Hyperechoic pattern in breast cancer--its bio-acoustical genesis and tissue characterization.

Clinical breast echography is increasingly being used as it offers a high degree of diagnostic accuracy rate. Internal echoes of usual breast cancer is hypoechoic in the majority of cases, however hyperechoic mass lesion may appear in rare instances. This echo-pattern was seen in a 38-year-old housewife with invasive ductal carcinoma. Also, its bio-acoustical mechanism, that is, why hyperechoic pattern appears, was investigated from the viewpoint of ultrasonic tissue characterization and its related papers were reviewed. Tumor heterogeneity in cellularity such as cribriform pattern, tubular structure, solid nests and a scirrhous pattern of cancer cells may play an important role in producing the hyperechoic breast pattern.

[An autopsy case of malignant histiocytosis-like disorder following hypersensitive reaction to mosquito bite].

An autopsy case of an 18-year-old Japanese girl with a malignant histiocytosis-like disorder that developed during the course of a hypersensitive reaction to mosquito bite is reported. Episodes of hypersensitive reactions to mosquito bite had been repeated since she was 12 years old and at the age of 18 years she died of acute respiratory failure only 11 days after a mosquito bite. On autopsy, the dermal reaction to the last mosquito bite had already calmed down, and 'poorly differentiated histiocytes', which were presumed from their histochemical characteristics, had remarkably infiltrated into multiple organs, appearing like leukemic infiltration. Only a small number of them were noted in the lymph nodes and the bone marrow. The results of histochemical examination of 'poorly differentiated histiocytes' was as follows: (1) Neither markers for granulocytes (peroxidase and naphthyl AS-D Cl esterase) nor markers for lymphocytes and plasma cells (leukocyte common antigen and immunoglobulins) were detected. (2) Some markers for histiocytes (peanut lectin agglutinin and lysozyme) were positive in some of the proliferated cells. (3) A marker for T-zone histiocyte (s-100 protein) was negative. These results suggested that the proliferated cells included cells of the monocyte-macrophage system. These cells morphologically showed no phagocytic activity and were suggested to be immature histiocytes. Jurco et al. reported (poorly differentiated) malignant histiocytosis consisting of immature histiocytes without phagocytic activity, by using histochemical methods.(ABSTRACT TRUNCATED AT 250 WORDS)

Diffuse pleural rhabdomyosarcoma with persistent pleural effusion.

A unique case of embryonal rhabdomyosarcoma arising at the left pleura of a 7-year-old Japanese girl is reported. The present case was characterized by persistent pleural effusion, and the malignant cells incidentally found in it were the first diagnostic clue. The tumor showed a rare growth pattern involving diffuse thickening of the parietal pleura. Biopsy of the thickened parietal pleura upon thoracotomy revealed embryonal rhabdomyosarcoma largely composed of immature mesenchymal cells. Immunohistochemical demonstration of creatinine phosphokinase-MM was most helpful among several types of immunostain for the histopathological diagnosis. Ultrastructurally, thin filaments with primitive Z bands could be seen in some tumor cells. Intensive clinical examination revealed only diffuse thickening of the parietal pleura, which was reduced by chemotherapy. This is the first documented case of rhabdomyosarcoma arising at the pleura. Previous reports of rhabdomyosarcoma arising at unusual sites are reviewed and the histogenesis of this tumor is briefly discussed.

Isoenzymes of alkaline phosphatase in germinoma cells.

Utilizing 18 germinomas, the characteristics of isoenzymes of alkaline phosphatase (ALP) in germinoma cells were examined by light and electron microscopic enzymohistochemical and immunohistochemical techniques and biochemical methods including inhibition tests, electrophoresis and electrosyneresis. ALP in germinoma cells was generally located on the cell surface. In a few germinoma cells, however, ALP was seen not only on the cell membrane but also in the endoplasmic reticulum. Enzymohistochemically as well as biochemically, ALP in germinoma cells was little sensitive to L-phenylalanine, moderately sensitive to L-homoarginine and somewhat resistant to heat in its inhibition tests. Immunohistochemical examinations and electrosyneresis showed the presence of the placental type of ALP in germinoma cells and biochemical analyses revealed that the heat-stable component of ALP in germinoma cells was consistent with the D-variant of the placental type of ALP. Therefore, germinoma cells possessed mixed isoenzymes of ALP, consisting mainly of the liver/bone type and a small amount of D-variant of the placental type on the cell surface. The prominent expression of ALP in germinoma cells may be due to the enhanced expression of gonadal genes active in the germinoma genome.

Studies on lymphocyte subsets in thymic tissue and functions of peripheral lymphocytes in patients with tumorous thymus with special reference to myasthenia gravis.

In order to obtain a better understanding of the immunological abnormalities present in myasthenia gravis (MG), which is often accompanied by thymoma or thymic hyperplasia, we investigated lymphocyte subsets and their functions using samples of thymoma or thymic hyperplasia tissues from 11 patients (6 cases with MG), and peripheral blood from 6 patients (4 cases with MG). In most thymic tissues from patients with MG, a maturating tendency of lymphocytes was generally observed. Especially in the medulla of thymic hyperplasia, an entirely peripheral blood type of T-lymphocytes, which were Leu-6- and either Leu-2a+ or 3a + 3b+, were encountered abundantly. Therefore, the presence of abnormal maturation of lymphocytes in the thymus or destruction of the barrier between the thymus and the peripheral blood in MG cases was indicated. In cases without MG, no such tendency was noted. As to the peripheral blood in patients with MG, concanavalin A-induced suppressor cells were significantly decreased (p less than 0.01). All of these changes were considered to be intimately related to the appearance of MG.

Effects of diltiazem on suppression and regression of experimental atherosclerosis.

The effects of diltiazem (a calcium antagonist) on the suppression and regression of atherosclerosis were studied. Thirty-one rabbits were fed a 1% cholesterol (atherogenic) diet together with saline (n = 22) or diltiazem (n = 9) injections. After 10 weeks, seven rabbits that received saline and nine rabbits that received diltiazem were killed. The remaining 15 saline-treated rabbits were then put on a standard (regression) diet for the next 15 weeks with saline (n = 7) or diltiazem (n = 8) injections. Sixteen rabbits given a standard diet were used as controls. At 5 and 10 weeks, the plasma LDL cholesterol level in rabbits on the atherogenic diet with diltiazem was significantly lower than in those on the atherogenic diet with saline. The aortic total cholesterol, esterified cholesterol and calcium contents were also significantly lower in rabbits on the atherogenic diet with diltiazem. After 25 weeks (15 weeks on the regression diet), the differences in aortic total cholesterol and calcium contents between the two groups on the regression diet were not significant; however, the aortic esterified cholesterol content was significantly lower in the regression diet with diltiazem. The results suggest that diltiazem has a favourable effect both on regression and on suppression of atherosclerosis.

Identification of testicular atypical germ cells by an immunohistochemical technique for placental alkaline phosphatase.

The identification of atypical testicular germ cells is often difficult by by routine histologic examination. By immunohistochemical detection of placental alkaline phosphatase (PLAP) and by periodic acid Schiff staining of glycogen, atypical germ cells were easily identified in testicular samples. Forty-one fetal and adult testes were used for a preliminary study, and 121 testes from infants and adults with either cryptorchidism or germ cell tumors were studied for the presence of atypical germ cells. Two types of clear germ cells were differentiated histochemically, and one with PLAP-positive cell surfaces and glycogen-rich cytoplasm was considered to be atypical. The alkaline phosphatase of atypical germ cells appeared to be similar to that found in a few germ cells of early fetal testes. The atypical germ cells seemed to be multi-potential malignant cells capable of developing not only into seminoma but also into other germ cell tumors. Only in yolk sac tumor of infants were the atypical germ cells absent from tumor-adjacent seminiferous tubules.

[An autopsy case of lymphoid interstitial pneumonia with monoclonal gammopathy].

An autopsy case of a 75-year-old female with lymphoid interstitial pneumonia with monoclonal gammopathy of IgA kappa type is reported. The patient was admitted to UOEH Hospital suffering from a cough, sputum and fever. After examining a specimen of transbronchial lung biopsy and chest X ray films, lymphoid interstitial pneumonia was suspected. Serum immunological examination showed monoclonal IgA kappa type hypergammaglobulinemia. Marked clinical and radiological improvement was attained after prednisolone administration. After 16 months, however, the patient deteriorated and expired on January 21, 1986. On postmortem examination, a number of plasma cells as well as lymphoid cells were seen to have infiltrated in the thickened interalveolar septa and the parenchyme of the lymph nodes throughout the whole body. The architecture of lymph nodes remained preserved, while the cortex as well as the medulla was almost completely replaced by plasma cells and lymphoplasmacytoid cells. Immunohistochemical examination demonstrated the presence of IgA kappa in the plasma cells and some of the lymphoplasmacytoid cells in both lymph nodes and lungs. These findings suggest that the proliferating plasma cells and lymphoplasmacytoid cells were in neoplastic or preneoplastic states and support the hypothesis that lymphoid interstitial pneumonia may be an expression of the systemic lymphoproliferative disorders in the lung.

Suppression of atherogenesis in hypercholesterolemic rabbits by chondroitin-6-sulfate.

The effect of chondroitin-6-sulfate, obtained from shark cartilage, on atherogenesis in rabbits fed a high-cholesterol diet was studied. Male Japanese white rabbits were housed for 10 weeks in three groups, one group was fed ordinary pellets and was injected intraperitoneally with saline (standard-diet group), one was fed pellets containing 1% cholesterol and was injected intraperitoneally with saline (cholesterol-diet group), and the third group was fed pellets containing 1% cholesterol, and was injected intraperitoneally with 10 mg of chondroitin-6-sulfate (C-6-S group). Injections were done daily. The plasma total cholesterol, and cholesterol from very low-density lipoprotein in the C-6-S group after 5 weeks in the test period, and low-density lipoprotein cholesterol in the C-6-S group at the end of the test period were lower than those of the cholesterol-diet group. Significantly fewer atherosclerotic lesions of the aortic surface were found macroscopically in the C-6-S group than in the cholesterol-diet group. The cholesterol, esterified cholesterol and calcium concentrations of the aortic intima-media in the C-6-S group were significantly lower than in the cholesterol-diet group. Hydroxyproline levels in these three groups were not different. The uronic acid concentration of the intima-media in the cholesterol-diet group was significantly higher than in the C-6-S group (P less than 0.02). Though the percentage of heparan sulfate on total glycosaminoglycans (GAGs) of the C-6-S group was lower than in the cholesterol-diet group, there were no significant differences in the percentages of dermatan sulfate and chondroitin-4/6-sulfate in total GAGs between the cholesterol-diet and C-6-S groups. These results suggest that chondroitin-6-sulfate suppresses cholesterol deposition in the aorta of rabbits fed a 1% cholesterol diet, probably partly due to a decrease in the plasma low-density lipoprotein cholesterol, and partly due to a change in arterial metabolism.

[A pathologic review of thymic neoplasms].

Pathology, particularly histology and histogenesis, of thymic neoplasms is reviewed. In this report, thymic neoplasms include thymoma, lymphoma, carcinoid and germ cell tumors (teratoma group) originating from the thymus. Ultrastructures, enzymohistochemistry and immunohistochemistry have increased the knowledge on the relation between the histogenesis, morphology and function of each neoplasm. Recently, various kinds of monoclonal antibodies have easily been available, and the use of them in immunohistochemistry is useful for differentiating the subpopulation of lymphocytes in thymomas. Some interpretations referring to the microenvironment by thymoma cells and developing T lymphocytes are described. We stressed that peanut lectin was useful for identifying cortical thymocytes in paraffin sections, and functions of S-100 protein positive cells in thymomas should be investigated for elucidating one of the questions concerning the microenvironment by thymoma cells.

Intranuclear membranous profiles in germinoma cells--a variant of nuclear pockets and intranuclear annulate lamellae.

When ultrastructurally examining 24 germinomas comprising 12 seminomas, 4 dysgerminomas, 1 mediastinal germinoma, and 7 intracranial germinomas, intranuclear membranous profiles were noticed in 17 germinomas, ranging from 20-100 nm in width and 3 microns in length. With occasional connections to the nuclear envelope through a small hole, intranuclear membranous profiles in germinoma cells were considered as clefts of the nuclear envelope. While most frequently situated under the inner nuclear membrane, they varied in configuration as well as distribution. As sequestered round mass of nuclear material, they were a variant of nuclear pockets containing nucleoplasm. Intranuclear annulate lamellae were occasionally present apart from the nuclear envelope and connected with nuclear clefts. Eleven of the twelve seminomas and 6 of the twelve non-seminomatous germinomas showed intranuclear membranous profiles, and the incidence of such profiles was much higher in seminomas than in non-seminomatous germinomas. Intranuclear membranous profiles facing the inner nuclear membrane were also noted in spermatogonia in adolescents and adults. It was suggested that intranuclear membranous profiles in germinoma cells could be structures following ones occasionally seen in spermatogonia.

Hepatocellular carcinoma associated with familial polyposis of the colon. Report of case.

A case is reported of a 33-year-old man with hepatocellular carcinoma associated with familial polyposis of the colon. During subtotal colectomy for diffuse colonic polyposis, a small tumor was excised from the right lobe of the liver. Histologic examination revealed hepatocellular carcinoma. The association of primary hepatoma in familial polyposis of the colon is very rare. However, it is well known that familial polyposis of the colon has a potent oncogenicity not only in the colon but also in the extracolonic organs. The hepatoma may also be one of the extracolonic malignant manifestations in this inherited disease.

The biological behaviour of gastric cancer.

A characterisation of the biological behaviour of gastric carcinoma was attempted by relating the histological classification to the mean number of metastases for each case (metastatic index). The material was chosen from a consecutive series of 294 gastric cancer autopsies in which only conservative therapy had been given or in which the cancer was first diagnosed at autopsy. According to the extent of metastasis it was possible to divide the nine histological types into two groups of limited invasiveness and generalised invasiveness. Unfavourable biological behaviour of the cancer was determined by the presence of the latter group of histological types in the primary site even in cases where this histological type formed the minor component of the tumour. The variation in histology within the primary tumour and between the primary site and the metastatic sites was related both to histological affinities and to the metastatic index associated with the primary site histology. It is suggested that this dual classification system adds significantly to our understanding of the natural history of gastric cancer.

An ultrastructural study on germinoma cells.

Germinoma cells were ultrastructurally studied in 17 cases composed of 8 cases with testicular seminoma, 3 cases with ovarian dysgerminoma, 4 cases with intracranial germinoma and one case with mediastinal germinoma. The tumor cells were generally polyhedral in shape, and has frequently a cytoplasmic process, occasionally a long process like a pseudopod. Regarding several components of the cytoplasm, most of the tumor cells in all but few cases had varying numbers of glycogen granules and fatty droplets, some of tumor cells in all but few cases had annulate lamellae, junctional apparatuses and microvilli, while stacks of rER or large autolysomes were observed in some of the tumor cells in several cases. In some of the seminoma cells and dysgerminoma cells observed were one or more of 4 types of intranuclear pseudoinclusions. Definite differences among germinoma cells associated with their originating sites were not recognized except for a few features of the nucleus.
