[Effect of a new Soviet-made antioxidant on the processes of lipid peroxidation in the treatment of patients with transient cerebro- vascular diseases and circulatory encephalopathy].

The authors present for the first time the results of investigation and treatment of 55 patients with transitory cerebral circulation disorders (TCCD) and circulatory encephalopathy by the new antioxidant drug emoxipine . The drug was noted to produce a beneficial clinical effect in 39 patients. After emoxipine treatment the patients demonstrated an increase of the endogenous antioxidants, evidence of the formation of the antioxidant depot. In the patients suffering from TCCD, the content of endogenous antioxidants did not differ from normal both before and after the treatment. Improvement of the patients' clinical status correlated with the drug dose and changes in lipid peroxidation. In patients with deterioration of the health status and without any changes, the dose-dependent effect was not recorded.

[Mild forms of multi-infarct dementia: effectiveness of cerebrolysin].

The randomized double-blind placebo-controlled study of the effectiveness of cerebrolysin in a mild form of multi-infarct dementia was carried out. Sixty patients were divided in two equal groups differing significantly at the beginning of the study by none of the compared parameters. They received for 28 days daily drop intravenous doses of cerebrolysin (15 ml of the drug in 200 ml of a 0.85% NaCl solution: 10 ml in the morning and 5 ml in the evening) or placebo in the same amount of the solution. The control of the therapy effectiveness was instituted clinically (by means of a special scale), electroencephalographically, and using the psychological test of Arnold-Kohlmann and the test for response time. The study found a significant improvement of the memory (p-0.01), abstract thinking and time of reaction in the patients on cerebrolysin, confirmed also by the EEG-mapping. It is believed advisable to use the drug cerebrolysin in mild forms of multi-infarct dementia.

[Mortality in the workers in the tire industry].

The article is concerned with a study of the mortality rates among the workers engaged at a tire-producing plant, for the 1950-1979 period. In comparison with the control population group, the contributors revealed increased mortality rates caused by cardiovascular diseases, malignant neoplasms, accidents, intoxications and traumas. Analysis of the labour conditions proved the influence of the unfavourable occupational factors on the mortality rates and mortality causes, excepting the indices of mortality caused by accidents, intoxications and traumas.

[Study of mortality among workers exposed to heating microclimate (epidemiological study)].

The article presents results of the study of mortality rates and causes of a retrospective cohort of workers engaged in a metallurgical plant, affected by different degrees of heating microclimate. The results of the study confirmed statistically verified high mortality risks among those engaged in the labour conditions characterized by the heating microclimate exceeding the admissible levels, as compared with the general male population in the Moscow region, with taking into account mortality from different causes (excepting accidents, traumas and poisonings), and from blood diseases, hypertension, IHD and respiratory diseases.

[Occupational morbidity due to the effects of vibration and noise in leading USSR industries and main objectives of its prevention].

The article presents data on the prevalence and dynamics of noise and vibration related occupational morbidity in different regions of the USSR, branches of industry and professional groups. Diseases caused by noise and vibration present major and ever growing occupational pathology in the Soviet Union. Particularly high morbidity rates were traced in the Russian Federation, Ukraine, Kazakhstan and Lithuania, primarily in coal mining, metallurgy, machinery construction branches of industry, and, as to occupational hypoacusis,--in metallurgy, some branches of machinery construction, automobile construction and light industry. Vibration related risk factors were revealed in such professional groups as choppers, emery polishers, tool sharpeners, loggers, drill operators; noise related risk factors--in blacksmith's works, weaving and some others. Certain decrease in occupational morbidity rate was registered before 1985 with a tendency to increase in the last 4-5 years. Prevention of vibration and noise related morbidity should include complex measures, modernization of industrial sites and health services improvement.

[Catecholamine metabolism in extrapyramidal disorders in patients with chronic hypertensive encephalopathy].

Total and cerebral metabolism of catecholamines (CA) was examined in patients with associated chronic hypertonic encephalopathy and extrapyramidal disorders. It was discovered for the first time that there was an increase in noradrenaline in the blood and cerebrospinal fluid together with a lowering of the content of dopamine and major metabolite thereof along the pathway of oxidative deamination of homovanillic acid. CA metabolism was shown to correlate with the patients' age, duration and gravity of extrapyramidal disorders. It is suggested that the revealed metabolic disorders of CA--neurotransmitters which are likely to underlie the development of neurological deficiency and characteristic mental disorders occur as a result of the diverse complex of biochemical alterations and are related both to the complex mechanisms of the pathogenesis of essential hypertension and ischemic injury to the brain as well as to the localization in the brain of lacunar infarctions. The complexity of the pathogenic treatment of the disease is emphasized. The main approaches to the pharmacological correction of the revealed disorders are outlined.

[Extrapyramidal syndrome in patients with hypertensive encephalopathy].

Sixty patients with associated discirculatory encephalopathy of hypertonic genesis and the extrapyramidal syndrome were examined. In order to compare the clinical manifestations with localization of foci of lesions, 51 patients underwent computer-aided brain tomography. The intensity of the extrapyramidal syndrome associated with has not been established to attain that in idiopathic parkinsonism. In patients with discirculatory encephalopathy, the extrapyramidal syndrome may be coupled with other symptoms pointing to diffuse impairment of the brain. In 49 cases out of 51 (81.6%), the computer-aided tomograms demonstrated different pathological alterations of the brain characteristic of chronic hypertonic encephalopathy, with those alterations primarily seen in the area of basal ganglia.

[Status of microcirculation and rheological properties of blood in patients with cerebral circulatory disorders].

Microcirculation and rheological properties of blood were investigated over time on the basis of the findings of biomicroscopy of the bulbar conjunctiva in 61 patients with different stages of circulatory encephalopathy in the presence of arterial hypertension and atherosclerosis. Rheological properties of blood were changed at all stages of disease. Correlations between the ability of erythrocytes to aggregation and the degree of their intravascular aggregation by the findings of biomicroscopy of the bulbar conjunctiva were not found. The importance of investigation of the intravascular aggregation of erythrocytes and theological properties of blood for assessment of the cerebral vascular microcirculation, control of therapy, and verification of some aspects of the mechanism of action of various drugs was shown.

[Determining the leading occupational and non-occupational factors in the planning of priority measures for reducing the incidence of diseases with temporary disability].

The research showed the possibility of using Kulback's informatory indices for distributing the qualitative properties connected with the morbidity levels with temporary disability among the workers of an instrument-making plant. Among the factors studied, the most informative were: marital state (K-7.2), psychological climate in the family (K-6.8), satisfaction with one's profession (K-5.3). Measures for optimizing personal life provide a complementary source of lowering morbidity with temporary disability.

[Evaluation of the carcinogenic hazard in the manufacture of graphite articles].

As a result of complex hygienic, clinical and cancer epidemiologic studies, dust proved to be a major occupational hazard. Coke and graphite dust absorbs polycyclic aromatic hydrocarbons, including benz(a)pyrene in concentrations exceeding maximum permitted levels. Screening of 746 graphite workers followed by clinical examination established increased incidence of diseases of the upper respiratory tract (chronic rhinitis pharyngitis, laryngitis, etc.), bronchi and lungs (chronic bronchitis or pneumonia) and skin (hyperkeratosis, naevi, etc.). Among respiratory diseases were hypertrophic laryngitis, papillomatous bronchitis and angiofibroma of the larynx which are now considered to be precancer. Cancer epidemiologic studies referred graphite workers to a group of high risk of cancer.

[Dynamics of blood serotonin contents in acute disorders of cerebral circulation and circulatory encephalopathy].

Blood serotonin levels have been measured by spectrofluorometry in 145 patients with various manifestations of acute disorders of the cerebral circulation (ADCC): with ischemic stroke, cerebral hypertensive crises, transient impairments of the cerebral circulation, and with circulatory encephalopathy (DE) in the presence of arterial hypertension combined with atherosclerosis. In ADCC patients the blood serotonin has been measured 7 times, in DE ones 3 times. This level has been found changed in the patients with ischemic stroke and with cerebral hypertensive crises for a long time after the cerebral circulation impairment. Serotonin levels correlated with the condition severity and course in ADCC and DE. The vestibulocerebellar syndrome in DE has been treated more effectively, particularly with trental. The blood serotonin level may be regarded as a prognostic criterion of the disease outcome and of the treatment efficacy.

[Correlation of chronic idiopathic headache and decreased monoamine/oxidase activity of thrombocytes].

A total of 19 males with chronic idiopathic headache referred to in the literature as tension headache were examined. A secondary nature of headache was completely ruled out by additional studies. The activity of platelet monoamine oxidase was depressed to 14.33 +/- 1.17 n mol/mg of protein/h as compared with 28.1 +/- 2.38 n mol/mg of protein/h in the control group. The role of diminished activity of the enzyme in the genesis of headaches is considered.
