[Malignant pheochromocytoma with remarkable response to CVD chemotherapy--a case report].

A 60-year-old man was admitted to our hospital with the chief complaint of a left upper abdominal mass. 131I-MIBG scintiscan revealed a left upper abdominal mass, and the patient's serum catecholamine level was very high. Our diagnosis was pheochromocytoma of the left adrenal gland. On Oct. 19, 1995, a left nephrectomy and adrenectomy were performed. The serum catecholamine level then decreased and blood pressure was normalized. Two years later, it became difficult to control the patient's hypertension, and multiple metastatic liver cancer was found by an abdominal CT scan. The diagnosis of malignant pheochromocytoma was confirmed by the accumulation of 131I-MIBG in the liver. We started CVD chemotherapy. After 10 cycles of this chemotherapy, the serum catecholamine level was almost normalized and the metastatic liver cancer was reduced to one-third in size.

[Clinical study on adenocarcinoma of the prostate].

A clinical study was performed on 75 patients with untreated prostatic cancer who had been hospitalized over a 5-year period. The age range was 52 to 90 years (median age: 72.3 years). The main complaint was dysuria in 73% of the cases. Thirty-two (43%) patients were smokers, and complications included bladder cancer in two and rectal cancer in one. The stages were T1 in 18 cases, T2 in 10, T3 in 29 and T4 in 18, 28 cases were G1, 29 were G2, 11 were G3 and seven were G4. Among the tumor markers, PA sensitivity was the highest, but all markers showed high sensitivities in the T3 and T4 cases. Endocrinological treatment was performed on 74 patients. Survival rates showed significant differences only for the T and M classifications; no significant differences in survival rates according to the G classification, age of the patient or tumor markers were observed. Radiotherapy was suggested to be related to improvements in the prognosis only in patients with histological grading of G2.

[Three cases of double cancer: genitourinary and lung cancers].

Three patients who had genitourinary cancer combined with lung cancer are reported. One patient had pulmonary adenocarcinoma and bladder transitional cell carcinoma, one had small cell lung cancer and squamous cell carcinoma of the penis, and one had large cell lung carcinoma and prostatic adenocarcinoma. In Japan, where society is aging rapidly, cases of multiple primary cancer with lung and genitourinary cancers are expected to increase considerably in the future.

[Esophageal cancer metastatic to the kidney: report of a case].

On December 12, 1988, 51-years-old man was admitted to our clinic with asymptomatic gross hematuria. He had been undergone total esophagectomy for esophageal cancer in the surgery service of our hospital on June 22, 1987, and histopathological diagnosis was squamous cell carcinoma, Stage III, a2, n(-), m0, pl0, ly(+), V(+). X-ray CT revealed a irregular, obscure bordered tumor image in the left kidney and angiography showed a hypovascular tumor. On December 20, nephrectomy was performed. Histologically, it proved to be squamous cell carcinoma metastasized from esophageal cancer. The present case is the 11th of those cases which have been clinically diagnosed as renal metastasis of esophageal cancer in Japan. An extremely small number of clinically reported cases compared with autopsy cases are thought to be ascribable to scarce detectability while living of such metastasis, but not to its rare occurrence. The coming diagnostic improvement may well make such case more and more increasing.

[Renal cancer with bone metastasis as initial symptom].

With a progress of image diagnosis method asymptomatic renal cancers have much chance to be disclosed, but renal carcinoma of which symptoms were caused by metastatic lesion are discovered as an initial symptom has not been decreased. In this paper we reported additional 4 patients who visited our hospital with an initial symptom of metastatic bone pain and were diagnosed that the primary lesion was the kidneys. We further conducted a follow-up study on 4 patients reported previously and discussed on renal cancer with bone metastasis as the initial symptom. Case 1 was a 51-year-old man with lumbar metastasis, case 2 was a 34-year-old woman with vertebral metastasis, case 3 was a 77-year-old man with femur metastasis and case 4 was a 65-year-old woman with femur metastasis. In three of the 4 patients except for case 1, extirpations of the primary lesions were performed. Case 3 and 4 received internal fixation for metastatic lesion in bone. In occasionally four cases, the prognosis was relatively favorable even in advanced renal carcinoma with bone metastasis, so surgical procedures should be made positively for relief of pain in metastatic lesion, maintenance of function and prevention of bone fracture.

[Three-dimensional image display of the kidney from X-ray CT scan utilizing the 3DCT method].

Stereographic reproduction of human organs, now under development as an application of computer graphics, aims at delineating the three-dimensional spread of a lesion, simulating an operation and drawing up a radiotherapeutic plan. We are currently attempting to obtain a stereo display of the kidney using a 3DCT method based on X-ray CT data obtained by scanning a 4 mm slice in cases of bilateral renal calculus of the template type, renal bleeding and renal carcinoma. This 3DCT method was capable of displaying a stereo image of the kidneys relatively with ease since it made analysis possible, using a computer built inside the X-ray CT device. The development of a software program capable of stereographically displaying many organs on the same screen is an awaited future project.

[A trial of digitization and digital analysis of analog clinical images of bladder cancer].

Owing to the recent developments in computed radiography and the picture archiving and communication system, all clinical images of bladder cancer can now be digitized, and recording, archiving, transportation and analysis of these images is becoming popular. We analyzed various kinds of analog images of bladder cancer after they were digitized via a simple image analyzer. The image analyzers we utilized were very useful for us to understand and become familiar with digitization of analog images of bladder cancer as well as digital analysis.

[A trial of digitization and digital analysis from analogue clinical images of the renal cancer].

The advancement in image diagnostic methods has been great in recent years and image diagnosis has become indispensable for the diagnosis of renal and other cancers. However, the clinical images are fixed and recorded on film as analog images for storage and the expansion of storage space, the poor efficiency in arrangement and control and the inconvenience of transfer have become big problems. Digitization of the clinical images offers the advantages of solving these problems and of allowing a variety of image analyses to be made easily. The tendency is for the digitization of all images for medical use in the near future. We applied a handy image analysis system (Shonic GA) using a personal computer as the mainframe and tried to digitize the analog images of a pyelogram, DSA, ultrasonogram, and X-ray CT of renal carcinoma for digital analysis. The results were displayed. This system is handy and easy to operate and thus is useful for the purposes of understanding and gaining experience in digitization and digital analyses of images for medical use.

[A case report of renal cancer detected unexpectedly by X-ray CT during observations in the clinical course of cervical carcinoma].

The case is a 55-year old woman whose mother died of gastric cancer. In 1984 she underwent radiotherapy for cervical squamous cell carcinoma stage IIIb and had since been under the periodical observations. In 1987 she happened to undergo abdominal X-ray CT scans, and with an image of tumor mass revealed in the right kidney, she was admitted to our hospital. A clinical diagnosis of the right renal cancer was made, and radical nephrectomy was performed. Histological diagnosis was renal cell cancer pT2N0M0. This is the 7th reported case among the cases of double cancer of the cervix and the kidney in Japan. Multiple primary malignancies consist of these two cancers, and the renal carcinoma as an incidental finding with image procedure were discussed.

[Case report of a patient with advanced pelvic and ureteral cancer whose primary lesion could be enucleated after cytoreduction by modified CAP regimen].

A 52-year-old man got right renal pelvic and ureteral carcinoma involving metastases in lung, and para-aortic and supraclavicular nodes, he underwent courses of combination chemotherapy consisting of cyclophosphamide, adriamycin and cisplatinum, including bleomycin. The initial response was encouraging with some regression of the primary tumor and pulmonary metastases. Accordingly, the patient then underwent extirpation of the primary cancer. Two weeks after the surgery, relapse of the lung metastatic lesion was observed, and the lesion was refractory to the chemotherapy. This investigation reports a case and discusses the pre-and-postoperative chemotherapy for upper urothelial carcinoma.

[A case of renal cancer complicated with acute hypercalcemia and acute pancreatitis].

A 55-year-old man who had liver metastasis after undergoing surgery for renal cancer was hospitalized immediately on May 4, 1987 with complaints of general malaise, epigastric pain, nausea and vomiting. Because of abnormally high levels of blood calcium 15.6 mg/dl and serum amylase 2,069 IU/l, the case was diagnosed as hypercalcemic crisis and acute pancreatitis. Following recovery from the critical stage with administration of elcatonin and FOY, therapy for cancer initiated. We report the clinical course of this patient and discuss about hypercalcemia and acute pancreatitis as cases of oncologic emergency.

[Two cases of renal cancer detected by ultrasonic scan during mass screening and subsequent MRI].

Reported herein are the cases of two woman, aged 47 and 69, respectively, who were each found to have a right renal carcinoma, both carcinomas discovered incidentally by an abdominal ultrasonic scan during a mass screening, with no other signs or symptoms of renal cancer being indicated. Following clinical diagnostic verification, using X-ray CT, and an MRI, radical nephrectomies were performed. In each instance, the pathohistological diagnosis was renal cell carcinoma, pT2N0M0V0. The clinical imagings, including the MRI, are presented, and the diagnostic techniques used in this imaging and the screening modality in cases of asymptomatic renal cancer are discussed.

Breast metastases of prostatic carcinoma: immunohistochemical case study.

A breast mass developed in a patient receiving estrogen therapy for prostatic cancer. The prostate tumor was adenocarcinoma of small acinar type, whereas that of the breast was infiltrating medullary adenocarcinoma. Histological features of the two tumors differed and double cancer was suspected by conventional pathological study. However, immunohistochemical staining with prostatic specific antigen and prostatic acid phosphatase was positive in each tumor. These results indicate the breast tumor to be a metastasis from the prostatic cancer.

Immunohistological study of two malignant lymphomas found as testicular tumors.

Immunohistological studies of two testicular lymphomas are reported. In the first case, lymphoma cells bear T-cell markers such as Leu-4 and Leu-3a, while they do not react with antibodies against B-cell markers such as anti-Leu-14. In the second case, lymphoma cells do not react with anti-T or anti-B cell antibodies, but react with antibodies against B-4, OKT9, and C 13 (anti-HLA-DR). Thus, it is shown that the first case is a lymphoma of helper/inducer T-cell origin and the second case is a pre-B-cell or pre-pre-B-cell lymphoma.

Inverted papillary carcinoma of the urinary bladder.

A 47-year-old man presented with an inverted papillary carcinoma of the urinary bladder. Cystoscopy revealed an approximately 2 cm mass, with a small stalk and smooth surface, which was situated just medial to the right ureteral orifice. Transurethral resection of the tumor was performed on July 25, 1983. Malignant changes were recognized in the epithelial cords in the resected specimen. This is in contrast to the general view that inverted papilloma of the urinary bladder is benign in nature. The authors believe that careful pathologic examination should be done when an inverted type tumor of the urinary tracts is found. No recurrence has been recognized as of September 11, 1987.
