[Bone and joint lesions in burns in childhood].

Bone and joint lesions as an underlying cause of disability require exact and emergency diagnosis and treatment. This is a report on the case material of the Pediatric Clinic of Burns and Plastic Surgery, with emphasis on the characteristic features of the children's population, having an essential bearing on the course of osteoarticular changes in the various stages of burn disease. The basic principles of treating osteoarticular lesions in each of the three groups, adopted in the clinic, are outlined, with special attention focused on the therapeutic approach to each of the groups, and on the emergency operative procedures aimed at maximally prompt removal of necrotic tissues and resurfacing of the osteoarticular lesions in early terms with a view to a varying degree preservation of function. Emphasis is also laid on the important practical implications of early rehabilitation as an element invariably present in the complex of functional recovery of the osteoarticular apparatus.

Studies on the plasma cholesterol and triacylglycerols in chronic methemoglobinemia in rats.

Plasma cholesterol and triacylglycerols were measured in rats with modelled chronic two-stage (mild and moderate) intermittent nitrite methemoglobinemia for 15 and 30 days. It was found that at the moment of methemoglobinemic peak (60 +/- 10 min) the experimental animals had mixed (hemtoxic, anemic and hypoxic) hypoxemia. The every day "pulse" decrease of the total oxygen concentration during the 30-day methemoglobinemia was accompanied with a significant rise (p < 0.05) of cholesterol concentrations in the high-density lipoproteins and the total cholesterol, as well as a decrease in the amount of triacylglycerols. These changes are considered to represent the side effects of adaptation for whose elucidation further research is needed.

Combined dermatoplasty in deep burns in the region of flexion of the large joints in children.

In 13 children with deep burns in the region of functionally important anatomical flexions, the wounds were covered with rotation skin flaps from the vicinity. The purpose of the method used is the prevention of post-burn contractures, which are particularly expressed in the process of the child's growth. After observation of the operated patients over 3 years, the results of operation justify the conclusion that it is purposeful to utilize skin resources from the vicinity of the wound surface to achieve more rapid and more effective restoration of anatomical integrity and function in deep burns on flexions of the body.

Hypersensitivity reactions to ethylene oxide: clinical experience.

A hypersensitivity reaction occurring in the first minute of the dialysis procedure was observed in seven haemodialysis patients in one day. Hollow-fibre dialysers were used, five made of saponified cellulose ester (SCE) and two of cuprammonium cellulose (CC). All were sterilised with ethylene oxide (ETO) and used for the first time. The severity of the reactions was grade 2. The whole series of dialysers was examined for the presence of ETO concentration. A significantly higher concentration of ETO was found in the polyurethane potting than in the capillaries. The ETO concentrations were 122, 185, 440, 274, 342, and 280 p.p.m. in the following dialysers: Cordis Dow (cellulose acetate CA), Cordis Dow-Plivadial (SCE), Fresenius C-1.3 (CC), Fresenius E-2 (CC), Fresenius E-3 (CC), and Travenol-Medial S 11 flate plate (CC) respectively. According to the clinical signs, ETO concentrations in the dialysers and the lack of reaction when extensive rinsing was used, it can be presumed that these reactions are related to ETO although other mechanisms cannot be excluded.

[Clinico-chemical study of a representative group of employees in the city of Pazardzhik. The reference intervals of lipid indices].

540 healthy employees were examined by a clinicochemical analyzer "PA-1000, Technicon", enzyme methods and calculated ratio. With the help of a non-parametric method the reference ranges (0.025 and 0.975 fractiles) and their 90% confidence intervals were determined for serum triglycerides (0.49-0.60 and 2.60-2.79), total cholesterol (3.18-3.49 and 6.92-7.43), high density lipoprotein cholesterol (0.68-0.78 and 2.00-2.20), low density lipoprotein cholesterol (1.56-1.71 and 4.83-5.31)-im mmol/l and the ratio total cholesterol: high density lipoprotein cholesterol (2.21-2.48 and 6.56-7.15). 0.25, 0.50 and 0.75 fractiles are also pointed out and the analytical control, the time of the test, sex and age differences as well as the risk degree of atherosclerosis are discussed.
