The analysis of characteristics of elderly people with high VSC level.

OBJECTIVE: The purpose of this study was to examine the characteristics of elderly subjects who had objectionable levels of volatile sulfur compound (VSC). SUBJECTS AND METHOD: In 2002, a total of 115 85-year-old persons in Japan were subjected to oral examinations, tongue coat collections, measurements of VSCs levels inside the mouth using a portable gas chromatography (Oral Chroma, Abilit, Japan), and assessments of quality of life (QOL) using an SF-36 questionnaire. RESULTS: Sixty-six of the subjects were edentulous and 49 were dentulous. They were divided into two groups by VSC levels, those with oral malodor (both H2S > 112 ppb and CH3SH > 26 ppb; subjects with oral malodor, OM group; n = 7) and those without (n = 108). Our results showed that tongue coat deposit amounts and proportion of dentulous subjects were significantly higher in the OM group. Further, in an analysis of QOL, the SF-36 scores for vitality, social functioning and mental health were significantly higher in OM. CONCLUSION: We found that elderly subjects with oral malodor tended to be dentulous and had large deposits of tongue coating. However, oral malodor in the OM group subjects did not appear to cause a disadvantage in their social lives.

Relationship between the SF-36 questionnaire and patient's satisfaction following halitosis therapy.

OBJECTIVE: To assess the health-related quality of life (HRQOL) of patients complaining of halitosis at their first visit and at a later time when their complaint had diminished following therapy, using a self-administered questionnaire, the Medical Outcome Study Short Form-36 (SF-36). The aim of this study was to examine the relationship between HRQOL of patients before and after self-reported disappearance of their complaint following oral hygiene improvements for halitosis. SUBJECTS AND METHODS: Seventy patients of our special clinic for halitosis served as subjects. At the first visit, each completed the SF-36 before determination of volatile sulfur compound (VSC) concentration in mouth air. After excluding dropouts, the same measurements were performed for subjects whose self-reported complaint had disappeared following oral hygiene therapy. RESULTS AND DISCUSSION: At the initial visit, SF-36 scale scores for general health, vitality, social functioning, role-emotion, and mental health were significantly lower when compared with the national averages in Japan. For subjects with self-reported disappearance of complaint, only social functioning rose significantly among SF-36 scores at the end of the study. These results suggest that an awareness of improvement in social life could be related to patient's satisfaction with halitosis oral hygiene therapy.

A survey of potential risk factors for HIV transmission through dental practice in Japan.

A survey was conducted to find potential risk factors for HIV transmission through dental practice. Self-administered questionnaires were distributed to the members of the dental associations of various prefectures and small cities in Japan. A total of 747 questionnaires was returned by dentists. The findings revealed several problems. Gloves, masks, and other protective garments were generally worn, but most dentists did not always use them during the full course of treatment limiting usage to surgical treatment, and when treating patients in "high-risk groups" and in dental practice the exact percentage of the dentists who reused a used anesthetic liquid cartridge is 12.7%. The effectiveness of the education for dentists regarding AIDS was statistically clarified. It is obviously necessary to provide important information regarding AIDS and HIV transmission to the dentists. Potential risk for HIV transmission through dental practice will be prevented by AIDS education for the dentists.

[Asymptomatic cerebral infarction associated with a patent foramen ovale and atrial septal aneurysm].

We reported two young adults (a 42-year-old female and a 45-year-old male) with tension type headache who had a patent foramen ovale and atrial septal aneurysm demonstrated by transesophageal echocardiography, associated with asymptomatic cerebral infarctions. There were multiple subcortical infarctions in the frontal and parietal lobes in case 1, and cerebral infarctions in the right corona radiata, head and body of the caudate nucleus, and putamen in case 2. The two cases did not have hypertension, diabetes mellitus, hyperlipidemia, cardiac diseases detected by electrocardiography and transthoracic echocardiography, and abnormality of intracranial and extracranial arteries by ultrasound sonography and cerebral angiography. Transeosophageal echocardiography revealed atrial septal aneurysm, and showed right-to-left shunt (patent foramen ovale) by Valsalva maneuver. Two cases were diagnosed as paradoxical cerebral embolism associated with a patent foramen ovale. If asymptomatic cerebral infarctions are cryptogenic stroke, a patent foramen ovale and atrial septal aneurysm should be examined by transesophageal echocardiography with Valsalva maneuver.

Flavor characteristics of glutathione in raw and cooked foodstuffs.

The flavor of glutathione (gamma-L-glutamyl-L-cysteinylglycine, GSH) was examined by several sensory evaluations. The measurement of a point of subjective equality (PSE) showed that the peptide increases the flavor characteristics but did not affect the intensity of basic tastes, such as sweetness, saltiness, sourness, and umami. However, the threshold value of GSH decreased significantly in an umami solution containing 0.05% each of monosodium glutamate (MSG) and disodium inosinate (IMP). This suggests that GSH interacts with the umami substance and has a certain effect on the flavor. GSH had a characteristic kokumi flavor, such as continuity, mouthfulness, and thickness in the umami solution as well as in a model beef extract constructed from analyzed components at a concentration of 0.02% w/v. Some foodstuffs, including meat, were found to contain GSH above its threshold value, which implicates the contribution of GSH to the flavor. The thermal degradation study suggested that a part of GSH have changed into its disulfide, pyroglutamic acid (PCA), and cyclocysteinylglycine in cooked foodstuffs.

Spontaneous dissection associated with proximal vertebral artery anomaly.

A 47-year-old man was admitted because of acute lateral medullary syndrome with severe posterior cervical pain. Cerebral angiography was performed three hours after the onset, which demonstrated that two arteries branched separately from the right subclavian artery, ran upward and formed a single right vertebral artery (VA). One of the two arteries showed both stenosis and luminal dilatation. We thought the structure of these arteries was proximal vertebral artery anomaly and diagnosed him as having dissection of the vertebral artery. We consider that the proximal vertebral anomaly may be a risk for spontaneous VA dissection.

Autonomic dysfunction and anemia in neurologic disorders.

The effect of autonomic dysfunctions on anemia in various neurological disorders, such as familial amyloidotic polyneuropathy (FAP) Type I, pandysautonomia, and Shy-Drager syndrome was examined. As a control, hemograms of patients with amyotrophic lateral sclerosis (ALS), which is known to be free from autonomic dysfunction, was compared with patients with the above neurological disorders. FAP and pandysautonomia patients showed significant anemia comparable with the severity of the autonomic dysfunctions. Shy-Drager patients exhibited mild anemia. However, in ALS patients, no such anemia was recognized at all even in the end stage of this disease. In pandysautonomia patients, hypoplastic bone marrow was recognized, which was quite consistent with the data previously reported in FAP patients. Human recombinant erythropoietin improved orthostatic hypotension as well as anemia in 4 FAP patients. These results suggest that autonomic dysfunction may be deeply connected with erythropoiesis.

[A survey on infection control practices, knowledge and attitudes toward AIDS/HIV among dental practitioners].

A survey was conducted in December 1993 by sending questionnaires to all 566 dentists of the Iwate Dental Association. The questionnaires consisted of 68 items including infection control practices, knowledge, and attitudes towards AIDS/HIV. The response rate was 51.1 percent (N = 289). The average age of dentists in the sample was 43.7 +/- 9.5 (S.D.) (range: 28 to 85). Data was evaluated statistically by Kruskal-Wallis test, Mann-Whitney's U test and Chi-square test, and significant differences were observed. Gloves, masks, and other protective garments were generally worn, but most dentists did not always use them during the full course of treatment limiting usage to surgical treatment, and when treating patients in "high-risk groups". Other infection control procedures, such as instrument sterilization, did not comply with the guidelines of the Ministry of Health and Welfare. Two dentists responded that they had treated AIDS patients and three dentists treated possible AIDS/HIV patients in their dental offices. Despite 71.3% expressing a belief that they have a moral responsibility as a dentist to treat AIDS/HIV patients only 15.6% were willing to treat AIDS/HIV patients. Over 40% of the respondents were not certain as to whether they had treated AIDS/HIV patients or not, and over 60% believed AIDS/HIV patients would come to their dental offices in the near future. Therefore, to practice dentistry on AIDS/HIV patients safely, dentists must provide effective infection control in their dental offices on the assumption that all patients are AIDS/HIV positive, and additional information about AIDS/HIV and adequate training on procedures in the care of AIDS/HIV patients are needed.

[The intravascular mobile structure detected by duplex carotid ultrasonography in cardioembolic internal carotid artery occlusion].

We reported that duplex carotid ultrasonography demonstrated specific mobile intravascular mass in four patients with cardiac cerebral embolism. In all cases, duplex carotid ultrasonography clearly showed the presence of a mobile intravascular mass in the carotid artery within 2 days after the onset. In two of four cases, the follow-up duplex carotid ultrasonography showed disappearance of the previously observed "a mobile intravascular mass" 7 days after the onset. In the other two cases, a mobile intravascular mass became stable gradually and changed to non-mobile hyperechoic mass 10 days after the onset. We think that the intravascular mobile structure was probably a thrombus extending from the occlusive part of the internal carotid artery to the proximal side. We suspect that the specific "a mobile intravascular mass" detected by ultrasonography may help to diagnose embolic internal carotid artery occlusion.

[Study on the oral health status of schoolchildren in Korea].

In 1992, dental health examination was conducted in three areas in Korea; Seoul (large city), Cheonan (rural city) and Cheongju (water fluoridated rural city). We also examined the children of Tokyo in Japan. The subjects were the schoolchildren in the 6th grade of primary school and the 3rd grade of junior high school, and the number of the subjects was 202 in Seoul, 191 in Cheonan, 164 in Cheongju and 152 in Tokyo. We compared the oral health status among these four areas using many indices such as the prevalence rate of dental caries and malocclusion, DMFT, DMFS, DI-S and P-M-A index. In Korea the children in Seoul showed the highest caries prevalence rate, DMFT and DMFS. The Cheonan children were average. The children in Cheongju had the lowest caries experience. The children in Tokyo had more dental caries than the Korean children in both school grades. The children in Tokyo had more gingivitis and malocclusion than the Korean children in the three areas. It was revealed that the oral health status of the Korean schoolchildren was much better than the Japanese children. And water fluoridation project in Cheongju was effective to prevent the dental caries especially in the 6th grade children.

[Epidemiological study on distribution of plaque, calculus and gingivitis among Nigerian people].

In 1991, dental health examination was carried out in Nigeria: ILE-IFE (urban) and Ashipa (rural) located in the southwest of Nigeria. The number of the subjects was 205 (aged 8, 10, 12) in ILE-IFE and 511 (aged 8, 10, 12, 15 to 75) in Ashipa. The purpose of this study is to clarify the distribution of plaque, calculus and gingivitis and to reveal the correlation between the plaque score (DI-S, OHI-S), calculus score (CI-S, OHI-S) and gingivitis score (GI) among the Nigerian by age. Results were as follows: 1. In the comparison between urban and rural schoolchildren, CI-S was higher in the rural children statistically but DI-S was not. This evidence is to be expected as a consequence of using PACO (traditional twig) for tooth brushing, because 73.1% of rural children were using the PACO whilst 92.1% of urban children were using the tooth-brush. 2. Because of the poor oral cleanliness using PACO, the changing pattern of DI-S, CI-S and GI by age could be natural and physiological. 3. DI-S and CI-S among the rural people were of a high level in the children 8 and 10 years of age and decreased to the bottom level in the 20's. After the lowest scores in the 20's, these increased with age. 4. The correlations between DI-S and GI were higher in the young people than in the elder people.

Joint epidemiological longitudinal dental survey in Nigeria, especially in comparison with that of Japanese.

Since 1980 we carried out a longitudinal dental survey in Ile-Ife, as a joint study with the dental school of Ife University, Nigeria, being supported by A Grant under The Monbusho International Scientific Research Program for ten years. One thousand one hundred seventy-one children and adults were examined in the 1991 survey. The data were compared with the data in the previous survey and Japanese survey. Results were as follows: 1) Caries prevalence rate and the average number of DMFT were still very low, especially showing that both the caries prevalence and the average number of DMFT decreased in the rural areas because the attrition proceeded faster than the caries, 2) Nigerian deciduous and permanent dentition were larger than in the Japanese in all items measured, 3) the condylar head was transformed from the round shape to the ultra-flat shape with age, 4) there was a fewer incidence of severe periodontal diseases despite of the marked deposition of calculus, 5) with respect to Nigerian foods, there was no difference between the rainy and dry seasons in both the urban and rural communities, 6) the weaning period of the baby is decided by their mother, taking care of the health of the baby, almost all babies at one year to two years and a few at three years and 7) the menu for the breakfast, lunch and supper of the baby was made considering the nutritional aspect of the baby.

Comparative study of oral health status between Scottish and Japanese schoolchildren aged 6-11 years.

This study was carried out to examine and compare the differences in oral health status between Scottish (Fife) and Japanese (Motoyoshi) primary schoolchildren, and also to analyse some factors concerned with the differences found. It was observed that caries experience is higher in Motoyoshi than in Fife, despite Fife schoolchildren having poorer oral cleanliness and consuming snacks more frequently. However, Fife children had more fissure sealants and brushed their teeth more frequently with fluoridated dentifrice than did Motoyoshi children. The sugar content of snacks consumed by Motoyoshi children was higher than that of the Scots. Gingivitis (PMA positive rate) was higher in Fife than in Motoyoshi, and it was also noted that Fife children were less independent than Motoyoshi children in their daily behaviour.
