[The role of hepatitis C virus RNA detection in various substrates of patients with chronic hepatitis C].

Puncture biopsy of the liver and blood count were made in 72 patients with chronic hepatitis C (CHC). Morphological alterations in the liver were assessed by Knodell index. The blood serum, lymphocytes and hepatic tissue were examined for a genome form of hepatitis C virus (HCV) RNA, blood lymphocytes and hepatic tissue--for a relevant replication form. HCV RNA was detected using "nested" RT-PCR. Only 26% patients had symptoms of asthenovegetative and dyspeptic syndromes. Normal alaninaminotransferase (ALT) level was observed in 24% patients, the rest had it high. HCV RNA was encounted more frequently in hepatic tissue than lymphocytes or serum (83, 68 and 46%, respectively). A replication form of HCV RNA was present in hepatic tissue of 31% patients and was absent in the lymphocytes. The incidence of the RNA detection was not related either to the disease symptoms or morphological alterations in hepatic tissue. The occurrence of the genome and replication forms in hepatic tissue does not correlate to ALT level. HCV RNA occurs more often in the serum, blood lymphocytes and in three substrates simultaneously in patients with hyperalatemia.

[Clinical and prognostic significance of disturbed global and regional contractility of left ventricle in diphtheria myocarditis].

Examination of 159 diphtheria patients diagnosed myocarditis in 64 of them. The latter were divided into 3 groups: with mild, moderate and severe myocarditis. The patients with and without diphtheria have undergone two-dimensional echocardiography with estimation of the asinergia index (AI) and left ventricular (LV) ejection fraction. Pronounced systolic dysfunction was revealed only in severe diphtheric myocarditis (DM). AI rose in moderate and severe DM. By the degree of AI elevation it can be judged about myocarditis severity and the disease prognosis. Regional LV contractility and LV systolic dysfunction were correlated. A group of patients with severe DM was identified who had a high risk of death in akinesia of LV segments (IA > 2) and lowering of LV ejection fraction below 35%.

[Immunostimulating activity of eubiotic bioflor in intestinal dysbacteriosis of various origin].

The data are reported on novel eubiotic biofor's effects on immune status of 213 patients with intestinal dysbacteriosis (39 surgical gastroenterological patients, 43 patients with diarrheal infections, 50 AIDS patients, 81 cancer patients exposed to radiation and chemotherapy). It was found that bioflor acts as an antidysbacteriosis drug and as an immunostimulator. It stimulates T-cell immunity, phagocytosis, B-cell immunity. Immunostimulating effect of bioflor depends on dysbacteriosis degree and is maximal at its first stages.

[Emotional disturbances in patients with infectious mononucleosis and serous meningitis].

30 patients with infectious mononucleosis (19 females and 11 males) and 30 ones with serous meningitis (16 females and 14 males) aged 16-35 years admitted to hospital on the disease day 2-14 were examined clinicopsychologically, clinicofunctionally and using laboratory tests. The examination has revealed emotional disturbances in 40% of patients with serous meningitis and 20% of those with infectious mononucleosis. Clinical manifestations were characterized by polymorphism, combination of somatovegetative and anxiodepressive disorders. Personality accentuations in the patients are described.

[Viral hepatitis C and pregnancy].

AIM: To evaluate course and outcomes of hepatitis C in gravidae; introduction of optimal management of pregnancy, postpartum period in mothers and newborns in viral hepatitis C. MATERIAL AND METHODS: Case histories, general and obstetric statuses, obstetric and extragenital pathology, pregnancy course and outcomes were studied in 76 pregnant women with viral hepatitis C. RESULTS: A complicated course of pregnancy was observed in 38.2, 13.1 and 24.3% of women in trimester I, II and III, respectively. Therapeutic policy of removing symptoms of the pathology with consideration of hepatitis C severity and stage allowed to keep pregnancy in all the women. CONCLUSION: Hepatitis C virus can complicate pregnancy but the latter does not aggrevate the course of hepatitis C.

[Central hemodynamics and portal-hepatic blood flow in patients with acute and chronic virus hepatitis].

AIM: To characterize central hemodynamics and portal-hepatic blood flow in patients with acute and chronic viral hepatitis. MATERIAL AND METHODS: Ultrasound investigation, tetrapolar rheography, hepatic scintigraphy were made in 149 patients with acute and chronic viral hepatitis B, C and B + C. RESULTS: The above patients had disturbances in central hemodynamics manifesting with developing myocardiodystrophy and hyperkinetic hemodynamics syndrome; decreased intensity and increased amplitude of respiratory fluctuations of volumic hepatic blood flow; impairment of peripheral blood flow. CONCLUSION: The above changes were most prominent in patients with chronic hepatitis B + C.

[Clinical and instrumental diagnosis of cardiac affection in patients with toxic diphtheria].

AIM: To specify myocardial affection in adult patients with toxic diphtheria (TD). MATERIALS AND METHODS: Along with routine clinical examination 150 patients with toxic diphtheria underwent ECG, echo-CG and biochemical tests. 22, 81, 26, 14 and 7 patients had subtoxic TD, toxic form of the first degree, second degree, third degree and hemorrhagic TD, respectively. RESULTS: Myocardial impairment in TD develops very early, but is diagnosed in different time depending on the diagnostic technique. Early signs of myocardial involvement are activation of cardiospecific enzymes and echo-CG changes. Poor myocardial prognosis in TD is predetermined by progressive cardiac failure (primarily right ventricular), marked conduction disorders, diminished contractility, hyper- or hypoactivity of cardiospecific enzymes. CONCLUSION: The knowledge of myocardial affection variants in TD enables the clinician not only to predict the disease course but to conduct more adequate therapy resultant in less numerous lethal outcomes of toxic TD.

[The clinical manifestations and treatment of shock in acute intestinal infections].

Shock (hypovolemic and infectious-toxic) is a severe complication of acute intestinal infections. Of 35096 patients with acute intestinal infections (90.6% had food poisoning, 9.4% had salmonellosis), 24 (0.07%) patients developed hypovolemic, 26 (0.08%) infectious-toxic shock. Hypovolemic shock is based on decompensated dehydration, infectious-toxic one arises from generalized circulation disorders with severe affection of hemostasis. Hypovolemic shock was effectively managed with polyionic crystalloid solutions. In infectious-toxic shock hemodynamic and homeostatic defects should be corrected. In spite of intensive therapy, lethality in infectious-toxic shock in patients with acute intestinal infections reached 80.8%.

[The use of leukinferon in treating measles in adult patients].

The following mucosal are characteristic of measles in adult patients: dryness in the oral cavity, dryness, desquamation, and fissures in the red edge and in the mouth corners, edema and hyperemia, of the buccal mucosa, macular enanthema on the hard and soft palate, Bel'skiĭ-Filatov-Koplik spots, whitish deposit on the gingiva, edematous and bleeding gingiva, and, as a result of general intoxication, a lingual deposit. Leukinferon, a wide-spectrum immunocorrective drug, was used to improve the efficacy of treatment of adult patients with measles; besides alpha-interferon, it contains other cytokines of the first phase of immune response. Therapy with leukinferon led to a sooner improvement of the general status and a more rapid regression of the disease symptoms, including changes in the buccal mucosa, in comparison with the control leukinferon-untreated group.

[Diphtheria of the oral mucosa].

High incidence of diphtheria became a pressing problem in recent years. The age structure of patients has changed. A specific feature of diphtheria in adults is a high share of toxic forms and a higher incidence of grave combined forms and rare localizations of the disease focus. The oral cavity proper is one of rare localizations of this infection. The authors examined 12 patients in whom the process was localized in various parts of the buccal mucosa: on the upper and lower lips, on the hard and soft palate, on the cheeks and tongue. Morphologic disturbances presenting as necrosis and inflammatory infiltration were detected in sites involved in the diphtheritic process. Changes whose manifestation was related to the diseases severity were found in zones adjacent to the foci. The authors come to a conclusion on a higher incidence of heretofore rare localizations of diphtheria than previously, this making difficult the diagnosis and treatment of the disease.

[Acute intestinal infections and chronic alcoholism].

153 chronic alcoholics were given treatment against food poisoning. 55 of them developed withdrawal syndrome, 98 had alcoholic psychosis including 88 with delirium tremens. The acute intestinal infection in these alcoholics was managed with rehydration using polyionic crystalloid solutions under controlled diuresis. Previous relief of central excitation contributed much to a positive treatment outcome.

[Intestinal dysbacteriosis in yersiniosis patients and the possibility of its correction with biopreparations].

Intestinal dysbacteriosis was found in all 37 patients with intestinal yersiniosis or pseudotuberculosis. Administration of biopreparations containing lacto- or bifidobacteria produced a good clinico-microbiological response rate (above 90%). The authors think it valid to include biopreparations in combined treatment of yersiniosis to prevent unfavourable outcomes.
