Reverse perfusion-metabolism mismatch predicts good prognosis in patients undergoing cardiac resynchronization therapy: a pilot study.

BACKGROUND: Cardiac resynchronization therapy (CRT) improves glucose metabolism in the septum of patients with heart failure, so in the present study the predictive value of combined fluorodeoxyglucose (FDG)-positron emission tomography (PET) and metoxy-isobutyl isonitrile (MIBI)-single photon emission computed tomography (SPECT) for the prognosis of patients undergoing CRT was investigated. METHODS AND RESULTS: Fourteen patients (70.3+/-8.2 years) who underwent FDG-PET and MIBI-SPECT before implantation of a biventricular pacemaker were enrolled. The total number of matches, mismatches, reverse mismatches, summed difference score (SDS: sum total of FDG - MIBI scores) and SDS per segment (%SDS) in each of 5 areas of myocardium (septum, anterior, lateral, inferior area, apex) was calculated and compared between the survival groups (all survival: survival group; survival without ischemic heart disease (IHD): non-IHD survival group) and non-survival group. Both the number of reverse mismatch segments and the %SDS in the septum in the non-IHD survival group were significantly greater than in the non-survival group (3.2+/-1.6 vs 0.5+/-0.6, p<0.05; 0.62+/-0.61 vs -0.11+/-0.19, p<0.05). The receiver-operating characteristics curves for prognosis showed that the area under the curve for the number of reverse mismatch segments in the septum (0.93; confidence interval 0.61-0.98) was significantly greater. CONCLUSION: A reverse mismatch pattern in the septum can predict a good prognosis for patients treated with CRT.

Targeted functional imaging of estrogen receptors with 99mTc-GAP-EDL.

PURPOSE: To evaluate the feasibility of using (99m)Tc-glutamate peptide-estradiol in functional imaging of estrogen receptor-positive [ER(+)] diseases. METHODS: 3-Aminoethyl estradiol (EDL) was conjugated to glutamate peptide (GAP) to yield GAP-EDL. Cellular uptake studies of (99m)Tc-GAP-EDL were conducted in ER(+) cell lines (MCF-7, 13762 and T47D). To demonstrate whether GAP-EDL increases MAP kinase activation, Western blot analysis of GAP-EDL was performed in 13762 cells. Biodistribution was conducted in nine rats with 13762 breast tumors at 0.5-4 h. Each rat was administered (99m)Tc-GAP-EDL. Two animal models (rats and rabbits) were created to ascertain whether tumor uptake of (99m)Tc-GAP-EDL was via an ER-mediated process. In the tumor model, breast tumor-bearing rats were pretreated with diethylstilbestrol (DES) 1 h prior to receiving (99m)Tc-GAP-EDL. In the endometriosis model, part of the rabbit uterine tissue was dissected and grafted to the peritoneal wall. The rabbit was administered with (99m)Tc-GAP-EDL. RESULTS: There was a 10-40% reduction in uptake of (99m)Tc-GAP-EDL in cells treated with DES or tamoxifen compared with untreated cells. Western blot analysis showed an ERK1/2 phosphorylation process with GAP-EDL. Biodistribution studies showed that tumor uptake and tumor-to-muscle count density ratio in (99m)Tc-GAP-EDL groups were significantly higher than those in (99m)Tc-GAP groups at 4 h. Among (99m)Tc-GAP-EDL groups, region of interest analysis of images showed that tumor-to muscle ratios were decreased in blocking groups. In the endometriosis model, the grafted uterine tissue could be visualized by (99m)Tc-GAP-EDL. CONCLUSION: Cellular or tumor uptake of (99m)Tc-GAP-EDL occurs via an ER-mediated process. (99m)Tc-GAP-EDL is a useful agent for imaging functional ER(+) disease.

Correlation between plasma brain natriuretic peptide concentration and lung thallium-201 uptake on exercise thallium perfusion images.

The plasma brain natriuretic peptide (BNP) concentration at rest correlates with left ventricular end-diastolic pressure (LVEDP), left ventricular ejection fraction (LVEF), and pulmonary capillary wedge pressure (PCWP). High lung thallium-201 uptake has been reported to be associated with hemodynamic variables such as LVEDP, LVEF, and PCWP. However, there is no study that has investigated the correlation of plasma BNP concentration with lung thallium-201 uptake. We examined whether the plasma BNP concentration was related to lung thallium-201 uptake. Before exercise, venous blood samples were obtained from 39 patients with old myocardial infarction. We investigated the correlations between plasma BNP concentration and lung thallium-201 uptake, and whether they were related to LVEF, extent of nonviable myocardium, and ischemic myocardium, respectively, with thallium-201 exercise stress testing. The plasma BNP concentration significantly correlated with lung thallium-201 uptake (P < 0.05), nonviable segments (P < 0.01), and LVEF (P < 0.01). Lung thallium-201 uptake correlated with nonviable segments (P < 0.01). Our results suggest that increased secretion of BNP is related to increased lung thallium-201 uptake, and they are related to the extent of nonviable myocardium and decreased left ventricular function. Plasma BNP concentration and lung thalium-201 uptake may reflect the extent of myocardial fibrosis causing myocyte stretch.

Evaluation of myocardial glucose metabolism before and after recovery of myocardial function in patients with tachycardia-induced cardiomyopathy.

BACKGROUND: We assessed left ventricular (LV) function and myocardial glucose metabolism by fluoro-18-deoxyglucose (18F-FDG) positron emission tomography (PET) in patients with tachycardia-induced cardiomyopathy (TC). METHODS: The subjects were 42 patients with heart disease, consisting of 7 patients with TC (61.4 +/- 19.0 years, LVEF 34.1%+/- 10.6%) and 35 with ischemic heart disease (IHD) (63.1 +/- 10.8 years, LVEF 49.9%+/- 13.5%). Five volunteers with normal ECG were the control group. All of the patients underwent 18F-FDG PET and echocardiography, and all of the patients with TC underwent 18F-FDG PET and echocardiography before and 6 months after antitachycardia therapy. Six patients underwent radiofrequency catheter ablation (RFCA) and 1 patient was medically treated with antitachycardia therapy. Myocardial glucose metabolism was assessed semiquantitatively by using the % dose uptake of 60 kg of BW (% dose uptake). RESULTS: Mean % dose uptake of the control group was 5.52 +/- 0.54%. After antitachycardia therapy, LVEF significantly improved (34.1 +/- 10.6% vs 54.3 +/- 13.6%, P < 0.01), and % dose uptake also significantly improved (1.26 +/- 0.55% vs 1.49 +/- 0.62%, P < 0.05). Patients with IHD showed higher % dose uptake than those with TC before antitachycardia therapy (3.18 +/- 1.36 vs 1.26 +/- 0.55%, P < 0.01), controls showed higher value of % dose uptake than TC before antitachycardia therapy (5.52 +/- 0.54% vs 1.26 +/- 0.55%, P < 0.01). CONCLUSION: Semiquantitative analysis of 18F-FDG PET showed that antitachycardia therapy improved myocardial glucose metabolism in patients with TC.

Prostate cancer: a comparative study of 11C-choline PET and MR imaging combined with proton MR spectroscopy.

PURPOSE: Prostate cancer is difficult to visualise in its early stages using current imaging technology. The present study aimed to clarify the utility of 11C-choline PET for localising and evaluating cancer lesions in patients with prostate cancer by conducting a prospective comparison with magnetic resonance (MR) imaging combined with proton MR spectroscopy. METHODS: PET and MR imaging combined with proton MR spectroscopy were performed in 20 patients with prostate cancer. Correlations among the metabolite ratio of choline + creatine to citrate (Cho+Cr/Ci) on MR spectroscopy, serum PSA and maximum standardised uptake value (SUVmax) of (11)C-choline were assessed. The location of the primary lesion was assessed by the site of SUVmax and the laterality of the highest Cho+Cr/Ci ratio and confirmed by examination of surgical pathology specimens (n=16). RESULTS: PET exhibited a diagnostic sensitivity of 100% (20/20) for primary lesions, while the sensitivities of MR imaging and MR spectroscopy were 60% (12/20) and 65% (13/20), respectively. Weak linear correlations were observed between SUVmax and serum PSA (r=0.52, p<0.05), and between SUVmax and Cho+Cr/Ci ratio (r=0.49, p<0.05). Regarding the localisation of main primary lesions, PET results agreed with pathological findings in 13 patients (81%) (kappa=0.59), while MR spectroscopy results were in accordance with pathological findings in eight patients (50%) (kappa=0.11). CONCLUSION: This preliminary study suggests that 11C-choline PET may provide more accurate information regarding the localisation of main primary prostate cancer lesions than MR imaging/MR spectroscopy. A further clinical study of 11C-choline PET in a large number of patients suspected of prostate cancer will be necessary to determine the clinical utility of 11C-choline PET in patients who clinically require biopsy.

Value of 18F-FDG PET in the detection of peritoneal carcinomatosis.

PURPOSE: Peritoneal carcinomatosis can be difficult to diagnose using computed tomography (CT). The purpose of this study was to evaluate the role of 2-(fluorine 18) fluoro-2-deoxy-D-glucose (FDG) positron emission tomography (PET) in the detection of peritoneal carcinomatosis. METHODS: We reviewed the CT and FDG PET radiological reports and clinical charts of 18 patients with peritoneal carcinomatosis and 17 cancer patients without peritoneal carcinomatosis. We also assessed FDG PET scans from 20 healthy volunteers as a baseline study. The maximum standardised uptake values (SUVmax) over peritoneal lesions in cancer patients and over the area of most intense intestinal uptake in healthy volunteers and cancer patients without peritoneal carcinomatosis were measured. RESULTS: The sensitivity and positive predictive value (PPV) of combined FDG PET and CT were superior to those of CT alone for the detection of peritoneal lesions (sensitivity: 66.7% vs 22.2%, p<0.025; PPV: 92.3% vs 50.0%, p<0.05). The most frequent pattern of FDG uptake in patients with peritoneal carcinomatosis was abnormally intense focal uptake near the abdominal wall. An SUVmax threshold of 5.1 produced a diagnostic accuracy of combined FDG PET and CT of 78%. The additional information provided by FDG PET allowed a more accurate diagnosis in 14 patients (40.0%), and led to alteration of the therapeutic strategy in five (14.3%) of the enrolled cancer patients. CONCLUSION: We found that use of an intra-abdominal FDG uptake cut-off value for SUVmax of >5.1 assists in the diagnosis of peritoneal carcinomatosis. FDG PET may play an important role in the clinical management of patients with suspected peritoneal carcinomatosis.

The clinical relevance of thymic fluorodeoxyglucose uptake in pediatric patients after chemotherapy.

The purpose of this study was to clarify the clinical implications of thymic fluorodeoxyglucose (FDG) uptake after chemotherapy in pediatric patients with malignant disease. Twenty-two pediatric patients, aged 0-17 years (mean 7.0 years), who had undergone FDG positron emission tomography (PET) were examined retrospectively. A total of 48 FDG-PET scans of the 22 patients were reviewed. Seven PET scans were recorded before the initial chemotherapy, and the remaining 41 scans were conducted during and after chemotherapy. Thymic FDG uptake was evaluated using standardized uptake value (SUV) analysis. The effect of chemotherapy on thymic FDG uptake was assessed by comparing SUV before, during, and after chemotherapy. The change in thymic FDG uptake with increasing time after the completion of chemotherapy was also assessed. Clinical laboratory data including number of white blood cells (WBCs), erythrocytes (RBCs), platelets, plasma glucose level and differential white blood count were analyzed for their association with thymic FDG uptake. Thymic FDG uptake in patients during chemotherapy was significantly lower than that in patients before chemotherapy (mean+/-SD 1.71+/-0.48 vs 2.27+/-0.32, respectively, P<0.01). Thymic FDG uptake in patients after chemotherapy was significantly higher than that in patients during chemotherapy (mean+/-SD 2.74+/-0.70 vs 1.71+/-0.48, respectively, P<0.01). A significant relationship between thymic FDG uptake and interval after completion of chemotherapy (r=0.74, P<0.0001) was observed. Significant relationships between blood counts (WBCs, RBCs, and platelets) and thymic FDG uptake were also observed. Comparison of the differential white blood count and thymic FDG uptake revealed a significant relationship only with lymphocyte count. Thymic FDG uptake in pediatric patients after completion of chemotherapy should not be mistaken as recurrent or metastatic thymic malignancy. Thymic FDG uptake appears to be associated with thymic function or hematopoietic function in bone marrow. Interpretation of FDG-PET must be made with this consideration in mind.

Clinical usefulness of ECG-gated 18F-FDG PET combined with 99mTC-MIBI gated SPECT for evaluating myocardial viability and function.

OBJECTIVES: This study sought to evaluate an imaging approach using gated 99mTc-MIBI (MIBI) SPECT and gated 18F-FDG (FDG) PET for assessment of myocardial viability and cardiac function. METHODS: Forty-eight patients (38 men, mean age 68.1 +/- 9.6 years) underwent ECG-gated FDG PET and MIBI SPECT within a week. The baseline diagnoses were coronary artery disease (31), mitral regurgitation (1), paroxysmal arrhythmia (10), and dilated cardiomyopathy (6). The gated FDG PET data were analyzed using pFAST software, and the gated MIBI SPECT data were analyzed using QGS software. Fifteen patients were diagnosed with myocardial infarction, and follow-up study was performed to assess the functional outcome four months later. An improvement in LVEF of >5% was defined as significant. The LV myocardium was divided into 17 segments, and regional defect scores were visually assessed using a 4-point scale for each segment (0 = normal, 1 = mildly reduced, 2 = moderately reduced, 3 = absent). A segment with a greater defect score on MIBI SPECT than on FDG PET was defined as a mismatch. The patients were divided into two groups: those with at least two mismatched segments (MM-group), and those with none or one (M-group). RESULTS: LVEF, EDV and ESV measured by gated FDG PET were highly correlated with those obtained by gated MIBI SPECT (r = 0.848, 0.855 and 0.911, p < 0.0001, respectively). The mean values of LVEF did not differ significantly, but EDV and ESV obtained by gated FDG PET were significantly grater than those obtained by gated MIBI SPECT (p < 0.0001). In 15 patients diagnosed with myocardial infarction, a significant association (p < 0.05) was found between the relative uptake of FDG PET and MIBI SPECT and the functional outcome 4 months later. Global LV function improved in 6 of the 8 patients showing mismatch but in only 1 of the 7 patients with matched defects, resulting in a sensitivity of 86% and specificity of 75%. The overall accuracy to predict global functional outcome was high (80%). CONCLUSION: This imaging approach allows accurate evaluation of myocardial viability. Furthermore, the high correlations of gated FDG PET and gated MIBI SPECT measurements hold promise for the assessment of left ventricular function using gated FDG PET.

Diagnostic use of T2-weighted inversion-recovery magnetic resonance imaging in acute coronary syndromes compared with 99mTc-Pyrophosphate, 123I-BMIPP and 201TlCl single photon emission computed tomography.

BACKGROUND: The incidence of missed diagnoses of acute cardiac ischemia in the emergency department could be reduced by a new imaging modality. In the present study, the clinical significance of (99m)Tc-pyrophosphate (PYP), (123)I-beta-methyl-p-iodephenyl-pentadecanoic acid (BMIPP), (201)TlCl scintigraphy (imaging) and T2-weighted inversion-recovery magnetic resonance imaging (MRI) for the detection of culprit lesion in patients with acute coronary syndromes (ACS) was compared. METHODS AND RESULTS: The study group comprised 18 patients with ACS: 12 patients with acute myocardial infarction (AMI) (11 males; mean age, 63+/-11 years) and 6 patients with unstable angina (UA) (3 males, mean age, 67+/-5 years). Of the 12 patients with AMI, 10 underwent (201)TlCl and PYP single photon emission computed tomography (SPECT) studies as a dual-energy acquisition ((201)TlCl/PYP) and 8 underwent (201)TlCl SPECT within 1 week of the BMIPP study. All 18 patients underwent BMIPP SPECT and MRI. The MRI pulse sequence was black blood turbo short-inversion-time inversion recovery (STIR) (breath-hold T2-weighted studies). The T2-weighted inversion-recovery MRI showed higher sensitivity and negative predictive value than PYP and (201)TlCl, and higher specificity and positive predictive value than BMIPP and (201)TlCl. The area under the receiver-operating characteristic curve for PYP, BMIPP, (201)TlCl and MRI was 0.787, 0.725, 0.731 and 0.878, respectively. The difference between the areas of MRI and BMIPP was significant (p<0.05). CONCLUSION: Accurate detection of culprit lesion is improved by using MRI rather than BMIPP, particularly for patients with ACS.

FDG-PET scanning after radiation can predict tumor regrowth three months later.

PURPOSE: Positron emission tomography (PET) with 2-[(18)F]fluoro-2-deoxy-D-glucose (FDG) is well known for providing excellent clinical information regarding malignant tumors. We investigated whether dual-time FDG-PET performed immediately post radiation could predict early regrowth of malignant tumors. MATERIALS AND METHODS: Twenty patients with malignant tumors were included in this study. All patients received radiation, and each underwent FDG-PET before the initiation of therapy and within 10 days of completing their course of irradiation. PET images after irradiation were obtained at 60 min and 180 min post FDG injection. For 26 lesions in 20 patients, standardized uptake value (SUV) before and after treatment was calculated and then correlated with postradiation tumor response and outcome at 3 months status post irradiation. RESULTS: Retention index [RI = (SUV on delayed image - SUV on early image)/SUV on early image] after irradiation showed a significant difference between patients with residual tumor and those without residual tumor at 3 months status post irradiation (p < 0.0025). All 9 lesions in 6 patients with residual tumors showed more than 0.1 of RI, whereas none of the lesions with less than 0.1 of RI revealed residual tumors. CONCLUSIONS: Dual-time FDG-PET imaging just after irradiation is potentially useful for predicting early regrowth of malignant tumors.

Performance evaluation of a hand-held, semiconductor (CdZnTe)-based gamma camera.

We have designed and developed a small field of view gamma camera, the eZ SCOPE, based on use of a CdZnTe semiconductor. This device utilises proprietary signal processing technology and an interface to a computer-based imaging system. The purpose of this study was to evaluate the performance of the eZ scope in comparison with currently employed gamma camera technology. The detector is a single wafer of 5-mm-thick CdZnTe that is divided into a 16x16 array (256 pixels). The sensitive area of the detector is a square of dimension 3.2 cm. Two parallel-hole collimators are provided with the system and have a matching (256 hole) pattern to the CdZnTe detector array: a low-energy, high-resolution parallel-hole (LEHR) collimator fabricated of lead and a low-energy, high-sensitivity parallel-hole (LEHS) collimator fabricated of tungsten. Performance measurements and the data analysis were done according to the procedures of the NEMA standard. We also studied the long-term stability of the system with continuous use and variations in ambient temperature. Results were as follows. INTRINSIC ENERGY RESOLUTION: 8.6% FWHM at 141 keV.LINEARITY: There was excellent linearity between the observed photopeaks and the known gamma ray energies for the given isotopes. INTRINSIC SYSTEM UNIFORMITY: For the central field of view, the integral uniformity and the differential uniformity were, respectively, 1.6% and 1.3% with the LEHR collimator and 1.9% and 1.2% with the LEHS collimator. SYSTEM SPATIAL RESOLUTION: The FWHM measurements made at the surface of the collimator were 2.2 mm (LEHR) and 2.9 mm (LEHS).CONTRAST TEST: The average S/N ratios (i.e. counts in the irradiated pixel divided by counts in the surrounding pixels) for the inner ring pixels (8)/outer ring pixels (16) using the LEHS collimator and LEHR collimator were 3.2%/0.2% and 3.7%/0.3%, respectively. COUNT RATE CHARACTERISTICS: We could not determine the maximum count rate and the 20% loss count rate from these data because the plateau was not reached while using the solutions measured. SYSTEM SENSITIVITY: The average acquisitions were 11,052 cpm/MBq (LEHR) and 28,590 cpm/MBq (LEHS). TEMPERATURE DEPENDENCE: The system displayed minimum corresponding shift in cps with temperature changes in the measured temperature range. We designed and developed a semiconductor-based gamma camera using CdZnTe. The basic performance of this camera compares favourably with the existing gamma camera technology that is deployed in the medical field today. The most significant differences include the spatial resolution, sensitivity, high count rate characteristics and energy resolution. We believe that this device will be of value for a number of clinical applications including sentinel node detection and radiopharmaceutical-guided surgery.

Scoring analysis of salivary gland scintigraphy in patients with Sjögren's syndrome.

The purpose of the present study was to evaluate the validity of a scoring system based on excretion rate of salivary gland scintigraphy in patients with Sjögren's syndrome (SjS). Total scintigraphic scores were compared with the results of the Saxon test. One hundred and twenty-four subjects who were clinically diagnosed with SjS and 11 normal ones underwent salivary gland scintigraphy and the Saxon test. In salivary gland scintigraphy, the difference between maximum and minimum counts after stimulation using vitamin C divided by maximum counts was defined as the excretion rate. We then defined a scoring system with 4 grades: severe dysfunction = 3 (excretion rate < 25%), moderate dysfunction = 2 (25% < or = excretion rate < 40%), mild dysfunction = 1 (40% < or = excretion rate < 50%) and normal function = 0 (50% < or = excretion rate). The summation of the total scintigraphic score (0-12) of all 4 salivary glands was used as a semi-quantitative index indicating total salivary gland function, and total scintigraphic scores were compared with the results of the Saxon test. A significant inverse linear correlation (R2 = 0.95) was observed between total scintigraphic scores and mean values of the Saxon test within a range of abnormal scintigraphic scores (> or = 4). The scoring system developed in the present study is a clinically available, objective, and reproducible method for evaluation of salivary gland function in patients with SjS.

Effect of biventricular pacing on myocardial glucose metabolism in patients with heart failure using fluoro-18-deoxyglucose positron emission tomography.

Biventricular pacing has recently been found beneficial in the treatment of congestive heart failure (CHF). Meanwhile, positron emission tomography (PET) has emerged as a new method to analyze glucose metabolism in the heart. Five patients (mean age 68.8 +/- 8.1 years, 4 men) who received biventricular pacing therapy for 5.8 +/- 6.6 weeks for CHF were studied. Myocardial glucose metabolism was evaluated by PET with fluoro-18-deoxyglucose (18F-FDG), and percent uptake (%uptake) of 18F-FDG was calculated during biventricular pacing and compared with that during 1 hour of conventional RV pacing. Biventricular pacing was associated with a significant decrease in NYHA functional Class from 3.67 +/- 0.52 to 2.50 +/- 0.55. After 18F-FDG PET, three of five patients remained clinically stable, and two died during follow-up. Mean 18F-FDG %uptake during biventricular pacing was not different than during short-term RV pacing (62.1 +/- 18.4 vs. 63.6 +/- 17.0%). However, patients who remained clinically stable had a lower value of 18F-FDG %uptake in the septal region than patients who died (46.9 +/- 5.6 vs 80.3 +/- 1.3%, P < 0.01). One patient whose cardiac function improved significantly also had a small septal region of decreased 18F-FDG uptake during RV pacing. In conclusion, biventricular pacing therapy was effective in this small group of patients with severe, drug-resistant CHF. An evaluation of the effects of biventricular pacing on glucose metabolism in the subacute phase may help identify patients with a favorable long-term response to this therapy.

Clinical value of iodine-123 beta-methyliodophenyl pentadecanoic acid (BMIPP) myocardial single photon emission computed tomography for predicting cardiac death among patients with chronic heart failure.

In the present study, the effectiveness of 123I-beta-methyliodophenyl pentadecanoic acid (BMIPP) single photon emission computed tomography (SPECT) for predicting cardiac death of patients with chronic heart failure was evaluated. Abnormalities of fatty acid metabolism are found in patients with chronic heart failure and BMIPP was developed as a tracer for scintigraphic assessment of myocardial fatty acid utilization. The study group comprised 74 patients with chronic heart failure with a left ventricular ejection fraction (LVEF) <45% on left ventriculography or radionuclide angiocardiography. They underwent both 201Tl SPECT and BMIPP SPECT. The uptake of tracer was scored semiquantitatively from 0 (normal) to 4 (defect) in 20 segments and a total defect score (TDS) for all 20 segments was calculated. On planar images the mediastinum to heart count ratio (H/M) was calculated for the BMIPP and Tl studies, and the H/M(BMIPP):H/M(Tl) (H/M(BMIPP) divided by H/M(Tl)) was also calculated. The mean follow-up period was 660 days and there were 17 cases of cardiac death. Multivariate analysis identified H/M(BMIPP):H/M(Tl) (p<0.05) and LVEF (p<0.05) as independent predictors of cardiac death. The receiver-operating characteristic curve of H/M(BMIPP):H/M (Tl) was situated to the left relative to LVEF. Analysis of the myocardial metabolism by BMIPP SPECT can predict the high-risk patients with chronic heart failure.

Anti-tachycardia therapy can improve altered cardiac adrenergic function in tachycardia-induced cardiomyopathy.

We investigated whether anti-tachycardia therapy might improve the altered cardiac adrenergic and systolic function in tachycardia-induced cardiomyopathy (TC) in contrast to dilated cardiomyopathy (DCM). The subjects were 23 patients with heart failure, consisting of 8 patients with TC (43.6 +/- 10.0 yrs) and 15 with DCM (45.3 +/- 8.2 yrs). TC was determined as impairment of left ventricular function secondary to chronic or very frequent arrhythmia during more than 10% of the day. All patients were receiving anti-tachycardia treatment. Cardiac 123I-MIBG uptake was assessed as the heart/mediastinum activity ratio (H/M) before and after treatment. LVEF was also assessed. In the baseline study, H/M and LVEF showed no difference between TC and DCM (2.21 +/- 0.44 vs. 2.10 +/- 0.42, 35.3 +/- 13.1 vs. 36.0 +/- 10.9%, respectively). After treatment, the degree of change in H/M and LVEF differed significantly (0.41 +/- 0.34 vs. 0.08 +/- 0.20, 20.5 +/- 14.4 vs. -2.1 +/- 9.6%, p < 0.01). In TC, heart failure improved after a shorter duration of treatment (p < 0.05). In conclusion, anti-tachycardia therapy can improve altered cardiac adrenergic function and systolic function in patients with TC over a shorter period than in those with DCM.

[Efficacy of biventricular pacing on myocardial glucose metabolism in a patient with heart failure using 2-deoxy-2[18F]fluoro-d-glucose positron emission tomography: a case report].

A 61-year-old-male with dilated cardiomyopathy and congestive heart failure was treated by implantation of a biventricular pacemaker. Myocardial glucose metabolism was evaluated by positron emission tomography with 2-deoxy-2[18F]fluoro-d-glucose (FDG) during atrial-right ventricular pacing or atrial-biventricular pacing. Percentage uptake of FDG decreased in the lower septal area of the left ventricle during right ventricular pacing, but the decrease was improved during biventricular pacing (74.7% vs 91.6%). Biventricular pacing may improve myocardial glucose metabolism in patients with congestive heart failure.
