Diagnostic and intraoperative immunoscintigraphy in ovarian cancer.

A new strategy for tumor detection using immunoscintigraphic techniques was used in an attempt to improve the diagnostic potential of conventional external immunoscintigraphy. Twelve patients who were strongly suspected to either suffer from primary or recurrent ovarian cancer were investigated as follows: radioimmuno-scintigraphy (RIS) by means of radiolabelled antibodies was performed in all patients prior to operation (first or second look). During the operative procedure, which always took place immediately after completion of the diagnostic RIS, a hand-held gamma-ray detection probe, especially developed for this purpose was used for the purpose of identifying radiolabelled tumor sites intra-operatively in the open abdomen. In 10 out of 12 of the investigated patients, Radioisotope-Assisted Surgery (RAS) could be performed successfully. The count rates in cancer sites ranged from 150-250 cts per 6 s vs. 30-50 cts per 6 s in normal tissues. While the presence of malignant-tumor sites could always be predicted by conventional RIS, RAS failed twice, but revealed more accurate information concerning the real extent of the disease.

Radio-immunoscintigraphy and intraoperative tumour detection by means of anti-tumour antibodies in patients with ovarian cancer.

A hand-held gamma-ray detection probe was developed by us to circumvent the problems of conventional radio-immunoscintigraphy (RIS). The intraoperative use of this probe in terms of radionuclide-assisted surgery (RAS) should help to overcome problems associated with insufficient accumulation by the tumour of the radio-active antibodies, unfavourable localization of tumour sites, collimation problems, etc., in patients with ovarian cancer. The probe was designed as a scintillation counter. Twelve patients were operated upon and investigated by RAS. It could be shown that RAS is able to detect tumour tissue intraoperatively, although we have already seen that improvements in the probe are necessary to make the method of real clinical value.

[Practical use of blood flow measurement with pulsed ultrasound in the 3d trimester of an at-risk pregnancy].

Using a pulsed Doppler technique measurements of blood flow of the arcuate artery, umbilical artery and foetal aorta were performed in the third trimenon in a group of 163 patients with risk pregnancies and compared with the weights of the newborn. In normal values of blood flow of the arcuate artery (n = 144) less than 10% of the weights of the newborn were below the tenth percentile. In pathological values (n = 19) about 42% newborn werde below the tenth percentile. In normal values of blood flow of the umbilical artery (n = 158) about 10% of the newborn were below the tenth percentile. In pathological values (n = 5) all newborn were below the tenth percentile. In normal values of blood flow of the foetal aorta (n = 139) about 12% of the newborn were below the tenth percentile. In pathological values (n = 4) all newborn were below the tenth percentile. Of the group with "normal" blood flow data a sub-group was formed with flow values between the first and second standard deviation. These were labelled "suspicious" and had shown a significant correlation with the birth weights of the newborn. Based on these experiences a schema was prepared comprising the blood flow measurements and cardiotocography, from which recommended clinical procedures were derived.

Prostaglandin injection for termination of tubal pregnancy: preliminary results.

A variety of conservative surgical techniques is available for the termination of ectopic pregnancy with preservation of the tube. We report on a new treatment by means of intratubal prostaglandin F2 alpha application followed by intramuscular administration of a prostaglandin E2 derivative (sulprostone) for 3 days. Only 5 of the 30 patients enrolled had to undergo laparotomy later. The results were compared with those obtained by different surgical procedures during the 2 preceding years. Hysterosalpingography was performed in 12 patients of the prostaglandin treated group and 14 of the surgically treated group. Tubal patency was demonstrated in 11 patients of the PG group but only in two patients of the surgically treated group.

[Immunoscintigraphy in oncology exemplified by ovarian cancer].

Diagnostic and therapeutical work increasingly uses immunologic and nuclear medical methods, which is especially true in oncology. Recently tumor localization has gained much from immunoscintigraphy, a noninvasive method combining immunologic and nuclear medicine techniques. After application and accumulation of radiolabelled tumor-associated monoclonal antibodies, it is possible to obtain malignant tumor sites by scintigraphy. In the following a short introduction of the method and its use in ovarian cancer patients is presented.

[Immunolymphoscintigraphy. A new procedure for noninvasive lymph node staging illustrated by the example of breast cancer].

In the course of a prospective study 43 women underwent axillary immunolymphoscintigraphy (ILS) shortly before a planned operation for suspected carcinoma of the breast. The aim was to test the feasibility of this procedure for the noninvasive staging of lymph-nodes. Three antibodies were used: HMFG-1 (3 patients), HMFG-2 (13) and 3C6F9 (27). The diagnosis was confirmed in 35 patients. They underwent radical mastectomy with removal of the lymph-nodes. There were marked differences in the usefulness of HMFG-2 and 3C6F9 for ILS in these circumstances. There were too few patients to evaluate HMFG-1. Sensitivity and specificity of the test were 0.57 and 0.83, respectively, for HMFG-1 and 0.83 and 0.93 for 3C6F9.

[Sonographic detection of increased perfusion in placental insufficiency following TNS therapy using pulsed Doppler ultrasound].

In the last years transcutaneous electric nerve stimulation (TNS) has been used at the first Department of Gynaecology and Obstetrics in Vienna for the therapy of placental insufficiency. With the new method of Doppler Ultrasonography a direct measurement of blood flow in the Arteriae umbilicales is possible. In 16 patients with reduced placental perfusion between 34 to 38 weeks of pregnancy TNS-therapy has been monitored by Doppler Ultrasonography. In 14 patients an impressive improvement to normal values of perfusion could be observed. Only 2 patients with morphological changes in the placenta were resistant to therapy. A control group of 16 patients with normal values of perfusion did not show any response to TNS-therapy as expected. Therefore this safe and simple method of TNS is recommended for the therapy of functional placental insufficiency.

[Modification of uteroplacental circulation by solcoseryl].

Pulsed ultrasonic doppler flow measurements both on uterine arcadic arteries and umbilical arteries have been done in 10 pregnant women suspected for intrauterine fetal growth retardation before, during and after Solcoserylinfusions for ten days. Improvement of maternal uteroplacental blood flow may be caused by changed rheologic blood properties as well as by physical inactivity. Fetal umbilical cord blood flow could not be influenced by Solcoseryl.

Radical isotope surgery: an enrichment for the surgical therapy of gynecological malignancies.

Intraoperative lymphscintigraphy during radical isotope surgery was performed in 242 patients suffering from cervical cancer and 170 patients with breast carcinoma. The results show a decreased rate of recurrence mortality in cervical cancer stage I and a decreased rate of recurrencies and metastases in breast carcinoma. In ovarian cancer patients radiommunoscintigraphy was introduced in the diagnostic procedure and is now performed intraoperatively. The results show a high positive prediction value of 93%. In 5 out of 28 patients radioimmunoscintigraphy identified tumor regrowth while all other methods failed.

Radioimmunoscintigraphy using monoclonal antibodies before second-look surgery in patients suffering from ovarian cancer.

19 patients with a known history of ovarian cancer were investigated by radioimmunoscintigraphy (RIS) to look for recurrent disease a few days before second-look surgery. The tumor-associated monoclonal antibody HMFG-2 (400 micrograms/patient) was injected intravenously after labeling with radioactive 123I (0.5-2.2 mCi/patient). Scans were reviewed for activity accumulations due to uptake of the tumor-associated antibody by tumor sites. In 15 out of the 19 cases the scan results correlated with the intraoperative findings. There were 2 false-positive and 2 false-negative scans, the latter in patients with subclinical disease. The smallest lesion detected by radioimmunoscintigraphy had a diameter of 1.5 cm. In 3 patients, tumor sites were identified that had been missed by all other routinely performed methods of investigation including transmission computed tomography. These data indicate that RIS is of considerable clinical value in the early detection and localization of recurrent ovarian cancer and may, therefore, improve the management of these patients.

[Perinatal results in cases of EPH gestosis and "small placenta"].

The foetal outcome was examined in 24 pregnant females with EPH gestosis and a placental weight below the 20th percentile. The results show a high perinatal mortality in such cases. 11 newborns with a mean birth weight of 1092 g and a mean gestational age of 33.8 weeks died perinatally versus 13 newborns with a mean birth weight of 1705 g and a mean gestational age of 35.6 weeks, who survived. Perinatal mortality was equally high in cases with primary Caesarean section as in cases with spontaneous labour. Perinatological research should, therefore, focus on the development of techniques for estimating placental weight. Patients with a small placenta and EPH gestosis could then be monitored exactly and be delivered at the optimal moment.

[Initial results of immunolymphoscintigraphy in the diagnosis of axillary lymph node involvement in breast cancer patients].

Sixteen patients suffering from breast cancer were investigated by the method of immunolymphoscintigraphy (ILS) to evaluate the axillary lymph node status. Two monoclonal tumour associated antibodies (HMFG-1 and HMFG-2) that are known to react strongly with a wide range of epithelial malignomas were used as tumour seeking agents in our recent study. All patients were operated a few days after the scanning procedure including a radical axillary lymph node dissection. Therefore the scan reports could be correlated in each case with the histology of the surgically removed probes. While HMFG-1 failed to show correct results, HMFG-2 gave accurate results in 9 of 13 cases of questionable lymph node involvement. Among the 4 false results, one turned out to be false positive and three false negative. If better antibodies or their fragments, respectively, are used, we are convinced this noninvasive approach will greatly improve preoperative diagnosis of lymphatic involvement in breast cancer patients.

Uteroplacental blood flow measurement using radioisotopes in small placentas.

In 25 pregnant females with a placental weight below the 20th percentile uteroplacental blood flow measurements using 113mIn-transferrin were performed in the 3rd trimester of pregnancy. Twelve patients with pregnancy-induced hypertension (PIH) were compared to 13 females with normal blood pressures. The results of uteroplacental blood flow measurements showed normal flow patterns in only 6 of the 25 subjects and were significantly worse in the group with PIH. Babies of patients with PIH had a mean birth weight of 1,100 g at a mean gestational age of 34.2 weeks versus 1,752 g and 37.4 weeks for newborns without PIH. Perinatal mortality was significantly higher in the PIH group. In combination with a small placenta PIH compounds the fetal risk.

Monitoring the cytotoxic potential of a sequential polychemotherapy (adriamycin/cisplatin-vincristine/cyclophosphamide-high dose methotrexate) in patients with advanced ovarian cancer with the tumor marker CA-125.

The cytotoxic efficacy of a new sequential chemotherapy, called changing-scheme (adriamycin/cisplatin-vincristine/cyclophosphamide-HD-methotrexate), was investigated parallel to the course of CA-125 serum levels. Sixty-three percent of the women studied achieved remission under this chemotherapy. A correlation between clinically proven tumor stage and CA-125 serum level was found in 90%. The cytotoxic potential of the three single parts of the changing-scheme was demonstrated based on decreasing CA-125 serum levels. The value of the tumor marker CA-125 for the follow-up of patients with malignant ovarian tumors is discussed in detail.
