[Neuropsychological evaluation of long-term therapy of Alzheimer's disease using different cerebrolysin dosages].

An open comparative randomized clinico-neuropsychological study of 4 cerebrolysin treatment courses was conducted during 19 months. The differences in long-term effects of different medication dosages (10 and 30 ml) were revealed. The higher cerebrolysin dose was more effictive for cognitive functioning of patients. In patients receiving a dosage of 10 ml, the disease progress was significantly more pronounced. The results obtained indicate that a course of cerebrolysin treatment in higher dosages significantly inhibited neurodegenerative process.

[ApoE genotype and efficacy of neurotrophic and cholinergic therapy in Alzheimer's disease].

Correlation association between an ApoE4 genotype in patients with mild-moderate Alzheimer's disease and efficacy of neurotrophic (cerebrolysin) and cholinergic (exelon) therapy was studied in the groups of patients formed using case-control method. A 4-month treatment has shown that both types of therapy had a significant clinical effect, however clinical effect proved to be more higher and stable in patients treated with cerebrolysin. A number of responders in the cerebrolysin group was 1.7-fold higher comparing to that in the exelon group. Patients with the ApoE4(+) genotype did not differ in response to either drug but in those with genotype ApoE4(-) the number of responders was 3-fold higher in the group treated with cerebrolysin compared to the group given exelon. A follow-up estimation of cognitive impairment in ApoE4(-) patients revealed that long-term clinical effect of cerebrolysin treatment was 6.5 times higher than that of exelon.

Content of autoantibodies to bradykinin and beta-amyloid(1-42) as a criterion for biochemical differences between Alzheimer's dementias.

We measured serum content of autoantibodies to beta-amyloid protein Abeta(1-42), its neurotoxic fragment Abeta(25-35), vasopressin, bradykinin, thrombin, antithrombin III, alpha(2)-macroglobulin, and angiotensin II in patients with various forms of Alzheimer's dementias, including presenile and senile dementias of the Alzheimer type. The ratio of antibradykinin and anti-Abeta(1-42) autoantibody contents differed by 39% in these patients. Our results can be used for the development of a new biochemical method for differential diagnostics of dementias of the Alzheimer type.

[Tissue plasminogen activator and plasminogen activator inhibitor gene polymorphisms: possible relation to atherosclerosis and its complications].

In the present review the dates about association of genetic variants of a plasminogen activator inhibitor type 1 (PAI-1) and tissue plasminogen activator (tPA) with ischemic heart disease and atherosclerosis of other localization are toted. The singularities of synthesis of PAI-1 and tPA, possible paths of regulation of their activity briefly surveyed. Known polymorphisms of PAI-1 gene and tPA gene, their localization, frequencies in different ethnic groups are circumscribed. In operation the genetic influence of polymorphisms of PAI-1 gene and tPA gene on synthesis and plasma concentration of PAI-1 and tPA, including under an operation of risk factors of atherosclerosis, and as possible clinical consequences of sectional violations is featured. In the review the possible role of polymorphisms of PAI-1 gene and tPA gene in a pathogenesis of atherosclerosis, association with hazard thrombotic complications for different diseases is considered. Many clinical examinations are circumscribed which have confirmed or not confirmed a possible connection of genetic markers with development and current of ischemic heart disease and other vascular diseases. We considered that there are no enough dates to one valued estimation of possible role of polymorphisms of PAI-1 and tPA genes in pathogenesis of atherosclerosis. It is necessary to get new genetic studies in groups of patient with high risk of ischemic heart disease and atherosclerosis.

Autoantibodies to beta-amyloid and neurotransmitters in patients with Alzheimer's disease and senile dementia of the Alzheimer type.

The content of autoantibodies to beta-amyloid protein Abeta(1-42), its neurotoxic fragment Abeta(25-35), and neurotransmitters were studied in the blood of patients with presenile Alzheimer's disease and senile dementia of the Alzheimer type. Significant differences in the relative content of autoantibodies to Abeta(1-42)and autoantibodies to biogenic amines were demonstrated. These results can be used for the development of a biochemical method for differential diagnosis of Alzheimer dementias.

Association between angiotensin-converting enzyme and Alzheimer disease.

BACKGROUND: Angiotensin-converting enzyme has been reported to show altered activity in patients with neurologic diseases. An insertion-deletion polymorphism in ACE has recently been linked to heart disease, cerebrovascular disease, and AD. OBJECTIVE: To determine whether the angiotensin-converting enzyme (ACE) is associated with risk of Alzheimer disease (AD). METHODS: We investigated the ACE polymorphism as a potential risk factor for AD in 151 patients with AD and 206 ethnically matched controls from Russia and in 236 patients with AD and 169 controls from North America by means of allele association methods and logistic regression. RESULTS: None of the ACE genotypes was associated with increased susceptibility to AD in the total sample or in subsets stratified by apolipoprotein E gene (APOE) epsilon4 status. However, the D allele was more frequent among AD cases between ages 66 and 70 years compared with controls in both the Russian (P = .02) and North American (P = .001) datasets. In this age group, the effect of D (odds ratio, 11.2; 95% confidence interval, 2.9-44.0) appeared to be independent of and equal or greater in magnitude to the effect of APOE epsilon4 (odds ratio, 7.8; 95% confidence interval, 3.5-7.4). CONCLUSIONS: Our results suggest that APOE and ACE genotypes may be independent risk factors for late-onset AD, but the ACE association needs to be confirmed in independent samples in which the time and extent of vascular cofactors can be assessed.

[Certain biochemical blood markers in the process of cholinergic therapy in Alzheimer's disease].

Acetylcholinesterase (AChE) activity and parameters of the system of regulation of lipid peroxidation (LPO) were estimated in blood of patients with Alzheimer's disease (AD) during therapy with amiridine and gliatiline. It was found that the therapy was accompanied by inhibition of AChE activity. A significant correlation was observed between clinical efficiency and changes of AChE activity. AD was characterised by essential changes in LPO parameters: the level of the primary products of oxidation was increased three times with a sharp increase (seven times) of total unsaturation of lipids. A significant correlation was found between AChE activity and the level of the primary products of oxidation in blood erythrocytes of AD patients before and after therapy with amiridine and gliatiline.

[Akatinol memantin in Alzheimer's disease: clinico-immunological correlates].

16 patients with Alzheimer's disease were treated with akatinol memantine. The levels of production of interleukin-1 (IL-1) in lymphocyte culture were evaluated both before and after the treatment that lasted for 2-4.5 months. Before the therapy a significant (p < 0.05) increase in the production of IL-1 was observed in patients with mild and moderate dementia, with early onset of the disease and with the duration of the disease up to 9 years as compared with a control group (24 healthy individuals). After the treatment the production of IL-1 was decreased in patients with moderate dementia, with early onset, and with hereditary form of the disease. Decrease of the level of IL-1 production after the treatment corresponded to improvement of the cognitive functions of the patient. In cases of unsatisfactory treatment (without any improvement of a clinical state) IL-1 production was at higher levels (p < 0.05) as compared with the control values. It is suggested that initial high level of IL-1 production could serve as a predictor of efficiency of therapy with akatinol memantine in patients with early onset of Alzheimer's disease.

[Quantitative EEG in early and differential diagnosis of mild dementia of different genesis].

Quantitative EEG was used to reveal the specific features of its amplitude-frequency parameters and topography in patients with mild dementia of different genesis versus healthy elderly individuals. All four study groups of patients differ in EEC relative spectral density, they also differ from the healthy persons while alpharhythm was suppressed in Alzheimer's disease (AD) and vascular dementia (VD) and there was slower alpha-rhythm and higher theta-activity in senile AD (SAD). Patients with VD were characterized by desynchronized EEG. Factorial and stepwise discriminant analyses of EEG parameters showed that the control group greatly differed from patients with mild dementias. Patient groups mainly with atrophic atrophic AD and SAD or vascular (VD and mixed) dementia were also different whereas the mixed vascular-atrophic group is intermediate in EEC parameters between the VD group and the partially overlapped AD and SDA patient groups. Quantitative EEC data may be thus used for differential diagnosis to optimize therapy and prognosis even in mild dementia.

[Brain dysfunction in subjects at risk of Alzheimer's disease].

Visual evoked potentials versus flash (VEP) and brain DC-potentials were studied in the first-order relatives of patients (mean age 42.6 +/- 1.6 years) with Alzheimer's disease (AD) and probands (mean age 60.9 +/- 0.9 years) and age-matched healthy controls for the relatives and probands. In the relatives of AD patients, the latencies of N2, P3, N3 components were found to be delayed as compared with the controls. There was a delay in the latencies of the same components in the patients and their relatives, but it was smaller in the relatives of AD patients. The brain DC-potentials were higher in the relatives of AD patients than in the controls. Neurophysiological changes in the relatives of AD patients may be considered to be signs of latent neurodegeneration in the limbicoreticulocortical pathways, which may be associated with the excitotoxic processes and abnormal hyperactivity in the limbicoreticulocortical structures.

[Analyzing correlation between neurophysiological parameters and level of stress hormone cortisol in aging].

A correlation between the serum levels of the stress hormone cortisole, neurophysiological parameters, and age was examined in 23 elderly healthy examinees (whose mean age was 65.7 +/- 2.1 years). The latency of the components of visual evoked potentials (VEP) was found to progressively increase with age. The individuals having high cortisole levels showed an increase in the latency of late VEP components associated with accelerated ageing and/or with the degenerative changes in the brain limbicoreticulocortical pathways. The DC potential level (DCPL) reflecting the intensity of brain energy expenditures correlated with the content of cortisole. Increased DCPL and longer latency of late VEP components were interrelated. It is suggested that high cortisole level produces a neurotoxic action and promotes accelerated ageing of brain regions having glucocorticoid receptors. Imbalance between the significant intensification of catabolic processes, the rise of energy expenditures and the limited increase in anabolism in elderly age is a course of this process.

[The influence of cerebrolysin on the efficiency of subsequent therapy with amiridine++ in Alzheimer's disease patients (neuropsychological investigation)].

The paper presents a study concerning an influence of cerebrolysin on the efficiency of subsequent therapy with amiridin in patients with mild and moderate dementia of Altzheimer's type (DAT). A study included 2 groups of patients similar in terms of clinical and demographic parameters. The patients of group 1 (23 cases) were treated by amiridin in daily dose of 80 mg during 10 weeks. In group 2 (26 patients) a session of 20 intravenous infusions of cerebrolysin (30 ml in 150 ml of physiologic solution during 4 weeks) preceded the same therapy with amiridin. The neuropsychological assessment was made according to Luria's method with a quantitative estimation of the regulatory and operational components of mental activity. Comparison of the results of neuropsychologic examination both before and after the therapy showed a more pronounced improvement in the states of all regulatory and separate operational components of mental activity in combined therapy with cerebrolysin and amiridin versus monotherapy with amiridin. In patients of group 2 with disturbances of higher mental functions the pathological symptomatology connected with anterior frontal and deep structures of brain was significantly decreased as compared with the patients from group 1.

[Correlation between functional activity of lymphocytes in patients with Alzheimer's dementia and their response to therapy].

16 patients with dementia of Alzheimer's type (DAT) were examined. Functional activity of lymphocytes was determined according to proliferating activity of T-lymphocytes, production of Interleukin-1 (IL-1) and estimation of subpopulations of T-leukocytes (T-helpers, T-suppressors). The patients were observed both before and 1-2.5 months after amiridin therapy. Considerable reduction of both IL-1 production and proliferating activity of T-lymphocytes was observed after the treatment in 8 patients of 11. Their cognitive functioning after the therapy was either improved or stable by the moment of repeated observation. It is quite important that initial functional activity of lymphocytes was considerably higher in this group of patients as compared with the age-matched controls. Change for the worse in cognitive functions by the moment of the second examination was accompanied in 5 patients with an increase in IL-1 production as compared to the initial level. It is proposed that high level of IL-1 production by blood lymphocytes may be used as a criterion in selection of DAT patients for amiridin therapy.

[System of natural killer lymphocytes in Alzheimer's type dementias].

Cytotoxic activity and quantity of natural killer (NK) lymphocytes were measured in 25 patients with Alzheimer's disease (AD), in 13 patients with senile dementia of Alzheimer's type (SDAT) and in 31 normals of different age (controls). In mentally healthy elderly individuals of the same age as the patients, functional activity of a subpopulation of NK lymphocytes was significantly decreased in comparison with healthy young people, meanwhile a number of these cells wasn't changed. In patients with AD and SDAT the tendency was found to increasing of functional activity of NK lymphocytes in comparison with elderly normals, which achieved the degree of significant differences only in SDAT patients with moderate dementia. Direct dependence of cytotoxic activity of NK lymphocytes on AD duration was determined. Such correlation was not observed in SDAT. This permitted to suppose a destructive influence of lymphocytes with increased cytotoxic activity on either CNS cells or their components in AD.

[The immune system characteristics during normal aging and in Alzheimer's disease].

The paper presents a review of data available in the literature and the authors' own findings on age-related physiology and pathology of the immune system. Almost all parameters characterizing the functional activity of the immune system in Alzheimer's disease (AD) were demonstrated to significantly differ from those in the healthy elderly, The magnitude of immunological changes clearly correlated with the severity of dementia in AD patients. A hypothesis of the gradual development of immunological changes in an AD patient in relation to his dementia formation is given.

[The neuroimmune relationships during normal aging and in Alzheimer-type dementias].

Relationships between neurophysiological and immune parameters were studied in normal middle-aged and elderly subjects and in patients with dementia of Alzheimer's type (DAT). The rate of such interaction was minimal in normal aging, in DAT neuroimmune relations increased. The correlation between the immune parameters and the characteristics of components of visual evoked potentials and auditory brain-stem responses suggests that in the middle-aged healthy subjects, a primary role in neuroimmune relations belongs to limbicoreticular structures and to the mesencephalon. In normal aging, the influence of limbicoreticular structures on immune structures decreases. In DAT, the modulation of immune processes by limbicoreticular and thalamocortical structures, as well as the pontobulbar part of the brain stem increases. In middle age and in DAT, persons with different functional brain asymmetry (as estimated from electrophysiological criteria) have different lymphocytic functional activities and rates of neuroimmune interactions.

[Changes in the immunological parameters in Alzheimer's disease: their relation to disease severity].

The peculiarities of the immune system in patients with Alzheimer's dementia (AD) and their relations with some clinical parameters were investigated. 21 patients with AD aged 51-70 and 16 age-matched healthy subjects were included in this study. The following parameters were examined: 1) total lymphocyte numbers, 2) the functional activity of T- and B-lymphocytes under conditions of stimulation by nonspecific mitogens phytohemagglutinin (PHA) and pokeweed mitogen (PWM) and 3) the production of interleukin-1 (IL-1) and interleukin-2 (IL-2). A statistically significant decrease (p < 0.01) in lymphocyte number and significant increase of proliferative activity of T-lymphocytes, stimulated by PHA (when the small mitogen doses in the range of 1.25-2.5 mg/ml were used) were found in patients with moderate and severe dementia (p < 0.05, p < 0.01). No significant changes of lymphocyte proliferative activity were detected in patients with mild dementia. Some tendency for higher IL-1 production was found in AD patients, but it was not statistically significant. No changes of functional activity of B-cells and of production of IL-2 in AD patients were found in comparison with the control group.
