Ovarian hemangioma associated with concomitant stromal luteinization and ascites.

A 62-year-old female presented with a pelvic mass and ascites. The Papanicolaou vaginal smear showed an unusual maturation, maturation index being 0/80/20. The serum level of estradiol was 48.7 pg/ml. The preoperative checkup suggested a pelvic malignancy with a differential diagnosis of hormone-secreting ovarian tumor. On surgical exploration, she had a hemangioma of the ovary without malignant cytology in the ascitic fluid. Histologically, this tumor was associated with stromal luteinization. This is the first case, reported in the literature, possessing ovarian hemangioma with stromal luteinization accompanying massive ascites. It should be noted that an ovarian hemangioma can be associated with stromal luteinization and ascites, and that MR imaging is sometimes of value for making a preoperative diagnosis of ovarian hemangioma.

Pelvic lymph node metastasis in endometrial cancer with no myometrial invasion.

OBJECTIVE: To analyze the incidence of pelvic lymph node metastasis in endometrial carcinoma with no myometrial invasion. METHODS: Between 1971 and 1995, 684 women with stage I endometrial carcinoma underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, and pelvic lymph-adenectomy. The incidence of pelvic lymph node metastases in 100 cases without myometrial invasion was examined. RESULTS: Histologic examination of the surgical specimens revealed a single pelvic lymph node metastasis in each of four cases. The incidence of pelvic lymph node metastasis was four of 83 in grade 1, zero of 13 in grade 2, and zero of four in grade 3 tumors. CONCLUSION: Pelvic lymph node metastasis in endometrial cancer with no myometrial invasion is not rare, even with grade 1 tumors. Lymphadenectomies may be necessary in all patients with endometrial cancer, except when clinical or operative factors increase the procedure's risk of morbidity.

Serum levels of squamous cell carcinoma antigen and carcinoembryonic antigen as tumor markers of vulvar cancer.

OBJECTIVE: To assess whether squamous cell carcinoma (SCC) antigen and carcinoembryonic antigen (CEA) are useful as tumor markers of vulvar cancer. METHODS: The serum levels of SCC antigen and CEA were determined for 30 patients with invasive vulvar carcinoma. RESULTS: There was a significant difference not only between tumor sizes of 2 cm or larger and those of less than 2 cm (p < 0.001), but also between patients with lymph node metastasis and patients free from it (p < 0.001). Tumor marker levels well reflected the response to neoadjuvant chemotherapy. The sensitivity to recurrence was 63%, and the specificity was 94%. In 2 of 8 recurrent cases, it was only the re-increase in tumor markers that indicated the recurrence. CONCLUSION: The measurement of serum levels of SCC antigen and CEA is useful not only for preoperative evaluation of the risk of lymph node metastasis, but also for the detection of recurrence.

Cardiorespiratory responses to cycling exercise in trained and untrained healthy elderly: with special reference to the lactate threshold.

The fastest growing age group in the United States and Japan is the elderly. There is a need to develop appropriate exercise training guidelines designed specifically for healthy older persons. Recent reports have shown that the lactate threshold (LT) can be used to evaluate the clinical significance of aerobic power (VO2max) and its effect of exercise training in the elderly. However, there is a lack of research comparing the LT between well-trained and sedentary elderly individuals. Also, the effect of exercise training on the heart rate (HR) at LT needs further investigation. The purpose of this study was to compare the LT levels between the older trained men (T group; n = 72, age = 71.3 +/- 5.8 yr, range 60-85 yr) and apparently healthy but untrained elderly men (U group; n = 172, age = 72.2 +/- 5.7 yr, range 60-93 yr). The LT was measured during an incremental cycle ergometer test. A low relationship was found between VO2 corresponding to LT (VO2LT) and age in the T (r = 0.20, P < 0.05) and U groups (r = 0.43, P < 0.05). A significant difference was found in the VO2LT between the T and U groups. The absolute VO2LT corresponded to approximately 6 and 4 METs for the T and U subjects, respectively. However, there was no significant difference in HR corresponding to LT (HRLT) between the two groups (T; 109 +/- 19 b.min-1, U; 107 +/- 13 b.min-1). The data show that the absolute VO2LT is higher for T than U elderly subjects and is associated with a HR of approximately 108 b.min-1 for both groups. Recommended exercise intensity in terms of HR may not differ between trained and untrained elderly men.

The relative utility of health-related fitness tests and skilled motor performance tests as measures of biological age in Japanese men.

In the present paper we report the results of a study in which we compared 2 different approaches to the computation of biological age (BA) in a sample of 322 Japanese men (age range 20 to 79 years). In the first approach, 4 commonly used measures of health-related fitness (VO2peak, trunk flexion from a standing position, body fat, and grip strength) were reduced to a single BA score (HRF Age) using principal component analysis. In contrast, in the second approach, 3 commonly used measures of skilled motor performance and agility (vertical jump, stepping side-to-side, and balancing on one leg with eyes closed) were reduced to a single BA score (SMP Age) using similar multivariate procedures. The criterion-related validity of both of the BA measures was examined by assessing each measure's ability to discriminate between healthy and active groups of subjects. This was achieved by classifying the original subject pool into regularly active (ACT; n = 108) and healthy (HLTH; n = 169) subgroups on the basis of self-reported activity levels. Analyses revealed that HRF Age was a more powerful discriminator between the two activity groups than SMP Age. While HRF Age of HLTH subjects was very close to their chronological age (CA), in the ACT group, HRF Age was on average 15 years less than their CA (P < 0.05). In a separate analysis, we assessed the HRF Age of patients with ischemic heart disease, hypertension, obesity, or diabetes (PAT; n = 45). The HRF Age of these subjects averaged 10 years above their CA. Our data suggest that commonly used measures of health-related fitness can be usefully employed as indices of BA which differentiate between individuals of similar ages but differing health and physical activity status. In contrast, measures of skilled motor performance were found to be less valuable measures of BA. The implication of our findings for future experimental design in exercise and aging research is discussed.

[A clinicopathological study in clear cell adenocarcinoma of the endometrium].

We clinicopathologically reviewed cases of clear cell adenocarcinoma (CCA) of the endometrium and adenocarcinoma of the endometrial type mixed with a clear cell component (AMC). 1. Among 1,152 cases of endometrial carcinoma, CCA and AMC were found in 16 (1.4%) and 21 (1.8%) cases, respectively. 2. As revealed by cumulative survival rates based on the Kaplan-Meier method, the prognosis was significantly poorer in CCA and AMC than in endometrial carcinoma (p < 0.001). 3. As for the subtype of CCA, 5 (31%), 9 (56%) and 2 (13%) cases were papillary, solid, and tubulocystic, respectively. 4. The frequency of epithelial hyaline body, bizarre nucleus, psammoma body, necrosis, lymphoplasmacytic infiltration, lymphatic invasion, vessel invasion, and atypical hyperplasia among cases of CCA was 8 (50%), 7 (44%), 5 (31%), 6 (38%), 8 (50%), 5 (36%), 4 (29%) and 1 (7%) cases, respectively. Histologically, lymphoplasmacytic response was the most closely correlated with the prognosis. 5. Among the cases of AMC, there were 6 cases of metastasis and recurrence. In 5 (83%) of these, the clear component increased markedly in metastatic and recurrent lesions although the component was very minor in the primary lesions.

Prediction of endurance running performance for middle-aged and older runners.

The purpose of this study was to develop regression equations that would sufficiently predict the endurance running performance (ERP) of middle-aged and older runners (n = 55, 43-79 years). Among many independent variables which were selected as possible predictors of the ERP, oxygen uptake corresponding to the lactate threshold (VO2@LT), or age was found to be the single best predictor. Some variables representing training habits correlated significantly but only moderately with the ERP. Linear multiple regression equations developed in this study were: V5km = 4.203 + 0.054X1 - 0.028X2 (r = 0.87) V5km = 4.436 + 0.045X1 - 0.033X2 + 0.005X3 (r = 0.89) V10km = 4.252 + 0.042X1 - 0.026X2 (r = 0.79) V10km = 4.371 + 0.037X1 - 0.031X2 + 0.005X3 (r = 0.82) VM = 3.207 + 0.048X1 - 0.022X2 (r = 0.91) VM = 3.707 + 0.038X1 - 0.031X2 + 0.005X3 (r = 0.93) where V5km, V10km and VM are the mean running velocity at 5 km, 10 km and marathon races, respectively, and X1 = VO2@LT (ml kg-1 min-1), X2 = age (year), and X3 = average running duration per workout (min). We suggest that the ERP of middle-aged and older runners can be predicted from a linear combination of VO2@LT and age or a combination of these variables plus average running duration per workout.

[Endometrial carcinoma without myometrial invasion].

Between 1974 and 1991, 621 patients with endometrial cancer were treated with hysterectomy without any preoperative treatment. Subsequent pathological examinations revealed that 92 cases had no myometrial invasion. Clinicopathological analysis was carried out in the 92 patients. The results are as follows: 1. Complete surgery was carried out in all cases except one case with intraperitoneal dissemination. The relapse rate in the 91 cases was 2.2% (2/91). The 2 cases with recurrence had grade 1 (G1) and grade 2 (G2) adenocarcinoma and both cases manifested local recurrence. 2. Of 70 cases treated with pelvic lymphadenectomy, two cases (2.9%) with G1 adenocarcinoma exhibited a single node metastasis. The metastatic site was an external iliac lymph node in both cases. Despite the lymph node metastasis, the 2 cases have exhibited no relapse. 3. None of the 92 cases showed any evidence of adnexal metastasis. The positive rate of peritoneal cytology was 13.4% (9/67). Except for one case with peritoneal dissemination who also had positive peritoneal cytology, 8 cases with positive peritoneal cytology have not developed recurrence. This study showed that some endometrial cancer patients without myometrial invasion manifest recurrence, peritoneal dissemination or lymph node metastasis. It is therefore considered that lymphadenectomy and careful follow-up are necessary in every case of endometrial cancer, even in G1 cases without myometrial invasion.

[Clinical and pathological study of 24 patients with fallopian tube malignancy].

Clinical and pathological analyses were carried out in 24 patients with fallopian tube malignancy. The results are as follows: 1. Twenty-three patients had adenocarcinoma (well differentiated 11, moderately 6, poorly 6) and one patient had MMT (Müllerian mixed tumors). 2. There were 11 cases in stage I, 4 in stage II, 8 in stage III and 1 in stage IV according to Dodson's classification. 3. Atypical genital bleeding was the most common symptom, whereas 12.5% of patients had no symptoms. 4. Positive rates in cytology were 25.0% in cervico-vaginal smear and 55.0% in endometrial aspiration cytology. 5. The overall 5-year survival rate is 60.0%. The 5-year survival rates for stage I/II and stage III/IV were 88.9% and 16.7%, respectively. 6. The relapse rate of 16 patients treated with complete surgery was 25.0% (well differentiated 11.1%, moderately 0%, poorly 75.0%). Based on these observation, it is likely that patients with stage I/II have a good prognosis, however, patients with poorly differentiated adenocarcinoma have a high relapse rate, indicating that intensive adjuvant chemotherapy would be necessary in these cases. Cytologic examination, particularly endometrial aspiration cytology, is an effective method to use in detecting cases with fallopian tube malignancy early.

Combined assay of serum levels of CA125 and CA19-9 in endometrial carcinoma.

Serum levels of CA125 and CA19-9 were examined in 225 cases with endometrial carcinoma before treatment and 32 cases with recurrent endometrial carcinoma. The positive rates in the 225 cases were 27.1% for CA125, 24.0% for CA19-9, and 38.7% for the combined assay. The serum levels of both CA125 and CA19-9 significantly increased with surgical staging. The presence of lymph node metastasis and extrauterine spread exhibited a marked influence on the serum levels of both CA125 and CA19-9. Myometrial invasion and vessel permeation also increased serum levels of CA125, whereas peritoneal cytology and adnexal metastasis exhibited no influence on CA125 levels or CA19-9 levels. Twenty-five of 33 cases who showed either more than 100 U/ml of CA125 level or more than 100 U/ml of CA19-9 level were classified as surgical stage III or IV. The combined assay demonstrated a 71.9% positive rate at the time of detection of the recurrence (65.6% for CA125, 43.7% for CA19-9). In 34.4% of the 32 recurrent cases, elevated levels of the tumor markers were the first sign of recurrence. These data indicate that the use of CA19-9 in combination with CA125 is noteworthy in the management of patients with endometrial carcinoma.

[Pelvic lymph node metastasis in endometrial cancer].

Various factors associated with pelvic lymph node metastasis were analysed in 393 cases with stage I endometrial cancer treated with extended total hysterectomy, bilateral salpingo-oophorectomy and pelvic lymphadenectomy. The results are as follows. 1. The positive rates for lymph node metastasis and the relapse rate in 393 cases were 8.4% and 12.5%, respectively. 2. Myometrial invasion, tumor size, vessel permeation, cervical involvement, peritoneal cytology and adnexal metastasis were significant factors for lymph node metastasis. 3. Internal iliac nodes were the most common metastatic sites for cases with a single node metastasis, whereas external iliac nodes were the most common in all cases with lymph node metastasis. 4. The relapse rate for cases with single node metastasis was 40%, while the relapse rate for cases with three or more metastatic lymph nodes was 70%. 5. The relapse rate for 33 cases with lymph node metastasis was 54.5% and all of the relapse cases showed distant recurrence, whereas the relapse rate for 360 cases without lymph node metastasis was only 8.6%. It is therefore considered that pelvic lymphadenectomy is important in detecting high-risk patients and to reduce the relapse rate. Cytotoxic chemotherapy seems to be more adequate as adjuvant therapy for cases with lymph node metastasis.

[Recurrent endometrial cancer after complete surgery].

Between 1974 and 1991, 576 patients with endometrial cancer were treated with complete surgery. 1. The overall relapse rate was 14.4%. No recurrence was observed in surgical stage Ia, whereas 56.8% relapse was noted in stage IIIc. 2. The relapse rate in cases with adenocarcinoma was 13.0%. The relapse rate increased with poorer differentiation. Five of 6 cases with serous adenocarcinoma developed recurrence. 3. Myometrial invasion, size of tumor, vessel permeation, cervical involvement, adnexal metastasis and lymph node metastasis were significant factors for relapse. 4. Distant recurrence was noted in 63.9% of recurrent cases and 28.9% developed pelvic recurrence. The most common recurrent sites were lung, peritoneal cavity, and para-aortic lymph node in distant relapse and vaginal vault and parametrium in pelvic relapse. 5. The time to relapse ranged between 106 days and 5,518 days after complete surgery. Thirty one point three % of the recurrent cases manifested relapse within the first year, 60.2% within 2 years and 93.9% within 5 years. The mean time to relapse was 561 days. It was concluded that adjuvant therapy for cases with adnexal metastasis, lymph node metastasis, cervical involvement, adenocarcinoma (G3), and serous adenocarcinoma should be established urgently.

Assessment of sympathetic nerve activity controlling blood pressure in the elderly using head-up tilt.

In order to assess age-related changes in sympathetic nerve activity controlling blood pressure, we recorded muscle sympathetic nerve activity, blood pressure, and heart rate during head-up tilt in 10 healthy elderly (69-75 years) and 16 healthy young (19-23 years) subjects. The elderly had significantly lower responsiveness of muscle sympathetic nerve activity to postural change than did the young subjects. In the elderly, marked rise in blood pressure without increase in muscle sympathetic nerve activity was observed in nearly upright position during head-up tilt, whereas this phenomenon was not observed in the young. We conclude that neural control function of blood pressure during head-up tilt in the elderly differs from that in the young, which may be due to age-related change in baroreflex function.

Effects of aerobic exercise conditioning at intensities corresponding to lactate threshold in the elderly.

In this study we attempted to determine the effects of exercise training at the intensity corresponding to lactate threshold (Thla-) on various health-related variables in sedentary but apparently healthy elderly subjects. Six men and five women volunteers [mean age 68.9 (SD 3.4) years] performed supervised endurance-type training on stationary cycle ergometers for 30 min and recreational activities for 30 min, 3 days a week for 12 weeks. Four men and four women served as the control group [68.8 (SD 4.4) years]. As a result of the training programme, statistically significant increases in maximal oxygen consumption (10%), oxygen consumption at Thla- (18%), distance covered in 12-min walk, side step, and leg extensor power were found in the training group, while no changes occurred in the control group. The changes in serum cholesterol and triglyceride concentrations from the pre- to post-training period were statistically significant. High-density lipoprotein cholesterol remained unchanged, and low-density lipoprotein cholesterol tended to decrease following the training programme. These data would indicate that exercise training at the intensity corresponding to Thla- may have favourable effects on overall physical fitness and some serum lipid variables in older individuals.

Origin of CA125 and SCC antigen in human amniotic fluid.

Levels of CA125 and SCC antigen in amniotic fluids were examined in 26 cases. All samples showed high levels of CA125 and SCC antigen. Statistically significant decreases in CA125 (p < 0.05) and increases in SCC antigen (p < 0.01) from the second to the third trimesters were recognized. We focused on amnion cells as likely sources of the 2 tumor markers in amniotic fluid. Analysis of in vitro culture of amnion cells and amniotic membranes revealed an accumulation of CA125 and no accumulation of SCC antigen in the culture supernatant. A Northern blot analysis using a cDNA probe of SCC antigen ensured that there was no mRNA expression of SCC antigen in the amnion, the cord or the placenta. It is likely that the amnion is a major source of CA125 in amniotic fluid, and that the fetus is the origin of SCC antigen.

The relationship between the ambulatory variability and the laboratory reactivity of blood pressure and heart rate.

We examined the relationship between the ambulatory variability and the laboratory reactivity of systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) in 21 male college students. The mean increase during the waking period from that during sleep was defined as variability 1, and the standard deviation of the ambulatory measurements during waking was defined as variability 2. The reactivity to laboratory tests was measured by the pretask-to-task increase in variables. The high SBP variability 1 group showed a significantly higher reactivity of SBP and noradrenaline in the bicycle exercise test (70w). This suggests the possibility that SBP variability 1, can be predicted by increased reactivity to a mild limb exercise test. The high HR variability 1 group, and the high SBP variability 2 group showed significantly lower reactivity of stroke volume (SV) and cardiac output (CO) in the cold face test. Thus, parasympathetic responses such as trigeminal-brainstem-vagal pathway function or baroreflex sensitivity seemed to be reduced in these groups. The high DBP variability 2 group showed significantly lower DBP reactivity in the cold face test, and low noradrenaline reactivity in the mental arithmetic test, which indicated a reduced alpha-adrenergic response in this group. No other significant differences in reactivity to the laboratory mental stress tests were found between the variability 1 and variability 2 groups.
