Unruptured ventricular septal wall dissection. A case report.

Dissection of the interventricular septum (IVS) is a rare condition, which can uncommonly complicate an acute myocardial infarction (AMI). We describe a case of unruptured IVS dissection observed 16 days after 2 close episodes of AMI. The diagnosis was made by transthoracic echocardiography. An echo-free space within the thickness of IVS, extended from the apex to the mid-portion, for a total length of about 30 mm was evident. The careful examination of the left ventricle did not reveal any discontinuity of the myocardial wall. The stable clinical condition, the absence of flow within the dissection, the demonstration of its favourable evolution during the hospitalisation and the characteristics of the underlying coronary disease (left anterior descending artery occlusion without myocardial viability) led to the decision of avoiding surgery. The predischarge contrast echocardiographic examination (Levovist) showed clearly the border of the infarcted zone and demonstrated an area reduction and echogenicity increase of the neocavitation, with partially organised thrombi. The patient recovered uneventfully and was discharged on medical therapy with a clinical and echocardiographic follow-up program. We believe that for IVS hemorrhagic dissection a nonsurgical option can be proposed; surgery should only be considered for myocardial revascularization when indicated. A close echocardiographic follow-up is mandatory.

Semiclassical simulations of azomethane photochemistry in the gas phase and in solution.

We present semiclassical simulations of the dynamical events which follow the n --> pi excitation of an azomethane molecule: decay to the ground state, isomerization, and dissociation. Ab initio potential energy surfaces and couplings, slightly modified according to available experimental data, are employed in conjunction with a trajectory-surface-hopping method. For an isolated molecule, we show that dissociation takes place in the ground state, because the radiationless decay is very fast. The dissociation is mainly sequential: the methyldiazenyl radical first produced fragmentates very rapidly to N(2) + CH(3)*. The results of previous experiments by Lee's and Zewail's groups, from which partially conflicting conclusions had been drawn, are reproduced and interpreted. Molecular dynamics, coupled to the surface-hopping algorithm, allows us to simulate the photochemistry in acqueous solution. Here the fragmentation is suppressed because of vibrational energy loss to the solvent: only the isomerization takes place.

The sterols of Cucurbita moschata ("calabacita") seed oil.

From the sterol fraction of seed oil from commercial Cucurbita moschata Dutch ("calabacita") delta 5 and delta 7 sterols having saturated and unsaturated side chain were isolated by chromatographic procedures and characterized by spectroscopic (1H and 13C-nuclear magnetic resonance, mass spectrometry) methods. The main components were identified as 24S-ethyl 5 alpha-cholesta-7,22E-dien-3 beta-ol (alpha-spinasterol); 24S-ethyl 5 alpha-cholesta-7,22E,25-trien-3 beta-ol (25-dehydrochondrillasterol); 24S-ethyl 5 alpha-cholesta-7,25-dien-3 beta-ol; 24R-ethyl-cholesta-7-en-3 beta-ol (delta 7-stigmastenol) and 24-ethyl-cholesta-7, 24(28)-dien-3 beta-ol (delta 7,24(28)-stigmastadienol).

[Application of bioelectric impedance measurement in the evaluation of body fat].

We compared the data referring to the percentage of body fat in 40 healthy adults (20 males and 20 females) of different weight, aged between 18 and 33 years, measured by biological impedance analysis and skinfold thickness measurement. Of these, 26 were shown to be of normal weight, 9 overweight and 5 underweight according to the body mass index (Weight/Height). All subjects were analysed for bioelectrical impedance with a BIA-103 (Ryl-Detroit) appliance and the skinfolds were measured with a Harpenden Caliper. The correlation between the values obtained with the two methods was shown to be linear and highly significant for both sexes (males: r = 0.71; females: r = 0.77). The study also showed that the content of body fat was higher than the mean values indicated in the literature for the age range considered in 23% of the males and 31% of the females of normal weight.

The pyrazine-binding protein and olfaction.

1. The present results provide circumstantial evidence, but not a proof, that the Pyrazine-binding Protein is an odorant carrier molecule of fundamental importance. 2. At first sight a role for a secretory protein in olfaction is not obvious. 3. Odorants freely diffuse in air, in water and in lipids, and the use of carrier proteins, would seem superfluous unless a very special combination with the odorant occurs [Gaupp E. (1902) In Anatomie des Frosches, 2nd Edn, pp. 673. Vieweg-Verlag, Braunschweig]. 4. The possibility should be considered that the Pyrazine-binding Protein and the urinary proteins belong to a large family of species-specific secretory molecules which, with the odorant bound, directly stimulate the receptor cell.

[Regional anesthesia and pharmacological hypnosis].

Regional - mostly spinal and epidural - anaesthesia associated with pharmacological hypnosis in approximately fifty general surgery and orthopaedic patients is described. The advantage of this type of management is that patients are sedated during the operation and in the post-operative period, coupled, in the latter situation, with a good antalgic effect.

[Pediatric anesthesia for adenotonsillectomy].

Seventy fifty cases of anesthesia in children for tonsillectomy are described. About twenty of these are induced to the narcosis with propanidid, thirty with Ketamine, fifteen with Althesin and a little number using mask with N2O/O2 and Halothane. The results obtained showed that these four methods are satisfying and confirmed by a long experience in a large number of cases.

Inhibition by paromomycin of R-factor transfer of tetracycline resistance between Escherichia coli and Salmonella pullorum.

In vitro assays showed that low concentrations (5--10 and 20 microgram/ml) of the antimicrobial paromomycin sulfate are able to block or diminish significantly the transfer of the tetracycline resistance R-factor between Escherichia coli and Salmonella pullorum. This observation is important because it offers the possibility of preventing the formation of tetracycline-resistant pathogens, a limiting factor of tetracycline use in both human and veterinary medicine.

[Comparison of the antibacterial activity in vitro of a combination of penicillin and other antibiotics on 127 strains isolated in an Oncological Institute].

127 pathogenic, opportunist and saprophytic strains isolated from patients and material in a Milanese oncological department were tested. An association of Ampicillin and Dicloxacillin ana (Diamplicil) proved effective against all strains, particularly Gram-negative bacteria notoriously responsible for hospital infections. A statistically inferior performance was observed for Rifampicin and Cephaloridine.

[Indications for the use of external fixator in orthopedics and traumatology during the developmental age].

The paper illustrates the characteristics of external fixators (EF) (Wagner, FEA, Ilizarov) regularly used in pediatric surgery. Attention is focused on the indications for their use in orthopedics and traumatology during the developmental age. In conclusion, their use, even though in traumatology this is more restricted than in adults, reduces many complications which may arise from traditional treatment. In addition, it is well tolerated and allows pathologies to be treated which were difficult to cure in the past.
