[Changes of neutrophil myeloperoxidase in coronary circulation among patients with acute coronary syndrome].

OBJECTIVE: To investigate the changes of neutrophil myeloperoxidase (MPO) blood concentration gradient between the systemic circulation and the coronary circulation among patients with acute coronary syndrome and its clinical value. METHODS: Fifty patients underwent coronary angiography, which including 10 patients in AMI group, 20 patients in UA group, 10 patients in SA group and 10 subjects served as control. The levels of MPO and hs-CRP were measured in the serum of blood collected from femoral vein, aortic artery root and coronary sinus. RESULTS: Compared with the control, concentrations of LDL in the AMI, UA and SA groups were significantly increased, while the latter three groups did not differ from each other. In the UA patients, the in-gate percentage of MPO decreased in the coronary sinus compared with that in the root of aortic artery (P < 0.01); the in-gate percentage of MPO decreased through coronary circulation more than through systemic circulation (P < 0.001); the average fluorescent intensity of MPO and the concentrations of hs-CRP showed no difference between samples from the coronary sinus and that from the root of aortic artery. In the AMI patients, the average fluorescent intensity of MPO in the coronary sinus was weakened compared with that in the root of aortic artery (P < 0.05); it decreased through coronary circulation more than through systemic circulation (P < 0.001); neither the in-gate percentage of MPO nor the concentrations of hs-CRP showed significant difference between samples from the coronary sinus and that from the root of aortic artery. In the control and SA groups, samples from the femoral vein, the root of aortic artery, and the coronary sinus did not show differences at the serum level of MPO and hs-CRP. In the UA group, the in-gate percentage of MPO correlated positively with the concentration of hs-CRP (r = 0.78, P < 0.01), and with the level of LDL as well (r = 0.52, P < 0.05); In the AMI group, the average fluorescent intensity of MPO correlated negatively with the concentration of hs-CRP (r = -0.80, P < 0.01), and showed no correlation with the level of LDL (r = 0.22, P > 0.05). CONCLUSIONS: MPO is a better marker for inflammation of the local plaques. It may be one of the mechanisms that MPO induces the transforming from LDL to ox-LDL in plaques vulnerability.

N-retinylidene-phosphatidylethanolamine is the preferred retinoid substrate for the photoreceptor-specific ABC transporter ABCA4 (ABCR).

ABCA4, a member of the family of ATP binding cassette (ABC) proteins found in rod and cone photoreceptors, has been implicated in the transport of retinoid compounds across the outer segment disk membrane following the photoactivation of rhodopsin. Mutations in the ABCA4 gene are responsible for Stargardt macular dystrophy and related retinal degenerative diseases that cause a loss in vision. To identify the retinoid substrate that interacts with ABCA4, we have isolated ABCA4 from rod outer segment disk membranes on an immunoaffinity matrix and analyzed retinoid compounds that bind to ABCA4 using high performance liquid chromatography and radiolabeling methods. When all-trans-retinal was added to ABCA4 in the presence of phosphatidylethanolamine, approximately 0.9 mol of N-retinylidene-phosphatidylethanolamine and 0.3 mol of all-trans-retinal were bound per mol of ABCA4 with an apparent K(d) of 2-5 microm. ATP and GTP released these retinoids from ABCA4, whereas ADP, GDP, and nonhydrolyzable derivatives, adenosine 5'-(beta,gamma-imido)triphosphate and guanosine 5'-(beta,gamma-imido)triphosphate, were ineffective. One mole of N-retinyl-phosphatidylethanolamine, the reduced form of N-retinylidene-phosphatidylethanolamine, bound per mol of ABCA4, whereas 0.3 mol of all-trans-retinal were bound in the absence of phosphatidylethanolamine. No binding of all-trans-retinol to ABCA4 was observed. Our results indicate that ABCA4 preferentially binds N-retinylidene-phosphatidylethanolamine with high affinity in the absence of ATP. Our studies further suggest that ATP binding and hydrolysis induces a protein conformational change that causes N-retinylidene-phosphatidylethanolamine to dissociate from ABCA4.

[The study of the invasive biologic behavior of ameloblastoma].

OBJECTIVE: To investigate the invasive biologic behavior of ameloblastoma (AB) and to analyze its correlative factors. METHODS: The specimens of 43 cases of AB (primary AB 16 cases, recurrent AB 21 cases, malignant AB 6 cases) were examined immunohistochemically using the streptavidin-biotin method to determine the expression of E-cadherin (E-cad), matrix metalloproteinase (MMP)-2, MMP-9 and vascular endothelial growth factor (VEGF). RESULTS: The cells in malignant AB scattered more, grew invasively, and the basal membrane ruptured or lost. The expression of E-cad in AB descended, MMP-2 and MMP-9 were strongly expressed in the epithelia cells of 28/41, 30/43 cases of AB, respectively. The positive rate and intensity of VEGF increased as AB recurred and transformed malignantly. The expression of E-cad, MMP-9 and VEGF were related to recurrence or malignant transformation of AB (r(s) = 0.309, 0.519, 0.381, P < 0.05). CONCLUSION: AB is a high invasive tumor. The biological behavior of AB is related to lost or abnormal expression of E-cad, the high expression of MMP-2, MMP-9, and VEGF.

[Expression of pRb and E2F-1 and telomerase activity in ameloblastoma].

OBJECTIVE: To investigate the expression of pRb and E2F-1, and the association between their expression and the activity of telomerase (hTERT) or cyclin E in human ameloblastoma (AB), and to explore the clinical biological characteristics of AB. METHODS: The expressions of pRb, E2F-1, cyclin E and hTERT mRNA in human AB were detected by in situ hybridization or immunohistochemistry (SP method). RESULTS: The positive expression ratio of pRb in the cell nucleus of AB was 20.4% (11/54). The positive ratio of E2F-1, cyclin E and hTERT mRNA was 92.6% (50/54), 66.7% (36/54) and 94.4% (51/54), respectively. With AB recurrence and malignant transformation, the expression of hTERT, E2F-1, cyclin E was up-regulated. hTERT and cyclin E or E2F-1 mRNA had high positive relation (Spearsman'r(s) = 1.000, P = 0.0001). CONCLUSIONS: The regulatory pathway of Rb/E2F-1 is associated with the cell proliferation and in differentiation of AB. The activity or release of telomerase may be related to the lower expression of Rb and higher expression of E2F-1, and is up-regulated in G(1) late phase by cyclin E.

[Effect of static magnetic field on proliferation and cell cycle of osteoblast cell.].

Rat calvarias osteoblasts were cultured and put in the static magnetic field of 0 Gs, 400 Gs, 620 Gs, 830 Gs and 1080 Gs for 24 h, 48 h and 72 h, respectively. MTT method was used for detecting osteoblast proliferation and flow cytometry for detecting cell cycle. When the magnetic exposure time extended to 48 h or 72 h, the corresponding OD value of osteoblast with magnetic treatment of 400 Gs or 620 Gs increased obviously with significant difference as compared with 0 Gs group (P<0.05), but such proliferative effect was not shown in the 1080 Gs group (P>0.05). After magnetic exposure with intensity of 620 Gs, G0/G1 phase percentage decreased, but S phase and G2/M phase percentage was significantly higher as compared with control. Proliferation index (PI) increased as well and there was significant difference in contrast to control (P<0.05). Within a certain range of magnetic intensity, magnetic field enhances cell growth in a dose-dependent pattern. And static magnetic field can activate static G1 phase into S phase, increase DNA synthesis, and accelerate cell proliferation.

[Feasibility study of controlled hypotension induced with combination of propofol and desflurane in craniotomy].

OBJECTIVE: To evaluate the feasibility of controlled hypotension induced with combination of propofol and desflurane in craniotomy. METHODS: Thirty-five ASA I approximately II patients undergoing elective craniotomy were randomly selected. Anesthetic induction was achieved with midazolam, thiopental, fentanyl, and vecuronium. Anesthesia was maintained with infusion of propofol and inhalation of desflurane. The propofol infusion rate and desflurane inhalation concentration were raised during isolation and resection of the tumors so that the mean arterial pressure (MAP) was reduced by 30% - 40% and > 50 mm Hg and maintained at this level for 30 - 45 min. After the resection of tumor, the propofol infusion rate and desflurane inhalation concentration were reduced so as to let the MAP increase to normal level. MAP, heart rate (HR), cerebrospinal fluid pressure (CSFP), propofol infusion rate, and inhalation concentration of desflurane were recorded before controlled hypotension, at the 1st, 3rd, 5th, 10th, 20th and 30th minute of the procedure of controlled hypotension, and just before the increase of blood pressure and 1, 3, 5, 10, 20, 30, 40 and 50 minutes after controlled hypotension. The awakening status was observed. Arterial and jugular bulb venous blood samples were taken before, during and after hypotension and the arterial to jugular bulb venous oxygen content difference [D(a-jv)O(2)] and cerebral oxygen extraction rate (ERO(2)) were measured. RESULTS: The time needed to reach controlled hypotension was 6 - 35 minutes and that needed to reach the normal blood pressure was 12 - 41 minutes. The propofol infusion rate and inhalation concentration of desflurane needed to maintain controlled hypotension were 51 micro g x kg(-1) x min(-1) +/- 27 micro g x kg(-1) x min(-1) and 6.4% +/- 4.1% respectively. HR increased significantly in the initial stage and returned to baseline value after 3 - 14 min during the controlled hypotension. CSFP decreased significantly after incision of dura mater of brain and during controlled hypotension (P < 0.05). The recovery time after weaning from propofol infusion and desflurane inhalation until eye opening in response to order and autonomous breathing was 25 min +/- 19 minutes. The time needed to regain autonomous breathing with the tidal volume > 300 ml/time was 17 min +/- 10 minutes. Compared with those before hypotension, the SjvO(2) was increased significantly, and D(a-jv)O(2) and ERO(2) were decreased significantly during hypotension (P < 0.05). CaO(2) and CjvO(2) remained unchanged throughout the process of hypotension (P > 0.05). CONCLUSION: With low propofol infusion rate and low desflurane inhalation concentration, rapid hypotension, rapid recovery of breathing after operation, a short recovery time from anesthesia, and decreased CSFP and brain oxygen metabolism, controlled hypotension with propofol infusion and desflurane inhalation is suitable to intracranial surgery.

[Origin of SARS from accelerated evolution of a virus by combined pollution].

The SARS virus origin which obviously differs from the theory of out-space origin and the theory of wildlife origin was theoretically discussed based on principles of pollution-evolution ecology, pathogenesis and infectant characteristics of SARS, and some relevant experimental evidence. It emphasizes on the fact that increasingly combined pollution in China provides a substantial basis for virus evolution. Gene recombination and fusion of more than one virus in ecosystems should take charge with the origin of a SARS virus. In particular, a common cold or influenza virus can acceleratedly changed into a superpower common cold or influenza virus through genetic mutation and gene recombination by increasingly combined pollution. This work can be useful for human beings to overcome SARS in theory.

[Expression of matrix metalloproteinases and tissue inhibitor of metalloproteinase in ameloblastoma].

OBJECTIVE: To study the expression of matrix metalloproteinase(MMP) and tissue inhibitor of metalloproteinase (TIMP) in ameloblastoma (AB) and to determine the relationship between biological behavior of AB and clinical pathology. METHODS: The specimens of 43 cases of AB, 10 cases of odontogenic keratocyst (OKC), 8 cases of normal oral mucosa were examined by streptavidin-biotin method. RESULTS: In 8 cases of normal oral epithelial, MMP-2 was negative or weak positive. In OKC, MMP-2 was extensively positive in stratum spinosum of 2 cases, and weekly or negative in stratum basale. MMP-2 was strongly expressed in the central and peripheral cells of the tumor islands in 28 cases of AB. There were difference among these three groups (P < 0.001). Comparing AB with normal oral mucosa and OKC, there was significant difference (P < 0.05). MMP was positive expressed in cells of stroma. The positively rate and intensity increased as AB recurrence and transformed malignantly, but were not associated with age, sex, pathological type and location. TIMP-1 was weakly or not expressed in normal oral mucosa, stoma, OKC and AB. CONCLUSIONS: The high expression of MMP-2 and MMP-9 is related to the biological behavior of AB. Imbalances of the expression of MMP-2 and MMP-9 protein maybe be one of the facts of the invasion of AB. The MMPs activation produced by stromal cells may be also be related to the invasion of AB.
