Pregnancies after intrafallopian transfer of embryos.

Six patients with a history of infertility of more than 4 years were offered in vitro fertilization (IVF) followed by translaparoscopic embryo transfer to the fallopian tubes. Three of the patients became pregnant. In one patient the oocytes did not fertilize in vitro. Intrafallopian transfer of embryos may be an alternative to gamete intrafallopian transfer (GIFT) or IVF, especially in those cases where confirmation of fertilization is wanted.

Isolation by using albumin columns of a cohort of fast-swimming human spermatozoa.

Unwashed human spermatozoa enter columns of 20% albumin in cohorts and not in a continuous stream. Using normospermic ejaculates, a single cohort, representing approximately 10% of the total number of spermatozoa added, appeared within 1 h of addition. In contrast, when ejaculates from asthenozoospermic men were tested using the same incubation period (1 h), the cohorts contained less than 5% of the total. Exposure of ejaculates to 2-deoxyadenosine, a potent stimulator of sperm motility, doubled the number of spermatozoa present in the cohort, but not the average velocity of the group. We suggest that spermatozoa also enter the cervical mucus and pass through the female reproductive tract in cohesive groups.

Oocyte retrieval in an IVF program. A comparison of laparoscopic and transvaginal ultrasound-guided follicular puncture.

From 1 April 1986 to 20 May 1987, 160 oocyte retrievals were performed in our IVF program. 80 oocyte retrievals were done by laparoscopy under general anaesthesia and 80 under light sedation and local analgesia using transvaginal ultrasound guided follicle puncture. More oocytes were recovered with the ultrasound procedure than with laparoscopy, 616 versus 478. The fertilization rate was higher when the oocytes were collected with the ultrasound procedure, vis-à-vis laparoscopy, 70.6% versus 59.6%. The number of embryos transferred was greater in the former than in the latter group, 300 versus 182. There were 13 clinical pregnancies in the laparoscopy group versus 17 in the ultrasound group. There were no serious complications in either group. Transvaginal ultrasound guided follicle aspiration is a safe and simple procedure.

Zeta waves: a special type of slow delta waves.

A special type of delta waves with a duration of 1-3 sec which, because of their saw-tooth or zed shape in the EEG, we have named 'zeta waves' has been described. They occur particularly in cases with rather severe brain lesions, usually with an acute or subacute onset and a space occupying character. In a period of 2 years during which 2500 EEGs have been reported we have seen zeta waves in 20 patients in whom 76 EEGs have been recorded. The characteristics of these waves and the types of lesions with which they occurred are described. The importance of an adequate recording technique for proper presentation of this EEG pattern is emphasized.

Ejaculate composition after masturbation and coitus in the human male.

The levels of fructose, acid phosphatase, citric acid, zinc, maltase, testosterone, prostaglandin E, putrescine, spermidine and spermine were compared in ejaculates collected by masturbation and after coitus in the same individuals. Coitus was associated with a significantly larger semen volume and increased concentrations and total amounts of prostaglandin E and polyamines in the ejaculate. In contrast, the concentrations of the conventional glandular parameters remained relatively unaffected, although a slight dilution of the seminal vesicle contribution to the ejaculate occurred after coitus. In the majority of cases, ejaculates provided by masturbation were significantly enriched in spermatozoa, but the total contributions under both conditions remained the same. It is concluded that the assessment of semen quality in male individuals using the conventional parameters of accessory gland function can be made with ejaculates provided by masturbation or coitus.

Biochemical correlates of asthenozoospermia: a lesion at the level of the sperm adenylyl cyclase.

Studies of the adenylyl cyclase in spermatozoa from 10 asthenozoospermic and 10 normal men revealed major differences with regard to the activation of the enzyme by various drugs and metal ions. While the response to acetate ions was normal, activation of the enzyme by forskolin and the GTP analogue [Gpp(NH)p] was uniformly impaired. In addition, in 4 of the patients the ratios of the Mn2+- and Mg2+-dependent activities of the enzyme were significantly higher than normal, suggesting a delay in the transformation of the enzymes catalytic unit from the soluble to the membrane-bound form. No differences could be recorded in a variety of seminal plasma components including prostatic acid phosphatase, citric acid, zinc, putrescine, spermidine, spermine, fructose, prostaglandin E, and testosterone. Furthermore, the DNA fluorescence patterns of the spermatozoa measured by flow cytometry were within the range of control variations, suggesting that chromatin condensation, and thus nuclear maturation, proceeded normally. The study suggests that a lesion in the adenylyl cyclase system of the germ cells may explain at least some of the motility disturbances seen in infertile males.
