[Surgical remodeling of the left ventricle in ischemic cardiomyopathy].

The paper covers early and long-term results of left ventricle geometric reconstruction and myocardial revascularization in 44 patients with ischemic cardiomyopathy. The authors have established clinical and hemodynamic criteria of ischemic cardiomyopathy, indications for surgical treatment and a surgical technique. The article presents a new approach to correction of mitral insufficiency in the given category of patients. According to the results of the study, the necessary conditions for a surgical intervention to be successful are: intraoperative monitoring of central hemodynamics using Swan-Ganz catheter, evaluation of myocardial contractile function by means of transoesophageal echocardiography and wide use of preoperative intraaortic balloon pumping. Geometric reconstruction results in normalization of left ventricle shape and its end-systolic and end-diastolic volumes, marked increase of ejection fraction and improvement of central hemodynamics. The stability of the long-term results has been confirmed in a 4-year follow-up.

[Application of sorbilact in complex correction of enteral insufficiency syndrome in surgical patients].

The most important teoretical aspects of the enteral insufficiency syndrome formation in an acute abdominal surgical diseases were suggested. Basing on the treatment results analysis of 19 patients with different abdominal surgical diseases, using hypertonic sorbitol solution, the authors confirm high therapeutic efficacy of the preparation, administration of which have had promoted the intestinal motor function restoration and optimization, as well as adapting action of the preparation in enteral probe nutrition conduction in patients during ealy postoperative period.

[Surgical treatment of acute intestinal obstruction].

The experience with treatment of acute ileus for the 5 years in 1032 patients is summarized. Operated on were 269 patients. The different types of operative interventions with a wide use of intubation, adsorbents, laparostomy were performed. The postoperative lethality was 9.2%.

[The effect of picolinic acid and iron ions on the proliferation of SPEV cells].

The effect of picolinic acid (PA) on SPEV cell proliferation is found to be different from that on normal and virus transformed NRC cells, and on spontaneously transformed CHO cells. It is shown that SPEV cells are arrested by PA at the end of G1-phase and at the beginning of S-phase and probably in G2-phase of the cell cycle. Ferrous ions remove the G1/S block induced by PA to permit the cell transfer through S-phase. On the one hand, PA chelates ferrous ions from the cells, and on the other one it inhibits the replicative DNA synthesis. It can be suggested that PA may arrest the SPEV cell growth affecting the iron-depend stable radical formation which is introduced into the active centre of ribonucleotiDE reduCTase. This results in the lower enzyme activity.

[The effect of the intracellular level of free iron on cell proliferation in culture].

Dependence of changes in the intracellular free iron content upon cellular proliferation has been studied on mammalian cells in vitro. It has been established that picolic acid (PA)--a natural metal chelating agent of variable valency--inhibits the proliferation of cultivated pig embryo kidney cells (SPEV). Simultaneously the free iron quantity was decreased 2-fold as compared with the norm. PA block was removed by substituting PA-containing cultivated media for the PA-free one. It was accompanied by complete recovery of the free iron quantity in the cells. The regulation of cell proliferation is likely to correlate with the intracellular free iron content.
