[The random urine estrogen, pregnanediol/creatinine ratio determinations for monitoring ovarian function].

Study was made to assess the potential clinical value of a random urine estrogen, pregnanediol/creatinine ratio (E/C, P/C) determinations as an index of monitoring ovarian function. Radioimmunoassay of total estrogens and pregnanediol was carried out on 24 hour and paired early morning urine samples in 14 healthy volunteers and 4 infertile patients treated at our clinic during 20 menstrual cycles. Estriol-16-glucuronide and pregnanediol-3-glucuronide antisera were used for radioimmunoassay of total estrogens and pregnanediol, and assays were made without any treatment of urine samples such as hydrolysis and/or extraction. E/C and P/C ratios correlated well with 24 hour excretion of these hormones: Total estrogens (micrograms/24 hr) = 0.89 spot E/C (ng/mg/ml) + 0.21, r = 0.924, p less than 0.01, pregnanediol (mg/24 hr) = 1.28 spot P/C (micrograms/mg/ml) -0.37, r = 0.791, p less than 0.01. The data indicate that ovarian activity in women could be predicted by measuring estrogen, pregnanediol to creatinine ratios in small samples of urine, and the collection of 24 hr samples is unnecessary. In view of this, the conventional basal body temperature method, used up to the present time as an infallible guide in monitoring ovarian function, needs to be reappraised.

Evaluation and management in consultation-liaison psychiatry.

The subjects were 95 medical and surgical inpatients referred to Kyushu University Hospital Psychiatric Consultation Service. The authors studied the mental status of each referred patient and elicited some Unclassified mental status cases. It was recommended, therefore, to bring in new concepts of normal condition and polymorphous condition in dealing with those Unclassified mental status cases which are often recognized as problematic both by the patient's primary physician and the psychiatric consultant. The ordinary style Single contact consultation was also compared with the Active follow-up consultation regarding the function of psychiatric consultations. The authors confirmed that most referring physicians who were supported by the Active follow-up consultation appreciated highly the evaluating function of a psychiatric consultant.

Meiosis-inhibiting effects in vivo of antiserum to progesterone on follicular ova in immature rats treated with gonadotropins.

Effects of the neutralization of endogenous progesterone with rabbit antiserum to progesterone (anti-progesterone) on germinal vesicle breakdown of ova in follicles of small (less than 125 micrometers), intermediate (125-250 micrometers) and large (greater than 250 micrometers) diameter were examined by a quantitative histological technique. Immature rats were treated with 5 IU pregnant mare's serum gonadotropin (PMS) then with 10 IU human chorionic gonadotropin (hCG). Administration of anti-progesterone together with hCG 6 h later significantly decreased the incidence of germinal vesicle breakdown of ova in the large follicles, but not in the intermediate ones. This treatment did not affect the proportion of intermediate to large follicles in the population. Replacement with progesterone 1 h after the simultaneous injection of hCG and anti-progesterone partly reversed the reduced incidence of meiosis. An injection of rabbit antiserum to estrone, in addition to the replacement with progesterone 1 h after the simultaneous injections of hCG and anti-progesterone, restored the incidence of meiosis to a value comparable to the values found for control rats treated sequentially with PMS and hCG. We concluded that the hCG-induced preovulatory rise in progesterone has a limited but definite stimulatory effect on the resumption of meiosis in the ova of large follicles and that it mediates the meiosis-inducing action of hCG.

Discordant pregnancy tests in detection of alpha-subunit--producing tumors.

Ectopic production of isolated alpha-subunit of glycoprotein hormones was detected in a 32-year-old Japanese woman with metastatic lung tumor by a discordance in the immunologic pregnancy tests for native human chorionic gonadotropin (hCG) assays. Quantitative discrepancy of titrations among different types of pregnancy tests, ie, competitive (hemagglutination or latexagglutination inhibition) and noncompetitive (hemagglutination or latexagglutination) reactions, was also confirmed experimentally using a highly purified ectopic alpha-subunit from the patient's urine. Thus the discrepancy between the 2 types of assay systems was due, at least partly, to a cross-reaction of high concentrations (20,000 ng/ml) of ectopically secreted free alpha-subunit of glycoprotein hormones.

[Laboratory and clinical studies on latamoxef in the field of obstetrics and gynecology].

Latamoxef (LMOX) is a new antibiotic synthesized by Shionogi Research Laboratory. Chemically LMOX is especially unique with a sulfur atom replacing the oxygen atom in the 1 position of the conventional cephalosporin nucleus, and in addition, this antibiotic has a cephamycin-like structure. The antibacterial activity of LMOX shows high potency against Gram-negative bacteria, but tends to be weak against Gram-positive bacteria. The tissue levels of LMOX in humans after intravenous injection of 1 g were examined. The levels in uterine and adnexa uteri tissue at 1 hour after administration were 25.4 and 27.4 micrograms/g respectively. LMOX was administered to 147 cases in infections of obstetric and gynecological field. The clinical effect according to disease was 94.6% for intrauterine infections, 95.0% for adnexitis, 87.0% intrapelvic infections, and 100% for external genital organ infections, making a total of 92.5%. The rate of occurrence of side effects or abnormal laboratory findings was similar to or slightly less than that seen with other beta-lactam antibiotics.

Ultrasonographic observation of ovarian follicular development via vaginal route.

Ovarian follicular growth during periovulatory period was measured using real-time ultrasound via vaginal route. In 15 patients examined preliminarily by the abdominal procedure, the mean follicular diameter increased from 15.6mm on four days before ovulation to a maximum of 21.8mm on the day of ovulation. A clear distinction of follicular development during periovulatory period could be made in 40 patients by the vaginal procedure. The mean distance between follicle and scanner was 5.6cm in the vaginal observation, which was significantly shorter than those observed in the abdominal observation. Thus, real-time ultrasound examination via vaginal route appears to be a useful technique for assessing the ovarian follicular development.

The characterization of alpha-subunit of glycoprotein hormone produced by undifferentiated carcinoma.

The glycoprotein hormone alpha-subunit was extracted and purified from the urine of a patient with undifferentiated carcinoma producing isolated alpha-subunit. Its final specific immunoactivity was 0.92 (mg alpha-subunit/mg protein). The alpha-subunit exhibited virtually identical immunoantigenicity to hCG-alpha antiserum with standard hCG-alpha. The molecular weight of the alpha-subunit determined by gel chromatography on Sephadex G-100 was greater than that of standard hCG-alpha dissociated by urea in vitro. By SDS disc electrophoresis, however, the alpha-subunit moved faster than hCG-alpha separated by mercaptoethanol reduction. The amino acid composition of the alpha-subunit was quite similar to that of standard hCG-alpha. In the isoelectric focusing, the major components of the alpha-subunit from undifferentiated carcinoma and the alpha-subunit from urine of normal pregnant women (third trimester) were distributed over the range from pH 3.5 to 6.0, while standard hCG-alpha was distributed in the fractions ranging from pH 6.0 to 8.0. The result of a combination study in vitro indicated that both alpha-subunits from undifferentiated carcinoma and from urine of normal pregnant women did not actively combine with hCG-beta. These results suggest that the alpha-subunit secreted by undifferentiated carcinoma is virtually identical with standard hCG-alpha as the protein moiety but differs in regard to carbohydrate moiety, and also suggest that the excess of alpha-subunit, which is not associated with beta-subunit, may have undergone some intracellular modification, and consequently, the electric charge of the freely secreted alpha-subunit changes and it no longer has the ability to combine with the beta-subunit.

[Discordance of competitive and non-competitive pregnancy tests for human chorionic gonadotropin, as related to alpha subunit producing tumor (author's transl)].

Ectopic production of isolated alpha subunit of glycoprotein hormones(s) in 32-year-old woman with metastatic lung tumor was first detected by a discordance in the immunological pregnancy tests for native human chorionic gonadotropin (hCG). Quantitative discrepancy of titrations among different types of pregnancy test kit, that i, competitive (hemagglutination or latex agglutination inhibition) and non-competitive (hemagglutination or latex agglutination) reactions, was also confirmed by the experimental study using highly purified ectopic alpha-subunit from the patient's urine. Thus, the discrepancy among the two types of assay systems was due by crossreaction of high concentrations (20,000 ng/ml) of ectopically secreted alpha-subunit of glycoprotein hormone(s). We report herein the typical behavior of immunological assay systems for native hCG in determining immunoreactive free hCG-alpha and stress the diagnostic value of the discordance observed in these assay systems.

[Management of retained secundines by hysteroscopy (author's transl)].

Seventy-three patients with retained products of conception were selected from a series of 112 women examined by hysteroscopy, to assess abnormalities of pregnancy. Thirty of the 73 were diagnosed as cases of retained secundines and treated effectively, under hysteroscopic control. This approach has proved to be an effective and a safe procedure for the management of retained secundines. Our study also suggested that traditional therapeutic and/or diagnostic dilatation and curettage (D & C) should be re-assessed and replaced with more effective, simple and safe procedures, such as suction curettage. In these patients with other abnormalities of pregnancy, such as ectopic pregnancy and trophoblastic disease, hysteroscopy does not supplant other diagnostic procedures: rather, it complements them.

[Implication of progesterone in the resumption of meiosis of rat follicular ova (author's transl)].

Gonadal steroids induced by ovulatory stimuli may be among the factors regulating resumption of meiosis even in mammals. Possible role of progesterone on the resumption of meiosis was studied by examining the effects of neutralization of endogenous progesterone induced by an ovulatory dose of hCG with rabbit antiserum to progesterone (anti-P) on oocyte maturation in rats. Immature rats received subcutaneous injection of 5 IU PMS on day 22 followed by intravenous injection of 10 IU hCG 56 hrs later. Animals were sacrificed 6 hrs after hCG injection and number of oocytes with or without germinal vesicle breakdown was counted on serial sections of both ovaries. Treatment with anti-P induced a marked decrease in incidence of meiosis of follicular ova in large (greater than 250 microns in diameter), but no appreciable change occurred in intermediate follicles (125-250 microns in diameter). With the replacement of exogenous P, percent incidence of resumption of meiosis recovered significantly, but not completely, in large follicles compared to those in the control group. The results can be interpreted as indicating the conversion of progesterone into estrogen which may exert the partial inhibition on meiosis by estrogen. When the replacement of exogenous P was supplemented with the simultaneous injection of antiserum to estrone to neutralize endogeneous estrogen, a complete recovery of resumption of meiosis in large follicles was observed. The present results suggest that the hCG-induced preovulatory progesterone has a localized facilitating effect on the resumption of meiosis of oocytes, predominantly in the large follicles of rats.

Inhibition by indomethacin of ovulation induced by human chorionic gonadotrophin in immature rats primed with pregnant mare serum gonadotrophin.

Treatment of immature rats with pregnant mare serum gonadotrophin followed by human chorionic gonadotrophin (HCG) caused an acute and temporary increase in concentrations of progesterone, testosterone and oestradiol in plasma with maximum levels 3 h after the administration of HCG. Concurrent injection of indomethacin and HCG reduced, in a dose-dependent manner, the mean number of ova shed and this was accompanied by a dose-dependent decrease in concentrations of plasma progesterone and testosterone but not of oestradiol when they were measured 3 h after the injection of HCG. The minimum effective dose that blocked ovulation completely at 0 h abolished the acute increase of progesterone and testosterone, suggesting that prostaglandins act on ovulation by stimulating steroidogenesis at an early stage in the preovulatory process. The anti-ovulatory action of the minimum effective dose at 0 h became progressively less potent as the time between injection of HCG and administration of indomethacin was increased, although plasma concentrations of progesterone and testosterone measured at autopsy 18 h after treatment with HCG had not changed appreciably. When indomethacin was administered 10 h after HCG, the relationship between the dose of indomethacin and the mean number of ova differed from that observed when simultaneous injections of indomethacin and HCG were given, and the minimum effective dose that prevented ovulation was much higher than that at 0 h, suggesting that prostaglandins act differently on ovulation in the later stage of the preovulatory process. It was concluded that prostaglandins may mediate the action of HCG on ovulation through two mechanisms which operate at different stages of the preovulatory process.

[Dual action of prostaglandins in the regulation of HCG-induced ovulation in immature rats (author's transl)].

With the aid of indomethacin, a potent inhibitor of prostaglandin synthesis, the mechanism of action of prostaglandins on follicle rupture was studied. Mean number of ova shed following treatment of immature rats sequentially with PMS and hCG was reduced in a dose-dependent manner by simultaneous injection with increasing doses of indomethacin. The minimum effective dose to block ovulation completely was 90 micrograms. However, the anti-ovulatory action of the minimum effective dose at 0 h became progressively less potent as the time between hCG injection and indomethacin administration was increased. Another dose-dependent relationship was found 10 h after hCG injection, the closest time prior to the onset of ovulation, the minimum effective dose necessary to prevent ovulation was 8 times higher than at 0 h, suggesting that prostaglandins acted differently on ovulation in the later stage of the preovulatory process. It is concluded that prostaglandins may mediate the action of hCG on ovulation at least through two mechanisms.

Capping of saccharides on the plasma membrane of lymphocytes as studied by fluorescein-labelled lectins.

The capping of saccharides on the plasma membrane of rat splenic lymphocytes was studied by means of fluorescein-labelled lectins. Treatment of unfixed splenic lymphocytes with any one of the three lectins, concanavalin A (Con A), Ricinus communis agglutinin (RCA) and wheat germ agglutinin (WGA) led to the formation of caps of each saccharide receptor on the plasma membrane. Treatment of unfixed lymphocytes with Con A was found to result in the formation of caps of saccharide receptors for RCA, whereas cap formations were never noted in such double treatment of the cells with all other combined uses of two lectins. These results are taken to indicate that the saccharide receptors for Con A are associated with those for RCA in the plasma membrane of rat splenic lymphocytes.
