Physiology of the vellus hair follicle: hair growth and sebum excretion.

The growth of vellus hair and the secretion of sebum from vellus hair follicles were measured on the forehead, cheek, chest, shoulder and back of healthy men and women aged 15-30 years. Hair growth was assessed by computerized image-analysis of photographs and sebum excretion by the use of Sebutape followed by image analysis. The density of vellus hairs and the percentage of growing hairs were higher on the face than on the thorax (439 hairs/cm2 with 49% growing hairs on the forehead compared with 85 hairs/cm2 with 31.5% growing hairs on the back). The rate of growth ranged from 0.03 mm/day on the forehead to 0.13 mm/day on the back. The maximum length of vellus hair significantly decreased with age; otherwise hair growth was not affected by age or sex. Some variations in hair growth and sebum secretion were observed over a period of 3 months, but no consistent rhythms were detected. There was no obvious link between vellus hair growth and sebum excretion.

Intraepidermal leakage of plasma proteins after tape stripping of normal skin and uninvolved psoriatic skin.

Following tape stripping of normal skin and uninvolved psoriatic skin, there was an early leakage of plasma proteins such as fibrinogen, fibronectin and immunoglobulins. This was accompanied by migration of mononuclear and polymorphonuclear cells into the epidermis. Both events appeared earlier, were more pronounced and lasted longer in psoriatic subjects than in controls.

Anthralin for psoriasis: short-contact anthralin therapy compared with topical steroid and conventional anthralin.

Anthralin is an effective topical drug for psoriasis therapy. Recent studies have suggested that anthralin may be effective in high concentrations for short periods of skin contact. The purpose of these studies was to compare psoriasis improvement rates in patients treated with 0.3% anthralin ointment for 10 minutes' skin contact daily with patients treated with a topical steroid, difluorosone diacetate. A further study conducted was a bilateral paired comparison between conventional strength (0.1%-0.5%) anthralin ointment overnight with high concentration anthralin (0.5%-3%) for short contact periods of 10 minutes daily. The results showed good responses in all treatment groups. Short-contact anthralin is confirmed as a practical means of outpatient therapy for psoriasis.

[Clinical trials in dermatology. The phase I trials].

Application of the new topical product on the diseased skin should be preceded by its safety evaluation on the healthy skin in human volunteers. We propose here guidelines for the evaluation of the irritation, sensitization, phototoxicity and photoallergy potentials for topical products. The methods for evaluation of percutaneous absorption are also discussed. The studies presented here are not the object of any regulations. Therefore, we propose here an approach for the safety evaluation of topical products in human volunteers.

[Clinical trials in dermatology. Evaluation of the tolerability and efficacy of a topical antipsoriatic treatment].

There has been considerable variations between different authors in the evaluation of antipsoriatic therapies. Improvement homogeneity must be achieved in this field. The main specific methods have been investigated: inclusion criteria, assessment of disease progress and safety parameters in phase II and III clinical trials, evaluating antipsoriatic treatment.

[Clinical trials in dermatology. Statistical support].

To determine the efficacy or safety of new topical treatments, more or less standardized studies must be performed. This article presents several examples of statistical analyses of the data currently seen in the dermatology area, and some guidelines for study sample size determination. More importantly, it indicates, for as soon as phase I studies, the need for preliminary informations such as development strategy, historical data, literature references or pilot studies, to determine or adapt appropriate designs and sample sizes. This need can be filled by close collaboration among clinicians, statisticians and sponsor.

[Clinical trials in dermatology. Evaluation of the tolerability and efficacy of a topical anti-acne].

Acne is a frequent dermatologic disease of the teenagers. Methodology of antiacne preparations clinical trials has evolved recently, leading to better comprehension of acne treatment on acne lesions. The main rules for good clinical evaluation of acne treatments are: an objective counting of each individual lesions on a defined area (face, back), a global acne assessment, a therapy duration from 1 to 3 months or more, a skin safety evaluation for erythema, desquamation, dryness, itching, burning and oiliness with a 0 to 3 scoring system.
