[Coping with illness by young diabetic patients: a comparison from the viewpoint of patients, their parents and personal physicians].

108 adolescents with type 1 diabetes as well as their parents and physicians in charge were studied in a prospective longitudinal design. Most of the juvenile patients were of good metabolic control. The results from different questionnaires of coping are presented. The opinions of the patients, their parents and the physicians in charge differ in some important aspects. Furthermore there are significant differences between the patients of best versus worst metabolic control as far as the extent of self reported psychosocial problems related with the disease is concerned.

[The psychosomatic consultation and liaison service. Evaluation, research approach and contribution to education].

Consultation and liaison work is one of the major fast growing fields of psychosomatic medicine in the Federal Republik of Germany. According to basic principles "integration by cooperation or by increasing psychosocial competence", we try to psychologize medicine as practized in the hospital. Results from a psychooncological study shown an urgent need for further research especially concerning process studies. The importance of CL-work for medical students' training is demonstrated with different examples.

[Psychosomatics of breast cancer - present state of research and new developments].

Cancer of the breast is a malignant disease most intensively studied under a psychosomatic perspective. But validated results are rare. Further research should follow three guidelines: 1. True prospective studies of non-selected populations, 2. interdisciplinary research of endocrine, immunologic, genetic, and psycho-social influencing factors, 3. the clinical course of the disease should be investigated to improve the psycho-social rehabilitation. Main emphasis lies here on the involvement of the patient's family.

[Breast cancer in context - results of a prospective study and implications for therapy].

56 patients admitted to the surgical clinic for a breast biopsy were interviewed on the day of their admission. The interviewer recorded his impression of the conversation in eight rating scales. The assessments were repeated by an independent rater from edited tape recordings of the interviews. There was a statistical difference between breast cancer patients and patients with a benign node in all eight scales used and the diagnosis cancer was predicted by the interviewer and rater in 80-90% of all cases. A specific mode of therapeutic procedure is discussed in which the cancer is seen as the final link in a chain of similar occurrences, and in which inclusions of the surrounding field is more conclusive to success than working with the individual alone. The basic idea of this approach has for a long time played a part in the work with "difficult" patients: it very positively sanctions the behavior exhibited as a survival feat in an extremely difficult situation and it accepts that for the time being any other behavior is hardly possible. Thus, a paradoxical effect often ensues: the families begin to offer resistance to the doctor's one-sided representation and at the same time call their own behavior into the question.

[Aspects of patient selection in psychosomatic ambulatory care].

A study of selective social factors was conducted among a sample of 350 out-patients of the Psychosomatic Clinic Heidelberg. The findings indicate that the age of a patient, his education and the type of his contacting the psychosomatic ambulance are relevant to a further therapeutical treatment by the clinic. A higher education, an age under 30 and a contact with the ambulance on one's own initiative were found as factors of positive influence for a further therapy. A subsequent study showed an obvious correlation between these aspects of selection and the therapeutical situation, in particular the frequency of a psychotherapeutical treatment, of all patients after 6 months.
