Bicaudate index in computerized tomography of Huntington disease and cerebral atrophy.

Ventricular sizes on computerized tomographic (CT) scans were compared in seven patients with Huntington disease, 20 patients with cerebral atrophy, and 20 normal controls. The bicaudate index--the ratio of the width of both lateral ventricles at the level of the heads of the caudate nuclei to the distance between the outer tables of the skull at the same level--significantly discriminated among the three groups. The bicaudate indices were: Huntington disease 0.209 +/- 0.007, cerebral atrophy 0.121 +/- 0.006, and controls 0.092 +/- 0.003.

Embolization and surgical excision of giant arterio-venous malformation.

A case of a giant arteriovenous malformation involving almost the entire dominant hemisphere in a 32-year-old white male with progressive neurologic deterioration, presumbaly due to cerebral steal, is described. Special management included surgical intravascular embolization with latex spheres. and at a later date, resection of the arteriovenous malformation.

Benign pituitary adenoma associated with hyperostosis of the spenoid bone and monocular blindness. Case report.

The authors describe a case of benign chromophobe adenoma associated with hyperostosis of the lesser wing of the sphenoid bone and monocular blindness in a 38-year-old woman. The endocrinological and radiological evaluations were all suggestive of a meningioma. The diagnosis was established by biopsy of the tumor mass. After orbital decompression and removal of the tumor, the patient was treated with radiation therapy. Her postoperative course was uneventful, and her visual defects remained fixed.

Elephantiasis of eyelids following repeated craniotomy. Case report.

A case is reported in which craniectomy for removal of right frontal meningioma was complicated by porencephaly. A peculiar granulomatous area was found over the brain at reoperation for cranioplasty. Iodine solution was used to paint the reactive material under the scalp flap. Giant swelling of the isilateral eyelids gradually developed, ascribed to interference with lymphatic drainage from the eyelids. There was no communication between eyelids and cerebrospinal fluid spaces. The enlarged eyelids were removed by ophthalmic plastic surgery.

Bilateral optic canal meningioma.

A case of bilateral optic canal meningiomas which presented with progressive visual deterioration and optic atrophy is described. Complete neurologic investigation was negative, only surgical exploration disclosed the tumors.

Development of above normal language and intelligence 21 years after left hemispherectomy.

Neuropsychologic follow-up studies of a 5 1/2-year-old boy who had left hemispherectomy for seizures showed that he had developed superior language and intellectual abilities. These findings contrast with recent reports indicating that the plasticity of the nervous system may have been overrated and contradict reports of restricted development of language functions after left and of nonlanguage functions after right hemispherectomy for perinatal lateralized brain lesions. Following removal of the left hemisphere, including "the classical language zones," the right hemisphere and other intact residual structures may provide the necessary substrata for the development of above normal adult language and intellectual capacities.
