKC subspecies so far isolated, although it resembled the brain type II PKC having beta-sequence. With H1 histone as substrate, this platelet enzyme was not very sensitive to Ca2+, and activated partly by phosphatidylserine plus diacylglycerol or by free arachidonic acid. Both platelet enzymes could phosphorylate the P47 protein in vitro, but the enzyme physiologically responsible for the P47 protein phosphorylation in the activated platelets remains to be identified.

Three types of protein kinase C, designated types I, II, and III, were purified from rat brain cytosol, and have been shown to correspond to the cDNA clones gamma, beta, and alpha, respectively. Their relative activities in the whole brain tissue were roughly 26, 49, and 25% with H1 histone as a substrate. Type II enzyme was an unequal mixture of two subspecies (roughly 1:7) encoded by beta I and beta II sequences which differ from each other only in a short range of their carboxyl-terminal end regions. Although the three types have closely similar structures, they showed slightly different modes of activation and kinetic properties. Type I enzyme was less sensitive to diacylglycerol but was significantly activated by low concentrations of free arachidonic acid. Type II enzyme exhibited substantial activity without elevated Ca2+ levels, and responded well to diacylglycerol and, to some extent, arachidonic acid. The type III enzyme responded to diacylglycerol as well as to arachidonic acid. The mode of activation of the enzyme by arachidonic acid required elevated levels of Ca2+ but not phospholipid. In the presence of phospholipid, phorbol esters could activate all three types in a manner similar to diacylglycerol. Among various phospholipids tested, phosphatidylserine was the most effective for all three types. Type III enzyme was most sensitive to 1-stearoyl-2-arachidonylglycerol for activation. Conversely, type I enzyme was activated most efficiently by synthetic permeable diacylglycerols, such as 1,2-didecanoylglycerol and 1,2-dioctanoylglycerol. Many heavy metal ions exerted variable and distinct effects on the catalytic activities of these three types.(ABSTRACT TRUNCATED AT 250 WORDS)

Although the three distinct forms of protein kinase C isolated from rat brain soluble fraction are structurally very similar, they respond differently to free unsaturated fatty acids such as arachidonic acid to exhibit their catalytic activity. Type I enzyme encoded by gamma-sequence, as predicted by cDNA clone analysis, responds to these fatty acids only slightly, whereas Type III enzyme determined by alpha-sequence is activated by free unsaturated fatty acids in the presence of Ca2+ in a comparable manner to phosphatidylserine plus diacylglycerol. Type II, a mixture of two enzymes encoded by beta I- and beta II-sequence, resulting from alternative splicing, shows properties in between those of Type I and Type III. Some of these forms of protein kinase C may function at a relatively later phase of cellular responses when large amounts of unsaturated fatty acids and Ca2+ are mobilized.

In 20 cases of head and neck carcinomas, we have studied the immunological alternation by combined chemotherapy with Cisplatin, Vincristine, and Peplomycin (COP therapy). After COP therapy, the number of peripheral lymphocytes decreased, but the OKT4/8 ratio, the PHA-blastogenesis and the positive rate of skin tests increased. The cellular immunity was distinctly augmented by COP therapy. When compared with results from radiation therapy and operation. This augmentation was suspected to have been caused by the suppressive change of the suppressor T cells.
