Prostacyclin does not influence placental blood pool in vivo.

In 20 healthy pregnant women placental blood pool was estimated by means of placental scintigrams before, during and after infusion of either prostacyclin (PGI2) or placebo for 10 min to study the effect of PGI2 on the local regulation of uterine blood flow. There was no difference in the mean number of counts between the PGI2- and the placebo-treated group. Therefore no effect of PGI2 on uteroplacental blood pool could be detected in healthy gravidae.

[Effect of short duration euphylline infusion on the uteroplacental blood pool].

In 19 late pregnant women with suspected placental insufficiency uteroplacental perfusion was measured and then the effect of a shortlasting infusion of aminophyllin on uteroplacental blood pool was examined. The results show evidence that in normal perfusion typ uteroplacental blood flow is not influenced by aminophyllin. But in pathologic perfusion typs there are two groups after aminophyllin. In the first one there is an increase of activity measured over the placenta (positive test). In these cases the cause of the lowered perfusion may be a functional one, treatment will be successful. In the second group there was no change or a decreasing activity following administration of aminophyllin (negative test). In these cases the reduced perfusion may be caused by morphological factors. Treatment will be without success with a high likelihood. The aminophyllintest following the isotope perfusion measurement is useful both for prognosis of newborn weight and appropriateness of therapy.

Radioimmunodetection in patients with suspected ovarian cancer.

Twenty-five patients, having either unilateral ovarian tumors of unknown etiology or suspected of having ovarian cancer recurrence were investigated by the method of immunoscintigraphy to rule out primary and/or metastatic tumor sites. Four-hundred micrograms of the tumor-associated monoclonal mouse antibody HMFG-2, raised against human milk fat globulin membranes and labeled with 123I, were used for each patient to display the tumor sites by external scintigraphy. The dose ranged between 0.5 and 2.2 mCi, the specific activity between 1.25 and 5.5 mCi per mg of antibody. Nineteen of the patients underwent operations a few days after immunoscintigraphy. The remaining six patients were investigated by transmission computed tomography (TCT) to establish the presence or absence of tumor of the imaging. In 22 of the 25 cases the scintigraphic results correlated with the situation found at the subsequent operation, or by TCT, respectively, as well as with the histological diagnosis of the tumor type. Overall, there were just two false-negative and one false-positive scan report, the latter due to faulty reading of the scintigrams. Sixteen out of 18 tumor sites in 25 patients could be revealed by immunoscintigraphy, the smallest one being 1.5 cm in diam. In four of the patients immunoscintigraphy was the only noninvasive investigation method that could reveal the malignant tumor sites prior to the operation.

[Visualization of malignant tumors in the abdomen using radiolabelled monoclonal antibodies].

The production of monoclonal antibodies by the hybrid technique introduced by Köhler and Milstein in 1975, led to new dimensions in the diagnosis and therapy of malignant diseases. Labelling such monoclonal antibodies, directed against tumour cells, by a radioactive isotope, makes them usable for visualising malignant tumours and their metastases. Early detection of cancer may represent a major step forward in cases where early detection is not possible with common noninvasive methods, such as ovarian cancer in gynaecological oncology. Radioimmunoscintigrams (RIS) were performed in seven patients, all of whom were suspect of having ovarian cancer. The RIS-results showed very good correlations with the operation reports. According to these first results we assume that the RIS method using I123-labelled monoclonal antibodies directed against tumour cells may be successful in the early detection of ovarian cancer. Other important information supplied by radioimmunoscanning is determination of tumour status, localisation, extent or metastatic spread.

Effects of smoking on uteroplacental blood flow.

The effects of nicotine consumption on uteroplacental blood flow were studied in 20 pregnant women by means of placental blood flow measurements using indium-113m-transferrin. Smokers were found to have a higher rate of poor perfusion patterns than subjects of a control group. In addition, the number of cigarettes smoked daily was significantly correlated with the placental perfusion type. The mean birth weight of infants born by smokers was 250 g lower than in the control group. The results suggest that smoking during pregnancy may compromise uteroplacental blood flow and thus result in poor fetal development.

Intraoperative lymph scintigraphy during radical surgery for cervical cancer.

Intraoperative lymph scintigraphy during radical surgery for cervical cancer was developed in the course of a program covering three periods. During the last period technetium-99m antimony sulfide has been used to visualize pelvic lymph nodes. Surgery is done with a modified gamma camera serving as an operating table. This ensures intraoperative monitoring and greater thoroughness of lymphadenectomy. The introduction of the technique has improved the rate of total lymphadenectomies and has increased both the yields of involved nodes and the 3-yr survival rates.

[Radiotherapy of recurrences of endometrial carcinoma].

In a retrospective study the data of 79 patients undergoing radiation-therapy for recurrent endometrial cancer during the years 1972-1978 were evaluated. Group I consisted of 43 patients, who primarily underwent irradiation, group II consisted of 36 patients, who primarily underwent surgery (total abdominal hysterectomy) mostly without irradiation. All patients were primarily classified as FIGO-stage I, the median age was 69 and 67 years respectively. In 32 cases (74%) of group I the localisation of the recurrence was the uterus, in 8 (19%) the vagina and in 3 (7%) the small pelvis. In group II a vaginal recurrence was found in 25 patients (69%), in 11 (31%) a pelvic recurrence. In two thirds of all cases the recurrences were diagnosed within three years of the primary treatment. Late recurrences after 5 years occurred in group I in 10%, in group II twice as often. The treatment of all recurrences was irradiation. 54 patients (68%) died within 2 years, 25 survived their recurrence for more than 2 years, (32%). In group I 23% survived for more than 2 years, in group II 42%. At the time of performance of the study the 5-year-survival-rate is 13%. Thus in two thirds of patients we succeeded in extending the survival for at least 2 years. The prognosis of a recurrence following surgery is distinctly better than following primary intracavitary irradiation. 56% of vaginal recurrences following primary surgery survived more than 2 years. The 5-year-survival-rate was similar in these vaginal recurrences however to the uterine recurrences following primary irradiation. The prognosis of patients undergoing primary surgery was the better, the later the recurrence occurred.

[Psychosomatic aspects of females with placental insufficiency].

Significant differences occurred on various scales by comparing 30 pregnant women who suffered from dysfunction of placenta chosen at random. Comparison was made by means t-tests and cross tables in GT, FPI, SSG sociological and medical data. Additionally a controlled experiment was performed. Dominating parts on a body level are the masochistic and the rigid one, both high charged structures. They seem to be more frequent than schizoid, oral and psychopathic parts. Looking to psychosomatics of dysfunction of placenta aggressive wishes against the child could be found.

Clinical value of immunolymphscintigraphy in patients with breast cancer.

Thirty four patients suspected of suffering from breast cancer were investigated by immunolymphscintigraphy (ILS) a few days before a planned operation. Patients received 500 micrograms of the BCD-F9 monoclonal antibody or its F(ab')2 fragments containing 1 mCi of Iodin-123. Each preparation was given by a subcutaneous injection into the fingerwebs between the 2nd and 3rd fingers of both hands. Three static scintigrams were taken 4, 8 and 24 hours following injection of the radiolabelled antibodies. ILS results were always compared to histopathological findings and gave for the intact antibody a sensitivity of 83%, a specificity of 93% and an accuracy of 89%, the positive predictive value was 91% and the negative predictive value was 80%. On the other hand, when the F(ab')2 fragments were used the sensitivity of the ILS technique was 75%, the specificity was 100% while the accuracy was 86%, the positive predictive value was 100% and the negative predictive value was 75%.

[Attempt to diagnose a "small" placenta by HPL determination before the 18th week of pregnancy].

In a prospective study covering 175 gravidae it was investigated whether the risk of a "small" placenta could be detected by determining HPL or serum estriol once during the first 18 weeks of pregnancy. The results show that neither the HPL value nor the serum estriol value correlate with the weight of the placenta or the child. Therefore, the problem of early detection of a "small" placenta cannot be resolved with these hormone parameters.

Immunolymphscintigraphy of axillary lymph node metastases in breast cancer patients using monoclonal antibodies: first clinical findings.

The results of preliminary investigations into the immunolymphscintigraphic (ILS) detection of axillary lymph node metastases in nine breast cancer patients by means of the 3C6F9 monoclonal antibody (MAs) are presented. The IgG2a monoclonal antibody detects a 37 KD antigen, consistently found on the surface of primary and metastatic breast tumors. Each patient received 1 mCi of I-123 (specific activity, 2 mCi per mg of antibody) as a subcutaneous injection between the 2nd and the 3rd finger of both hands, i.e., the healthy side serving as a control for the affected side. Clear images of lymph node metastases were visible 4 to 8 hours after injection of the antibody. Seven of the nine patients studied were positive by scanning and six showed positive lymph node involvement by histopathology (6/7; true positive = 86%). Two patients did not show any iodine uptake in the axilla and were subsequently found to be free of metastases (2/2; true negative = 100%). These data give an overall accuracy of ILS of 89% and demonstrate that 3C6F9 localizes preferentially in affected axillary lymph nodes compared to normal lymph nodes.

[Measurement of uteroplacental circulation in antihypertensive therapy with clonidine].

40 Gravidae with hypertension were treated with clonidine . Placental blood flow measurements were performed to study the effect of clonidine on uteroplacental blood flow. The results show a significant decrease of the gestosis-index and a significant improvement of placental perfusion type during medication. Therefore, clonidine is suitable for treatment of hypertension in pregnancy and has no negative effect on uteroplacental blood flow.
