Study of the role and function of the right heart and the peculiarities of their remodeling in a range of diseases (heart valvular diseases, pulmonary thromboembolism and its complications, cardiomyopathy, combined cardiorespiratory pathology, heart failure, essential hypertension etc.) is of great scientific and practical interest. An optimal right ventricular (RV) examination should be precise, reflect its function objectively, be feasible in everyday clinical practice, and provide accurate prognosis. The last decade has been marked by search for possibilities to evaluate the systolic and diastolic R Vfunction, especially after clinical introduction of Doppler EchoCG. However, publications dedicated to the protocol of EchoCG study of the RV, the normal values of its structure and function, and their clinical interpretation are still rare. The authors of the present work analyze studies on the investigation of the R V and offer a protocol with a method of calculation of EchoCG parameters and their reference values concerning the right heart.

AIM: To analyse clinical and cost efficacy of 6-year (1999-2005) activity of the outpatient "Hypertensive Patient School" (HPS) at the consultative and diagnostic center N 52 of the RF Ministry of Defense. MATERIAL AND METHODS: A total of 232 hypertensive patients followed up for 2 years were divided into two groups: 120 patients of group 1 participated in the education antihypertensive program at HPS and received combined therapy according to chrono = pharmacology; 112 patients did not attend the HPS and were treated conventionally. RESULTS: The lectures at HPS raised awareness of the patients about their disease and its risk factors from 45.8 to 100% (p < 0.05), compliance from 71.7 to 99.2% (p < 0.001), treatment adequacy from 15.8 to 88.3% (p < 0.01), improved quality of life from 13.3 to 95.0% (p < 0.001), finally resulting in reduction of hypertensive crises and hospitalizations from 32.5 to 2.5% (p < 0.01), side effects frequency from 30.8 to 3.3% (p < 0.05). The cost of outpatient management of one patient fell by 3661.0 roubles. CONCLUSION: Learning at HPS prevents complications of essential hypertension, promotes adequate control over systolic and diastolic pressure, improves quality of life, raises cost efficacy of the disease therapy.
Training of hypertensive patients is a widely disputable problem. It is shown that education of hypertensive patients and population at risk can effectively prevent hypertension both at population and individual levels. The authors believe that programs designed for the education of hypertensive patients and population have marked drawbacks as they lack consistency, vary in strategy (group or individual learning), methods of teaching (interactive multimedia presentation of the material, self-education according to certain recommendations, video- and audio materials), evaluation of short- and long-term effects (6 and 24 months), patients management, etc. This explains the necessity of further studies into the problem of the approaches to education of the patients and population concerning hypertension.
The authors propose an original program of learning in the school of a hypertensive patient (SHP) which includes 12 sixty-minute lessons devoted to causes of the disease, cardiovascular complications, methods of their prevention at treatment, and rehabilitation. The analysis of the results of 2-year follow-up showed that most of the learners became informed about the risks of hypertension, better followed recommendations of their doctor, improved their life quality. The number of hypertensive crises and urgent hospitalizations decreased significantly. In the control group of 112 patients no improvement was seen in the level of blood pressure, quality of life and prognosis. Moreover, learning in the SHP proved highly cost-effective.
Approaches to home self-monitoring of arterial pressure (AP) are reviewed. Such monitoring is cost effective, facilitates adjustment of antihypertensive treatment, assessment of the response. The perspectives of AP self-monitoring lie in learning at "schools for hypertensive patients" that promotes control of hypertension using regular adequate treatment; in design of standards for such monitoring, in perfection of its techniques; in application of monitoring results in cardiovascular research.
The authors present the results of experimental and clinical trials of recombinant genes coding the synthesis of endothelial growth factors VEGF, bFGF. Single transendocardial administration of these genes' DNA into the zone of hybernating myocardium in some patients with CHD caused in 3-6 months significant increase in perfusion, left ventricular ejection fraction, decrease in stenocardic pang rate, increase in exercise tolerance. The marked clinical effect was achieved when these drugs were introduced into ischemized tissue in patients with obliterating atherosclerosis of lower extremity arteries. Development of the methods for studying endothelial function available for wide clinical practice will permit to raise the efficiency of initial and secondary prophylaxis of arterial hypertension, CHD. Today the evaluation and correction of endothelial dysfunction is the new and the most perspective direction in cardiology development.

Lipid peroxidation (LPO) and antioxidant system (AOS) were examined in 40 inpatients with chronic obstructive bronchitis (COB) on conventional treatment. The above parameters were considered in relation to clinical picture. It is shown that inflammation in the bronchial tree is associated with AOS functional deterioration and marked LPO intensification. A correlation was established between LPO activity, concentration of alpha-tocopherol in red cell membrane and degree of bronchial obstruction as well as remission quality. Persistent high intensity of LPO is suggested as an underlying cause of the disease transition to chronic form and progression of bronchial obstruction. Basing on the above observations, the authors insist on antioxidants introduction in COB treatment.
Within 1979-1989 the authors treated 26 wounded soldiers and officers with brain injury inflicted by mine explosion. 5 patients were in severe condition, 18 in extremely grave and 3 in critical condition. The patients had concomitant blast injuries and contusions. Pulmonary complications observed in such patients compused to DIC and respiratory distress syndromes running in the presence of marked metabolic disorders, hypoxia and intoxication. It is thought important that conservative therapy of respiratory disorders in patients with brain traumas were not constricted with correction of bronchial obstruction. The additional intensive treatment of the above syndromes is needed including extracorporeal detoxication (plasmapheresis) and hyperbaric oxygenation.
Lipid peroxidation (LPO) and antioxidant system (AOS) were investigated in 40 patients with chronic obstructive bronchitis (COB) in relation to clinical features of the disease. Bronchial tree inflammation was associated with reduced AOS function and intensification of LPO which reached a new high level. LPO activity, tocopherol concentration in red cell membranes and the severity of respiratory obstructive syndrome were correlated. It is suggested that high LPO activity may underlie the disease chronicity and development of progressive irreversible bronchial obstruction. Antioxidant drugs are believed necessary for COB patients not only in acute, but also in regression and remission phases of the disease.
