[Chemotherapy of ovarian cancer with a combination of low-dose consecutive CDDP and cyclophosphamide].

Sixty-six patients with ovarian cancer were treated with low-dose consecutive CP (LDC-CP) consisting of cyclophosphamide (CPM: 500 mg/m2, day 1) and CDDP (10 mg/m2, days 1-7). Two-9 (median: 4) courses of LDC-CP were given following reduction surgery (42 cases) or preceding primary debulking (24 cases). Among 66 cases, 12 with stage Ic were not evaluable (NE). The response rate (CR + PR/evaluable) for stages II-IV was 57.4% (12 + 19/54). Histologically, serous and endometrioid type showed a significantly (p < 0.001) higher response rate (77.5% among 40 evaluable) compared to the other histologic type (0% among 14). Toxicities including nausea/vomiting and renal impairment were markedly mild or almost absent despite the lack of any particular care. Grade 3 leucopenia and thrombocytopenia were observed only in 4.2%, and 2.5% of total 284 courses, respectively. Mean survival time by stage was 1,309 days for stage I, 809 days for stage II, 1,180 days for stage III, and 691 days for stage IV, with a significant difference among stages (p = 0.0452). In stages III and IV disease, significant prognostic factors included 1) response to chemotherapy, 2) no. of LDC-CP courses, 3) histologic subtype, 4) performance status, and 5) tumor size. Thus, LDC-CP is considered to be a useful chemotherapeutic regimen for serous and endometrioid type ovarian cancer.

Expression of mRNA of SCC antigen in squamous cells.

The expression of SCC antigen mRNA was studied by Northern blot analysis and in situ hybridization in human gynecologic tissues. Northern blot analysis revealed that mRNA of SCC antigen was expressed strongly in normal squamous epithelium and columnar epithelium of the uterine cervix, but not in the endometrium, fallopian tube or ovarian tissue. Among gynecologic malignancies, squamous cell carcinoma of the uterine cervix expressed mRNA of SCC antigen, whereas endometrial and ovarian adenocarcinoma were negative. With in situ hybridization, mRNA of SCC antigen is located in the basal and parabasal layers of the normal squamous epithelium, in dysplasia, and also in carcinoma in situ and invasive squamous cell carcinoma. These results confirmed the previous findings by immunohistochemical studies that SCC antigen is closely related to squamous cells, but also raised a new puzzle regarding the different localization of mRNA and its product, SCC antigen, in the normal squamous epithelium.

Critical determinants of endurance performance in middle-aged and elderly endurance runners with heterogeneous training habits.

The current investigation was designed to determine which factor or what combination of factors would best account for distance running performance in middle-aged and elderly runners (mean age 57.5 years SD +/- 9.7) with heterogeneous training habits. Among 35 independent variables which were arbitrarily selected as possible prerequisites in the distance running performance of these runners, oxygen uptake (VO2) at lactate threshold (LT) (r = 0.781-0.889), maximal oxygen uptake (VO2 max) (r = 0.751 approximately 0.886), and chronological age (r = -0.736-(-)0.886) were found to be the 3 predictor variables showing the highest correlations with the mean running velocity at 5 km (V5km), 10 km (V10km), and marathon (VM). When all independent variables were used in a multiple regression analysis, any 3 or 4 variables selected from among VO2 at LT, chronological age, systolic blood pressure (SBP), atherogenic index (AI), and Katsura index (KI) were found to give the best explanation of V5km, V10km, or VM in a combined linear model. Linear multiple regression equations constructed for predicting the running performances were: V5km = 0.046X1-0.026X2-0.0056X3+5.17, V10km = 0.028X1-0.028X2-0.190X4-1.34X5+6.45, and VM = -0.0400X2-0.324X4-1.16X5+7.36, where X1 = VO2 at LT (ml.min-1.kg-1), X2 = chronological age, X3 = SBP, X4 = AI, and X5 = KI.(ABSTRACT TRUNCATED AT 250 WORDS)

Individualization of the cutoff value for serum squamous-cell carcinoma antigen using a sensitive enzyme immunoassay.

A sensitive enzyme immunoassay (IMx) for the squamous-cell carcinoma (SCC) antigen was used to evaluate low concentrations of this marker in the blood circulation. The assay can detect 0.05 ng/ml of serum SCC antigen, with intra- and interassay variabilities of 4.8 and 8.9%, respectively. Daily changes in serum SCC antigen were determined in healthy volunteers and in patients with cervical squamous-cell carcinoma after complete resection of the primary tumor, which revealed that, although the absolute values were variable from one case to another, the daily changes were relatively constant in each case, with a mean daily variation of 24.1 +/- 3.7% (mean +/- SE). The determination of the cutoff value for each individual patient may be effective for the early detection of an abnormal rise of circulating tumor marker, and will be useful in detecting small tumor foci during follow-up after treatment.

[Discriminant function analysis for evaluating the status of coronary heart disease risk].

We investigated to discriminate those individuals categorized by 1. obesity, 2. hypercholesterolemia, 3. hypertension, 4. low maximal oxygen uptake, 5. an abnormal electrocardiogram reflecting ischemic patterns, and/or 6. real sedentary life, from relatively healthier individuals without coronary heart disease (CHD) risk factors. One hundred and six Japanese women, aged 30 to 72 years, all of whom were in the postabsorptive state, were recruited in a series of tests for anthropometric and physiologic profiles both during the resting state and during the submaximal-maximal cycling exercise. Subjects were categorized into two groups--those who possessed four or more of the above 1, 2, 3, 4, 5, and 6 (high-CHD-risk group, n = 15) and apparently healthy individuals with a minimum number of risk factors (low-CHD-risk group, n = 83). Analyses of the data revealed that a combination of 8 variables extracted from among original 25 variables accurately classified 13/15 (87%) of high-CHD-risk group and 77/83 (93%) of low-CHD-risk group (mean = 90/98 or 92%) into their respective groups. The 8 variables were double product, Katsura index, waist girth, chest girth, TG, TC, and skinfold thicknesses at the subscapular and abdominal sites. Subsequent t-test identified significant differences between groups not only for VO2max, SBP and TC but also for DBP, LDLC, TG, Hb, HR, and HRmax. Most of these differences were of a much greater magnitude compared to the existing difference in chronological age. These findings suggest the usefulness and importance of anthropometric and blood lipid variables in the explanation of differences in the health status between high-CHD-risk women and their counterparts.

[Screening for endometrial cancer using a curettage stamp method].

The present study shows the value of a routine screening program for the detection of endometrial malignancies. From 1980 to 1985, 5,640 subjects were screened for endometrial malignancies using the curettage stamp method. Of 60 cases whose smears were abnormal, 25 were found to have malignant lesions. These patients included endometrial cancer (15 cases), sarcoma (3 cases) and metastatic cancer (4 cases). Three cases were found to have cervical cancer. Among 22 patients, excluding 3 cases with cervical cancer, 17 cases had negative results in a cervical smear test. Also 3 cases of endometrial cancer showed no history of abnormal genital bleeding. All 15 patients with endometrial cancer were over 40 years of age. These results indicated the clinical importance of the routine endometrial cytological smear in the detection of malignancies of the uterine body, especially in women over 40 years of age.

[Treatment of gynecological cancer with cis-platinum].

Seven patients with gynecological cancer were treated with cis-platinum. Cis-platinum at 50 mg/m2 was administered by intravenous infusion for 2 hours, followed by 2,000 ml of intravenous fluid. This treatment was repeated every three weeks. Five of the seven patients were evaluable. These patients included those with ovarian cancer (3 cases), metastatic ovarian cancer (1 case), and endometrial cancer (1 case). One patient attained a complete response, two a partial response, and two showed progressive disease. The response rate for both complete and partial response was 60%. Gastrointestinal symptoms were most frequently found. Nephrotoxicity was generally mild, but one patient had to discontinue cis-platinum treatment due to severe nephrotoxicity. Cis-platinum treatment is considered to be effective and useful for gynecological cancer. Second-look operation and two-channel chemotherapy were also discussed in this paper.

Clinical value of tumor markers for early detection of recurrence in patients with cervical adenocarcinoma and adenosquamous carcinoma.

OBJECTIVE: The clinical value of tumor markers for early detection of recurrence was investigated in 32 patients with cervical adenocarcinoma or adenosquamous carcinoma who had recurrent tumors. METHODS: Serum levels of CA 125, CA 19-9, carcinoembryonic antigen (CEA), and squamous cell carcinoma antigen (SCC), in addition to clinical status at the time of recurrence were investigated. RESULTS: Among the 32 patients, 26 had no symptoms at the time of recurrence. In 20 patients, elevated serum levels of tumor markers were the first sign of recurrence. In 21 patients with recurrent adenocarcinoma, the positive rates were 14% (CA 125), 62% (CA 19-9), 29% (CEA), and 5% (SCC). There were 71% of cases positive for CA 19-9 and/or CEA. In 11 patients with recurrent adenosquamous carcinoma, the corresponding positive rates were 37% (CA 125), 46% (CA 19-9), 64% (CEA), and 55% (SCC), with 100% positive for CA 19-9, CEA, and/or SCC. CONCLUSIONS: The combination of CA 19-9 and CEA is probably the most promising for detection of recurrent cervical adenocarcinoma. For adenosquamous carcinoma, the additional use of SCC is recommended.
