[Viral hepatitis type A as an important all-clinical problem].

The article contains present-day data on type A viral hepatitis, whose incidence in Russia has increased significantly in the recent years. The authors consider the pathogenesis of type A hepatitis, taking into account the peculiarities of immune response to the virus. Clinical manifestations of various variants, including anicteric one and combinations of type A hepatitis with hepatites of other etiologies, are described. The authors stress the diagnostical significance of anti-HAV IgM detection, the aspects of differential diagnosis, treatment, as well as specific and non-specific prophylaxis of type A viral hepatitis.

[Clinical significance of intracellular cytokines in patients with yersiniosis].

AIM: To evaluate clinical value of intracellular content of interferon-gamma (If-g), IL-2 and IL-4 in different forms of yersiniosis. MATERIAL AND METHODS: The number of If-g, IL-2 and IL-4 producing cells was estimated in 35 patients with generalized (n = 21, group 1) and secondary-focal (n = 14, group 2) forms of yersiniosis. RESULTS: The control over the number of If-g, IL-2 and IL-4 producing cells in the blood enables valid prognosis of the course and outcome of yersiniosis. CONCLUSION: Pathological processes developing in acute yersiniosis are based on interleukin-dependent immunopathology.

[Diagnostic significance of type IV collagen and hyaluronic acid in the serum of patients with chronic hepatitis C for staging hepatic fibrosis].

AIM: To evaluate diagnostic value of serologic fibrosis markers (hyaluronic acid--HA and type IV collagen C-IV) in patients with chronic hepatitis C (CHC) and hepatic cirrhosis (HC). MATERIAL AND METHODS: HA and C-IV were measured in 88 CHC patients with fibrosis stage 1 (n = 63) and 3 (n = 25), 13 patients with acute hepatitis C (AHC), 28 patients with hepatic cirrhosis (HC), 19 patients with pulmonary fibrosis (PF). The control group consisted of 32 healthy subjects. RESULTS: HA concentrations in the serum of CHC patients with mild to severe inflammation and fibrosis (F1 and F3) were normal (100 ng/ml). For HC diagnosis, HA test proved highly sensitive and specific (in HA 100 ng/ml sensitivity was 100%, specificity 84.6%), but this method cannot stage hepatic fibrosis. HA test was inferior to C-IV test. A mean C-IV concentration in the serum of CHC patients at the stage of marked fibrosis (F3) is significantly higher than in F1, in HC (A) patients higher than in patients with CHC F3. CONCLUSION: It is shown than concentration of C-IV above 196 ng/ml can differentiate fibrosis stage 1 from stage 3 with specificity 58.7 and sensitivity 88%.

[Development of immunopathology in patients with yersinia infection].

AIM: To evaluate clinical significance of changes in IL-6 concentration and neopterin in peripheral blood of patients for prognosis of the course and outcome of yersinia infection (YI). MATERIAL AND METHODS: Clinical examination and laboratory tests, 1-5-year follow-up were conducted in 68 patients with a cyclic, recurrent and lingering course of the disease. IL-6 and neopterin concentrations were measured in the course of the disease. RESULTS: Dynamic control over IL-6 concentrations in the blood of patients with yersinia infection provides information predicting the disease course and prognosis. Blood concentrations of neopterin are of help in prognostication of long-term outcomes of acute infection. CONCLUSION: Pathological syndromes forming in acute stage of yersinia infection are related to interleukin-dependent immunopathology.

[Immune status of patients with different outcomes of acute hepatitis C].

AIM: To ascertain parameters of immune response significant for prognosis of the disease outcome in patients with acute hepatitis C (AHC). MATERIAL AND METHODS: Seventy two examinees were divided into three groups: 30 patients of group 1 had AHC; 29 patients of group 2 had chronic hepatitis C (CHC) with the disease history 3-10 years; 13 AHC convalescents who had the disease 3-10 years ago. The control group consisted of 10 healthy subjects. The following parameters of immune status were studied: CD3+, CD4+, CD8+, CD16+, CD20+, HLA-DR+CD3+, CD95+, O-lymphocytes. RESULTS: By clinicobiochemical picture and the results of 12-month follow-up RT-PCR investigations of blood HCV RNA, AHC patients were divided into two groups: in 18 (72%) of 25 patients AHC became chronic, 7 (28%) patients achieved convalescence. The proportion and absolute number of subpopulations of lymphocytes CD8+, CD20+, CD16+ as well as CD4/CD8 in patients with AHC with different outcomes did not differ from the controls. AHC convalescents and patients with AHC transformation into chronic hepatitis had in the acute period of the disease much higher absolute number of subpopulations of the lymphocytes CD3+, CD4+, HLA-DR+CD3+ vs the controls. In patients with chronic hepatitis C with elevated levels of AlAT, 3-10 year follow-up registered significantly higher absolute amount of CD8+, CD3+, CD4+, HLA-DR+CD3+ vs the controls. Only patients with AHC which later became chronic had for 6 months of the disease significantly elevated absolute number of O-lymphocytes vs the controls. CONCLUSION: A high absolute count of O-lymphocytes in AHC patients may indicate probable development of chronic hepatitis C.

[The phospholipid spectrum of erythrocyte membranes in hemorrhagic fever with renal syndrome].

The subjects of the study--patients with severe hemorrhagic fever with renal syndrome--were divided into two groups: those who were on hemodialysis, and those who were not. The study included evaluation of the phospholipid spectrum of erythrocyte membranes in the acute period and during recovery. The results revealed conformational shifts in the structure of the bilipid membrane layer, which were maximal during the acute phase of the disease, as well as less prominent and varied changes in the phospholipid spectrum during recovery. This allows determination of the terms of rehabilitation of the patients and substantiates administration of membrane stabilizers as a part of complex therapy of residual syndrome.

[Nervous system involvement in hemorrhagic fever with renal syndrome].

The authors of the article describe two cases of hemorrhagic fever with renal syndrome (HFRS) with prevalence of signs of nervous system involvement. The first case was a 40-year-old woman with moderate HFRS, who developed Guillain-Barre syndrome of axonal-demyelinating polyneuropathy. An important observation was the absence of hemorrhagic or renal syndrome; combined therapy including plasmapheresis was successful. The second case demonstrated polymorphism of HFRS clinical manifestations with prevalence of neurological symptoms, which consisted in encephalopathy and no renal failure signs; hemorrhagic syndrome was moderate. In both cases the diagnosis was confirmed by elevated titer of antibodies to HFRS virus, belonging to the group of hantaviruses.

[Diagnostic significance of antibody detection to various hepatitis C virus antigens in patients with acute and chronic HCV-infection].

AIM: To examine diagnostic value of antibodies to various HCV antigens in patients with acute and chronic HCV-infection. MATERIAL AND METHODS: Enzyme immunoassay has tested blood sera from 136 patients with icteric acute hepatitis C (AHC) and 45 patients with chronic HCV infection for IgG antibodies to antigens of proteins core, NS4, NS5, HCV. Synthetic peptides core-16, NS4-20, NS5-23 were used as antigens. RESULTS: Patients with icteric AHC had IgG antibodies to antigens of both structural protein core and non-structural proteins NS4, NS5 of HCV as early as the first 10 days of jaundice. Occurrence of anti-core and anti-NS4 increases with the disease duration. Incidence of anti-NS4 correlated with duration of previous intravenous drug addiction. In patients with AHC early in the icteric period anti-core, anti-NS4, anti-NS5 were present less frequently than in patients with chronic HCV infection having elevated levels of AlAT. Significant differences were found neither with the group with normal AlAt nor in the spectrum of the detected antibodies between patients with acute and chronic HCV infection. CONCLUSION: Despite different frequency of anti-core, anti-NS4, anti-NS5 detection in patients with icteric AHC and patients with chronic HCV-infection and high AlAT, their high incidence rate in this or that group and absence of differences by the spectrum of the studied antibodies do not allow the fact of their detection to be a diagnostic marker differentiating acute HCV-infection with chronic one.

[Clinical and structural parallels in changes of the myocardial conduction system in diphtheria patients].

AIM: Comparison of the results of clinical device investigations of the heart with morphological autopsy evidence in diphtheria. MATERIAL AND METHODS: Hearts of 309 patients with diphtheria aged 25 to 80 years and 60 hearts of patients who died of diphtheria were investigated using ECG, echo-CG, doppler echo-CG, Holter ECG monitoring, biochemical tests of blood. Structural study of cardiac conduction included examination of the sinus-atrial node, atrioventricular node and bundle regarding the adjacent myocardium. RESULTS: Variants of structural-functional state of the myocardium and conduction system are explained by variability of the pathological processes which arose due to individual features of the conduction system structure and location, relationships with the myocardium. In acute diphtheria (day 1-10) dystrophic, necrobiotic and vascular disorders prevailed followed on days 11-30 by inflammation ending in myocardiosclerosis. Nodes and bundles of the conduction system are affected less frequently than the myocardium. CONCLUSION: A correlation exists between structural state of the cardiac conduction system and variants of clinical affection of the heart in diphtheria.

[Diagnosis and approaches to therapy of emotional disorders in patients with infectious diseases].

AIM: To examine psychological and emotional disorders in patients with infectious diseases, to specify indications for their pharmacological correction. MATERIAL AND METHODS: Clinicopsychological, clinicofunctional and laboratory tests were made to examine 30 patients with infectious mononucleosis (19 females and 11 males) and 30 patients with serous meningitis (16 females and 14 males) aged 16-35 admitted to hospital on the disease day 2-14. RESULTS: Shmishek's questionnaire revealed various types of personality accentuations with dominating hyperthymic (30%) and cyclothymic (20%). According to the data of the clinical scale SCL-90, the greatest number of cases with values over normal was in patients with serous meningitis. Beck's questionnaire revealed clinical depression in 12 patients (40%) with acute serous meningitis, subdepression in 14(46.7%) patients, severe depression in 6(20%) patients with infectious mononucleosis. In convalescence, emotional disorders persisted in 4 patients with serous meningitis. CONCLUSION: Affective disorders in the above patients require consultation of the psychiatrist to decide on psychopharmacotherapy inclusion in combined treatment of infectious diseases to prevent lingering course.
