[Clinical and microbiological efficacy of metronidazole and ornidazole in the treatment of urogenital trichomoniasis in men].

The aim of the study was to compare the efficacy of metronidazole and ornidazole in the treatment of urogenital trichomoniasis in men. The drugs were used randomly in usual doses as monotherapy: 210 patients, metronidazole and 217 patients, ornidazole. The clinical efficacy of metronidazole and ornidazole was stated in 57.6 and 94.5% of the patients and the microbiological efficacy was stated in 77.1 and 98.2% of the patients respectively. The side effects were recorded in 59.0 and 3.7% of the patients respectively. Thus, ornidazole proved to be a more efficient and safe agent vs. metronidazole in the treatment of urogenital trichomoniasis.

[Relationship between CagA-bearing strains of Helicobacter pylori and gastrointestinal pathology].

AIM: To study the prevalence of CagA-positive strains of Helicobacter pylori in Novosibirsk residents and to assess their relation to some clinical parameters. MATERIAL AND METHODS: Random sample of the population (218 males and 220 females aged 25-64 years) was examined. Antibodies against Helicobacter pylori and CagA protein were tested in the sera using enzyme immunoassay. Gastrointestinal symptoms were evaluated with questionnaire. RESULTS: The prevalence of H. pylori infection was 88%, among H. pylori-positive persons CagA-positivity was revealed in 58%. Presence of antibodies to CagA protein was associated with dyspepsia (OR = 2.5, 95% CI 1.2-2.5) and history of peptic ulcer (OR = 3.6, 95% CI 1.0-14.5). CagA-positive subjects more frequently reported bowel disturbances, diarrhea, in particular (OR = 2.9, 95% CI 1.2-7.4). CONCLUSION: The data showed similar distribution of H. pylori and CagA-positivity among adult population in Russia and Eastern European countries. CagA-positivity is associated with peptic ulcer and dyspepsia.

[Significance of different methods in evaluation of prevalence of cholelithiasis in population: pathoanatomical and lifetime ultrasonic diagnostics].

AIM: To evaluate the occurrence of cholelithiasis (CL) diagnosed by routine autopsy and to compare these data with the results of ultrasonographic examination of the general population. MATERIAL AND METHODS: Autopsy protocols (n = 1124) from a large general hospital were analysed, and a representative sample of Novosibirsk population (1678 examinees aged 25-64 years) was examined by ultrasonography. RESULTS: Prevalence of CL increased with age and was five-fold higher in females than in males. Both methods showed good concordance. The age-adjusted CL prevalence rates in 25-64 year-old groups was 1.94% in males and 9.53% in females in autopsy series, and 2.16% and 11.16%, respectively, in ultrasonographic study. CONCLUSION: Routine autopsy may serve as a reliable tool in evaluating prevalence of CL among the population provided the values are gender- and age-standardized.

[Association between the level of serum pepsinogen I and markers of the Helicobacter pylori infection in teenagers].

The concentration of pepsinogen I as well as antibodies to Helicobacter pylori and CagA protein were determined in the blood serum in 84 schoolchildren at the age of 14-17 using the immune-enzyme analysis. The level of pepsinogen I did not depend on the sex, age, gastroenterological symptoms and ulcer occurrence in parents. However, the concentration of pepsinogen was higher in teenagers infected with H. pylori (89.6 ug/l) than in non-infected ones (69.4 ug/l, p = 0.012), this increase taking place mostly due to strains containing CagA.

[Dyspepsia and gastroesophageal reflux in adolescents].

AIM: To study the prevalence and risk factors for dyspepsia and gastroesophageal reflux (GER) among adolescents. MATERIAL AND METHODS: Senior schoolchildren (forms 9-11) of four randomly selected secondary schools of Novosibirsk participated in the study (189 boys and 260 girls aged 14-17 years). They filled in the Bowel Disease Questionnaire, and sera were tested for antibodies against Helicobacter pylori infection. RESULTS: Dyspepsia was reported by 21% of the schoolchildren, being more frequent in girls (25%) than boys (14%, p = 0.004). GER occurred in 22% of adolescents with the same frequency in both genders. The combination of dyspepsia with GER was found in 8%. The prevalence of H. pylori infection was 56%. No association was found between the infection and abdominal symptoms, except heartburn. Among factors associated with dyspepsia were female gender, irregular meals, family history of dyspepsia. GER was related to H. pylori infection, obesity, smoking and irregular meals, 46% of adolescents with dyspepsia and 36% of those with GER have consulted a physician, and school absenteeism was reported by 55 and 40%, respectively. CONCLUSION: Dyspepsia and gastroesophageal reflux are widespread among the adolescent population and result in frequent use of health care resources.

Seroprevalence of Helicobacter pylori infection in Siberia.

BACKGROUND: Despite many population-based epidemiological studies of Helicobacter pylori prevalence in different parts of the world, Russia still represents a 'blank spot' in this map. The aims of the present study were to evaluate the seroprevalence of H. pylori infection among the population of Siberia and to assess whether there is an association between abdominal symptoms and H. pylori positivity in our population. METHODS: Six hundred and forty-nine adults from five regions of Siberia and the Far East, and 79 children and adolescents from Novosibirsk participated in the study. Antibodies against H. pylori (IgG) were detected in serum using ELISA. RESULTS: The total percentage of infected adults varied from 70% to 92% depending on the region, and was somewhat lower in three populations living to the north of the 60th latitude compared with those under the 60th latitude. A lack of association was found between the presence of H. pylori and the gender, age and abdominal symptoms. Almost maximum prevalence of H. pylori infection occurred by 30 years of age with only a minor increase in older age groups. Half the children were infected with H. pylori by the age of 15. CONCLUSIONS: Helicobacter pylori infection is widespread among the Siberian populations with some trend in its prevalence to decrease northward. In Siberia age-specific prevalence rates of H. pylori infection are similar to those usual for developing countries.

[Prevalence and risk factors of the development of irritable bowel syndrome in adolescents: a population study].

AIM: To determine prevalence of irritable bowel syndrome (IBS) and associated factors in a community-based population of adolescents. MATERIAL AND METHODS: 189 schoolboys and 250 schoolgirls aged 14-17 years from four randomly chosen schools of Novosibirsk participated in the study. RESULTS: The prevalence of IBS defined according to Rome criteria was higher in girls (24 vs 14% in boys, p = 0.01). Colonic pain frequency was similar in both genders (11% in girls vs 9% in boys, p = 0.46), however disturbed defecation occurred significantly more often in girls than in boys (16 vs 7%, p = 0.003). Half of the subjects who reported IBS had also dyspeptic symptoms. Fiber-rich diet and/or laxatives to prevent constipation were used by 2% of boys and 7% of girls (p = 0.012). However, only one third of those having less than 3 stools a week thought it abnormal. IBS is associated with the age over 15-16 years, female gender, abdominal pain in childhood, appendectomy, irregular meals and smoking (in girls). No association was found with other socio-demographic factors and serological markers of some infections and invasions (H. pylori, G. lamblia, O. felineus, hepatitis viruses B and C). Of the persons with diagnosable IBS, 49% have consulted a physician about it, 21% have undergone upper endoscopy. School absenteeism was reported by 62% of the IBS sufferers. CONCLUSION: IBS is common among adolescents (especially girls) with the prevalence rates similar to those described in adults from West Europe and USA. These disorders are associated with significant disability and health care costs.

Prevalence of Helicobacter pylori infection in the Asian part of Russia.

The purpose of the present study was to evaluate the prevalence of H. pylori infection in various regions of Siberia and the Far East. A representative sample of adult Novosibirsk population participated in the study and four groups of native population of various regions of Siberia were examined, and patients' sera were tested using ELISA. H. pylori is widespread among Siberian populations with prevalence rates ranging between 71% and 92%. Almost maximum prevalence rates of H. pylori infection occurred by the age of 30 years, with only minor increase at older ages.

Prevalence of dyspepsia and irritable bowel syndrome among adolescents of Novosibirsk, western Siberia.

The study was undertaken to determine the prevalence of gastrointestinal symptoms in a community-based population of adolescents. All students in grades 9-11 of four randomly chosen schools in one of the typical districts of Novosibirsk were invited to participate. A total of 449 students participated (189 boys, 260 girls aged 14-17), all of them completed the Russian version of the Bowel Disease Questionnaire. Dyspepsia was reported more frequently by girls than boys (27% vs 16%). The prevalence of irritable bowel syndrome defined according to the Rome criteria was 24% in girls and 14% in boys. Among students with dyspepsia, 49% had sought medical advice, and 37% had undergone upper gastrointestinal endoscopy or X-ray series, for IBS the corresponding figures were 49% and 24%. School absenteeism was reported by 57% of dyspepsia sufferers, and by 62% of subjects with IBS. In conclusion, dyspepsia and IBS are common among adolescents of Novosibirsk with prevalence rates similar to those described in adults in Western Europe and the USA. These disorders are associated with significant disability and health care costs.

Diffuse liver lesions among the general population of western Siberia.

The study was aimed at ascertaining the prevalence of diffuse liver lesions and associated factors in the general population of Novosibirsk, Western Siberia. A representative sample of 362 men and 870 women aged 25-64 years was investigated using ultrasound examination and blood tests for liver-derived enzymes (AST, ALT, GGT). Diffuse liver lesions (DLL) were detected in 42.8% of males and 18.7% of females. A significant association was found between DLL and elevated levels of liver enzymes. In conclusion, the prevalence of chronic liver disease is relatively frequent in the unselected urban population.

[Prevalence of and risk factors for gallstones in female population of Novosibirsk].

AIM: To investigate the prevalence of gallstones and associated factors in female population of Novosibirsk (Western Siberia). MATERIALS AND METHODS: A representative sample of 870 women aged 25-64 years was drawn from general population according to WHO "MONICA" protocol. The subjects were screened for the presence of gallstones by gallbladder ultrasonography, completed a questionnaire relating to food and alcohol consumption, smoking, gastrointestinal symptoms and obstetric history. They also underwent physical examination and blood chemistry tests. Age-adjusted prevalence of cholelithiasis was 9.5%. Increasing age, obesity, diabetes mellitus, consumption of animal fat, pregnancies and opisthorchiasis positively correlated with gallstones in univariate analysis. Serum lipids, family history of gallstones, consumption of alcohol and tobacco were not predictors of gallstones. Only association with age and obesity was significant in multivariate analysis. Among subjects with cholelithiasis 52.1% were not aware of having gallstones. Subjects with gallstones more frequently suffered from biliary colics and non-specific dyspeptic symptoms. However, their predictive value was poor. Cholecystectomized patients revealed more often upper abdominal pain and dyspeptic symptoms. CONCLUSION: Prevalence and risk factors for gallstones in female population of Novosibirsk are similar to those reported in Western European countries. Cholecystectomy is not recommended in patients with symptomless disease.

Helicobacter pylori in a Chukotka Native male population.

Helicobacter pylori (HP) is a widespread human pathogen closely associated with gastroduodenal diseases. Thirty-four males from one of the settlements of coastal Chukotka (72% of total adult male population, mean age 31.6 years) underwent upper endoscopy, and HP was examined histologically in antral biopsies (Giemsa stain). Histology revealed normal mucosa in 16%, antral superficial gastritis in 29%, and atrophic gastritis in 55%. HP was found in 40% of histological specimens of normal mucosa, in 100% of superficial gastritis, and in 77% of atrophic gastritis. Correlation was found between the degree of bacterial contamination and both the activity of gastritis and grade of mononuclear infiltration. Data indicate a high prevalence of HP among Chukotka Natives (77%) which may partly explain the significant frequency of antral gastritis in this population.

[The clinical and morphofunctional characteristics of different types of stomach ulcer].

AIM: The study of a complex of anamnestic, clinicoendoscopic and functional-morphological characteristics in type I and II (according to Johnson) gastric ulcer. MATERIALS AND METHODS: Esophagogastroduodenoscopy, gastric secretion tests, determination of blood group and Rh factor were performed in 91 patients (52 patients with ulcer type I and 39 with ulcer type II). RESULTS: Ulcers type I have arisen in the presence of long-term chronic gastritis. They were associated with marked changes in the mucosa of gastric body, its atrophy and intestinal metaplasia, persistent recurrences in the same gastric zone. Ulcers type II are characterized by hereditary loading, 0(I) blood group, combination with gastroduodenal erosions, season occurrence, trend to migration and recurrence in different gastric or duodenal zones, HCl hypersecretion, high occurrence of Helicobacter pylori infection. CONCLUSION: It is thought valid to include types of gastric ulcer in current classification of ulcer.
