[Stress to the lateral ankle joint ligament complex in confined imaging with an a.-p. radiation path].

Physical definition and laws of rotating systems will be stated. The application for supination in ankle joint shows, that lever action increases the force applied up to four times in case of stress X-rays in the a.-p. view. As quoted in literature, the test power of up to 25 kp means a loading of the lateral collaterals up to 100 kp. An additional iatrogenic rupture of already traumatically partially sprained structures seems possible. The critical judgement of five different methods producing stress X-rays of the ankle joint shows the weak points of this test system. Beck's method seems sufficient for precise statements and judgement of the stress X-ray tests without endangering ligaments so far unsprained.

[Arterial spasm caused by administration of DHE-heparin in a tibial fracture].

A 38 years old white woman suffered a tibial fracture which was primarily managed by traction therapy. 3 X 5000 IU Heparin DHE was given as prophylaxis against thromboembolism. 6 days after administration of the drug a severe arterial spasm (documented by arteriography) with high-grade ischaemia of the left leg occurred. Successful arterial dilatation with a Fogarty catheter within the entire infragenual region down to the foot arteries was performed and consecutively during the same anaesthesia the intramedullary stabilisation using an interlocking nail was also performed. 8 months later the patient was free from symptoms, the fracture had healed, and the function was unrestricted. All pulses were palpable and the ultrasound Doppler index was greater than 1. In conclusion, we recommend in arterial spasm during heparin DHE therapy, immediate operative exposure of the vessel--if possible under local anaesthesia--and adequate vascular surgery as balloon catheter dilatation.

[Local treatment of post-traumatic infections with debrisorb].

Debrisan is an highly hydrophilic dextran polymer in the form of dry, spheric beads. When dry Debrisan beads are placed on a wound, the wound secretions are sucked up into and between the beads. No crust is formed on the wound. Bacteria and granular substances are removed with the secretions. In a prospective study 20 patients were treated with Debrisan. 8 patients had local wound infections and there was a healing-up inert 6.1 days in every case. 12 patients with osteomyelitis had a poor result in 3 cases, because they had still fistulization after 10 days procedure. The treatment of osteomyelitis must be surgical in the first place. Debrisan may only have the character of an adjuvans.

[Surgical treatment of fresh injuries of the Achilles tendon].

We report 35 injuries of the Achilles tendon that have been operated on immediately. Clinical follow-up examinations of 23 patients showed the following results: very good and good in 19 cases, satisfactory in 3 cases and unsatisfactory in one case. Immediate surgery of an injury of the Achilles tendon is clearly preferable to conservative therapy. The functional and the cosmetic results are better after an interwoven Bunell's suture than after the interweaving with the plantaris muscle's tendon. Because of the high rate of infection we do not use the "fibrin adhesive" any more.

Classification of tibial shaft fractures and correlation with results after rigid internal fixation.

A classification of tibial shaft fractures based on etiology, morphology, and clinical features is documented in a series of 291 fractures treated by AO/ASIF rigid internal fixation. The fractures were placed in nine main fracture groups, each with three subgroups according to location in the proximal, middle, or distal segment of the shaft. Group A includes all simple fractures, and Group B includes fractures with butterfly fragments and cortical contact between the main proximal and distal fragments. In Group C there is no such contact. For all degrees of comminution, Group 1 includes the spiral fractures and Groups 2 and 3 the nonspiral bending fractures. Group C2 contains segmental and Group C3 crush and high-velocity projectile fractures.

[Traumatic intraclinoidal aneurysm of the internal carotid artery].

A case of traumatic supraclinoidal aneurysm of the internal carotid artery is presented. The patient with adventitious multiple lesions of the midface was successfully treated by a team consisting of a neurosurgeon, a traumatologist, and a dental surgeon. As an example, this case presents the diagnostic problems, and the operative treatment. Problems of post-traumatic intracranial vasospasm are discussed.

[Material testing of modern plaster substitutes].

4 Plaster substitutes were tested in order to obtain some information about their reaction to stress. 2 materials are made of glass fibre with resin (Scotchcast and Lightcast), one from another artificial product (Baycast) and the fourth consists of a mesh of pure resin (Hexelite). Their resistance to bending forces was correlated to the weight of the samples used. The results indicated that a product containing glass fibre as carrier for the resin is the most robust.

[Complications of arthroscopy. A review of 3714 cases (author's transl)].

This review is drawn from a total of 3,714 knee arthroscopies carried out on patients from six clinics. The diagnostic value of arthroscopy at this site is established for it can improve prognostication, prevention and treatment of derangements of the knee. The complications of the procedure, both theoretical and in practice, have been considered by subdividing them into four groups. Firstly, the infection rate following arthroscopy alone was nil. Secondly, minor articular cartilage damage attributable to the instrument occurred in just over 2% of cases. This complication is more common when the technique is first being learned and is usually avoidable when experience has been gained. Thirdly, complications of a general nature included four cases of subcutaneous emphysema produced by the insufflation of gas, and 22 instances of an allergic skin reaction to the disinfectant preparation. Finally, failures in the technique itself resulted from the instrument bending or breaking when negotiating the condyles. This occurred on 26 occasions. In a further seven instances the fat pad was entered and distended in error, thus preventing examination of the joint. Overall, the complication rate was acceptable low and confirmed that arthroscopy can be safely applied clinically.
