Laryngeal papillomas: the epidemiology in a Danish subpopulation 1965-1984.

The incidence rate of laryngeal papillomas in a Danish subpopulation (approximately 2.8 million inhabitants) was 3.84 x 10(-6) per year in the period 1968-1984. For juvenile papillomas the incidence rate was 3.62 x 10(-6), compared with 3.94 x 10(-6) for laryngeal papillomas of adult onset. When comparing different time periods a significantly low incidence was found in the time 1965-1968, while the incidence remained constant in 1969-1984. The low incidence rate in the early period may be real, but selectional bias may have played a part. It is general anticipated that maternal genital HPV-infections may serve as an HPV-reservoir, and that juvenile laryngeal papillomas is a result of HPV transmission from the mother to the child during birth. In the period in question cervical HPV-infections have been recorded with increasing frequency in younger women, indicating that the prevalence is rising. However, this is not reflected in the incidence of laryngeal papillomas.

Squamous cell carcinoma of the oral cavity: a retrospective analysis of treatment and prognosis.

A retrospective study of 267 patients with an oral cavity squamous cell carcinoma is presented. It includes all patients from a geographically well-defined area during an 11 and one-quarter year period. Thirty-five percent of the patients were treated with radical surgery, 40% with curative-dosage radiotherapy and 22% with combined therapy. One-year survival with No Evidence of Disease (NED) was 48%. Almost all recurrences were within the first year following completion of the initial treatment. Treatment results were especially ineffective in large local tumour cases (T3 and T4) and in instances with extensive regional lymph node metastases (N2 and N3). The observed 5-year survival ('crude survival') was 44%. All parameters for the course of disease following diagnosis and treatment were significantly correlated to disease stage.

Treatment of glomus tumours. A retrospective survey.

A retrospective survey of symptomatology, treatment and course of disease in 45 patients with glomus tumours treated between 1959 and 1986 at the Radium Centre and ENT Department of the Aarhus University Hospital is presented. Nine patients were treated surgically, 7 had surgery and irradiation combined, and 25 patients were treated solely with radiation therapy. Six patients developed recurrence of tumours. Two patients died of tumour, one of them with pulmonary metastases.

Laryngeal papillomas: clinical aspects in a series of 231 patients.

The records of 231 patients with laryngeal papillomas were reviewed. Considering age at onset and sex distribution, the patients were divided into four groups: juvenile solitary, juvenile multiple, adult solitary, and adult multiple papillomas. The main conclusions are (1) it is in general impossible to predict the course in individual cases, but solitary papillomas in adults follow a less severe course than do juvenile and adult multiple cases; (2) late recurrences (up to 42 years) makes it impossible to declare a patient cured; (3) there is no linkage between puberty and the disappearance of laryngeal papillomas; (4) a latent infection, acquired at or before birth, is unlikely to be responsible for adult papillomas.

Acoustic rhinometry: evaluation of the nasal cavity with septal deviations, before and after septoplasty.

We introduce acoustic rhinometry as a new, objective method to assess the geometry of the nasal cavity. The cross-sectional area of the nasal cavity as a function of distance from the nostrils was obtained. A group of 21 patients with septal deformities was examined with acoustic rhinometry preoperatively and postoperatively. These values were compared with those of 21 normal control subjects. The minimal cross-sectional area (MCA) is located in the anterior part of the nose, and it shifts anteriorly under the effect of decongestion. The preoperative value of MCA is related to the location and severity of the anterior septal deformity. Postoperative smaller MCA found in the opposite side of that narrowed by a severe anterior septal deformity may be explained by the impact of septoplasty without reduction of a hypertrophic turbinate. A highly significant relation between MCA and the subjective feeling of nasal patency, before and after surgery, suggests that MCA is a valuable parameter to express nasal patency. Correction of posterior septal deformities is found to increase significantly the cross-sectional area posteriorly. The effect of decongestion in the postoperative values, however, suggests that the mucosa contributes even more to the cross-sectional area of the posterior part of the nose. Acoustic rhinometry seems very suitable for evaluation of the nasal cavity in cases where septoplasty and turbinoplasty is considered, as well as for the postoperative evaluation.

Pharyngo-cutaneous fistulae after laryngectomy. Influence of previous radiotherapy and prophylactic metronidazole.

The development of a pharyngocutaneous fistulae is a major complication after total laryngectomy. In Denmark radiotherapy is the primary treatment for all laryngeal carcinomas. Based on the experience with conventional daily irradiation, a split-course radiation schedule was introduced in 1978. The charts of 106 consecutive patients laryngectomized for recurrence in the years 1975 to 1984 were examined. Thirty-four patients developed a fistula. An evaluation of the different radiotherapy schedules used during this period allowed a dose-response curve to be constructed. It showed a pronounced increase of fistulae with high doses of radiotherapy. Split-course radiotherapy caused a rise in late complications and did not improve tumor control. Large field sizes increased the number of fistulae. High-dose fractions showed a surprisingly high incidence of late complications. Prophylactic metronidazole (introduced in 1980) resulted in a highly significant decrease in the frequency of postoperative fistulae. Patients in whom fistula formed were hospitalized for an average of 54 days, patients without, for 22 days.

Prospective epidemiological study of common colds and secretory otitis media.

To evaluate how often a common cold induces secretory otitis media (SOM) two groups of young children were studied. One comprised 373 children aged 9 months to 7 years who were being minded in institutions. The other group consisted of 210 children not minded in day institutions or private day care. This latter group was subdivided into children minded at home or in private day care attended by not more than 2 children (117 children), and the remainder (93 children). Common colds were defined as nasal discharge, and tympanometry was carried out 6 times at 2-week intervals. Among the 1-year-olds common colds induced SOM in 83% of those minded in institutions and in 56% of those minded at home. At the age of 5 years this applied to 20% of both groups.

A prospective epidemiological study of secretory otitis media in young children related to the indoor environment.

To assess the influence of housing upon the middle ear status in young children in the form of secretory otitis media (SOM), 210 children not looked after in kindergarten or municipal day-care were investigated. Tympanometry was performed 6 times at 2-week intervals, and the parents filled in a questionnaire concerning housing conditions. In addition to the number of times the children had suffered from SOM, we evaluated long-lasting cases, and the frequency of common colds. Age affected all 3 parameters and sex 1, the boys having a larger number of long-lasting episodes. As to housing factors, the type of housing was of importance, children living in flats being most apt to get SOM, and children on farms least, but this difference disappeared after a regression analysis. Both major and minor airing resulted in fewer cases of SOM than did average airing. Children with a history of allergy (but not of allergic rhinitis) caught colds more often than others.

Prospective epidemiological study of secretory otitis media in children not attending kindergarten. An incidence study.

To assess the incidence of secretory otitis media in children minded at home we examined 210 children not attending kindergarten or municipal day care. Of these children 117 were minded exclusively at home or in private day care with up to 2 children. Tympanometry was carried out 6 times at 2-week intervals. Secretory otitis media (SOM) of more than 3 months' duration was found in 17% of the one-year-olds and in 6% of the total series. In the age range 1-5 years the incidence was from 0.35 to 0.19/child/month, and the mean for the total group was 0.26/child/month.

Prospective epidemiological study of secretory otitis media in children not attending kindergarten. A prevalence study.

This prevalence study of secretory otitis media (SOM) comprised 210 children who were not attending or had not attended kindergarten. Children in home care/private day care with up to two children were analyzed separately. In this group the prevalence peak was at one year, being at that age about 40%. The prevalence decreased from the age 1-2 years, but thereafter it was steady at about 15-20%. The rest of the children showed another at the age of 4-5 years. Bilateral SOM was most common at the age of one year, but also showed another peak at 5 years of age. [corrected]

Hearing improvement after stapedectomy: up to 19 years' follow-up period.

The long-term results of 925 stapedectomies were analysed after a mean follow-up period of 15 years. Patients who had the operation before the age of 30 were analysed separately. The entire group deteriorated by an average of 0.91 dB/year at 500-2000 Hz, while the under-30 group deteriorated by 0.67 dB/year. There were but slight differences in the conductive component, considering frequencies and age. The perceptive loss/year was not far from that in a normal population.
