[Differential cytological diagnosis of pleural exudate].

Results of cytological investigation of pleural exudates were analyzed in 644 patients for the 2000-2000 yrs period. Meticulous description of principal kinds of pleural exudates is presented, differential signs of cytologic composition in patients with nonspecific, tuberculous, stagnant and oncologic pleuritis were delineated. The changes in cytological composition of exudates in lingering of the disease course are depicted. High diagnostic value of cytological investigation of pleural fluid, especially in presence of contraindications, was established.

[Significance of cytohistologic examination of pleural biopsy samples obtained through diagnostic thoracoscopy for exudative pleurisy of obscure etiology].

Significance of results of cytologic (CE) and histologic (HE) examinations of the biopsy material, received during diagnostical thoracoscopy for the exudative pleurisy (EP) of nonestablished etiology was studied. There were 69 observations of EP analyzed. Following diagnosis was established: nonspecific, tuberculous, metastatic pleurisy and in pleural mesothelioma. The values of CE and HE characteristics were presented. The complex general value of cytohistologic diagnosis had constituted 94%. Causes of diagnostic mistakes occurrence were considered.

[The pleural effusion syndrome in patient with tumor of thymus].

Five observations of the thymic gland tumor and concurrent exudate pleuritis were presented. In one patient the pleuritis was total and in the others--restricted one, in two of the patients exudate was diagnosed during performance of operation. In two observations in exudate there were revealed tumoral cells. In two patients tumor turned out to be inoperable one, the rest of the patients were operated on successfully.

[Pulmonary resection of arteriovenous fistula].

Arteriovenous fistula (AVF) is rare congenital pathology of the bronchopulmonary system vessels. The results of treatment of 12 patients with AVF were analyzed. In 2 (16.6%) patients Randu-Veber-Osler syndrome was diagnosed. In 10 observations the diagnosis was confirmed by the angiography data. All the patients were operated on. One (8.4%) patient died.

[Chylothorax].

Six cases of chylothorax of their own observation were depicted. In one of them the cause of the disease was not disclosed. In four patients a pleural puncture or pleurodesis were accomplished, in two--an operation was performed.

[Exudative pleurisy as the manifestation of multiple myeloma].

For the 15-year period of time 2 patients with myelogenetic disease (MD) and concomitant exudative pleuritis were observed in the clinic. In one of them with MD diagnosed 3 years before there was nonspecific (occurred after the pleuropneumonia) pleuritis. In the other patient the MD was diagnosed after revealing of characteristic cells in pleural exudate.

[Pseudochylothorax].

Pseudochylothorax--a rare disease, occurring in prolonged persistence of exudate in the cavity of fibrotically changed pleura. Three observations of pseudochylothorax, including as the complication of viral hepatitis A, are adduced. Diagnosis was established after biochemical analysis of exudate, obtained during pleural puncture.

[Tracheal bronchus].

Three patients were operated on for the tracheal bronchus (TB) in the clinic. All the patients had right-sided TB localization and other inborn concomitant pathology of bronchopulmonary and cardiovascular systems. One patient, who had concomitant pathology of cardiovascular system with TB, died due to continuing pulmonary hemorrhage.

[Intralobar pulmonary sequestration].

For the period of 21 years the intralobar pulmonary sequestration was revealed in 6 patients aging from 11 to 47 years old. In all the observations of the intralobar sequestration the only affection of lower pulmonary lobes was noted. In 4 patients the symptomless course of the disease was present, while in 2 it was manifested by the recurrent pneumonia. The correct diagnosis before the operation was established in one patient only. All the patients were operated on, in 2 two aberrant vessels to the sequestrated pulmonary region were revealed and in all the patients--bronchogenic cyst in the affected region.

[Carcinosarcoma: a rare mixed tumor of the bronchopulmonary system].

Six patients with carcinosarcoma were observed in the clinic. In 4 patients with roentgenological signs of tumor were determined and in 2--the signs of pleuritis. The diagnosis was established after pulmonary resection performance or after thoracoscopy with pleural biopsy conduction.

[Clinical course, diagnosis and treatment of mesothelioma of the pleura].

Retrospective analysis of 58 patients history cases, in whom pleural mesothelioma was diagnosed, was conducted. Clinical signs of the disease were: progressive pain in the chest on the affection side, permanent fever, dyspnoea, weakness, rapid accumulation of pleural liquor. The combined application of clinico-roentgenological, laboratory and instrumental methods of examination has great significance in diagnosis. The informativity of transthoracic puncture with pleural biopsy is 36.2%, and of thoracoscopy-84.3%. Diagnostical thoracotomy was performed when indications were present.
